Abstract (sommario):
Historic parts of the city, Old Town, New Town, and the new buildings on Gedimino and Konstitucijos avenues form the city centre of Vilnius. These parts are situated on the opposite banks of the River Neris. The above mentioned avenues are the main streets and the river is the spatial axis of the centre. In the process of development of Vilnius city centre, the principal problems should be solved: 1. to further preserve cultural heritage; 2. to use a stable expansion in the development; 3. to strengthen the architecture and singularity of its natural surroundings in the city centre. Specific suggestions are presented for the development or regeneration of separate building spaces in the streets of the city centre relating them to the Official Plan of Vilnius (from 2007 to 2015).

Abstract (sommario):
Centres, which are spaces of interaction in urban area where social, political and economic relations condensed, have importance with their publicity characteristic beyond being social. In the modernisation of cities, functional and structural elements supporting the publicity of space have been evaluated as a dimension of development. Especially throughout the twentieth century, movie and movie theatres became the important components of the urban centres with their publicity characteristic. The relation with the urban centre or structural–spatial properties of movie theatres are realised in the context of growth and development processes of urban. It is possible to read this synchronisation through the relationship between formation of urban centre and movie theatres in certain periods in Ankara, an Ottoman small town by beginning of the century, and then, had become the space of modernisation politics after 1923. Beyond the morphological, this positioning reflects the pressure of infrastructure factors that affect its formation. Keywords: Modernisation, publicity, urban centre, movie theatres, Ankara.

Abstract (sommario):
Konya developed as a city with a single centre in historical period, the mound (höyük) known as Alaeddin’s Hill. This is still the central feature of the urban settlement of Konya. The tomb of the great humanist, poet and philosopher, Mevlana Celaleddin-i Rumii, is also here. The most prestigious section of Konya during the Seljuk and Ottoman periods was the Konya Türbeönü, the space in front of his tomb. The historical city centre maintains its liveliness in the area between Alaeddin’s Hill and the tomb of Mevlana. This historical city centre is the most accessible area in the city and functions fully as such while bearing its historical identity. The transformation of Konya from single-centred city to multi-centred city resulted in considerable changes in the city’s silhouette. The Konya Türbeönü Square was redesigned and presented for use. While defining the effect of outdoor spaces on human life, they are evaluated as places that meet basic physical, social and psychological requirements. Since the planning process of the Konya Türbeönü Square was not participative, it has remained a focal point of intense discussions and debates about its meeting users’ needs have arisen. This article studies user satisfaction with the design of the Konya Türbeönü Square and investigates the effects of changes on users. The study survey, asked for demographical information about the users, the sensorial effects of the outdoor space, the effects of its landscape and general satisfaction. The questionnaire was administered to 300 people, including 100 architects who are educated in designing, shopkeepers who are interaction with this area everyday and laypersons. Evaluating the questionnaire data revealed the physical landscape quality and the behavioural and functional qualities of the users along with the visual qualities of the historical urban square’s former/older condition. This study aims to show how participative planning can improve future projects.

Abstract (sommario):
This paper compares two cases: It relates to the development of new urban sub-centres in China, and the relationship of these sub-centres to ‘Network City’ theory. As cities move towards more polycentric systems, the case of Potsdamer Platz Berlin, compared to Zhenru Sub-Centre in Shanghai, is discussed. Both are transport-oriented developments promoting mixed-use density and transport-oriented development. According to the documentation by Shanghai municipality, this new urban centre, which is currently in its planning phase, is supposed to become a ‘sustainable sub-centre for a growing metropolis.’ The author, who has intimate knowledge of the Berlin case, was asked to advice on the Chinese project, based on the Potsdamer Platz experience. After some hesitation, a series of careful recommendations were formulated for the design and development of the Zhenru Sub-Centre, knowing that it is rather difficult to translate from one case to the other. The conclusion includes five recommendations for the urban design of such sub-centres, to ensure a delivery of economical, social and environmental sustainable outcomes.

