Introduction There is a paucity of knowledge regarding adolescent's preferences for care. The health related problems they face have implications for individuals and nations. Objectives To determine the health problems and needs of adolescents, their knowledge, use of, and preferences for health related services and service providers. Study design A descriptive study involving a two-stage probability sample. An interview schedule was designed for data collection. Setting Eight randomly selected main-stream high schools in Stoke-on-Trent, England. Subjects One hundred and eleven males and 142 females aged 14 and 15 years between 1 st April and 30th June 1994. Results The adolescents had unmet problems and needs relating to lifestyle and risk-taking behaviour, sexual and reproductive health, and emotional problems, influenced by socio-economic and legislative factors. Services were used primarily for physical problems. Knowledge of the location and opening times of two local contraceptive services for adolescents was poor (10/253,4%). Factors they associated with confidentiality were identified. Preferences for service providers varied with the nature of the problem. The girls were more likely to give advice to peers regarding substance abuse, and issues relating to sexual and reproductive health, and expressed a greater preference for advice from peers on these issues. The services the adolescents wanted to see provided were appropriate to their needs and reflected a holistic concept of health. Conclusions • The Health of the Nation targets will not be met unless these problems and needs are addressed. • Potential intervention points for health promotion are being missed. • Local services must be widely advertised. • Adolescents need specific reassurance from service providers that their care will be confidential. • Positive actions adolescents are prepared to take need reinforcing. • Peer counselling programmes might be expected to have a greater positive impact on girls. • Adolescents' opinions regarding service provision must be taken into account.

The purpose of this study was to determine the service needs being met and the needs not being met as perceived by the chronically homeless who call the park their home. The authors thought this an important study to examine to better understand the service needs of the homeless. The study used a qualitative design for collecting data which involved face-to-face interviews with ten of the homeless people at the park. Four primary themes, three of which had two subthemes each, were identified through a thematic analysis. The primary theme of mistrust of services had subthemes of safety and restrictions and ineffective services. The primary theme of services needed was subdivided into problem identified and potential solutions. The families theme contained subthemes fractured families and park community as family. The theme of hopelessness did not have any subthemes. It was concluded that research should continue in this field and funding should be used to focus on providing services as specified through these themes.

The mental health needs of patients entering the program at ward one, Valkenberg Hospital, are not routinely measured. Their presenting problems are frequently complicated by basic and social needs, which impact on recovery and re-integration into the community. The Camberwell Assessment of Need is a valid and reliable instrument used to measure the mental health needs of psychiatric patients. In this study, this instrument was used with some modifications to assess the needs of patients on admission to ward one. At discharge, these needs were explored to measure the extent to which services and interventions were useful in meeting those needs. Where not, questions aimed at elucidating impediments to care were asked. All 60 patients completing the program agreed to participate. The average number of total needs on admission was 9.2, which included an average of 6.7 unmet needs. Psychiatric needs were the most commonly reported, with a need in the area of "depression" being declared in 59 patients. Help received prior to admission was scant, and patients had high expectations of ward one. Help received from ward one was highest in areas of psychiatric needs, with 89.8% of patients receiving moderate or high help in the area of depression. More than two-thirds of patients reported receiving low help in basic areas such as accommodation, self-care, and sexual expression. The services most often regarded as useful were organised group activities and therapies. Paired t-tests revealed that the patients without borderline personality had more unmet needs on admission. Regression analysis suggests that younger age and lower level of education confer a greater risk of unmet needs on admission and discharge. The routine use of a needs assessment instrument is recommended as an aid to guide clinicians. Attention needs to be given to basic and social needs, such as accommodation, daily activities and company, prior to admission to hospital. Where these needs persist, the active involvement of a social worker and occupational therapist is suggested. The high numbers of unmet needs in domains other than psychiatric problems, requires the input of the multidisciplinary team.

