75 Background: Beginning in 2015, the Commission on Cancer (CoC) requires accredited facilities to provide eligible patients with a Survivorship Care Plan (SCP). While the benefits of SCPs in the quality and continuity of care are well documented in the literature, there is a dearth of information regarding best practices for implementation. Here we describe lessons learned in the development of a SCP at the Kaiser Permanente Oakland Medical Center (OMC). Methods: Adult patients with a diagnosis of stage 1-3 cancer who received curative treatment were identified by the Northern California Kaiser Permanente Cancer Registrar. Consistent with CoC guidelines for 2015, 10% of all eligible patients (n = 1160) are required to receive a SCP 3-6 months following active treatment. Thus, a total of 116 patients with a diagnosis of breast, colon, lung/bronchus, prostate, or melanoma of the skin at OMC were given a SCP. The implementation timeframe was July to December 2015. Use of a modified ASCO template was recommended for its rigor across the required domains. Following template approval by the Comprehensive Cancer Committee (CCC), a workflow consisting of identifying patients’ last treatment and manual data entry by a Physician Assistant (PA) as well as automated data population in the Kaiser Permanente electronic medical record system was developed. Completed SCPs were then provided to patients during a clinic visit. The workflow process was refined as appropriate based on feedback from staff & physicians. Results: Depending on the scope of treatment, mean preparation time is 10 minutes per plan using specific diagnosis codes via an electronic medical record system. Further, it takes 30 minutes for a PA or other Ancillary personnel to discuss the plan with a patient. Patient response was generally positive upon receipt of the plan. Conclusions: Successful SCP implementation is contingent upon engagement from multiple stakeholders. Per standard CoC 3.3, the implementation rate will increase annually from 10% in 2015 to 100% by 2019. Careful consideration to workflow processes and resources planning be must factored into achieving 100% compliance across all disease sites by 2019.

77 Background: Beginning in 2015, the National Accreditation Program for Breast Centers (NAPBC) requires accredited facilities to provide Survivorship Care Plans (SCP) to breast cancer patients treated with curative intent. Here we describe lessons learned with the use of telemedicine in the delivery of SCPs at Kaiser Permanente East Bay. Methods: SCP implementation followed a two-step process: patients who are diagnosed and complete treatment in 2016 are given a SCP at their last surgical or medical oncology appointment (YTD, n=8); 2) patients diagnosed in 2015 who completed treatment in 2015-2016 were contacted by a physician assistant and offered an appointment by telephone or in-person to deliver the care plan (n=152). Both clinic and telemedicine appointments consist of a detailed review of treatment received, post-treatment follow-up care, discussion of health education resources, and provision of referrals as appropriate. Patients also have the opportunity to ask questions about treatment and follow-up care. Results: Among patients diagnosed in 2015 who were offered a TAV or in-person visit, 44.0% preferred a telephone appointment. Actual appointment duration ranged from 10 to 30 minutes. Lack of co-payment and fees associated with transportation as well as loss of income due to taking time off work were the primary factors cited choosing telemedicine over in-person clinic appointments. Interestingly, no differences in member satisfaction scores for in-person vs. telephone visits were observed. Conclusions: Findings suggest telemedicine may be effective in the delivery of SCPs. Similar to rural settings, use of telemedicine in urban settings has a unique capacity to increase service to breast cancer patients.

ObjectiveTo describe the safety and efficacy of rituximab (RTX) in MS and pregnancy, we conducted a retrospective cohort study of 74 pregnancies among 55 women treated with RTX for MS and their offspring.MethodsWe used prospectively collected information from the electronic health record at Kaiser Permanente Southern California between 2012 and 2019 of mother and baby to identify treatment history, pregnancy outcomes, and relapses.ResultsLast RTX exposure before conception occurred between 1.8 and 5.2 months in 32 (49%) of 65 pregnancies and accidentally during the first trimester in 9 (12%). Among 38 live births, adverse pregnancy outcomes were as follows: 3 preterm deliveries (including 1 set of twins), 1 neonatal death (preterm twin), and 1 perinatal stroke (full-term). No stillbirths, chorioamnionitis, or major malformations were found. Fifteen (27%) women had at least one first-trimester miscarriage, of whom 8 (53%) had a history of infertility. Cumulative dose or timing of last RTX infusion was not associated with an increased risk of miscarriage. Only 2 (5.4%) women experienced relapses, one during pregnancy and the other postpartum.ConclusionWe observed no increase in adverse pregnancy outcomes compared with expected national incidence rates and remarkably little disease activity in RTX-treated women with MS, particularly when compared with periconceptional natalizumab-treated cohorts. However, larger studies are needed to fully assess the safety of RTX use before pregnancy, especially risks associated with prolonged B-cell depletion and hypogammaglobulinemia. Until these data are available, we recommend restricting RTX use before pregnancy to women who require highly effective MS treatments.Classification of evidenceThis study provides Class IV evidence that for pregnant women with MS, RTX controls disease activity and does not increase adverse pregnancy outcomes.

Hindemith's setting of Whitman's When Lilacs Last in the Dooryard Bloom'd has been called his only "profoundly American" work. However, the double entendre of its original subtitle, "An American Requiem," alluding to Brahms's Ein deutsches Requiem, mirrors Hindemith's ambivalence about his own postwar cultural identity. Although the work's intertextual links with the German polyphonic tradition extend back to Bach, "Taps" is the only overt "American" reference. But the phrase in quotation marks within the final subtitle, "A Requiem 'For those we love,' " is the incipit of a World War I hymn of commemoration, "For those we love within the veil." Hindemith quotes verbatim the melody for this hymn from the 1940 Episcopal Hymnal, which identifies it as "Gaza," a "Traditional Jewish Melody" (in turn derived from a Yigdal). The Requiem may be reinterpreted as a covert commentary on Whitman's text from the post-Holocaust perspective of Hindemith's conflicted personal and artistic circumstances.

