Le dose-banding des anticancéreux constitue une standardisation des doses de chimiothérapies à partir d'une dose calculée individuellement sur base d'une donnée spécifique, qu'il s'agisse d'une donnée anthropométrique ou d'un paramètre physiologique. Etant donné l'index thérapeutique étroit des molécules anticancéreuses, la définition initiale du dose-banding tolère un écart maximal de ± 5% de la dose individuelle. Le dose-banding ouvre la voie à une production en série des préparations d'anticancéreux mais l'applicabilité du dose-banding nécessite le respect d'un certain nombre de critères parmi lesquels la stabilité à long terme des préparations de chimiothérapie est essentielle. Le recours au dose-banding pour la production de préparations d'anticancéreux en série et à l'avance dans des unités centralisées de production constitue une opportunité de rationalisation, de fluidification et de sécurisation des processus en pharmacie, tout en réduisant les délais d'attente des patients dans les unités de soin. Le dose-banding se développe dans un contexte qui se partage entre l'utilisation classique de doses individualisées et la généralisation progressive du concept de médecine « personnalisée ». L'objectif de ce travail interdisciplinaire est triple et consiste dans (i) la réalisation d'études de stabilité physicochimiques au long cours de préparations de molécules anticancéreuses; (ii) la comparaison des coûts de production de poches de gemcitabine sur base d'une préparation individualisée et d'une préparation en série à partir de données rétrospectives ; (iii) le questionnement éthique quant à la justification du dose-banding comme pratique de soin et à son positionnement dans la diversité des stratégies de soins. Au niveau technique, ce travail a permis de développer trois études de stabilité physicochimiques démontrant la stabilité au long cours du 5-fluorouracile et de la gemcitabine dans des conditions de préparation et de conservation typiques d'un dose-banding. Il a en outre mis en évidence l'existence d'un biais possible lors du recours aux dispositifs de transfert en système clos pour les études de stabilité. Au niveau économique, l'intérêt pour le département de pharmacie de l'implémentation d'un dose-banding pour la production en série de préparations de gemcitabine a pu être confirmé. Au niveau éthique, l'élaboration d'un concept de « juste » dose des chimiothérapies a éclairé les éléments de justification d'une application pratique du dose-banding tout en l'intégrant aux autres approches de soin
Anticancer dose-banding is a standardization of chemotherapy doses on the basis of a dose calculated individually according to a specific anthropometric or physiological parameter. Given the narrow therapeutic index of anticancer molecules, the initial definition of dose-banding tolerates a maximum deviation of ± 5% of the individual dose. In practice, the applicability of dose-banding depends on various criteria, including the long-term stability of chemotherapy preparations. The use of dose-banding allows the centralized and in advance production of anticancer drugs and represents an opportunity to rationalize, streamline and secure pharmacy processes, while reducing patient waiting times in care units. Dose-banding is developing in a context split between the common use of individualized doses and the gradual generalization of the concept of “personalized” medicine. The aim of this interdisciplinary study is triple: (i) to carry out long-term physicochemical stability studies on preparations of anticancer molecules; (ii) to compare the production costs of gemcitabine bags based on individualized and serial produced preparations, using retrospective data; (iii) to examine the ethical justification for dose-banding as a care practice, and its position within the diversity of care strategies. On a technical level, this work enabled us to carry out three physicochemical stability studies demonstrating the long-term stability of 5-fluorouracil and gemcitabine under preparation and storage conditions typical of dose-banding. It also highlighted the potential bias associated with the use of closed-system transfer devices for stability studies. From an economic point of view, the value to the pharmacy department of implementing dose-banding for the serial production of gemcitabine preparations was confirmed. On an ethical level, the development of a concept of “right” chemotherapy dose has shown the justification for a practical application of dose-banding, while integrating it with other care practices

Through qualitative analysis with basis in a theoretical framework constructed by perspectives from sociology of childhood and Freeman and Dwyer’s understanding of children’s rights, this thesis contributes to in depth knowledge and critical analysis of actors perspectives and experience with Statens Barnehus forwarding children’s rights. This thesis has the aim to explore and examine the role of Statens Barnehus and their work with abused children. Through 8 in-depth interviews this thesis set focus on the impact of establishment of Statens Barnehus and their process related to forensic interview and problematize the concept of understanding children’s rights in different institutions. The aim of the in-depth interviews is to explore different actors understanding of `Statens Barnehus´ and how Statens Barnehus can contribute to forwarding children’s rights to be heard.