Foster children are already at a disadvantage given that they tend to live in a home without their parents. Social workers have direct interactions working with children with special needs, and through interviews with the workers, we aimed to understand whether or not the services available are able to meet foster children’s special needs. The research on special needs children in foster care is quite limited. The purpose of the study was to identify social workers’ perceptions of the needs of special needs foster children and their access to services. This study used qualitative interviews with open-ended questions to gather qualitative data from ten social worker participants. The researchers employed thematic analysis techniques to analyze the qualitative data. The themes that emerged from analyzing the data were social workers’ challenges in providing services to special needs foster children, social workers’ access to services, and the need for institutional support to effectively manage service delivery for special needs foster children. This study found that social workers face challenges in obtaining training for parents and social workers, navigating complex funding processes, managing inconsistent desires between parents and court officials, and dealing with the limited number of capable foster homes. The social workers interviewed identified different tactics that could be implemented in order to improve services for the special needs foster children through changes and improvements within the department. Understanding large-scale policy and practice issues, such as organizational change or resource management, can lead county agency social workers to advocate for the challenges of special needs children in child welfare.

Seniors 75 years of age and older, the majority of whom live in the community, constitute a segment of the population that is vulnerable to loss of autonomy. Indeed many community dwelling seniors have difficulty performing daily living activities, such as bathing, toileting, walking, preparing meals and housekeeping.
In the setting of a population based cohort study of community-dwelling seniors 75 years of age or older, we examined the effect of unmet needs for community services for activities of daily living (ADL) and instrumental activities of daily living (IADL) on health services utilization. Self-perceived unmet need status was determined using a baseline in-home interview. A total of 839 subjects were recruited from the Greater Montreal Region, Quebec, Canada, using random telephone number dialling.
Health services utilization data were obtained from administrative databases from the Quebec Health Insurance Board (Regie de l'Assurance-Maladie du Quebec - RAMQ). Multivariable negative binomial regression models were used to examine the association between unmet need status and health services utilization during the six month period following the baseline interview.
The results of this study indicate that unmet needs are associated with higher rates of emergency department visits, hospitalization and prescription drug use. No statistically significant association was found between unmet needs and physician utilization among single seniors, although married seniors with unmet needs in activities of daily living had 2.8 times the rate of medical specialist visits as compared to those who reported no unmet ADL needs.
Unmet need for community services among the elderly has implications for the use of more expensive acute and long-term health care services. The results of this research suggest that developing programs to address unmet needs in the elderly population can potentially reduce health services utilization by the elderly.

This document is a review of literature on needs assessments and the benefits of conducting one. Communicare is a mental health agency that serves the Lincoln Trail Region. Currently, most of the revenue from their children programs comes from Medicaid, which is a fee-for-services payer source. The Kentucky Medicaid Program is in the process of contracting with managed care organizations to oversee services that have been paid directly from Kentucky Medicaid. With these changes, mental health organizations must identify specific community service needs as well as expanding revenue sources. Applying for grants is one way mental health agencies can expand revenue sources. Communicare has identified the KY SEED grant that focuses on prevention and providing services to early childhood programs as a potential funding source. A needs assessment was conducted to gather information on children services implemented by Communicare. It was conducted in order to assess current programs and seek out potential areas of future program growth. The needs assessment further sought to identify gaps in services for the early childhood programs and assist in the grant application process. A review of existing data on children’s services offered at Communicare, including a satisfaction survey and a System of Care Assessment Report, was conducted as part of the needs assessment. A Community Forum with community partners from the Lincoln Trail region was held to gather additional data for the needs assessment.

