Individuals who identify as lesbian, gay, or bisexual (LGB) are at elevated risk for anxiety disorders and obsessive–compulsive disorder, likely due to the mental health impact of stigma-related stress. Effective psychosocial interventions, especially cognitive–behavior therapies, can reduce symptoms and improve quality of life. This chapter reviews the research and clinical literature to describe adaptations that may be needed at each point of therapy from initial assessment and case conceptualization to effective intervention. Consideration of intersectionality between LGB identities and other identities, such as race, ethnicity, and HIV status, highlights the importance of recognizing the multiple identities a client may have. Case vignettes bridge the gap between the research and clinical literature and implementation of the recommendations.

Lesbian, gay, and bisexual (LGB) parenthood post-heterosexual relationship (PHR) dissolution was the focus of much of the early social science research on sexual minority parenting from the 1980s onward, and remains relevant today. The initial legal cases contesting LGB parenting involved mainly lesbian mothers seeking custody of their children PHR. Yet the focus of most research on LGB parenting and legislation in the twenty-first century has been on children brought up in planned LGB-parent families. This chapter highlights the key issues of sexual identity disclosure and identity intersection for LGB parents PHR. It considers family transition through single parenthood, nonresidential parenting, and stepfamily formation that might particularly marginalize LGB parents and their children PHR. The review focuses on studies of LGB parents, new same-gender partners, and children in these families. When relevant, it considers how family law relates to sexual minority identity empowerment and disempowerment PHR.

Sexual and gender minority (SGM) patients are often assumed to be cisgender and heterosexual by healthcare professionals, resulting in missed opportunities for a patient-centered experience. Although lesbian, gay, bisexual, and transgender (LGBT) is the common parlance, SGM is the preferred terminology in science-based settings (e.g., the National Institutes of Health and the Centers for Disease Control and Prevention) because it includes individuals with minority sexual attraction, sexual behavior or gender identity whose sexual or gender identity is something other than LGBT. Despite bioethics placing anti-LGBT bias below professional standards, disclosure places SGM patients at risk of bias and discrimination, such as refusal to treat, stereotyping, and explicit and implicit bias. SGM patient disclosure is frequently inhibited by the anticipation of bias and discrimination from healthcare workers. By establishing a patient-centered practice, informed by the needs of SGM patients and their support network, physician assistants in palliative care medicine can enhance the end-of-life experience of their SGM patient population while optimizing community and individual resilience.

Japan does not have a cultural history of strong stigma against homosexuality and gender nonconformity in the ways that are true in West, but there is growing evidence that homophobia and transphobia do exist. In this chapter, the history of LGBT issues in Japan is overviewed, focusing mainly on the experiences of gay men and lesbians. Lately, more LGBT-related research has focused on studies on persons with gender identity disorder (GID) due to the approval of gender reassignment surgery as a treatment for GID. Many studies showed that sexual minority youth reported suicidal wishes or behavior because of bullying experiences, feelings of isolation, physical dysphoria, or internal homophobia or transphobia. Moreover, most teachers reported that they did not include any material on LGBT issues in classroom, thus highlighting an urgent need to educate school teachers on these issues.
