Background. Providing equitable access to the full range of reproductive health services over wide geographic areas presents significant challenges to any health system. We present a review of a service provision model which has provided improved access to abortion care; support for complex issues experienced by women seeking nonjudgmental family planning health services; and a mechanism to collect information on access barriers. The toll-free pregnancy options service (POS) of British Columbia Women’s Hospital and Health Centre sought to improve access to services and overcome barriers experienced by women seeking abortion.Methods. We describe the development and implementation of a province-wide toll-free telephone counseling and access facilitation service, including establishment of a provincial network of local abortion service providers in the Canadian province of British Columbia from 1998 to 2010.Results. Over 2000 women annually access service via the POS line, networks of care providers are established and linked to central support, and central program planners receive timely information on new service gaps and access barriers.Conclusion. This novel service has been successful in addressing inequities and access barriers identified as priorities before service establishment. The service provided unanticipated benefits to health care planning and monitoring of provincial health care related service delivery and gaps. This model for low cost health service delivery may realize similar benefits when applied to other health care systems where access and referral barriers exist.

Background/Aims The quality of information technology (IT) services is key to effective healthcare delivery. However, the high aspirations of health ministers for IT services in hospitals may not be aligned with clinicians' perceptions. This study aimed to assess frontline clinicians' perceptions of the quality of IT services in their institutions. Methods The British Orthopaedics Directors Society online forum was used to invite a group of trauma and orthopaedic clinical leads from a range of hospitals to complete a short questionnaire regarding their perceptions of IT service quality in their practice. Results Negative perceptions of IT service quality were found to be common, with 45% of respondents rating their trusts' overall IT quality as poor or very poor. Of these, 13% deemed their trust's IT service quality to be so poor as to put patients at increased risk. Wide disparities were also reported between respondents' ratings of IT infrastructure quality and institutional responsiveness to concerns. Conclusions This small initial evaluation highlights concerning variations in clinicians' perceptions of IT service quality across different trusts. It also suggests the need for further, more detailed assessment and monitoring of IT quality improvement, for which the same questionnaire method may be useful.

ABSTRACTNational surveys show that people from minority ethnic groups tend to be less satisfied with social care services compared with the white population, but do not show why. Research indicates that barriers to accessing services include lack of information, perceptions of cultural inappropriateness and normative expectations of care. Less research has examined the experience of minority ethnic service users after they access services. This study conducted in-depth interviews with 82 South Asian and White British service users and family carers, the majority of whom were older people. Thematic analysis was used. The key theme was understanding the social care system. Participants with a good understanding of the system were more able to adapt and achieve control over their care. Participants with a poor understanding were uncertain about how to access further care, or why a service had been refused. More White British than South Asian participants had a good understanding of the system. There was more in common between the South Asian and White British participants' experiences than might have been expected. Language was an important facilitator of care for South Asian participants, but ethnic matching with staff was less important. Recommendations include better communication throughout the care process to ensure service users and carers have a clear understanding of social care services and hence a better experience.

Post‐secondary students with print disabilities (blindness, visual impairments, learning disabilities, physical handicaps) need information access to learning resources in alternate formats such as Braille, electronic text, large print, taped books, and tactile graphics. British Columbia College and Institute Library Services (CILS) is a provincial clearinghouse for resources for students and faculty with print impairments. CILS supplies resources tocomply with the “duty to accommodate” under Canadian Human Rights legislation. As a coordinated service, CILS provides services by direct loans, interlibrary loans with partner agencies, and production of new alternate format materials. This article describes the range of services and technological applications that are used to locate and produce resources to accommodate the students’ needs.

This is as true for hand trauma as it is for any other trauma specialty. however, the resources to manage the trauma caseload are relatively fixed. The aim of our study was to investigate the optimum way to manage hand trauma given its unpredictable workload. This information could be used to improve the efficiency of our local service and construct guidelines for other services to supplement the current guidance in circulation.

