Abstract (sommario):
Placement of postextraction dental implants has become a common practice. Here, we reviewed current literature, along with clinical procedures, outcomes, and incidence of complications, associated with immediate implants in infected postextraction sites. The YSGG (yttrium, scandium, gallium, and garnet) laser can significantly reduce the bacterial concentration after extracting a compromised tooth. We treated a 40-year-old woman with a compromised tooth in the esthetic zone, presenting clinical and radiological signs of infection, particularly a periapical periodontitis. The tooth was extracted after administering local anesthesia using Optocain® (mepivacaine and adrenalin 1 : 100,000), following which the site was treated with an ErCr : YSGG (erbium, chromium-doped yttrium, scandium, gallium, and garnet) 2780 nm laser device (Biolase iPlus®). The implant (Straumann® fixture) was inserted with minimum 35 N torque, 1 mm below the most apical bone peak. Bio-Oss® and resorbable membrane were applied to improve bone healing. The use of ErCr : YSGG laser has ensured success of implant therapy performed on an infected site. There were no complications such as peri-implantitis or loss of peri-implant bone. The implant achieved good primary stability, immediate placement into an infected site did not increase complications, and the 5-year follow-up confirmed the treatment success.

Abstract (sommario):
Introduction: Several techniques such as using citric acid, plastic curettes, ultrasonic devices, and lasers have been suggested for debridement of contaminated implant surfaces. This comparative investigation aimed to assess and compare the effects of Er, Cr: YSGG laser and super-saturated citric acid on the debridement of contaminated dental implant surfaces. Methods: In this in-vitro study, 12 contaminated failed implants were collected and randomly divided into 2 groups (6 in group A, and 6 in group B). Also, one implant was considered as the control. The implants were horizontally sectioned into coronal and apical portions and subsequently irradiated by Er, Cr: YSGG laser in coronal and citric acid in apical in group A and the opposite in group B. In order to evaluate the effect of water spray on the laser section, half the laser portion of the implants was irradiated using water, while the other half was irradiated without water with an irradiation time of 1 minute. Results: Results revealed that calculus and plaque removal was greater in the laser part of both groups (with and without water) compared to citric acid parts and the correlation between calculus removal and surface roughness were statistically significant. Furthermore, the surface roughness in the citric acid parts was significantly higher than in laser parts. Water spray during irradiation had a very small influence on understudy factors. Conclusion: Based on the results of this study, the Er, Cr: YSGG laser was more effective in calculus removal and caused less surface roughness compared with citric acid application.

Abstract (sommario):
Introduction: The removal of ceramic veneers is a time-consuming procedure in a dental office. Little research has been done in alternative removal techniques for ceramic veneers. The objective of this study was to evaluate the removal of feldspathic and lithium disilicate reinforced glass ceramic veneers by Er, Cr: YSGG and to measure debonding time and pulpal temperature increase during veneer removal. Methods: Fifty-seven bovine incisor teeth were prepared and divided into 3 groups. Ceramic specimens with a thickness of 0.7mm, a width of 4mm and a length of 8 mm were fabricated from feldspathic ceramic, lithium disilicate reinforced glass ceramic HT (high translucency) and lithium disilicate reinforced glass ceramic MO (medium opacity) (19 for each group). Specimens were cemented on the labial surface of incisors using resin cement. The Er, Cr: YSGG laser was applied to each specimen at 2.5 W and 25 Hz. Debonding time was measured for each specimen, and the intrapulpal temperature was detected in 3 specimens for each group. Data were analyzed via one-way analysis of variance (ANOVA) at significance level of 0.05 (α = 0.05). Results: Mean debonding time was 103.68 (26.76), 106.58 (47.22) and 103.84 (32.90) seconds for feldspathic, lithium disilicate MO, and lithium disilicate HT respectively. There was no significant statistical difference among the groups (P value = 0.96). The intrapulpal temperature increase was less than 1°C in all groups. Conclusion: Er, Cr: YSGG can successfully be used to efficiently debond feldspathic and lithium disilicate reinforced glass ceramic veneers. There was no significant difference for debonding time among these ceramic materials. During ceramic laminate veneer removal by laser irradiation, no irritating temperature rise was detected.

