With approximately 400,000 patients on hemodialysis in the United States alone, there are increasingly more patients who eventually develop central venous occlusions, mostly because of numerous central venous catheterizations and the placement of arteriovenous fistulas and grafts. With improving technical abilities to obtain central venous access for catheters and maintain fistula/graft patency, increasingly more patients present with signs of central venous stenosis/occlusion. For many, the underlying occlusion is chronic, with well-formed collateral vessels that have developed over time, thus allowing continued dialysis or asymptomatic function. However, for many patients there is a gradual progression of symptoms. This chapter discusses several techniques to cross chronic central venous occlusions.

Endovascular thrombectomy (EVT) for acute ischemic stroke is a new and powerful treatment modality that restores functional independence to many victims. Although it has been proved of value in large-vessel occlusion of the anterior circulation, it is also used in basilar artery embolism. Time to successful reperfusion is a major determinant of recovery. A subset of patients has robust collaterals and will benefit from treatment up to 24 hours after stroke onset; the presence of salvageable brain tissue (penumbra) must be ascertained by specialized imaging. The number of patients who can benefit from EVT is estimated at 100,000/year in the United States alone in more than 300 designated Thrombectomy-Capable Stroke Centers. EVT is a new anesthetic emergency. Anesthesiologists must be actively involved in creating protocol-driven care for acute ischemic stroke patients.

‘Alarms and Safety Mechanisms’ breaks that puzzling black box, which includes those critical functions designed to safeguard the ventilated patient. It describes the categories of alarms, including application alarms (also called clinical alarms), which may indicate problems with the patient; a leak or occlusion in the breathing circuit; or an improper ventilator setting. It goes on to describe technical alarms that point to an actual malfunction within the ventilator system. It also discusses alarm priorities and alarm settings or alarm limits. The chapter details each common alarm, listing alternative alarm messages as they appear on different ventilators, and describing what the alarm means and how to troubleshoot it. Finally, the chapter discusses a ventilator’s emergency states, such as the ambient state or the safety valve open state, and the ventilator inoperative alarm.

When the vertex of an occluded angle geometrically belongs to the side of the occluding surface, the occluded angle looks distorted. This characteristic effect of coincidental occlusion—called the Gerbino illusion—is consistent with the phenomenal rounding of angles observed under conditions of symmetrical occlusion. Both effects are robust and appear in static and dynamic displays. The Gerbino illusion differs from distortions observed in Poggendorff-like displays, runs against the tendency to global Prägnanz, and reveals important aspects of amodal completion processes. Alternative explanations based on visual interpolation and visual approximation are discussed. According to the approximation-based explanation, the possible discrepancy between internal models and the sensory input is perceptually represented as a phenomenal distortion.

Acute ischemic stroke (AIS) is the leading cause of acquired neurological disability worldwide. AIS most commonly occurs when a cerebral artery is occluded, leading to irreversible brain injury and neurologic disability. Acute supportive physiological interventions and close monitoring on a stroke unit are beneficial to optimize overall recovery and functional outcome. Phamacological treatment options are limited though as the only FDA-approved therapy for AIS is the thrombolytic agent intravenous recombinant tissue plasminogen activator (Alteplase, rtPA), which improves functional outcome in therapeutic time windows ranging up to 3–4.5 hours. Several clinical trials assessing the efficacy of endovascular therapy have shown a benefit in carefully selected patients with a documented large vessel occlusion (LVO), and subsequently are becoming part of the standard practice in this AIS subset. Clinical trials using various imaging paradigms to enhance patient selection for thrombolytic therapy, endovascular therapy and neuroprotection therapies are all progressing.

In the previous chapters we described the basic principles of density functional theory, gave examples of how accurate it is to describe static magnetic properties in general, and derived from this basis the master equation for atomistic spin-dynamics; the SLL (or SLLG) equation. However, one term was not described in these chapters, namely the damping parameter. This parameter is a crucial one in the SLL (or SLLG) equation, since it allows for energy and angular momentum to dissipate from the simulation cell. The damping parameter can be evaluated from density functional theory, and the Kohn-Sham equation, and it is possible to determine its value experimentally. This chapter covers in detail the theoretical aspects of how to calculate theoretically the damping parameter. Chapter 8 is focused, among other things, on the experimental detection of the damping, using ferromagnetic resonance.

Humanity has oftentimes been defined as a laboring being. This chapter looks at how musical play undoes this determination. Using Deleuze, Guillaume, and Malabou, it revisits the relation between repetition and variation in musical form, focusing on the refrain cadences in Mozart’s piano concertos. The analyses reveal that repetition, far from being static, restores musical material to its potentiality. They show how repetition is related to virtuosic play and hence how repetition challenges the privilege of productivity and labor. To this end, this chapter reexamines what is at stake in recnet Formenlehre over debates between Schoenbergian functional and sonata-theory approaches. The result is to develop this important body of work via a theory of potentiality.

Algorithms exist and operate on multiple levels. They are technical as much as they are social, cultural as much as they are functional. The chapter examines the multiplicity of algorithms, arguing for an expanded view on algorithms that takes it variable ontology into account. Providing the conceptual groundwork by merging perspectives from computer science, social sciences, and the humanities, the chapter explains the different meanings of algorithms as technical entities, and phenomena of social concern, respectively. Algorithms do not merely have power and politics; they are fundamentally productive of new ways of ordering the world as part of a much wider network of relations and practices. Offering a rich overview of critical algorithms studies, the author suggests that the multiplicity of algorithms is not about providing different perspectives on one static object called an algorithm but about realizing how the algorithm is already many things at once.

Spinal cord stimulation (SCS) has been a long established therapy for various pain conditions including low back pain, failed back surgery syndrome, complex regional pain syndrome, and other neuropathic and nociceptive pain states. Since the first report of SCS in 1967 by Shealy, advances have occurred in the technology used to achieve clinical analgesia. Developments in both the hardware and software involved have led to significant improvements in functional specificity, as seen in dorsal root ganglion stimulation, along with increasing breadth and depth of the field of neuromodulation. The patient experience during the implantation of the systems, as well as post-procedurally has been enhanced with improvements in programming. These technological improvements have been validated in quality evidenced-based medicine: what was a static area now is a dynamic field, with neuromodulation poised to allow physicians and patients more viable options for better pain control for chronic painful conditions.

This book treats instrumentation used in meteorological surface systems, both on the synoptic scale and the mesoscale, and the instrumentation used in upper air soundings. The text includes material on first- and second-order differential equations as applied to instrument dynamic performance, and required solutions are developed. Sensor physics are emphasized in order to explain how sensors work and to explore the strengths and weaknesses of each design type. The book is organized according to sensor type and function (temperature, humidity, and wind sensors, for example), though several unifying themes are developed for each sensor. Functional diagrams are used to portray sensors as a set of logical functions, and static sensitivity is derived from a sensor's transfer equation, focusing attention on sensor physics and on ways in which particular designs might be improved. Sensor performance specifications are explored, helping to compare various instruments and to tell users what to expect as a reasonable level of performance. Finally, the text examines the critical area of environmental exposure of instruments. In a well-designed, properly installed, and well-maintained meteorological measurement system, exposure problems are usually the largest source of error, making this chapter one of the most useful sections of the book.