Abstract (sommario):
Palavras- chave: Imperatriz. Centralidade Urbana. Shoppings Centers. NEW COMMERCIAL FORMS AND EXPRESSIONS OF URBAN CENTRALITY IN IMPERATRIZ – MA: an analysis of the installation and dynamism of shopping centers ABSTRACT In this article it is discussed the key aspects of the new commercial forms and expressions of the urban centrality of Imperatriz. At first, some considerations about the urban phenomenon in Brazil, that poses diverse and complex, are made. This scenario was influenced by different determinations; some of them were the development of many economic activities, that promoted greater dynamism of the cities, as in the case of industrialization, that favored the expansion of this process in the Center-South of this country, the modernization of the countryside in the Central-West, influenced by advancement and the use of new techniques. The reflections of this phenomenon is visible in the Amazon reality, the region where it enters the city of Imperatriz, which is the object of this study. Because of that, we had the anxiety to understand the expressions of the urban centrality developed by this city by the installing and dynamism of the shopping malls. This research is supported by the adoption of the theoretical Marxist approach and on the use of the dialectical method for understanding the urban reality is built in a contradictory way. Those methodological constructions were based on the contributions provided by Trivinos (2011) and Gil (2008). As techniques for data collection the simple observation and the adoption of semi-structured interviews were used. Keywords: Imperatriz. Urban centrality. Shopping malls. Abstract (sommario):
Abstract. The research in the geospatial data structuring and formats interoperability direction is the crucial task for creating a 3D Geodatabase at the urban scale. Both geometric and semantic data structuring should be considered, mainly regarding the interoperability of objects and formats generated outside the geographical space. Current reflections on 3D database generation, based on geospatial data, are mostly related to visualisation issues and context-related application. The purposes and scale of representation according to LoDs require some reflections, particularly for the transmission of semantic information. This contribution adopts and develops the integration of some tools to derive object-oriented modelling in the HBIM environment, both at the urban and architectural scale, from point clouds obtained by UAV (Unmanned Aerial Vehicle) photogrammetry. One of the paper’s objectives is retracing the analysis phases of the point clouds acquired by UAV photogrammetry technique and their suitability for multiscale modelling. Starting from UAV clouds, through the optimisation and segmentation, the proposed workflow tries to trigger the modelling of the objects according to the LODs, comparing the one coming from CityGML and the one in use in the BIM community. The experimentation proposed is focused on the case study of the city of Norcia, which like many other historic centres spread over the territory of central Italy, was deeply damaged by the 2016-17 earthquake.

Abstract (sommario):
The role of town centre management (TCM) schemes in the UK has expanded to incorporate a more overt and explicit focus on marketing and promotion. This paper considers the marketing/promotional activities of TCM schemes in the UK. TCM schemes operate at the interface of the public and private sectors. The implications of this are discussed, including the need for a consensual approach by a wide range of urban stakeholders, and the actual activities undertaken, influenced by the funding imperative under which such schemes operate (which impacts on the feasibility of certain activities and the efforts made to evaluate them). Comparisons are drawn between specific place marketing practice by TCM schemes and wider place marketing strategies.