The critics that have analyzed Jorge Luis Borges’ “Deutsches Requiem” (1946) have usually focused on the moral inconsistencies committed by its protagonist and narrator, a Nazi officer: Otto Dietrich zur Linde. Otto explicitly states that the goal of Nazism was the destruction of Judeo-Christian values, but he is apparently unable to think outside them. All his autobiography is told through the ascetic ideal, which would make his last message incongruous. The asceticism of Otto, of the "fascist subject", is not to my view a deliberate incongruity of Borges's text. I show in this article that Otto’s deeply ascetic nature is the ultimate result of the model of subjectivity developed by western philosophy.

The 'fourth freedom' of freedom of movement of persons – somewhat misleadingly labelled 'European citizenship' – lay at the normative heart of the European project. Although sceptics have often suggested it was part of the building of a European fortress, or even a last gasp of elite European colonial privilege, the essential point of EU freedom of movement was its revolutionary introduction of a regionally expansive non-discrimination by nationality, going well beyond established abstract notions of 'personhood' and human rights on which other global egalitarian movements depend. For sure, it had been battered by roll back in national courts, suspension of Schengen, and new external borderings, well before the Brexit vote. Yet the practice of the fourth freedom in terms of everyday transactions and interactions struck at the heart of the core of the modern Hobbesian nation state: its sovereignty to decide on the boundaries of its own, increasingly de-territorialised population, which was also its power to shore up the most potent source of global inequalities – the birthright lottery which protects the 'wealth of nations' and the privileges of democratic 'peoples' from the unbounded effects of de-territorialised mobilities. As we are also seeing – and hearing among many ostensibly progressive academic voices – the putatively egalitarian voice of people's democracy can be used to further bolster the shrinkage of moral community within the nation state. The essay takes upon itself to evaluate what is being lost normatively in terms of the return of the national – methodologically as much as politically – as the slow motion car crash of Brexit happens and after it takes place.

Brahms's German Requiem stands at the end of a long line of Lutheran funerary music. Luther reworked funeral responsories into a new, totally Biblical form, and later Lutherans collected anthologies of Biblical texts on death and dying. Such sources were used by later composers, including Schüütz and Bach, to compose funeral pieces on Biblical texts together with appropriate chorales. Brahms's opus 45 is similar in that its text is made up of Biblical verses assembled by the composer, and connections may be drawn between chorale usage in this work and the composer's Protestant upbringing in Hamburg on one hand, and in his knowledge of two cantatas by Bach (BWV 21 and 27), on the other. The text and structure of the work accord with general, north German Protestantism, and the famous letter to Reinthaler, which many have taken as a demonstration of Brahms's general humanistic tendencies, shows Brahms to be standing aloof from the theological controversies of his day in favor of a basic understanding of Biblical authors. Part of the problem was that the first performance was scheduled for Good Friday in Bremen cathedral; Reinthaler, the organist, and the cathedral clergy would have preferred passion music of some kind and what Brahms gave them was something different. Brahms surely knew of the distinctive Lutheran observance of "Totensonntag," the commemoration of the dead on the last Sunday in the church year (the Sunday before Advent). There are many similarities between Brahms's Requiem and Friedrich Wilhelm Markull's Das Gedäächtnis der Entschlafen (The Remembrance of those Who Sleep) of ca. 1847. Since Markull's work is subtitled Oratorium füür die Todtenfeier am letzten Sonntage des Kirchenjahres (Oratorio for the Celebration of the Dead on the Last Sunday of the Church Year), it is possible that Brahms had the same occasion in mind when composing his German Requiem.

Polemics about criteria for nontrivial principal components are still present in the literature. Finding of a lot of papers, is that the most frequently used Guttman Kaiser?s criterion has very poor performance. In the last three years some new criteria were proposed. In this Monte Carlo experiment we aimed to investigate the impact that sample size, number of analyzed variables, number of supposed factors and proportion of error variance have on the accuracy of analyzed criteria for principal components retention. We compared the following criteria: Bartlett?s ?2 test, Horn?s Parallel Analysis, Guttman-Kaiser?s eigenvalue over one, Velicer?s MAP and CHull originally proposed by Ceulemans & Kiers. Factors were systematically combined resulting in 690 different combinations. A total of 138,000 simulations were performed. Novelty in this research is systematic variation of the error variance. Performed simulations showed that, in favorable research conditions, all analyzed criteria work properly. Bartlett?s and Horns criterion expressed the robustness in most of analyzed situations. Velicer?s MAP had the best accuracy in situations with small number of subjects and high number of variables. Results confirm earlier findings of Guttman-Kaiser?s criterion having the worse performance.

Niels Krabbe: Paul von Klenau and his ninth Symphony – the sources, the work, the reception In 2001, the Royal Library learned about a comprehensive private collection in Vienna that contained music, letters and lecture manuscripts, photographs and other archive materials of the Danish composer Paul von Klenau (1883–1946). A preliminary survey of the collection revealed that the contents included a number of music manuscripts (symphonies, chamber music concerts and more), which were not known from the rest of the library’s major collection of Klenau works. The collection’s greatest and most interesting work was a major complete “Ninth Symphony” for orchestra, choir and four soloists in eight movements, for a Latin text with a mix of liturgical texts from the Catholic requiem and texts of unknown provenance.In 2005, the library succeeded in acquiring the collection and it was transferred to the Royal Library. Subsequently, the Danish Centre for Music Publication (DCM) organised a philological adaptation and published Symphony No. 9 for the purpose of the premier performance of the work, which duly took place 70 years after it was written, performed as a Thursday Concert in March 2014 and conducted by Michael Schønwandt.Klenau had worked in Germany as a composer and conductor in the 1920s and 1930s. He returned to Denmark in 1939 where he stayed for the rest of his life. Because of his extensive German background he did not receive high recognition in Danish music, despite the range and nature of his musical output. This was mainly because of his relationship with the Third Reich and Nazism, which affected his last years and his posthumous reputation.Symphony No. 9 was composed in the years 1944–45, and is a mix of requiem and a symphony, each in four movements. Due to the text, the work is both a traditional requiem and a requiem about the war. Both in its expression and in its length, it is probably the greatest symphony ever written by a Danish composer.The premier in 2014 received mixed reviews, and Klenau’s attitude to Nazism was discussed once again. The work was criticised for its eclectic character with its mix of late romantic forms of expression on the one side and its accomplished dodecaphonic passages on the other.The newly available Klenau collection from Vienna, including the treated Symphony No. 9, has nuanced and problematised Klenau’s position in Danish music history.