The present article examines the modernization of the Youth Service – a specific instrument in the implementation of state policy in the sphere of youth socialization in the UK. The author aimed to reveal the causes of modernization, which was achieved after the analysis of statutory documents of British authorities concerning young people and children; information taken from the British government’s official website about cooperation with voluntary organizations; works of foreign and national authors: experts in the implementation of state policy in the sphere of youth socialization – Anthony Giddens and Tom Wiley; researchers of the evolution of youth policy in foreign countries in the second half of the 20th century – A.A. Popov and V.S. Efimov, and researchers of the history of the British Youth Service creation – S.V. Fomenko and Graham Brotherton. Despite the existing opinion about the ultimate formation of a “democratic approach” to the evaluation of the place and role of young people in society by the middle of the 1990s, the author tends to consider the middle of the 1990s not as the end, but as the beginning of the rethinking of the British government’s position towards young people. As a result, the efforts of state and non-state social services began to focus on assistance not only to youth from the “risk group”, but also to such categories of young people which are oriented to active work, study, professional growth. Hence, the specific instrument of state policy implementation in the sphere of British youth socialization – the Youth Service – had also exercised radical modernization in 1990–2010. Among the reasons for the modernization, the author reckons the following: (1) the shift of youth policy emphasis from teenagers and young people towards infancy according to the loss of personal life benchmarks together with the lack of the “ontological security” feeling; (2) the extension in the domain of government’s actions on youth problems in a “horizontal” direction. In the author’s opinion, the first reason contributed to the legislative recognition of such a phenomenon as Children’s and Youth Trusts, which at the beginning of the 2000s opened a perspective for the development of an intersectoral approach in the solution of youth problems at the local level and led the trusts to become the major structure, at the local level, that operates a contact between various agencies influencing the youth socialization process. The second reason removed the sole responsibility of the Department of Education for the implementation of state youth policy allocating it between other departments.

Purpose – This paper aims to describe the transition of EThOS, the British Library’s E-Theses Online service, from its original role as a transactional document supply service to the service seen today where it forms part of the UK’s network of institutional repositories, open access and still-developing research funder mandates. Design/methodology/approach – The constituent parts of the EThOS service are described, and an analysis is given of the development of open access repositories, electronic theses and the way that PhD theses have become an important resource for cutting-edge research content for researchers worldwide. Findings – The value of doctoral theses for researchers continues to grow and be recognised. Many UK institutions have moved to mandatory open deposit of electronic theses, and many are digitising their older print thesis collections. Public funders are starting to track open deposit of the theses they fund; and research organisations are analysing the full UK metadata collection to understand trends in PhD research areas.

The British and Scotish public extension programs are currently “privatized,” after a decade-long process aimed at this objective. While the British system is owned by a private corporation, the Scotish one is still operated by a public entity. In both situations, information is not freely dispensed, but sold through a subscription process. For a fee, a basic level of service, including newsletters, production/marketing/farm management bulletins, and a limited amount of telephone time with disciplinary/commodity experts, is provided. For an additional fee, farm visits or problem diagnostic services can be secured. The government is one of the largest customers in both systems, funding major “public good” natural resource projects, rural reviatization projects, and agricultural sector job re-training programs. This has significantly impacted the way that information is obtained and delivered to primary producers. These issues, and their implications, will be discussed in this presentation.

The internet and NHS Net are used increasingly in UK general practice. A questionnaire survey conducted in southern England examined these applications. 77 (55%) of 141 practices responded. Of these, 71 were connected to one or other service and 27 offered a practice website. Only a small minority used a website for direct patient booking or access to pathology results. Moreover, among those with a practice website, none paid the necessary attention to data security. The survey revealed some fundamental misunderstandings that may partly account for the slow uptake of these technologies in British general practice.

Prescriptions for psychotropic drugs for in-patients in the Dundee Psychiatric Service were studied for three five-month periods in 1991, 1992 and 1993 using computer data and drug sheets. Information about Journal advertising and advice to doctors was obtained for the same period. A possible effect of Journal advertising was detected in the first year but there was no further Increase in the second year in the total of prescriptions. Information is presented of a similar survey in 1975 and 1976. A high proportion of prescriptions was for formulary drugs and doctors rated the British National Formulary as the strongest influence on their prescribing.