Abstract (sommario):
In 1999, the Governing Council of the Universitat de Barcelona approved the creation of the POLIS Research Center. Later, in 2016, the Center was ratified after passing an assessment of the Catalan Accreditation Agency (AGAUR). The Centre has an interdisciplinary vocation and brings together researchers from different research groups at the University of Barcelona and cooperates with nine European and Ibero-American universities in the fields of Arts, Architecture and Human and Social Sciences. Academically, the Centre has promoted the doctoral program Public Space and Urban Regeneration (1998–2017) and the Master’s Degree in Urban Design: Art, City, Society (since 2007) and the publication of the journal On the w@terfront. The research object of the Centre is the city and its public space and, more specifically, the role of citizens in the production of Public Art and Urban Design. For this reason, the work of the Centre covers the topics related to Urban Regeneration, Sustainability, Urban Governance, Civic Remembrance, Heritage. Throughout its twenty-year history, the Centre has developed a series of projects for citizen participation in various areas of the periphery of Barcelona: River Besòs (municipality of Sant Adrià de Besòs) La Mina neighbourhood (municipality of Sant Adrià de Besòs) and the Barcelona’s neighbourhoods of Baró de Viver and Bon Pastor. A characteristic of the work of the centre has been, and is, the endorsement of citizen participation, through an innovative approach based on enabling the creative empowerment of the neighbours within the framework of Participatory Action Research. This approach is based on a project methodology, as it is understood in various project disciplines from Art to Architecture, from Design to Engineering. This article, associated with the itinerant exhibition “20 years working with neighbours,” reviews the founding project carried out by the Centre, “Social Uses of the River Besòs” (1997–1999), analysing the lessons learned, with the aim of clarifying the research criteria that the Centre follows for the development of citizen participation projects.

Abstract: The aim of this article is to present the main analytical findings of the application of GIS techniques to urban planning in Mauá, São Paulo. The paper is centre on several types of exploratory cartography related to demography, urban risk and accessibility to public policies in the 1990s. In certain census sectors of the municipality, the results show a superimposition of extreme poverty and urban risk conditions, pointing to the existence of very strong cumulative effects of urban risk and precarious socio-economic conditions. This cumulative effect seems to be more important than that indicated by the academic literature: a much more heterogeneous urban periphery than is commonly considered was identified, including areas that are very well served and included in social and urban terms, as well as others in which the population is submitted to more adverse conditions than those that characterised the metropolitan peripheries of previous decades. Keywords: urban space; living conditions; urban periphery; population and environment environmental risk; socio-spatial segregation.

Abstract (sommario):
Karbala is one of the metropolitan cities in Iraq, its historic and religious centre has a long history, yet many of its buildings are under threat because of unclear conservation management due to urban transformation. The history of religious rituals and processions reflects an array of values, concepts and planning philosophy that has used the power of religion and holiness of the city as a source of homogeneity and integration. By looking at the mass-pilgrimage spatial practices to the Holy Shrines in Karbala city centre and the adaptation by its residents of their domestic neighbourhoods, this paper analyses the spatial conditions of the city and offers insights into a set of factors that have shaped its historical evolution and urban spaces. The paper is in three parts; first, it discusses the causes of the urban transformation in this holy city. Secondly, it documents a set of everyday practices and problems in Karbala city, focusing on the urban level (the traditional fabric), following the analytical method of the historic evolution of Karbala as a religious centre as well as the incompatibility of the modern development with the centre’s historical heritage. Thirdly, it analyses the transformation of the urban structure by discussing the characteristics of the historical centre and the role of legislation in urban transformation of traditional city centres.

Abstract (sommario):
Care of the growing number of liver transplant recipients will increasingly fall on the referring centres. Thus, there is a need to define more clearly the demographic, clinical and laboratory profiles of liver transplant recipients, particularly in the setting of a centre where a liver transplantation program does not exist. The present study documented these features in 37 patients attending an adult ambulatory care clinic in an urban, nonliver transplant centre. Mean ± SD age of the study population was 44±11.9 years. Twenty-one patients (57%) were male. Annual enrolment in the clinic increased from three patients at the completion of the clinic's first year (1988) to 16 patients in the final year of the study (1993). Time between the transplantation procedure and the patient's return to the referring centre decreased from a mean of 12 weeks in 1988 to four weeks in 1993. During those seven years no patient required an unscheduled return to the transplant centre for surgical complications or problems associated with immunosuppressive therapy. In conclusion, these data provide a profile of liver transplant patients attending a nonliver transplant centre for follow-up and support the concept that nontransplant centres are capable of providing safe and, in terms of travel, less expensive care for liver transplant recipients.