With the death of Joonas Kokkonen in October of 1996, Finland lost one of its most important post-World War II composers. Almost certainly, Kokkonen is most widely known outside Finland for his 1975 opera Viimeiset kiusaukset (Tlie Last Temptations). Yet his orchestral compositions such as the Cello Concerto (1969), the song cycle Lintuijen Tuonela (The Hades of the Birds, 1958–59), the Requiem (1981), the chamberorchestra work …Durch einen Spiegel… 1977), or such chamber works as the Piano Quintet (1953) or the String Quartets Nos.l (1959) and 3 (1976) demonstrate that masterpieces may be found in virtually every genre of Kokkonen's output. (Oddly, piano music represents a small and minor position in Kokkonen's oeuvre – a surprising fact given his accomplishments as a pianist.)

Of Philip Glass's twelve symphonies - perhaps the most popular minimalist composer after John Cage – it is the choral Fifth, entitled "Requiem, Bardo, Nirmanakaya", composed in the years of full maturity (the first performance took place in 1994), that mostly impresses through its grand design, display of instruments and human voices, soloists and four choral ensembles may be considered a version of the theocratic scenario that projects the history of mankind in the order of the divine, characteristic of the premodern age. Glass's originality consists in the overlapping of several mythical structures - Swedish, biblical, Buddhist, Islamic, Sufi, etc. - in accordance with the holistic epistemology of the contemporary era. Despite the Buddhist references in the title, the narrative is modelled on the biblical archetype, from the genesis of the universe to the Last Judgment and it ends with the vision of a future humanity bonded in brotherhood, peace and compassion

The fates of the last Emperors of Russia and Germany are forever linked by the First World War and the revolutions. History knows the circumstances of their abdication; however, their motives still remain a matter of debate. The present research featured a comparative analysis of various points of view on the roles Nicholas II and Wilhelm II played in the revolutions and the political decisions they made. The biographical research clarified the general characteristics of the revolutionary processes that occurred during their reign and how both revolutions affected the institution of monarchy. The authors analyzed the contemporary historiographical discussions of the monarchy and the revolutions caused in both countries by the consequences of the First World War. The article also focuses on the role of social, class, and political groups in both revolutions, as well as on the attempts of monarchists to save the emperors. Tsar Nicholas II and Kaiser Wilhelm II lived in a very similar political context: complicated family ties, rapid economic development, the Great War, etc. Their abdications occurred in similar circumstances and had similar motives and socio-political consequences. However, their further destinies were very different.

Abstract Background With the advent of primary human papillomavirus testing followed by cytology for cervical cancer screening, visual interpretation of cytology slides remains the last subjective analysis step and suffers from low sensitivity and reproducibility. Methods We developed a cloud-based whole-slide imaging platform with a deep-learning classifier for p16/Ki-67 dual-stained (DS) slides trained on biopsy-based gold standards. We compared it with conventional Pap and manual DS in 3 epidemiological studies of cervical and anal precancers from Kaiser Permanente Northern California and the University of Oklahoma comprising 4253 patients. All statistical tests were 2-sided. Results In independent validation at Kaiser Permanente Northern California, artificial intelligence (AI)-based DS had lower positivity than cytology (P < .001) and manual DS (P < .001) with equal sensitivity and substantially higher specificity compared with both Pap (P < .001) and manual DS (P < .001), respectively. Compared with Pap, AI-based DS reduced referral to colposcopy by one-third (41.9% vs 60.1%, P < .001). At a higher cutoff, AI-based DS had similar performance to high-grade squamous intraepithelial lesions cytology, indicating a risk high enough to allow for immediate treatment. The classifier was robust, showing comparable performance in 2 cytology systems and in anal cytology. Conclusions Automated DS evaluation removes the remaining subjective component from cervical cancer screening and delivers consistent quality for providers and patients. Moving from Pap to automated DS substantially reduces the number of colposcopies and also achieves excellent performance in a simulated fully vaccinated population. Through cloud-based implementation, this approach is globally accessible. Our results demonstrate that AI not only provides automation and objectivity but also delivers a substantial benefit for women by reduction of unnecessary colposcopies.

Abstract In the diverse space of contemporary music, the fascinating and controversial personality of the Welsh composer Karl Jenkins, which is surprising from several perspectives, stands out. Open to assimilating and processing music from various sources (academic, liturgical, folk, entertainment, oriental, exotic), the all-round musician Karl Jenkins impresses the public with unexpected artistic choices, giving up the hypostasis of instrumentalist of the jazz-rock band Nucleus and of the group Softmachine in favour of the postmodern creator he has become today, synthetizing trends from musical compositions of the last decades of the 20th century. Once with the return to the functional system, either through minimalism or through neo-romanticism, the artist has successfully covered a potential sonority path of modern opposites, also evoking references to creative models of the past. We are referring to the musical valorizing of the sacred in a synthetic vision between tradition and innovation, in the works included in the Adiemus cycle, in the opus choir Missa for Peace and, more particularly, in the Requiem (2005), a significant score in the contemporaneity. The manner in which the composer, while resorting to a musical genre originating from the Roman Catholic cult and drawing on the liturgical text of the Mass for the dead, inserted Japanese poetry, written following the structure of haiku, belonging to representative authors - Gozan Koshigaya, Issho Kosughi, Hokusai Katsushika, Kaga-no-Chiyo, is highly surprising. This study aims to highlight the interweaving imagined by Karl Jenkins between the two cultures as well as to conduct a semantic analysis of an opus in which the relationships between music and words entail a highly emotional response.