Abstract (sommario):
BACKGROUND: Enamel demineralisation is an initial step of the serious dental problem including dental caries, white spot lesions and dental erosion. AIM: Compare the effect of Er, Cr: YSGG (λ = 2780 nm) and nanosecond Nd: YAG (λ = 1064 nm) laser on enamel acid resistance. MATERIAL AND METHODS: Thirty non-carious human premolars, extracted for orthodontic reasons, were used. The experimental groups (n = 10 each group) were: Group I, untreated (control); Group II, Er,Cr:YSGG laser irradiation (0.75 W, 20 Hz, 140 μs, 10 s); Group III, nanosecond pulsed Nd:YAG laser irradiation (0.8 W, 10 Hz, 7 ns, 10 s). Scanning electron microscope and Energy Dispersive X-ray Spectroscopy (EDX) were used to assess acquired enamel resistance to PH cycling. RESULTS: After subjecting the three experimental groups to PH cycling, scanning electron microscopic examination revealed irregular porous dissoluted enamel surface in group I. However, groups II and III demonstrated partially dissoluted enamel surface. EDX analysis demonstrated the lowest mean percentage decrease in calcium and phosphorus content in group II followed by group III, then the highest mean percentage decrease was observed in untreated group I. One-way ANOVA revealed significant differences (p < 0.0001) between the tested groups. CONCLUSIONS: Both Er, Cr: YSSG and nanosecond Nd: YAG laser irradiation were able to improve the acid resistance of enamel. However, enamel surface treated with Er, Cr: YSSG laser showed the lowest mean percentage decrease of calcium and phosphorus (highest acid resistance).

Abstract (sommario):
ABSTRACT Objective: The aim was to evaluate the effect of Er, Cr: YSGG laser and casein phosphopeptide-amorphous calcium phosphate (CPP-ACP) on surface micro-hardness of primary tooth enamel. Materials and Methods: A total of 30 freshly extracted caries free primary anterior teeth were cleaned and stored in 1% thymol. Teeth were embedded in acrylic resin such that only their buccal surfaces were exposed and were divided into four groups. Group I: Five intact teeth (negative control). The remaining 25 teeth were immersed for 30 min in 1% citric acid for demineralization. Group II: Five demineralized teeth (positive control), Group III: CPP-ACP (GC tooth mousse-GC International, Itabashi-Ku, Tokyo, Japan) application and Group IV: Etching using Er, Cr: YSGG laser + CPP-ACP application. Groups III and IV were subjected to pH cycling for 5 days. Surface micro-hardness of all the teeth was measured using Brinell hardness tester (Fuel Instruments and Engineers Pvt. Ltd.). Data were analyzed using ANOVA. Results: Mean surface micro-hardness of Groups I and II were 177.43 kgf/mm2 and 164.86 kgf/mm2, respectively. Group IV showed a higher mean surface micro-hardness (230.68 kgf/mm2) compared with that of Group III (190.28 kgf/mm2). In comparison to all other groups, laser etching prior to CPP-ACP application increased surface micro-hardness significantly (P < 0.001). Conclusion: Laser irradiation of primary teeth followed by CPP-ACP application increased surface micro-hardness of enamel.

Abstract (sommario):
Aim: The aim of the study was to investigate the shear bond strength (SBS) and compressive strength (CS) of Er Cr YSGG laser (ECL) treated, re-bonded lithium disilicate (LD) ceramic in comparison to standard conventional conditioning (hydrofluoric acid (HFA) and silane). Methods: One hundred LD ceramic disks were divided equally for SBS and CS testing. Eighty samples were conventionally surface treated and bonded to resin cement followed by de-bonding of the cement build-up. All de-bonded specimens were divided into four groups based on re-bonding surface treatments (HFA, primer, adhesive, and ECL). Resin cement build-ups were performed in 40 specimens for SBS testing (universal testing machine); however, the remaining 40 specimens were tested for CS. Ten specimens each were used as controls (surface treatment was performed once and no primary resin cement bonding) for SBS and CS assessment. Surface topography was assessed using a scanning electron microscope. Results: The maximum and minimum SBS values were shown by groups: control (33.42 ± 3.28 megapascals (MPa)); and ECL (17.50 ± 2.22 MPa) respectively. The maximum and minimum CSs were displayed by specimens in the ECL group (439.45 ± 70.68 MPa) and the control group (237.28 ± 19.96 MPa), respectively. For ECL specimens, SBS was significantly lower and CS was significantly higher as compared to control specimens. Conclusions: Application of the Er Cr YSGG laser significantly improved the CS of de-bonded ceramic specimens. However, it did not show a positive influence on the bond integrity of re-bonded ceramics in comparison to conventional surface treatment regimes.