Abstract (sommario):
Background: ICDS is an integrated program intended for Maternal and Child Care which adopts a holistic, lifecycle approach. Its main focus is on health, nutrition and education. Urban ICDS caters to the vulnerable urban slum population. Despite the program running for four decades its impact on its beneficiaries is still slow. The Anganwadi worker and helper are the grass-root functionaries running this program through Anganwadi centre. Hence this study was conducted to assess the functioning of the Anganwadi centre. Methodology: A Descriptive Cross-sectional study was conducted at all 15 urban-ICDS Anganwadi Centres of Khopoli during September-November 2019. The Anganwadi workers and helpers were interviewed regarding their sociodemographic details, knowledge and challenges perceived to run the centre. Observational Checklist designed based on guidelines and standard proforma for monitoring of the ICDS projects was used to assess infrastructure, equipment and registers. Results: Out of 14, 11(78.57%) Anganwadi workers had more than a decade experience. Indoor space of 600 sq.ft was available in 7(46.67%) centres. Toilets with running water were available in 9(60%) Anganwadi Centres and 6(40%) were linked to the school. Functional Salter’s weighing scale was available in 11(73.33%) Anganwadi centres. All 12 registers were available in 9(60%) Anganwadi centres. The utilization of services by pregnant women was the highest. Partially immunized children were present in 2(13.33%) Anganwadi centres. The service gap was highest (100%) with health check-up. Conclusions: There is a gap in the availability of infrastructure and utilization of some services. Keywords: urban ICDS, Maharashtra, Service gap.

Abstract (sommario):
Anthropogenic heat release is a key component of the urban heat island. However, it is often excluded from studies of the urban heat island because reliable estimates are not available. This omission is important because anthropogenic heat can contribute up to 4ºC to the urban heat island, and increases heat stress to urban residents. The exclusion of anthropogenic heat means the urban heat island effect on temperatures may be under-estimated. Here we estimate anthropogenic heat for four Australian capital cities (Brisbane, Sydney, Melbourne and Adelaide) to inform the management of the urban heat island in a changing climate. Anthropogenic heat release was calculated using 2011 population census data and an inventory of hourly traffic volume, building electricity and gas use. Melbourne had the highest annual daily average anthropogenic heat emissions, which reached 376 W/m2in the city centre during the daytime, while Brisbane’s emissions were 261 W/m2 and Sydney’s were 256W/m2. Adelaide had the lowest emissions, with a daily average of 39 W/m2 in the city centre. Emissions varied within and among the four cities and decreased rapidly with distance from the city centre, to 2 at 20 km from the city in Brisbane, and 15 km in Adelaide. The highest emissions were found in the city centres during working hours. The peak emissions reached in the centre of Melbourne are similar to the peak emissions in London and Tokyo, where anthropogenic heat is a large component of the urban heat island. This indicates that anthropogenic heat could be an important contributor to the urban heat island in Australian capital cities, and needs to be considered in climate adaptation studies. This is an important problem because climate change, combined with an ageing population and urban growth, could double the deaths from heatwaves in Australian cities over the next 40 years.

Abstract (sommario):
Since 2010, a team from the Tunisian Institut National du Patrimoine and the University of Oxford1 has been investigating Utica’s monumental centre, located at the tip of the promontory on which the city is built (fig. 1). The range and scale of architectural elements littering this area were remarked upon by most antiquarian investigators of the site. Nathan Davis, working at the site in 1858, noted that, despite the fact that it “had been ransacked for building materials”, this part of the city was covered with “marble and granite shafts, capitals, and cornices, of every order, size, and dimension”.2 Alfred Daux even observed that local residents referred to the largest building of the zone as the “Dar Es Sultan” (Palace of the Sultan), such was its magnificence.3 Aerial photographs commissioned by A. Lézine in the 1950s (fig. 2) show the area at the head of the promontory almost completely robbed out during and immediately after the Second World War, giving it a rather desolate aspect.