Abstract By consulting monographies, musicological studies, specialty articles about the personality of romantic musician Hector Berlioz and implicitly linked to the relevance of his significant opera, one discovers researchers’ constant preoccupation for historical, stylistic, analytical, hermeneutical comments upon aspects related to established scores (the Fantastic, Harold in Italy symphonies, dramatic legend The Damnation of Faust, dramatic symphony Romeo and Juliet, the Requiem, etc.). Out of his compositions, it is remarkable that the cycle Les nuits d´été was rarely approached from a musicological point of view, despite the fact that it is an important opus, which inaugurates the genre of the orchestral lied at the end of the 19th century and the beginning of last century. In this study, we set out to compose as complete as possible an image of this work, both from an analytic-stylistic point of view by stressing the text-sound correspondences and, above all, from the perspective of its reception at a didactic level, by promoting the score in the framework of listening sessions commented upon as part of the discipline of the history of music. In what follows, I shall argue that the cycle of orchestral lieder Les nuits d´été by Hector Berlioz represents a work of equal importance to established opera.

Scanning tunneling microscopy has proven to be an enormously useful technique for probing the atomic scale electronic structure of clean semiconductor surfaces. In particular, application of STM current-voltage spectroscopy over the last several years has begun to yield detailed information about the energies and spatial locations of electronic bonds at surfaces. Recent work by Kaiser and Bell shows that STM can also be used to elucidate the nature of electronic transport across interfaces buried tens of nanometers beneth the surface. We are attempting to extend the STM current-voltage technique, which has been so successful at addressing near surface electronic properties, to allow the study of the more general class of opto-electronic properties near semiconductor surfaces: We have constructed a novel Scanning Tunneling Microscope/Electro-luminescence apparatus. Our instrument combines a high speed STM with a stationary tunnel tip/collection optics assembly and a standard spectrometer and photodetector. The system allows us to study the spectral composition and intensity of the light generated by various inelastic processes during electron tunneling.

A study conducted by the Kaiser Family Foundation in the United States during the turn of the millennium showed that children in America spent almost five hours watching television and two hours in front of the computer. As recently as June 2010, the World Summit on Media for Children and Youth held in Karlstad, Sweden held sway on the topic- ‘Towards the new global vision for children, youth and media’. Alberto Pellai, the Summit keynote speaker (representing the department of Public Health at the University Degli Studi Di Milano, Italy) touched upon the need for media education emphasizing on media content that had to educate and help in the growth and development of children rather than mislead the beliefs and value system of children. Statistics reveal new media as being a strong contender for top spot of favoured past time among children. It no longer surprises people that the time spent by children on new media usage has tripled in the last ten years in the US (Kaiser Family Foundation report). Children in urban India from all accounts are no different from their American counterparts. This trend needs to be pondered, debated and questioned. The researcher would like to explore the different issues related to children and new media usage from an urban perspective since new media is more accessible to urban Indians for obvious reasons- finance, socio-cultural readiness towards new, innovative medium of communication (new media) and educational background. Through interactive sessions with school students, parents and teachers the researcher would like to find out- why children use new media, the content used by children, parent perception of new media usage by their children, teacher’s view on new media usage by students. According to Bagman, a popular blogger, there are two kinds of people, “Whenever there is a question of a new medium, there are those who are excited because of it and those who are wary of it.” The researcher too, through this paper, would like to know which way people in urban India are inclined.

The government of Indonesia has long experienced an uneven pattern of budget realization. Our budget realization is characterized by small absorption in the first three-quarters and then piled up in the last quarter. An increase in spending at the end of the year eventually led to the quality of work on the national economy, which is not considered optimal. Through factor analysis, the researchers reviewed what factors are causing slow realization of the budget, especially for spending unit in the working area of KPPN Jakarta II. Several studies have been conducted to determine the problem, including Herriyanto (2012), BKF, LPEM-UI and IBRD (2012), Siswanto and Rahayu (2010), Miliasih (2012), Widjanarko (2013), and Fitriany (2015). Based on the factor analysis that has been conducted, it was found six factors that often slow down the realization of central government expenditure, especially for spending unit in working area of KPPN Jakarta II. The six factors include coordination, organizational culture, competence, technical constraints, administrative, and document. These six factors are derived from 27 indicators that were processed through the standard factor analysis, i.e. correlation between variables Kaiser Mayer Olkin (KMO), variables distribution and rotation of factors.