The present job intends to show the ideological impediments to the effectiveness of the economic, social and cultural rights, using the critical theory of the human rights and its consequences in the effectiveness of the worker s rights, in special in regards to the clauses of agreements and collective conventions of work. We look for to demonstrate as the theoretical constructions concerning human rights, fundamental rights and worker s rights are marked by elements of the bourgeois liberal hegemonic ideology and which way these theories allow the construction of practices that only are compromised with the reproduction of the status quo. On the other hand, with a hermeneutic proposal which is compromised with the political decisions which had chosen the principles which are in the Constitution of the Republic, we elaborate a proposal of criterions of validity for the conventional clauses that import in negotiation of conditions of work related to human and fundamental rights of the worker human being.
O presente trabalho tem por objetivo desvelar os entraves ideológicos à efetividade dos direitos econômicos, sociais e culturais, a partir da teoria crítica dos direitos humanos e seus reflexos na efetivação dos direitos dos trabalhadores, em especial no tocante às cláusulas constantes de acordos e convenções coletivas de trabalho. Procuramos demonstrar como as construções teóricas acerca de direitos humanos, direitos fundamentais e direito do trabalho encontram-se marcadas por elementos da ideologia hegemônica liberal burguesa e de que forma estas teorias permitem a construção de práticas comprometidas tão somente com a reprodução do status quo. Por outro lado, a partir de uma proposta hermenêutica compromissada com as decisões políticas que elegeram os princípios constantes da Constituição da República, elaboramos uma proposta de critério de validade para cláusulas convencionais que importam em negociação de condições de trabalho ligadas a direitos humanos e fundamentais do ser humano trabalhador.

This research examines the attitude that Zimbabwe portrays towards the protection, promotion and enforcement of the right to social security. Zimbabwe did not have a Constitution that provided for the realisation of the right to social security before 2013. With the enactment of the 2013 Constitution, many people now have a chance to enjoy economic, social and cultural rights (including social security) which ordinarily was not possible in the past. Provision of the right to social security plays a pivotal role by assuring human dignity for people when they are faced with circumstances such as the deteriorating and unpredictable economic situation in Zimbabwe in recent years. This study uses academic textbooks, journals, legislation, international treaties, newspaper reports and other sources of literature to analyse Zimbabwe’s outlook on social security and examines the structures, policies and institutions put in place to enforce the right to social security. The Constitutional provisions for social security are also analysed in the light of international instruments’ provisions of social security to find out if they are adequate and effective in the protection and promotion of social security in Zimbabwe. The study also examines the national polices; the national commission for human rights and the courts to determine their efficacy in their administration of the right to social security. The Constitution of Zimbabwe states that when provisions of the declaration of rights and any legislation such as the provisions for the right to social security are being interpreted, international law and all treaties and conventions that Zimbabwe is a party to must be taken into consideration. Thus, in this study the application and influence on the enforcement of the right to social security by international law and all the treaties and conventions that Zimbabwe is a party to, are scrutinised. Budgets mirror the government’s values and can be reflective of the attitude that a State has towards a priority. That is why the study also examines the budget allocation and spending patterns of Zimbabwe towards the protection and promotion of the right to social security.

The initial matching relationships between underwriters and bonds/issuing firms and the certification quality of underwriters, as determined by changes in the issuing firm’s financial strength post issue, are the two primary research topics in this dissertation. Based on total underwriter syndicate market share, two distinct categories, low market power (LMP) syndicates and high market power (HMP) syndicates were defined. Firm financial strength is examined based on a new factor developed in this research. A comparison of the two underwriting categories, or pools, indicates that the HMP underwriters take on firms of lower initial financial strength and additionally, the issuing firms decline more in financial strength two years following bond issuance than do firms using LMP underwriters. Notwithstanding these results, the more interesting findings are the relationships within each of these pools. In the LMP pool of underwriters, financially stronger firms used the larger LMPs to underwrite their bonds, while the weaker firms used smaller LMPs. In contrast, among HMP underwriters, the largest HMPs aligned with the firms of relatively lower financial strength. The relationships in both pools reverse when changes in financial strength are examined. Larger LMPs are associated with greater issuing firm financial decline while larger HMPs correlate with lower levels of decline in firm financial strength. Divergent patterns in initial underwriter-issuer matching and underwriter certification found in this research indicate that there are true differences in the “small” underwriting syndicates as compared to the larger syndicates. These patterns should be considered by both issuing firms and investors as both constituencies contemplate the corporate bond market.

