Background Discrepancies in controlled substance documentation are common and can lead to legal and regulatory repercussions. We introduced a visual analytics dashboard to assist in a quality improvement project to reduce the discrepancies in controlled substance documentation in the operating room (OR) of our free-standing pediatric hospital. Methods Visual analytics were applied to collected documentation discrepancy audit data and were used to track progress of the project, to motivate the OR team, and in analyzing where further improvements could be made. This was part of a seven-step improvement plan based on the Theory of Change with a logic model framework approach. Results The introduction of the visual analytics dashboard contributed a 24% improvement in controlled substance documentation discrepancy. The project overall reduced documentation errors by 71% over the studied period. Conclusion We used visual analytics to simultaneously analyze, monitor, and interpret vast amounts of data and present them in an appealing format. In conjunction with quality-improvement principles, this led to a significant improvement in controlled substance documentation discrepancies.

AbstractAll information obtained from a patient in the course of medical care is a potential part of clinical documentation. The documentation usually serves a number of different purposes. The task of a documentation system is to fulfil these purposes in a methodically correct manner and as economically as possible. This requires that the properties of the documentation system be planned systematically with a view to the goals pursued. To support systematic planning, a “documentation protocol” is proposed analogous to the “study protocol” used for controlled clinical trials. The individual sections of the proposed documentation protocol are described and the design options which exist in the corresponding planning phases are pointed out. Experience gained by the application of the documentation protocol is discussed.

The safety problem with information circulating in corporate informationcomputer nets is urgent under conditions of presentday information society. The authors have developed a generalized functional model of the process of controlled access differentiation. At the same time come forward users identified by accounts as access subjects in the model and files of documentation formats are objects. Rules for the differentiation of a subject access to objects are specified as a matrix of powers taking into account marks of confidentiality. A distinguishing feature consists in that a container storing data is protected on basis of the method of indistinguishable obfuscation. The model developed allows storing data in a uniformed kind and ensuring a single method for an access to them. For safe storing is used a format of the protected container where information is stored in an obfuscated form. A container represents an executable file having a number of preset properties and functions allowing unambiguously the user identification, differentiation of an access to data (rights: to read, write, and assignation), assurance of the security for a confidence of the document implemented. The container format ensures its safe storing and transmission through a network.