Abstract. The comment by Nicholson (2011a) questions the "consistency" of the "definition" of the "biological end-member" used by Kaiser (2011a) in the calculation of oxygen gross production. "Biological end-member" refers to the relative oxygen isotope ratio difference between photosynthetic oxygen and Air-O2 (abbreviated 17δP and 18δP for 17O/16O and 18O/16O, respectively). The comment claims that this leads to an overestimate of the discrepancy between previous studies and that the resulting gross production rates are "30% too high". Nicholson recognises the improved accuracy of Kaiser's direct calculation ("dual-delta") method compared to previous approximate approaches based on 17O excess (17Δ) and its simplicity compared to previous iterative calculation methods. Although he correctly points out that differences in the normalised gross production rate (g) are largely due to different input parameters used in Kaiser's "base case" and previous studies, he does not acknowledge Kaiser's observation that iterative and dual-delta calculation methods give exactly the same g for the same input parameters (disregarding kinetic isotope fractionation during air-sea exchange). The comment is based on misunderstandings with respect to the "base case" 17δP and 18δP values. Since direct measurements of 17δP and 18δPdo not exist or have been lost, Kaiser constructed the "base case" in a way that was consistent and compatible with literature data. Nicholson showed that an alternative reconstruction of 17δP gives g values closer to previous studies. However, unlike Nicholson, we refrain from interpreting either reconstruction as a benchmark for the accuracy of g. A number of publications over the last 12 months have tried to establish which of these two reconstructions is more accurate. Nicholson draws on recently revised measurements of the relative 17O/16O difference between VSMOW and Air-O2 (17δVSMOW; Barkan and Luz, 2011), together with new measurements of photosynthetic isotope fractionation, to support his comment. However, our own measurements disagree with these revised 17δVSMOW values. If scaled for differences in 18δVSMOW, they are actually in good agreement with the original data (Barkan and Luz, 2005) and support Kaiser's "base case" g values. The statement that Kaiser's g values are "30% too high" can therefore not be accepted, pending future work to reconcile different 17δVSMOW measurements. Nicholson also suggests that approximated calculations of gross production should be performed with a triple isotope excess defined as 17Δ#≡ ln (1+17δ)–λ ln(1+18δ), with λ = θR = ln(1+17ϵR ) / ln(1+18ϵR). However, this only improves the approximation for certain 17δP and 18δP values, for certain net to gross production ratios (f) and for certain ratios of gross production to gross Air-O2 invasion (g). In other cases, the approximated calculation based on 17Δ† ≡17δ – κ 18δ with κ = γR = 17ϵR/18ϵR (Kaiser, 2011a) gives more accurate results.

Assuming that the Uniformitarian Principle refers to processes of production and perception, I argue that it remains invaluable for work on language in the last five thousand years or so, the period of linguistic historical record. In this article I show that some proposals about the development of ‘insubordination’ (Evans 2007), particularly those that link it to degrammaticalization (e.g. Higashiizumi 2006 onbecause-monoclauses; Brinton 2014 onas ifclauses), are artifacts of theory (Kaiser & Struckmeier 2015), and do not conform to processes that can be projected from a Uniformitarian Process Principle that pays attention to interactional practices. I investigate evidence in the history of English for the development of finite independent monoclauses that are introduced by subordinators, for example,Because you don't understand,If we could see that picture again,As if you are not gorgeous. I conclude that, at least in English, such monoclauses are chunks that are used incrementally in on-line interaction, just like independent NPs and prepositional and adverbial phrases (Fordet al. 2002; Couper-Kuhlen 2011; see also Lindström & Londen 2008 on Swedish monoclauses with subordinators; Gras & Sansineña 2015 on Spanish monoclausalque-constructions). In such cases degrammaticalization is not relevant. The Uniformitarian Processes Principle can serve as an important corrective on artifacts of theory.

70 Background: Improving quality of death (QOD) is a key goal in palliative care. No instruments to measure QOD have been validated in Spanish. The goal of this study was to validate the Spanish version of the quality of death and dying questionnaire (QODD) and to develop a shortened version of this instrument suitable to be completed by phone interview. Methods: Translation/back-translation and adaptation of the QODD was performed following established standards. One question was removed due to cultural inadequacy. We enrolled caregivers (CGs) of consecutive deceased cancer patients from a single palliative care clinic in Santiago, Chile. CGs were contacted by phone between 4 and 12 weeks after patients’ death. Participating CGs completed the adapted QODD (QODD-ESP) by phone interview. A global rating question assessing quality of life (QOL) during the last week of life was included. A shortened version of the QODD (QODD-ESP-12) based on the model by Downey et al. (J Pain Sympt Manage 2010;39) was created. Measures of reliability (Cronbach-alpha), convergent validity (correlation with global rating question) and construct validity (factorial analysis [FA]) were estimated for both versions of the instrument. Results: 150 (50%) out of 302 CGs completed the QODD-ESP. Patients mean (standard deviation [SD]) age was 67 (14), 71 (47%) female and 131 (87%) died at home. CGs mean (SD) age was 51 (13), 128 (85%) female. Mean QODD-ESP score was 69 (range 35-96) with an alpha = .829. Correlation with last week QOL was .434 (p < .01). Kaiser-Meyer-Olkin measure of sampling adequacy was .585, not supporting the existence of a unique underlying construct. Mean QODD-ESP-12 score was 69 (range 31-97) with a alpha = .728. Correlation with last week QOL was .306 (p < .01). Confirmatory FA of QODD-ESP-12 showed that data fitted well Downey’s model (Chi2 4.85 (60), p = .311 Comparative Fit ndex = .98; Tuker-Lewis Index = .977 and root mean square error of approximation (RMSEA) = .023 (CI: 0-.056). Conclusions: QODD-ESP-12 is a reliable and valid instrument with good psychometric properties and can be used to assess QOD in a Spanish speaking cancer palliative care population by phone interview.

Abstract OBJECTIVE: Patients with brain arteriovenous malformation (AVM) are at life-threatening risk of intracranial hemorrhage (ICH). Identification of genetic variants associated with increased new ICH risk would facilitate risk stratification and guide therapeutic intervention. METHODS: Brain AVM patients evaluated at University of California, San Francisco or Kaiser Permanente Northern California were followed longitudinally. Primary outcome was new ICH after diagnosis; censoring events were any AVM treatment or last follow-up examination. The association of ApoE ɛ2 and ɛ4 genotype with new ICH was evaluated by Kaplan-Meier survival analysis and further characterized via a Cox proportional hazards model. RESULTS: We genotyped 284 brain AVM patients (50% women; 57% Caucasian; median follow-up time, 0.3 yr) including 18 patients with a history of new ICH). ApoE ɛ2, but not ApoE ɛ4 genotype, was associated with new ICH (P = 0.0052). ApoE ɛ2 carriers had fivefold increased risk of new ICH (hazard ratio, 5.09; 95% confidence interval, 1.46–17.7; P = 0.010; Cox proportional hazards model adjusting for race/ethnicity and clinical presentation). Subset analysis in the largest homogenous ethnic subcohort (Caucasians) confirmed the increased risk of new ICH in ApoE ɛ2 carriers (hazard ratio, 8.71; 95% confidence interval, 1.4–53.9; P = 0.020; multivariate model adjusting for clinical presentation). CONCLUSION: ApoE genotype may influence the risk of ICH in the natural course of brain AVM. The identification of genetic predictors of ICH risk may facilitate estimation of AVM natural history risk and individualize clinical decision-making and therapeutic recommendations.