Abstract (sommario):
Background: This study aimed to determine the impact of laser radiation on the repair bond strength of dental composite restorations by gathering, assessing, and systematically reviewing previous articles referring to this issue. Methods: Several previous studies relevant to the objectives of this research were found in PubMed, Scopus, and Web of Science databases. All prior articles indexed in these databases according to the selected keywords until 2018 were gathered and assessed. Some article abstracts showed the necessary basic conditions for inclusion in the study. Therefore, the full texts of these relevant articles were further evaluated in terms of the study objectives. Results: A total of 300 relevant articles were obtained by searching the databases. Eight studies remained highly relevant after performing a title review, eliminating the duplicate articles, and implementing the selection criteria. The latest study was conducted in 2018. A statistically significant difference was observed between the impacts of laser and other methods in the seven of these final relevant studies. Of these articles, five indicated a better impact in the case of other methods, particularly the dental milling technique, and one study was related to the impacts of the laser method. Additionally, the Er,Cr: YSGG laser was considered the most adequate laser in these studies. Conclusions: According to the review of prior studies on the impact of laser radiation on the repair bond strength of composite restorations, Er: YAG and Er,Cr: YSGG lasers are advised for surface preparation of composites. However, surface preparation by adopting the milling technique remains the adequate choice for repairing composites.

Abstract (sommario):
Eruption cysts are benign cysts that appear on the mucosa of a tooth shortly before its eruption. The majority disappear on their own. If they hurt, bleed or are infected they may require surgical treatment to expose the tooth and drain the content. Here we present a clinical case of a six-year-old child with an eruption cyst on the permanent maxillary central left incisor, which was handled using treatment with an Er,Cr-YSGG laser. It did not require suture, there was no haemorrhage, swelling, infection or postoperative pain. The treatment of eruption cysts with this technique facilitates obtaining the cooperation of pediatric patients and makes behavior management by the pediatric dentist easier.

Abstract (sommario):
AbstractThe objective of this in vitro study was to evaluate the influence of the Er,Cr:YSGG laser associated or not to a desensitizing agent in the treatment of erosive lesions. Forty specimens with dimensions of 4mm x 4mm and 3mm thickness were divided into 4 groups (n = 10): G1- no treatment; G2- 5% fluoride varnish; G3- Er,Cr: YSGG; G4 - fluoride varnish + laser. The specimens were immersed in erosive drink 3 times a day for 1 minute with an average interval of 2 hours between cycles for a period of 10 days. The treatments were performed according to the groups and the surface roughness and the wear profile were analyzed by scanning confocal microscopy. The normality (Kolmogorov-Smirnov) and homogeneity (Levene's) of the tests were evaluated. After these analyzes, the surface roughness data were submitted to the statistical analysis of Variance Analysis (ANOVA). All tests adopted a significance level of 5% (α = 0.05). At the representative images of the wear profile, the morphology of bovine dental enamel in its control and eroded areas were compared and qualitatively discussed. As regard surface roughness, there was no statistically significant difference between the groups. The qualitative analysis of the loss of volume showed that all experimental groups showed significant superficial morphology differences. Considering the limitations of an in vitro study, it can be concluded that the treatments performed were not able to treat dental erosion satisfactorily, indicating the need for more treatment sessions. Keywords: Tooth Erosion. Dental Enamel. Fluoride. YSGG Laser. Esmalte dental. Flúor. Laser de YSGG.