Abstract (sommario):
Objective: The aim of this study was to determine 1-year symptomatic outcome and its predictors in patients with first-episode psychosis (FEP) treated at 3 different publicly funded sites. Method: We evaluated FEP patients ( n = 172) treated in specialized programs in 2 medium-sized centres and 1 large urban centre with an identical protocol for demographic variables, diagnosis, and duration of untreated psychosis (DUP) at entry, and positive, negative, and general psychopathology symptoms at entry, 6 months, and 1 year. We used a mixed model analysis of variance, with time and centre and interaction between time and centre as fixed effects and sex and DUP as covariates, to analyze data. Results: A significant effect of time and time x centre interaction on positive, negative, and general symptom outcome was shown after controlling for ethnicity, education, and diagnosis. Patients showed significantly better outcome on all dimensions of symptoms in the 2 medium-sized centres, compared with the 1 large urban centre. Sex had a significant effect on negative and general symptoms, while DUP had no effect on any outcome measure. Conclusions: Similarly enriched early intervention services may produce different outcomes, even within a relatively homogeneous mental health system.

Abstract (sommario):
The urban heat island intensity is the difference in temperature between a site close to the centre of a city and a site close to but outside the city (the rural site). The urban heat island intensity varies continuously throughout the day and is strongly dependent on the weather conditions at the time. The most important weather parameters are the wind speed, the cloud cover and the solar radiation. We have developed an empirical model for the urban heat island intensity and applied it to a site near the centre of Manchester and a rural site at Rostherne, approximately 17 km away. Weather data from the Met Office station at Rostherne are available from the British Atmospheric Data Centre. Our model uses the measured wind speed, the measured cloud cover and the measured solar radiation from Rostherne. The parameters of the model are adjusted to give a best fit to the measured urban heat island intensity for the year 2014. The model is then used to predict the hourly urban heat island intensity for the first six months of 2015, obtaining good results especially as the values of the parameters are not changed throughout the year and the model does not make use of the temperatures at either site. The accuracy of the model is such that if used for a basic heating and cooling load calculations the accuracy of the annual demand is high. Practical applications: Many buildings that building services engineers and other building designers design are in urban or city centres. However, the weather data for their designs are based on near-rural weather data which do not include the urban heat island effect. This paper describes a method to ascertain the urban heat island effect in the centre of Manchester. A designer could apply this for Manchester and as an initial indication to other similar urban areas. This will allow the rural weather data to be adjusted on an hourly basis for the urban heat island effect throughout the year.

Abstract (sommario):
ObjectivesAs Canada’s second largest province, the geography of Quebec poses unique challenges for trauma management. Our primary objective was to compare mortality rates between trauma patients treated at rural emergency departments (EDs) and urban trauma centres in Quebec. As a secondary objective, we compared the availability of trauma care resources and services between these two settings.DesignRetrospective cohort study.Setting26 rural EDs and 33 level 1 and 2 urban trauma centres in Quebec, Canada.Participants79 957 trauma cases collected from Quebec’s trauma registry.Primary and secondary outcome measuresOur primary outcome measure was mortality (prehospital, ED, in-hospital). Secondary outcome measures were the availability of trauma-related services and staff specialties at rural and urban facilities. Multivariable generalised linear mixed models were used to determine the relationship between the primary facility and mortality.ResultsOverall, 7215 (9.0%) trauma patients were treated in a rural ED and 72 742 (91.0%) received treatment at an urban centre. Mortality rates were higher in rural EDs compared with urban trauma centres (13.3% vs 7.9%, p<0.001). After controlling for available potential confounders, the odds of prehospital or ED mortality were over three times greater for patients treated in a rural ED (OR 3.44, 95% CI 1.88 to 6.28). Trauma care setting (rural vs urban) was not associated with in-hospital mortality. Nearly all of the specialised services evaluated were more present at urban trauma centres.ConclusionsTrauma patients treated in rural EDs had a higher mortality rate and were more likely to die prehospital or in the ED compared with patients treated at an urban trauma centre. Our results were limited by a lack of accurate prehospital times in the trauma registry.