The Constitution of South Africa is often commended for the protection that it affords to socio-economic rights - including the right to access to adequate housing, 'Adequate' housing is said to comprise of more than just a roof over one's head, and requirements have also been set regarding the overall location of the housing provided. Processes such as 'gentrification' have presented a threat to the fulfilment of the abovementioned right. Gentrification results in escalated property prices in the areas undergoing gentrification, with original residents of these areas consequently being unable to afford to stay in these areas and facing possible eviction as a result thereof. Although legislation is in place to prevent homelessness for those facing eviction, individuals who stand to be evicted often find themselves being relocated to state-provided Temporary Relocation Areas (TRAs). As particularly evident in Cape Town, these TRAs are commonly located on the outskirts of cities (arguably in defiance of the Constitution's requirement in terms of the location of adequate housing), and those who have been relocated to these TRAs commonly criticize the remote locations of these TRAs (due to it being far from employment opportunities and other amenities). This has led to a continued demand by individuals facing eviction to TRAs (and advocacy groups) for the state to provide them with alternative accommodation in the inner city and surrounding areas, resembling a 'Right to the City'.

Esta tese dedicou-se a perscrutar como crianças escolarizadas estão sendo subjetivadas como sujeitos de direitos, a partir dos discursos sobre os direitos da infância em circulação no currículo escolar, tendo como referência a prática pedagógica de uma professora que se dispôs a desenvolver a temática dos direitos das crianças em seu planejamento didático-pedagógico. Constituiu-se como foco da pesquisa e corpus de análise e problematização as produções escritas (poesias, narrativas de histórias, diálogos, crônicas, cartas, desenhos, painéis e diários) de alunos/as de 3a. e 4a. séries do Ensino Fundamental, de duas escolas públicas estaduais de Porto Alegre, ambas sob regência da mesma professora. Também foram contemplados no corpus deste estudo o diário de campo da pesquisadora, contendo o registro das aulas acompanhadas e das interações envolvendo as crianças e a professora; o planejamento didático-pedagógico e o diário de classe da professora; livros paradidáticos e didáticos, bem como documentos oficiais que abordam os direitos das crianças. Esta investigação delineou-se como um estudo de caso com matizes etnográficos. Contou com a inspiração teórica de pensadores como Michel Foucault, Alain Renaut e Hannah Arendt, buscando promover uma aproximação entre suas perspectivas analíticas a fim de fundamentar as problematizações do estudo. A partir dos vestígios do empírico, a investigação possibilitou perceber que os discursos sobre a infância de direitos, ao posicionarem as crianças como sujeitos de direitos, constituem-se como verdadeiros e necessários para o contexto social contemporâneo. As técnicas de si implicadas e imbricadas com as tecnologias de poder, em alguma medida estão mobilizando as crianças, a partir de experiências oportunizadas pelo currículo escolar, em direção a um aprendizado que as potencialize a cuidarem de si, a preservarem suas vidas, pois a vida presente tem urgência em ser vivida e é vulnerável. Outra mirada que a imersão no empírico possibilitou vislumbrar foi a emergência de uma proliferação discursiva sobre a infância de direitos que tem inscrito as crianças como sujeitos de direitos, por meio da imbricação dos direitos-proteção com os direitos-liberdade, no sentido de compreendê-las para além da proteção e do cuidado, mas pelo registro da participação, da autonomia, da possibilidade de terem opinião, de serem ouvidas e de terem voz.
This thesis aimed to scrutinize how schooling children have been made subjective as subjects of rights, from discourses on the childhood rights within the school curriculum, having as reference the pedagogical practice of a teacher who accepted to develop the children’s rights thematic in her didactic-pedagogical planning. The focus of the research and corpus of analysis and questioning, the written productions (poetry, chronicles, dialogues, short stories, letters, drawings, panels and diaries) of students in the 3rd and 4th degrees of Elementary Education of two Public State Schools in Porto Alegre, both being taught by the same teacher. It was contemplated in this study the researcher’s field diary, with the registration of the observed classes and the interactions involving the children and the teacher; the didactic-pedagogical planning and the teacher’s class register; para didactic and didactic books, as well as official documents that encompass the children’s rights. This investigation was outlined as a case study with ethnographic hues. It was considered the theoretical inspiration of thinkers such as Michel Foucault, Alain Renaut and Hannah Arendt, trying to promote an approach among their analytical perspectives in order to fundament the questioning of the study. From vestiges of the empiric, the investigation made it possible to notice that the discourses on the childhood rights, by placing the children as subjects of rights, constitute themselves as actual and necessary to the contemporary social context. The self techniques implicated and imbricated with the technologies of power, to a certain extent, are mobilizing the children, from experiences offered by the school curriculum towards a learning that allows them to take care of themselves, to preserve their lives, because the current life urges to be lived and is vulnerable. Another look that the immersion in the empiric made possible to discern was the emergency of a discursive proliferation on the childhood rights that have inscribed children as subjects of rights, through the imbrication of the protection-right with the liberty-right, in the sense of understanding them beyond protection and care, but by the register of participation, autonomy, the possibility of having opinion, being heard and having voice.