Radio broadcasting has played an important role as a medium of information, news and entertainment for Australian military personnel in wartime and conflict situations. However, while many nations have comprehensive units tasked to the full-time provision of broadcasting services, such as the Armed Forces Radio and Television Service (AFRTS) in the United States and the British Forces Broadcasting Service (BFBS) in the United Kingdom, Australia has relied on more ad hoc measures. As contingencies have required, the Australian military has introduced radio broadcasting elements into its table of organisation, the most comprehensive having been the Australian Army Amenities Service (AAAS) during World War II. Now, in a new technological era, perhaps specialised radio for troops will fade completely from the agenda.

Art librarianship can still achieve new successes, and some of these are noted. AimsPERCEIVE is a service evaluation counting whether nurse demographics correlate with reported subjection to violence and verbal aggression. The setting was a large mental health, learning disability and neuropsychiatry NHS trust in England. This continues our work to understand correlations reported in the literature between temporary staff and violence.MethodWe consulted the Caldicott, legal, equality & diversity, teams and gained service evaluation permission SER-19-031 from CNTW R&D department. We briefly consulted with staff regarding themes relevant to temporary nurse workers. They expressed concern that staff perceived to be “other” would be at more risk.Employees’ age, ethnicity, employment status, nationality, length of service and seniority are routinely collected for the running of the trust. Therefore, these were anonymously collated then cross-referenced with violence and aggression incident reports (VA IR1s). Chi-squared was used to identify statistical significance. Ethno-national status was taken from self-report. We could not control for hours worked nor could we get agency staff demographic data.We compared “exposure to at least one violent incident” in June, July and August 2019 against the following demographic categories:Substantive vs bank staffBand 5 and above vs band 4 and belowStaff with < 1 year of service vs staff with ≥ 1 year of service“White British” staff vs Non-“White British” staff“British” staff on self-report vs “Non-British” staffAge ≤30 years vs ≥ 31yearsA minimum of 1682 nursing staff were analysed for each category in each month.ResultSubstantive staff, “White British”, “British”, younger, and staff of shorter employment length had greater frequencies of at least one VA IR1s compared to the complementary groups. Length of service was significant only in two months but judged significant overall. There was no statistically significant correlation with seniority. Substantive staff have three times the risk vs bank staff, perhaps mediated by hours worked. Other risk ratios were in the region x1.2 to x1.8.ConclusionBeing British, White British, younger, less experienced or substantive staff correlate with subjection to reported aggression. This did not fit with staff speculation during consultation. Survival effects may be relevant. We are working to get more detailed information. Induction may help reduce aggression against newer staff.

Its reputation is based upon formidable achievements in all areas associated with the responsibilities of the national library, traditional and non-traditional – in the provision of current and historical research materials in all disciplines, in the bibliographic control of the nation's current printed output, in pioneering information services, in sponsoring research, and in becoming a major centre for the electronic capture, storage and transmission of documents to remote users. It has however in the last ten years been bedevilled by the endless delays in, and rising costs of, the construction of its badly needed St Pancras building for accommodating the London-based reference collections and services; these have given rise to public concern and to official scrutinies, while the move from the present reading rooms has itself been opposed by organized groups of users. Only part of the original plan will be realized. The library has issued a series of strategic planning documents setting out its plans and intentions, each one adapted to the circumstances at the time, and also commissioned a study which has challenged traditional assumptions concerning collecting and retention policies.

We believe cellphone text messages are commonly used in medical practice whether in rural or urban settings and that clinical photos are often attached to them. Our interest is the use of this technology to provide dermatology service to rural and remote British Columbia. Concern has been expressed about the security of confidential information and adequacy of privacy protection in such an application. We have found little published information about the extent of texting in rural and remote settings (and none in our jurisdiction) or the number and nature of privacy breaches that have actually occurred as a result. To obtain such information, we first set out to survey medical practitioners about their actual use. The results reported here are from medical trainees enrolled with the University of British Columbia who are in both rural and urban settings.