This prospective, single-blinded study on tonsillectomy was done to compare the Bovie electrocautery dissection technique with the epinephrine-and-lidocaine injection technique and to document which technique is safer, faster, and less morbid. Twenty-nine patients who were scheduled for tonsillectomy at two Northern California Region Kaiser Permanente Medical Centers were enrolled in the study. Each patient served as his or her own control, and tonsillectomy technique was randomized on the basis of even or odd numbers of the last digit of the medical record number. The time of injection of epinephrine and time of dissection for each tonsil was recorded. Blood loss was quantified for each dissection, and pain was assessed by asking the patient which side hurt more. I found no statistically significant difference in operating time, intraoperative blood loss, or postoperative hemorrhage between these two methods. The electrocautery, or Bovie, technique produced more clinically significant eschar and delayed healing than the epinephrine-injection technique did. The Bovie technique produced more inadvertent burns to surrounding tissue than the epinephrine-injection technique did, but the epinephrine caused transient tachycardia in 14% of the study participants. The present study showed no difference between the two techniques in postoperative pain experienced by study participants, but other recent studies have shown that patients experience more pain when the Bovie technique is used. Study data do not support the superiority of Bovie tonsillectomy. I recommend epinephrine-injection tonsillectomy as a safe and expedient operation.

Background and Purpose: Currently, there is no guideline or standard of practice for performing the psychiatric/psychological evaluation that is a requirement for approval for bariatric surgery. The Readiness to Change for Bariatric Surgery Assessment Tool (RCB-SAT) establishes a means for psychiatric evaluators to objectively assess the patient’s cognition, beliefs, and motivation around the bariatric diet and lifestyle changes. Development of a clinical decision-making tool for assessing readiness to change in bariatric patients will be useful regarding The Strategic Plan for NIH Obesity Research. The strategic plan outlines 6 overarching themes, with the last 3 centering around creation of such a clinical decision-making tool to assess a bariatric patient’s readiness to change: evaluate promising strategies for obesity prevention and treatment in real-world settings and diverse populations, harness technology and tools to advance obesity research and improve health care delivery, and facilitate integration of research results into community programs and medical practice (National Institutes of Health, 2011). Methods: The pilot tool was administered to 153 potential bariatric patients, with 61 patients completing the survey a second time. Face and content validity of the items were established through an expert review process. Results: Principle axis factoring by means of varimax rotation with Kaiser normalization identified 15 items loading on 3 factors associated with Prochaska and DiClemente’s transtheoretical model of health behavior change: precontemplation, contemplation, and action (DiClemente & Prochaska, 1998). Test–retest reliability was also established for the tool. Implications for Practice: The proposed RCB-SAT demonstrates potential for assessing a patient’s readiness to change regarding the bariatric diet and lifestyle.

ObjectiveTo determine whether MS disease-modifying therapies (DMTs) can be safely discontinued in patients aged 50 years or older with suspected benign/burnt-out MS and to define criteria to identify such patients.MethodsWe conducted a retrospective cohort study of 136 patients with suspected benign/burnt-out MS who discontinued DMTs from the electronic health record (EHR) at Kaiser Permanente Southern California.ResultsThe majority discontinued an injectable DMT (n = 131, 96%). At the time of DMT discontinuation, mean and SD for age was 60.6 (6.2) years, disease duration 19.5 (10.7) years, and time since last relapse 11.0 (7.2) years. After a mean duration of follow-up of 5.0 years post-DMT discontinuation, 5 (3.7%) patients had a relapse, 2 (1.5%) had mild residual deficits, and 3 (2.2%) had asymptomatic MRI disease activity. Patients with MS disease activity following DMT discontinuation were younger (median = 53.6 years) than those who remained disease activity free. Fifty patients (36.8%) had only 1 lifetime relapse, of whom 1 relapsed post-DMT discontinuation. Sixty (56.6%) of 106 patients with spinal cord MRIs before discontinuation showed demyelinating lesions.ConclusionsDMT discontinuation in older patients with suspected benign/burnt-out MS appears safe. Our findings suggest that MRI evidence of spinal cord involvement does not preclude the possibility of benign/burnt-out MS, and for those with 2 or more lifetime relapses, a benign/burn-out classification is best reserved for those aged 55 years and older. Future studies to determine whether DMT discontinuation is safe at a younger age in patients with a single lifetime relapse are needed.Classification of EvidenceThe study provides Class IV evidence that DMTs can be safely discontinued in older patients with suspected benign/burnt-out MS.

147 Background: Kaiser Permanente (KP) is an integrated care delivery system that provides clinical services to over 9 million members in nine states and the District of Columbia with the goal of providing high-quality and affordable health care to our patients. Over the last decade, the rapid adoption of prostate cancer (Pca) screening practice and technology have resulted in increased detection of Pca and helped identify opportunities for care improvement. KP initiated improvements of continuum of services provided to Pca patients. Methods: Transforming the care provided to Pca patients evolved into the first population-based cancer program that manages the entire continuum of care by taking advantage of our integrated health delivery system. Major accomplishments include appropriate Pca screening to prevent over-diagnosis (Prostate Cancer Screening), improved patient safety and claims reduction to prevent missed abnormal cancer screening (PSA Safety Net), comparative effectiveness of cancer treatment choices to improve quality, and benchmark outcomes in efficiency and clinical quality utilizing a new technology (Robotic Surgery). In addition, the program has achieved superior outcomes and value through innovative chemotherapy management for advanced disease (Lupron Management). Evidence-based medicine, research, analytics and continuous quality improvementare cornerstones of the program, while the pinnacle is our patients, who receive informed, shared decision making and equitable unbiased care. Results: We strongly believe that measures of how well our patients are living with Pca are as important as the cancer-specific survival measures; because of this, we established a prospective registry to measure quality of life for every newly diagnosed patient and stratified by all treatment types. This practice is being disseminated across the Program. Conclusions: By systematically stratifying this diverse population, we have been able to achieve efficient reliable care, and spread each successful process to other regions through collaboration with KP’s Interregional Chiefs of Urology.

