Obesity and obesity-associated metabolic disorders are highly associated with cardiovascular disease. Abnormal ectopic deposition and accumulation of adipose tissue in organs, including perivascular space (perivascular adipose tissue, PVAT) in obesity are emerging to contribute to vascular disease development through pathological paracrine and/or endocrine secretion of cytokines, namely adipokines, which are vasoactive factors including vascular relaxing and contracting factors, smooth muscle growth promoting and inhibiting factors, and pro- and anti-inflammatory factors. In obesity, production of these factors from PVAT is altered and in imbalance which favours vascular contraction, pathological remodelling, and inflammation. In cross-talk with the endothelium, the functional changes of adventitia and PVAT are detrimental and importantly contribute to the acceleration of vascular atherosclerosis and complications associated with obesity and metabolic disorders

The motor unit represent the final output of the motor system. Each consists of a motoneuron, its axon, neuromuscular junctions, and muscle fibres innervated by that axon. The discharge of a motor unit can be followed by recording its electromyographic signature, the motor unit action potential. Motoneurons are not passive responders to the excitatory and inhibitory influences on them from descending and segmental sources. Their properties can change, e.g. due to descending monoaminergic pathways, which can alter their responses to other inputs (changing ‘reflex gain’). Contraction strength depends on the number of active motor units, their discharge rate, and whether the innervated muscle fibres are slow-twitch producing low force, but resistant to fatigue, fast-twitch producing more force, but susceptible to fatigue, or intermediate fast-twitch fatigue-resistant. These properties are imposed by the parent motoneurons, and the innervated muscle fibres have different histochemical profiles (oxidative, glycolytic, or oxidative-glycolytic, respectively).

This chapter provides a proposed scope of application of the NATO SOFA and compares this to other agreements. In the NATO SOFA, the definition of a ‘force’ covers both collective military units and individuals in active service and assigned to the military line of command. Nationality is not a condition to be met as a ‘member of a force’. The member of the armed service of one party must be present on the territory of another contracting party to fall within the definition, and as such, Receiving State personnel are excluded from the definition of a ‘force’. The force must furthermore be operating in the North Atlantic Treaty area. Finally, the member of the force must be present on the territory of another party in connection with official duties. This condition, particularly, has generated discussion in NATO SOFA practice.

This chapter covers two major fields of the blood circulation: ‘distribution’ and ‘exchange’. After a short survey of the types of vessels, which form the circulation system together with the heart, the chapter describes how hydrostatic pressure derived from the heartbeat and vascular resistance determine the volume of blood that is locally delivered per time unit. The vascular resistance depends on the length of the vessel, blood viscosity, and, in particular, on the diameter of the vessel, as formulated in the Poiseuille-Hagen equation. Blood flow can be determined in vivo by different imaging modalities. A summary is provided of how smooth muscle cell contraction is regulated at the cellular level, and how neuronal, humoral, and paracrine factors affect smooth muscle contraction and thereby blood pressure and blood volume distribution among tissues. Subsequently the exchange of solutes and macromolecules over the capillary endothelium and the contribution of its surface layer, the glycocalyx, are discussed. After a description of the Starling equation for capillary exchange, new insights are summarized(in the so-called glycocalyx cleft model) that led to a new view on exchange along the capillary and on the contribution of oncotic pressure. Finally mechanisms are indicated in brief that play a role in keeping the blood volume constant, as a constant volume is a prerequisite for adequate functioning of the circulatory system.

This case study provides an example of a collaboration between a university and a public health agency to build organizational capacity to spread data-driven decision-making, implementation, and evaluation of evidence-based cancer prevention strategies. The Office of Chronic Disease Prevention and Health Promotion at the South Dakota Department of Health provided the key management practices for scale-up of evidence-based decision-making (EBDM): leadership support, training, a supportive organizational climate and culture, inclusion of partners, and outcomes-based contracting with partnering organizations. A pre–post survey showed increased use of research evidence for several job tasks, including selection of interventions and evaluation. Perceived work unit access to skills in prioritization and adapting interventions also increased. The 16 staff and partners interviewed perceived leadership support, federal funding requirements, and an initial multi-day training as the key facilitators for spreading EBDM.

Relativity is a set of remarkable insights into the way space and time work. The basic notion of relativity, first articulated by Galileo, explains why we do not feel Earth moving as it orbits the Sun and was successful for hundreds of years. We present thinking tools that elucidate Galilean relativity and prepare us for the more modern understanding. We then show how Galilean relativity breaks down at speeds near the speed of light, and follow Einstein’s steps in working out the unexpected relationships between space and time that we now call special relativity. These relationships give rise to time dilation, length contraction, and the twin “paradox” which we explain in detail. Throughout, we emphasize how these effects are tightly interwoven logically and graphically. Our graphical understanding leads to viewing space and time as a unified entity called spacetime whose geometry differs from that of space alone, giving rise to these remarkable effects. The same geometry gives rise to the energy?momentum relation that yields the famous equation E = mc2, which we explore in detail. We then show that this geometric model can explain gravity better than traditional models of the “force” of gravity. This gives rise to general relativity, which unites relativity and gravity in a coherent whole that spawns new insights into the dynamic nature of spacetime. We examine experimental tests and startling predictions of general relativity, from everyday applications (GPS) to exotic phenomena such as gravitomagnetism, gravitational waves, Big Bang cosmology, and especially black holes.