AbstractAttending physicians and medical advisors, physicians contracting their services to the Workers' Compensation Board, have key roles in assisting injured workers to return to work. A literature review of the role of the physician in the compensable return to work process reveals a lack of information regarding the experiences of medical advisors. This descriptive phenomenological study was undertaken to explore the lived experiences of four medical advisors in a northern rural service delivery location. The purpose of the research was to gain an understanding of the medical advisors' experiences in the compensable return to work process and in the compensation system. Analysis of the interview data revealed a central theme of commitment to quality medical care for injured workers, along with three major themes and several minor themes subsumed within the major concepts: providing medical opinions — requiring factual information, clarifying the diagnosis, no previous relationship with worker, categories of injuries; working with attending physicians and specialists — building relationships, evidence based treatment plans, role of the attending physician, role of the medical advisor; and, working within the workers' compensation environment — structure and policies, expedited services, and case management/team environment. This research report presents the central theme as the foundation through which the major themes are interconnected. This study does not generalise to all medical advisors, but relays stories that contain the essence of a lived experience.

ABSTRACTDuring the nineteenth century the British consular service was often dismissed as an organization with purely commercial responsibilities. A succession of governments and diplomats insisted upon this notion, despite the fact that at certain times both relied very much on consular officials for information on foreign affairs. This dependence was especially evident in Italy during the decade after 1860, when British leaders had lent their moral and diplomatic support to the creation of the modern Italian state against considerable international opposition. During this period their desire not to see the achievement undone led them to maintain a close watch on Italian affairs. The contribution made in this area by the consular service, and the manner in which it was reorganized in response to Italian unification, show how such a role could take priority over its other functions. Although this state of affairs was no doubt exceptional on account of the remarkable level of British interest in the Unification of Italy, it nonetheless provides a clear demonstration of how the organization could be used under certain circumstances. The extent to which British consuls were used to monitor affairs in post-unification Italy also encourages reflection upon the widespread view that British foreign policy rejected interventionism in favour of isolation from European affairs during the 1860s.

BackgroundOptimal transition from child to adult services involves continuity, joint care, planning meetings and information transfer; commissioners and service providers therefore need data on how many people require that service. Although attention-deficit hyperactivity disorder (ADHD) frequently persists into adulthood, evidence is limited on these transitions.AimsTo estimate the national incidence of young people taking medication for ADHD that require and complete transition, and to describe the proportion that experienced optimal transition.MethodSurveillance over 12 months using the British Paediatric Surveillance Unit and Child and Adolescent Psychiatry Surveillance System, including baseline notification and follow-up questionnaires.ResultsQuestionnaire response was 79% at baseline and 82% at follow-up. For those aged 17–19, incident rate (range adjusted for non-response) of transition need was 202–511 per 100 000 people aged 17–19 per year, with successful transition of 38–96 per 100 000 people aged 17–19 per year. Eligible young people with ADHD were mostly male (77%) with a comorbid condition (62%). Half were referred to specialist adult ADHD and 25% to general adult mental health services; 64% had referral accepted but only 22% attended a first appointment. Only 6% met optimal transition criteria.ConclusionsAs inclusion criteria required participants to be on medication, these estimates represent the lower limit of the transition need. Two critical points were apparent: referral acceptance and first appointment attendance. The low rate of successful transition and limited guideline adherence indicates significant need for commissioners and service providers to improve service transition experiences.