Abstract (sommario):
This study was conducted to investigate the tree species richness and diversity of urban and periurban areas of the Federal Capital Territory (FCT), Abuja, Nigeria, and produce Normalized Difference Vegetation Index (NDVI) for the territory. Data were collected from urban (Abuja city) and periurban (Lugbe) areas of the FCT using both semistructured questionnaire and inventory of tree species within green areas. In the study location, all trees with diameter at breast height (dbh) ≥ 10 cm were identified; their dbh was measured and frequency was taken. The NDVI was calculated in ArcGIS 10.3 environment using standard formula. A cumulative total of twenty-nine (29) families were encountered within the FCT, with 27 occurring in Abuja city (urban centre) and 12 in Lugbe (periurban centre) of the FCT. The results of Shannon-Wiener diversity index(H′)for the two centres are 3.56 and 2.24 while Shannon’s maximum diversity index(Hmax)is 6.54 (Abuja city) and 5.36 (Lugbe) for the urban (Abuja city) and periurban (Lugbe) areas of the Federal Capital Territory (FCT). The result of tree species evenness (Shannon’s equitability (EH) index) in urban and periurban centres was 0.54 and 0.42, respectively. The study provided baseline information on urban and periurban forests in the FCT of Nigeria, which can be used for the development of tree species database of the territory.

Abstract (sommario):
A city centre is an integral part of the overall urban model of the city and its metropolitan area. Therefore, the centre is influenced by the same urban trends. Renovation of the centre and devastated inner areas is related to the chaotic suburbanization trend. This trend is known as urban sprawl. It sucks viable powers (people, investments) from the city, which reflects in the loss of density of the existing urban structure as well as unaesthetic landscapes and threatens the revitalization of the existing urban structures and centres. The research on master plans of cities show their nonconformity to the demographic situation, which makes it impossible to avoid negative consequences of urban spread, growing use of resources, car dependency and pollution. New housing is developed to satisfy market needs, whereas the public interest is ignored. Based on the theoretical and practical research, the authors came to the conclusion that there is a need for a coordinated urban/rural policy and a realistic integrated territorial planning system as well as related monitoring and implementation tools. The EU policy documents and resolutions of National urban forums, which serve as a strong public think tank and monitoring tool in Lithuania, can serve as the guide for the sustainable development. The urban issues should be integrated into the National strategy for 2030. Lietuvoje urbanistinis sprogimas išryškėjo XXI a. pradžioje, jį lemia įvairūs sociokultūriniai veiksniai. Atlikti tyrimai rodo, kad urbanistinis sprogimas reikalauja miestų renovacijos išteklių, blogina miestų demografinę situaciją, didina energijos vartojimą, automobilių skaičių bei viešosios infrastruktūros poreikį. Chaotiškos priemiesčių suburbanizacijos problemų neišsprendžia fizinės aplinkos projektavimas, atsietas nuo socialinio, ekonominio planavimo. Jis daugiausia sprendžia komercinius uždavinius. Remdamiesi pasaulinio garso urbanistų teiginiais, ES urbanistinės politikos gairėmis, atliktais Lietuvos urbanistinių tendencijų tyrimais, Vilniaus miesto monitoringo ataskaitos duomenimis, autoriai teigia, kad chaotiškas priemiesčių urbanizavimas yra nei funkcionalus, nei estetiškas, nei palankus miestų urbanistinių struktūrų, tarp jų ir centrų, renovacijai. Jis gali būti įveiktas tik numatant kompleksines, koordinuotas urbanistinės politikos, teritorijų planavimo ir įgyvendinimo priemones.