However, little has been said about the consequences of such a mass labor stoppage on undergraduate medical education and those students who aspire to the profession.

This thesis draws on the principle of moral equality to develop a novel critique of a range of practices apparently licensed by the idea that the state has a right to exclude. The state’s right to exclude can be understood as consisting of three conceptually distinct rights: a right to exclude outsiders from its territory, a right to exclude them from settling within the territory, and a right to exclude them from acquiring citizenship status. It is widely acknowledged that the right to exclude has its limits. But on what grounds do we think the right to exclude must be limited, and how extensive are those limits? It is common to argue that the right to exclude must be constrained by the principle of moral equality: the idea that all persons are of equal moral worth, by virtue of their shared humanity. Indeed, in the existing literature, there has been one powerful attempt to show that the right to exclude always violates the requirements of moral equality, by distinguishing between citizens and non-citizens on the basis of a factor that is morally arbitrary: their nationality. According to that line of argument, there can be no right to exclude. In this thesis, I make a different move. I argue that we need to pay attention to the ways in which exercises of the right to exclude violate another key demand that emerges from the idea that we are all of equal moral worth. I focus on the idea of social equality, and on the relationship between this and the demand for respect. The central claim of my thesis is that borders do not just passively stand in the way, blocking people from pursuing the equal opportunities to which they are entitled. The exercise of the right to exclude also actively licenses disrespectful policies towards non-citizens, and in some cases, citizens as well. I examine three such examples: the detention and deportation of non-citizens, discrimination against low-skilled migrants, and the denationalization of citizens who are dual nationals, for the purposes of national security. In order to uphold the demands of moral equality, we need to rethink the existing social structures and institutional contexts that are bound up with states’ exercise of the right to exclude.

AbstractCrystalloids are commonly used in medicine as solutions containing electrolytes dissolved in water, with or without glucose. They can be used as maintenance, replacement, or resuscitation fluids, but should be administered with caution. This chapter provides an overview of basic definitions, terminology, and concepts regarding crystalloids, including their categorization by tonicity, their balanced or unbalanced nature, and the importance of strong ion difference (SID). Improper administration of crystalloids can lead to morbidity, particularly hyperchloremic metabolic acidosis (HMA) and fluid overload. Moreover, saline with a SID of zero can cause a positive sodium balance and subsequent fluid accumulation, which can lead to renal dysfunction and the need for vasopressors and renal replacement therapy. Recent systematic reviews and post-hoc analyses of six major fluid trials have shown that balanced solutions (not containing glucose) reduce mortality by 1%, making them a good first choice for resuscitation in patients with sepsis and septic shock, burns, or diabetic ketoacidosis. Traumatic brain injury and gastrointestinal losses may be the only indications left for (ab)normal saline. The pediatric community still favors isotonic solutions for maintenance, although a growing body of evidence supports hypotonic crystalloids as a better choice. Hypertonic crystalloids have been described for small volume resuscitation in specific patient populations, such as post cardiac arrest, but their sodium burden may outweigh the temporarily beneficial hemodynamic effects. In case of excessive losses, fluids should be substituted or replaced by those that mimic the fluids that are lost, such as blood. Prescribing crystalloid solutions should be done with care. Fluid overload or accumulation and HMA should be avoided, as it can induce extra morbidity and mortality. Choosing the right fluid, indication, dose, and duration is crucial for preventing morbidity and mortality: it is all about giving the right dose of the right fluid at the right time for the right patient!