BackgroundMinority ethnic groups in the UK are reported to have a poor experience of mental health services, but comparative information is scarce.AimsTo examine ethnic differences in patients' experience of community mental health services.MethodTrusts providing mental health services in England conducted surveys in 2004 and 2005 of users of community mental health services. Multiple regression was used to examine ethnic differences in responses.ResultsAbout 27 000 patients responded to each of the surveys, of whom 10% were of minority ethnic origin. In the 2004 survey, age, living alone, detention and hospital admissions were stronger predictors of patient experience than ethnicity Self-reported mental health status had the strongest explanatory effect. In the 2005 survey, the main negative differences relative to the White British were for Asians.ConclusionsEthnicity had a smaller effect on patient experience than other variables. Relative to the White British, the Black group did not report negative experiences whereas the Asian group were most likely to respond negatively. However, there is a need for improvements in services for minority ethnic groups, including access to talking therapies and better recording of ethnicity.

Background In British Columbia (BC), Canada, clinicians are responsible for giving involuntary psychiatric patients rights information upon admission. Yet an investigation by the BC Office of the Ombudsperson found that clinicians are not always fulfilling this responsibility. The Ombudsperson recommended that the provincial government fund an independent body to give rights advice to patients. Methods To understand how clinicians feel about this recommendation, focus groups of clinicians who may give psychiatric patients rights information (n = 81) were conducted in Vancouver, BC, to probe their attitudes toward independent rights advisors. The focus group transcripts were thematically analyzed. Results Most clinicians believe that giving rights information is within their scope of practice, although some acknowledge that it poses a conflict of interest when the patient wishes to challenge the treatment team’s decisions. Participants’ chief concerns about an independent rights-advice service were that (a) patients may experience a delay in receiving their rights information, (b) integrating rights advisors into the workflow would complicate an already chaotic admission process, and (c) more patients would be counselled to challenge their hospitalization, leading to an increased administrative workload for clinical staff. However, many participants believed that independent rights advisors would be a positive addition to the admission process, both allowing clinicians to focus on treatment and serving as a source of rights-related information. Conclusions Participants were generally amenable to an independent rights-advice service, suggesting that the introduction of rights advisors need not result in an adversarial relationship between treatment team and patient, as opponents of the proposal fear. Clearly distinguishing between basic rights information and in-depth rights advice could address several of the clinicians’ concerns about the role that independent rights advisors would play in the involuntary admission process. Clinicians’ and other stakeholders’ concerns should be considered as the province develops its rights-advice service.

Objectives Previous research has largely attempted to explore breast screening experiences of South Asian women by combining opinions from Pakistani, Bangladeshi, and Indian women. This research often fails to reach the most underserved sub-groups of this population, with socioeconomic status not routinely reported, and English fluency being a participation requirement. With uptake low amongst British-Pakistani women, this study explores the experiences these women encounter when accessing the NHS Breast Screening Programme. Methods 19 one-to-one semi-structured interviews were carried out with British-Pakistani women from East Lancashire, UK. 14 interviews were conducted via an interpreter. Results Data were analysed using thematic analysis. Three themes were identified: ‘Absence of autonomy in screening and healthcare access’ describes how currently the screening service does not facilitate confidentiality or independence. Access requires third-party intervention, with language barriers preventing self-expression. ‘Appraisal of information sources’ makes distinctions between community and NHS communication. Whereas community communication was invaluable, NHS materials were deemed inaccessible due to translation incongruences and incomprehensible terminology. ‘Personal suppositions of breast screening’ explores the subjective issues associated with disengagement, including, the cultural misalignment of the service, and perceiving screening as a symptomatic service. Conclusions British-Pakistani women face some unique challenges when accessing breast screening. To promote uptake, the service needs to address the translation of screening materials and optimize upon community networks to disseminate knowledge, including knowledge of the screening environment within the context of culture to promote informed choice about attendance.

The main source of topical information in most parts of the world is through the mass media – principally newspapers and television. Television news and current affairs have a duty to remain impartial, as they provide a service for the whole population. Newspapers, on the other hand, have a greater journalistic and editorial freedom as they target subgroups. British newspapers are currently being scrutinised as to how they report both factual and speculative information of a general kind. At the same time there seems to be an increase in their interest in psychological matters. However, work on newspaper reporting of psychiatric disorders (Day & Page, 1986; Matas et al, 1986) has shown that the mentally ill are usually portrayed in a rather negative light with few positive images.

