Haematological malignancies examines the epidemiology, genetics, clinical presentation and classification of these diseases, and presents current treatment approaches for each. First are the acute leukaemias, and the management of acute lymphoblastic leukaemia (ALL) and acute myeloid leukaemia (AML). Chronic myeloid leukaemia, its genetics and sensitivity to tyrosine kinase inhibitors, is described. Myelodysplastic syndromes and their management, are followed by chronic lymphoid leukaemias, a large heterogeneous group of diseases, and their treatment. Hodgkin lymphoma, its pathology and presentation, staging and role of PET scanning, is described along with current treatment with chemotherapy and limited radiotherapy. Non-Hodgkin lymphoma is another heterogeneous group of diseases, divided into low-grade and high-grade pathology, and varying in their genetics, presentation, and management. Rituximab is a key component of chemotherapy regimens against B-cell lymphoma. Myeloma and other plasma cell dyscrasias are described, and treatment options reviewed. Myeloma remains incurable, but with appropriate management consistent with prolonged good quality life. Treatment includes chemotherapy, bisphosphonate therapy, analgesics and radiotherapy, Throughout this chapter is emphasised the importance of clinical trials in driving the rapid improvements in treatment of these diseases.

Injuries to the musculoskeletal pelvis and thigh in sport are extremely common. Injury can occur at one or multiple sites of the bony pelvic ring, and in the soft tissues of the groin, abdominal wall, and thigh. Athletes in certain sports are particularly prone to hip injury, especially those involved in running, soccer, hockey, rugby, and dancing. Although recognized as a common region of injury, the true epidemiology is not known, as the spectrum of injury is wide, diagnosis can be complex, and injury classification is still debated in some conditions. Nevertheless, soft tissue injury and dysfunction are the most common forms of injury seen and, indeed, hamstring injury is the most frequent injury in a number of sports, including athletics, soccer, rugby union, and Australian Football League. This chapter provides information on the classification and management of different types of fractures and dislocation which may commonly appear in the short-answer question (SAQ) paper. It also covers common rheumatological problems such as gout or infective arthritis and their associated diagnosis and management. The Royal College of Emergency Medicine (RCEM) curriculum includes both traumatic and atraumatic limb problems, which this chapter reflects. This chapter also covers the diagnosis and management of specific hand injuries and wound infections, which may appear as any aspect of the Intermediate FRCEM examination.

This chapter draws on historical statistics reporting financial outcomes for spending, taxation, debt, and deficit for the UK over a century to (a) identify quantitatively and compare the main fiscal squeeze episodes (i.e. major revenue increases, spending cuts, or both) in terms of type (soft squeezes and hard squeezes, spending squeezes, and revenue squeezes), depth, and length; (b) compare these periods of austerity against measures of fiscal consolidation in terms of deficit reduction; and (c) identify economic and financial conditions before and after the various squeezes. It explores the extent to which the identification of squeeze episodes and their classification is sensitive to which thresholds are set and what data sources are used. The chapter identifies major changes over time that emerge from this analysis over the changing depth and types of squeeze.

Product innovations are considered as global, local, or new to firm; new and original products are defined; and horizontal innovations are differentiated from vertical innovations according to whether they are preferred only by some buyers or by all. The significance of an innovation is addressed via a discussion of alternative approaches based upon either changes in the characteristics of the product or its market impact.

A theory of the structure of rock music is presented, addressing aspects such as tonality/key, harmony, rhythm/meter, melody, phrase structure, timbre/instrumentation, form, and emotional expression. The book brings together ideas from the author’s previous articles but also contains substantial new material. Rock is defined broadly (as it often is) to include a wide range of late twentieth-century Anglo-American popular styles, including 1950s rock & roll, Motown, soul, “British invasion” rock, soft rock, heavy metal, disco, new wave, and alternative rock. The study largely employs the informal, intuitive methods of conventional music theory and analysis, but it is also informed by corpus data. An important component of the theory is a representation of pitches—the “line of fifths”—that sheds light on issues such as stylistic distinctions within rock, effects of surprise, and emotion. The theory also entails a model of expression with three dimensions, representing valence, energy, and tension; this proves to be a powerful tool for tracing shifts in expressive effect within songs. The theory features novel approaches to issues such as cadences, melodic-harmonic coordination, the handling of sectional boundaries, and the classification of formal types. The final two chapters present analyses of six songs and a broader consideration of rock in its historical and stylistic context.