IMRT is the primary development of 3D-CRT, because it represents a dose distribution to the tumor cell in superior way also sending of highly conformal radiation to irregularly and concave shaped target volume. The purpose of the present study is to compare the dosimetric analysis of two plans of radiotherapy (IMRT) and 3D-CRT, which include target volume and organ at risk for both plans. The present study selected eight patients, seven cases with different types of brain cancer and one case with chordoma cancer, all the cases previously irradiated in Zhianawa Cancer Center. All selected cases were re-planned by both techniques. IMRT planning provides reducing the dose of both right and left optic nerve mean dose for right optic nerve 23.52 Gy and left 22.51 Gy compared with the 3D-CRT plan (right optic nerve 28.06 Gy and left 25.71 Gy). IMRT plan reduces dose of both right and left eye compared to 3D-CRT plan, the mean dose for right eye 17.53 Gy and left 13.71 Gy compared with the 3D-CRT plan (right eye 18.4 Gy and left 16.97 Gy). IMRT plan provided better target cover (PTV95%) (97.33± 1.16) for (prescribed dose of 40Gy) than 3D-CRT (96.96±1.83).

1 Introduction 1.1 The urgency of industrial decarbonization The last few years have seen several of the world’s largest carbon dioxide-emitting countries and leading heavy industry companies committing to mid-century net-zero targets (Buckley 2021; Denyer and Kashiwagi 2020; McCurry 2020; Myers 2020). Consequently, the discussion on economy-wide transition to net-zero is accelerating, with focus shifting from “if” to “when” and “how”, even for heavy industry sectors like steel, cement and chemicals. This makes it increasingly urgent to analyse not just whether it is technologically feasible to decarbonize heavy industry, but also investigate issues more directly related to practical implementation. This includes site-specific planning, infrastructure availability, and consultation with local authorities and other stakeholders. Many of the latter considerations are formalized as part of the permitting processes that are an essential vehicle to ensure that industrial interests are balanced against interests of society at large. However, doing this balancing act can turn out to be very complicated and associated with uncertainties as to their outcome, as well as being demanding in resources and time. At the same time, to ensure broad buy-in and support from society, the investments needed must be implemented in a way that takes a broad spectrum of sustainability concerns into account, not just climate change mitigation. A key question is if and how permitting processes can run more smoothly and efficiently while still ensuring inclusive consultations, fair procedures and adherence to legal certainty. This policy brief discusses this question from the starting point of Swedish conditions, but many of the points raised will be relevant for a broader international discussion on taking industrial decarbonization to implementation. 1.2 Industrial transition and permitting processes in Sweden Decarbonization of the industrial sector in Sweden essentially entails a relatively small number of investment projects in the cement, steel, petrochemical and refinery sectors, where the vast majority of carbon emissions are concentrated (Karltorp et al. 2019; Nykvist et al. 2020). However, while few in number, the size of these investments means that their implementation will by necessity become relevant to many other parts of society. In connection with the increasing focus on how to implement industrial decarbonization in Sweden, discussions about permitting processes have been brought higher up on the agenda. While there has been an active discussion on permitting processes in Sweden for quite some time, it has primarily been focused on aspects related to mining and wind power (Larsen et al. 2017; Raitio et al. 2020). The last few years have, however, focused increasingly on industrial projects, in particular related to a proposed – though eventually cancelled – expansion of an oil refinery in the southwestern part of the country (Blad 2020). In terms of political discussions, both the governmental initiative Fossil-free Sweden (2020) and the Swedish Climate Policy Council (2020) emphasize that permitting processes need to become faster in order for Sweden’s industrial transition to be implemented in line with the time plan set by the 2017 Swedish Climate Act. Business representatives and organizations are also voicing concerns about the slow speed of permitting (Balanskommissionen 2019; Jacke 2018). At the same time, criticism has been raised that much of the environmental damage done in Sweden comes from activities conducted within limits set by environmental permits, which could be a flaw in the system (Malmaeus and Lindblom 2019). Finally, recent public inquiries have also discussed permitting processes.

The right to decent work including social protection is guaranteed to all citizens in multiple international human rights and labour rights conventions, as well as in African Union resolutions. However, many marginalised workers, including informal and rural workers, are excluded from social protection provisions. The digitalisation of social protection promised to increase the convenience and efficiency of accessing social protection entitlements (and it does for many), but millions of informal workers are being further excluded by digitalisation. This is the first study of digital social protection from the perspective of informal marginalised workers in Africa. Six studies were carried out with and by domestic workers, disabled workers, migrant workers, and home-based workers in Ghana, Kenya, Nigeria, South Africa, Tanzania, Uganda, and Zambia. This IDS Research Report synthesises their findings and recommendations to establish a clear agenda for change.