The methodology and findings are presented of an empirical study comparing local citation patterns with the holdings lists of a number of sources of journal articles. These sources were the British Library Document Supply Centre (BLDSC) and the BL inside service, library holdings, ProQuest Direct, SearchBank, EiText and a linking system including both the Geobase database and the BLDSC. The value of local citation figures is discussed, as is the concept of a “core” of journal titles, from both theoretical and practical perspectives. Using these figures to represent the local use of journal articles, the coverage of the document sources was found to vary widely. Unsurprisingly, the BLDSC was found to offer the widest coverage. Newer, electronic systems generally fared less well, but may offer other advantages.

PurposeThe Comparative Outcomes And Service Utilization Trends (COAST) Study in British Columbia (BC), Canada, was designed to evaluate the determinants of health outcomes and health care services use among people living with HIV (PLHIV) as they age in the period following the introduction of combination antiretroviral therapy (cART). The study also assesses how age-associated comorbidities and health care use among PLHIV may differ from those observed in the general population.ParticipantsCOAST was established through a data linkage between two provincial data sources: The BC Centre for Excellence in HIV/AIDS Drug Treatment Program, which centrally manages cART dispensation across BC and contains prospectively collected data on demographic, immunological, virological, cART use and other clinical information for all known PLHIV in BC; and Population Data BC, a provincial data repository that holds individual event-level, longitudinal data for all 4.6 million BC residents. COAST participants include 13 907 HIV-positive adults (≥19 years of age) and a 10% random sample inclusive of 516 340 adults from the general population followed from 1996 to 2013.Findings to dateFor all participants, linked individual-level data include information on demographics, health service use (eg, inpatient care, outpatient care and prescription medication dispensations), mortality, and HIV diagnostic and clinical data. Publications from COAST have demonstrated the significant mortality reductions and dramatic changes in the causes of death among PLHIV from 1996 to 2012, differences in the amount of time spent in a healthy state by HIV status, and high levels of injury and mood disorder diagnosis among PLHIV compared with the general population.Future plansTo capture the dynamic nature of population health parameters, regular data updates and a refresh of the data linkage are planned to occur every 2 years, providing the basis for planned analysis to examine age-associated comorbidities and patterns of health service use over time.

In Britain as in other countries security service files have come to provide an important resource for the writing of communist history. This paper discusses some of practical, ethical and methodological challenges they pose for historians. In Britain the selective release of small batches of redacted files was undertaken from the 1990s as part of the post-Cold War rebranding of the MI5 state security service. No meaningful public consultation took place regarding the principles governing the release of these materials. Nor were details made available of the scope of state surveillance or of the individuals and organisations subjected to it. The lack of transparency and accountability was compounded by the asymmetry between observers and observed and the withholding of information regarding the identities, associations and career histories of security operatives. Drawing on published and unpublished examples, the paper characterises this as a project for the 'securitisation' of communist and wider left history: one that seeks to validate MI5's past and continuing role by accentuating issues of espionage while filtering and in many cases destroying evidence of the routine surveillance of political and social movement activism.

In this article, we report on the experiences of newly arrived South Asian international students (undergraduate and graduate levels) with accessing healthcare services in a small town setting in British Columbia, Canada. The objective of the study was to better understand the barriers and facilitators of accessing healthcare services. We used a qualitative design with an exploratory approach and conducted 8 semi-structured interviews with South Asians students that had newly arrived in the city of Kamloops and had been enrolled at Thompson Rivers University (TRU) for less than 12 months. Findings included barriers such as needing more information during on campus orientation, inefficiencies of the healthcare system, and accessing healthcare service. Facilitators of accessing healthcare services included attending campus orientation, having a curiosity, and a social support system. These findings provide new knowledge regarding the barriers and facilitators of accessing healthcare in Canada. In order to better meet the needs of international students programs and services must be tailored to this unique group of students.