Abstract Background Clostridium difficile is a major cause of severe diarrhea in the U.S. We described characteristics of Kaiser Permanente Northern California (KPNC) members with C. difficile infection (CDI), identified risk factors associated with CDI, and developed risk scores to predict who may develop CDI. Methods Retrospective cohort study with all KPNC members ≥18 years old from May 2011 to July 2014 comparing demographic and clinical characteristics for those with and without lab-confirmed incident CDI. We included CDI risk factors in logistic regression models to estimate the risk of developing future CDI after an Identification Recruitment Date (IRD), a time when an individual might be a good candidate for a C. difficile vaccine clinical trial. Two risk score models were created and cross validated (70% of the data used for development and 30% for testing). Results During the study period, there were 9,986 CDI cases and 2,230,354 members without CDI. CDI cases tended to be ≥65 years old (59% vs.. 21%), female (61% vs. 53%), and white race (70% vs. 53%), with more hospitalizations (42% vs. 3%), emergency room visits (51% vs. 14%), and skilled nursing facility stays (25% vs. 0.6%) in the year prior to CDI compared with members without CDI. At least 10 office visits within the prior year (53% vs. 16%), use of antibiotics in last 12 weeks (81% vs. 11%), proton pump inhibitors in the last year (36% vs. 7%), and multiple medical conditions within the prior year (e.g., chronic kidney disease, congestive heart failure, and pneumonia) were important risk factors for CDI. Using a hospital discharge event as the IRD, our risk score model yielded excellent performance in predicting the likelihood of developing CDI in the subsequent 31 – 365 days (C-statistic of 0.851). Using a random date as the IRD, our model also predicted CDI risk in the subsequent 1–30 days (C-statistic 0.658) and 31–365 days (C-statistic 0.722) reasonably well. Conclusion CDI can be predicted by increasing age, medications, comorbidities and healthcare exposure, particularly ≥10 office visits, hospitalizations, and skilled nursing stays in the prior year and recent antibiotics. The formation of scientific research activity (SRA) abilities in comprehensive school is undoubtedly, a very important sphere that has not received a proper attention yet. Current teaching and learning process has changed, in fact, from the point of view of paradigm – subjects taught are becoming a means of the realization of learners’ needs and interests. Scientific research activity is not an entertainment but responsible, thorough work requiring a lot of self-independence. During such an activity pupils’ analytical thinking gets stronger, information searching and using abilities are formed, they learn how to analyze gathered material, prepare reports, make research presentations and so on. The research was carried out between January and April, 2010. The method of the research was survey in written form (questionnaire). On the whole, 380 questionnaires were acknowledged acceptable. The article deals with Natural science and Mathematics teachers’ opinion about the situation of scientific research activity in comprehensive school. A factor analysis was performed in order to find out the structure of 41 items. The main aim of the factor analysis is to reduce the number of variables. Two methods were applied in order to evaluate whether the data set was appropriate for the factor analysis: Bartlett`s Test of Sphericity and Kaiser-Meyer-Olkin (KMO) test. Kaiser-Meyer-Olkin Measure of Sampling Adequacy test value is 0.844. Bartlett's Test of Sphericity tests the null hypothesis that the correlation matrix is an identity matrix. In this case approx. Chi-Square value is 3921.835 and p<0.000. These results clearly show that data can be used for factor analysis. The eight factors were extracted based on the Eigen Value Statistics (with the real value more than one). Also the significance (popularity) indexes (SI) and standards deviations (SD) for each factor were calculated. 1. Interest in scientific research activity (SI=0.67; SD=0.17). 2. Ignoring the scientific research activities (SI=0.59; SD=0.21). 3. Adverse (unfavourable) conditions of carrying out of scientific research activity (SI=0.43; SD=0.23). 4. Fragmentary of scientific research activity (SI=0.65; SD=0.20). 5. Adverse (unfavourable) material conditions of scientific research activity (SI=0.65; SD=0.20). 6. Lack qualification (competence) of teachers (SI=0.70; SD=0.25). 7. Employment (busyness) of learners and teachers (SI=0.68; SD=0.21). 8. Influence of the educational environment on scientific research activity (SI=0.76; SD=0.19). However, we need to emphasize, that significance indexes are relatively not high, though higher than 0.50. All these factors accounts for 47.00% of variance. There are 9 statements under the first factor, 6 statements under the 2nd factor, 5 statements under the 3rd factor, 4 statements under the 4th factor, 6 statements under the 5th factor, 3 statements under the 6th factor, 4 statements under the 7th factor and 3 statements under the 8th factor. The first factor forms 7.56% of total variance, the second factor constitutes 7.17% of total variance, the third factor forms 6.00% of total variance, the fourth factor forms 5.92% of total variance, the fifth factor constitutes 5.81% of total variance, the sixth factor forms 5.28% of total variance, the seventh forms 5.05% of total variance and the last one (eighth factor) constitutes 4.17% of total variance. It was found that in the teaching of scientific research activities the cooperation methods are used. It was found that in general the most favourable to SRA are biology and chemistry teachers, and the less favourable - teachers of geography. It was also found that highly significant factor in the development of scientific research activity is the educational environment. AIMS: To evaluate the psychometric properties of the satisfaction scale of patients with the care of the family physician (SatMF17).METHODS: Methodological study, in which users enrolled in Family Health Units and Personalized Health Care Units of the Marão and Douro Norte Health Centers. The criteria for inclusion of the users were: to be a patient enrolled in one of the two referenced units, to be over 18 and under 65 years old, to be able to read and write and to have had, at least, one visit in the last year in one of the two units. The SatMF17 scale consists of 17 items, answered by a Likert scale ranging from 1 (bad) to 5 (excellent), in which the sum ranges from 17 to 85. The data collection was carried out in the period between January and June 2016. The patient who met the inclusion criteria upon arrival at the health unit was invited to participate in the study by the administrative assistant. After a presentation and explanation of the objectives of the study, and signing of the consent term, the questionnaire was offered to the patient, who at the end of self-completion inserted it into an envelope. For statistical analysis, the statistical resources were Cronbach's alpha coefficient, exploratory factorial analysis, Kaiser-Meyer-Olkin criteria, Bartlett's sphericity test and Pearson's correlation coefficient.RESULTS: The sample included had 284 patients, of whom 159 (56,0%) were female. The mean age was 41,2 years, ranging from 18 years to 65 years. The exploratory factorial analysis in main components and varimax rotation revealed a factorial solution of two factors with eigenvalues greater than 1, which together explain 72,48% of the total variance of patient satisfaction with the care of the family doctor.CONCLUSIONS: The SatMF17 Scale is reliable and valid and can be used in populations with similar characteristics, constituting an instrument of practical utility to evaluate the satisfaction of patients with the care of the family doctor.