Abstract (sommario):
AimsTo compare the Emergency Department (ED) referrals to psychiatry in a suburban versus an urban setting over a one-month to evaluate mental health presentations characteristics across two locations.MethodThis study was a retrospective cross-sectional study examining ED referrals to psychiatry in an inner-city and suburban centre over one month; - one based in an inner-city setting, the other based in a suburban area outside the city. The anonymised data were collected from both hospital's electronic patient records and analysed. The authors collected data on gender, age, employment, housing, the clinical problem at presentation, time of assessment and admissions. Descriptive data and hypothesis testing were performed where appropriate using Statistical Package for Social Sciences SPSS® version 26.ResultThe total number referred was 213: inner-city n = 109 and suburban n = 104. The inner-city saw a younger population; 47/109 (43%) were aged between 20 and 29 years, compared with 28/104 (27%) of suburban presenters (P-value 0.0134). A higher number of presenters were aged over 60 years in the suburban centre n = 13/104 (12.5%) versus the inner-city centre 3/109 (2.8%) (P-value 0.0084). In the inner-city, the proportion of homeless presenters was significantly higher at 30/109 (28%) versus 5/104 (4.8%) in the suburban setting (P < 0.0001). Presentations related to substances were highest, a total of 73 (34.3%) across both centres, with no significant difference in clinical presentations across the two centres. The majority were seen in the on-call period, 74/109 (67.9%) in the inner-city centre and 66/104 (63.5%) in the suburban centre. The psychiatric admission rate was significantly different between the two centres, with 33/109 (30.3%) patients admitted to the inner-city centre and 13/104 (12.5%) patients admitted to the suburban centre (P-value 0.002).ConclusionA large proportion of ED referrals to psychiatry constitute patients with unmet social and addiction needs. The variance in capabilities of liaison psychiatry (LP) and ED services means the local population's needs may not always be adequately catered for within a typical LP setting, which in the Irish context is predominantly driven by medical and nursing staff. This study highlights many patients attend the ED who may be better assessed directly by the community as per our National Emergency Program policies. This prompts consideration of expanding both ED and community services to comprise a more integrated, multidisciplinary-resourced, 24/7 care model.

Abstract (sommario):
In recent years, the centres of many towns and cities have been reshaped by urban design projects, but little attention has been paid to how these transformations are experienced everyday by users of the city. In other words: how do the users of urban centers, such as shoppers, cleaners, or workers, perceive these changes, as embodied subjects in specific material environments? This paper analyses how bodies in two intensely designed urban spaces–the shopping centre of Milton Keynes, a 1960s new town, and Bedford’s recently redeveloped historic town centre–are affected by elements of the built environment. ’Affected’ is a term borrowed from Latour (2004),and the paper works with, and elaborates, some of his and others’ work on how bodies are effectuated by other entities. Such Latourian work pays a great deal of attention to how bodies are affected by both human and non-human entities of many kinds, and we examine how certain aspects of the built environment in these two towns affect bodies in specific ways. However, we also emphasise the variability in this process, in particular that bodies seem unaware–or ambivalently aware–of many entities’ affordances.

Abstract (sommario):

Abstract (sommario):
The aim of this project was to improve the detection, monitoring, and medical care of Indigenous patients with diabetes in an urban Aboriginal medical centre. The research design and methods entailed the diabetes register being cleaned and updated. A pre- and post-project non-random sample audit of medical records of patients diagnosed with diabetes evaluated the level of care received compared to best practice standard of care. A multi-disciplinary Diabetes Team established procedures to improve the patient care provided. Additional service providers, including a dietitian, podiatrist, ophthalmologist, and endocrinologist, joined the team. A holistic health care approach was implemented with an emphasis on opportunistic care. The percentage of patients having a care plan completed increased from 18% in the 2002 audit to 72% in the 2003 audit. There were significant trends between increased completion of all cycle of care activities and frequency of GP review. Doctors prepared care plans for 50% of patients who attended less than two review visits in 2003, but increased to 89% if receiving six visits or more (p = 0.000). No significant improvement in health outcome was noted. The project showed significantly improved care planning and medical management of urban Aboriginal and Torres Strait Islander patients with diabetes.