A wide variety of manmade and naturally occurring air pollutants are known to cause cancer. Diverse exposures such as tobacco smoke, radionuclides (radon), chemicals (benzene, mustard gas, and volatile organic compounds), fibers (asbestos), and metals and metalloids (chromium, nickel, and arsenic) have long been classified as carcinogenic to humans. Historically, these classifications were based predominantly on high levels of exposure in occupational settings. Over the last thirty to forty years, scientific attention has focused on quantifying the adverse health effects of indoor and outdoor air pollutants at exposure levels several orders of magnitude lower than were studied initially. These include secondhand smoke, household exposure to radon, residential and environmental exposure to asbestos, soot from diesel-powered engines, ambient exposures to small particles (PM2.5), and indoor air pollution from the combustion of biomass and coal. This chapter provides an overview of recent epidemiologic studies of air pollutants and cancer.

Chapter ten focuses on botanical thought of the Middle Ages in the Roman, Eastern and Islamic Empires when herbals devolved from practical field guides to decorative status symbols, and Dioscorides developed his classifications of sex in plants according to anthropomorphic criteria, such as hardness or softness. Dioscorides’ authority endured into the sixteenth century. Sources including The Book of Idols and the Ethiopian Book of Enoch inform the discussion of pre-Islamic vegetation goddesses Allat and Al-’Uzza, and the Satanic Verses. Zakariya Muhammad Qazwini suggested the palm was human-like, with two sexes and “a sort of copulation” required for fruit production. The Quranic story of Mary and the date palm reprised “The Cherry Tree Carol,” both evidencing her assimilation of pagan goddesses. After the Council of Ephesus’s sanction of the cult of the Virgin as Theotokos, her cult bourgeoned. Her imagery often centered on gardens, fruits, and flowers symbolizing her purity.

L’exceptionnelle richesse floristique de la Nouvelle-Calédonie est mondialement connue. Plus de 3 400 espèces de plantes vasculaires indigènes y sont répertoriées, dont les trois-quarts sont endémiques de l’archipel. L’endémisme ne concerne pas seulement les espèces, mais aussi les genres (près d’une centaine) et même trois familles. La diversité se décline aussi sur le plan écologique, en lien avec l’histoire géologique originale de la Nouvelle-Calédonie, qui a favorisé le micro-endémisme et les espèces inféodées aux substrats ultramafiques. De nouvelles espèces continuent à être découvertes, aussi reste-t-il nécessaire de poursuivre prospections et recherches botaniques, afin de mieux comprendre l’origine et l’évolution de cette flore, et contribuer à sa préservation. Le présent volume regroupe trois familles d’Angiospermes. Celle des Apocynaceae dont la classification a été profondément remaniée depuis la publication en 1981 du fascicule « Apocynaceae », volume 10 de la Flore de la Nouvelle-Calédonie et Dépendances : les trois sous-familles traitées ici (Periplocoideae, Secamonoideae et Asclepiadoideae) formaient auparavant la famille des Asclepiadaceae. Les Phellinaceae qui, avec 10 espèces ligneuses, constituent l’une des trois familles endémiques du territoire. Enfin, la Nouvelle-Calédonie héberge quelques espèces de la famille cosmopolite des Capparaceae, toutes appartenant au genre du câprier (Capparis). Conformément à la ligne éditoriale de la collection, cet ouvrage comporte, pour chaque famille traitée indépendamment : une présentation générale suivie de descriptions détaillées des genres et des espèces ; des clés d’identification, en français et en anglais ; une illustration variée comprenant des dessins au trait et des photographies des plantes vivantes ; des cartes de répartition et une évaluation des besoins de conservation selon les critères de l’UICN.

To understand public opinion and political behavior, researchers typically sort people by self-identified groupings such as party identification, race, gender, education, and income. This book advances gun owners as a new classification. It demonstrates a “gun gap,” which captures the differences between gun owners and nonowners, and shows how this gap improves conventional models of political behavior. The gun gap in fact represents an important explanation for voter choice, voter turnout, perceptions of personal and public safety, preferences for gun control policies, and support for the death penalty. Moreover, the gun gap is growing. During the 1970s and 1980s, it was small. However, legislative battles over guns in the early 1990s marked a significant growth in the gun gap that continues to this day. The 2016 presidential election witnessed the largest recorded gun gap in history. The gun gap in voter choice was nearly three times larger in 2016 than the gender gap, and it exceeded age and education gaps by notable margins. This book also focuses on variation among gun owners. Gun owners are not a monolith but exhibit attitudinal and behavioral differences that can be as large as the gap between gun owners and nonowners. The gun gap thus affords a new and compelling vantage point to evaluate modern mass politics.