ObjectivesPeople with HIV (PWH) have a high burden of bacterial sexually transmitted infections (STIs). We examined the relationship of alcohol and drug use and partner pre-exposure prophylaxis (PrEP) use to STI prevalence in a cohort of PWH with a history of unhealthy alcohol use.MethodsWe analysed data from a primary care-based alcohol intervention study at Kaiser Permanente Northern California (KPNC). Participants were recruited between April 2013 and May 2015 and were followed for up to 24 months. We linked participant responses to questions from the 24 month follow-up interview, including alcohol and drug use and partner PrEP use, with STI test results (ie, syphilis, chlamydia, gonorrhoea) in the KPNC electronic health record. Prevalence ratios (PR) were estimated using Poisson models fitted with robust variance estimators to evaluate the association of substance use and partner use of PrEP with STIs.ResultsIn the analytic sample (n=465), the median age was 52 years (IQR 45–59); 67% were white; 95% were men who have sex with men. Thirty-two per cent of participants had HIV-positive partners only; 31% had HIV-negative partners with at least one on PrEP in the previous year and 37% had HIV-negative partners without any on PrEP. Twenty-three per cent reported alcohol and drug use prior to sex in the last 6 months. Eight per cent of participants had an STI. Partner PrEP use (adjusted PR (aPR) 2.99 (95% CI 1.11 to 8.08)) was independently associated with higher STI prevalence. Participants who reported use of alcohol (aPR 1.53 (0.61 to 3.83)), drugs (aPR 1.97 (0.71 to 5.51)) or both (aPR 1.93 (0.75 to 4.97)) prior to sex had a higher STI prevalence.ConclusionsThe higher prevalence of STIs among PWH with unhealthy alcohol use who have partners on PrEP suggests that this subgroup may be a high-yield focus for targeted outreach, STI screening and sexual health counselling.

338 Background: Identifying participants for time-sensitive clinical trials such as those with newly diagnosed cancer presents challenges. Manual identification methods, the current standard, can be resource intensive and present challenges with data collection, quality assurance and can introduce bias to the selection process. Electronic health record (EHR) data, designed to optimize clinical care, quality, and billing, may also streamline participant identification and facilitate recruitment. We aim to develop an EHR-based study eligibility program (SEP) to identify participants for recruitment to the Cancer Financial Experience (CAFÉ) randomized trial. Methods: CAFÉ is a NIH-funded randomized trial to test the impact of a novel financial navigation intervention on financial hardship and quality of life for cancer patients. Inclusion criteria: currently enrolled at Kaiser Permanente (KP) Northwest or KP Washington, age ≥18, had a visit to a CAFÉ-participating oncology clinic within last 14 days (defined as qualifying visit), diagnosed with invasive cancer (new or recurrent) within 120 days of qualifying visit, 6 months continuous enrollment prior to qualifying visit, English or Spanish speaker. Exclusion criteria: diagnosis of non-melanoma skin cancer, diagnosis of benign or in-situ tumors, hospice care within last 12 months, unable to complete baseline survey, household member enrolled in CAFÉ study. To assess the validity of the SEP for use in CAFÉ, we extracted EHR data on study inclusion/exclusion criteria for n = 125 patients across both study sites between 3/1/19–2/29/20 to determine eligibility status (eligible, ineligible). Manual EHR review was then conducted on the n = 125 patients to assess inclusion/exclusion criteria and determine eligibility. Results of SEP vs. manual review were compared, noting reasons for discrepancies. Positive predictive value (PPV), negative predictive value (NPV), sensitivity and specificity values were calculated. We herein report results of one site (n = 75), with next steps to complete analysis of the total n = 125 at both sites. Results: The SEP had a PPV of 76%, NPV of 96%, sensitivity of 97% and specificity of 67%, based on n = 75 patients. We identified areas of improvement for the SEP and groups to conduct manual validation during the CAFÉ trial. Potential reasons for lower specificity and lower PPV include: the SEP was identifying patients with a hematologic malignancy (e.g., lymphoma) and a recent visit to oncology, but whose diagnosis date was outside the eligible period; additionally, the SEP was identifying patients with a recent oncology visit, but who had a non-cancer diagnosis (e.g., monoclonal gammopathy of undetermined significance [MGUS]). Conclusions: Development of a validated EHR-based tool to rapidly identify cancer clinical trial participants offers a low resource option that may overcome challenges associated with manual chart review.