Emergency Medical Services (EMS) is an institution and product of public health, health care, and public safety that is chopped and scattered across multiple jurisdictional deployment methodologies throughout Arizona. To fully-asses the EMS needs of the state, those jurisdictions are considered as a whole; for it is the whole that makes a system, and a system is what truly impacts patient outcomes. Evaluating the ""whole"" is the genesis and driver of the 2016 Arizona Statewide EMS Needs Assessment (ASENA). The primary objective of ASENA is to establish a current ""snap-shot"" of EMS in the state while simultaneously identifying needs and/or areas that can be targeted for further analysis and/or improvement as part of Population Health Management and Emergency Medical Services Integration under the AZ Flex Grant funded by the U.S. Health Resources and Services Administration (HRSA). In addition, the secondary objective of ASENA is to compare and contrast this current ""snap-shot"" with data obtained in a more narrow needs assessment conducted in 2001, allowing comparison of changes in Arizona's critical access EMS system over 15 years. To accomplish this, a 105-question needs assessment survey tool was developed and distributed to EMS agencies throughout the state. The fully-vetted survey tool collected information pertaining to sixteen core functional sections. Eighty-six agencies fully-completed the needs assessment survey tool, with respondents evenly distributed across the state's four EMS coordinating regions and representative of the various service-delivery methodologies. The combined service areas of the respondents cover over 85% of the state's population. Arizona's statewide EMS system is well organized and positioned to deliver advanced levels of prehospital care for the vast majority of its citizens and visitors, with some variation between urban and rural regions. Key needs identified relate to: patient care reporting between EMS providers, emergency departments and receiving hospitals; quality assurance activities; education and skills training programs; dispatch system capabilities; mass casualty and public health preparedness; equipment and supplies; and more robust use of data and analyses to inform continuous EMS system improvement.

Since the establishment of a democratically elected multi-party government in 1990, the need for advancing a national educational program has been evident if Nepal is to rise above its present poverty status. This dissertation provides a description of the existing status, the needs, and future proposals concerning special education for exceptional students in Nepal. The purpose of this study was to conduct a needs assessment for the delivery of special education services in Nepal. Specifically, this study provides: (a) a national overview of the status of special education services in Nepal; (b) a description of the specific issues with which school administrators are faced in trying to serve exceptional children and youth, and (c) recommendations for the future. The review of the literature addresses the limited Nepalese data and information on: (a) prevalence of exceptional school-age population; (b) the legal status of special education; and (c) exceptional students being served. The major components of comprehensive special education programs were discussed including: (a) the purposes of special education; (b) the alternative kinds of special education services; and (c) the organization of special education services. The methodology for this study included a description of a conceptual model; unit of analysis; subjects selection; instrumentation, data collection methods; and data analysis procedures. The results described the current status and needs of special education services in all seventy five school districts of Nepal. Interviews and questionnaires were used to collect data from district education officers. These data presented the responses of the district education officers regarding the: types and numbers of children with exceptionalities identified, served, and not served; the kinds of personnel and programs provided; operating procedures; barriers; and administrators' suggestions for improving services. The recommendations from this study included: information about strong and weak areas in existing services and needs to improve the special education services; a new model for the delivery of services; and suggestions for effecting further action. This study concluded with comments on the feasibility of implementing a national plan, with particular attention to the establishment of a nationwide delivery procedure for Nepalese children with special education needs.

Method: the Camberwell Assessment of Need Short Appraisal Schedule (CANSAS) was used to assess the needs and the demographic questionnaire was used to identify the patient’s demographic variables includes gender, age, income and level of education. Results: patient who meets any one of these criteria; being elderly, poor, and the uneducated are more likely to have higher total number of needs. Men and women are more likely to report similar met needs, but women are more likely expressed more unmet needs.

Reflecting an increased interest in service design and service science, this study develops a perspective that unites the Service Dominant Logic, the principles of service design and the foundations of market orientation in a triangular relationship. In this approach, the premises of the SD logic are used to provide support and structure to the principles of service design and market orientation. This perspective is used as a backdrop for the discovery of latent needs. Using an unconventional sample of primary school pupils, customer journey maps and critical incidents were created. The application of these service design tools generated insights into their needs, which could be categorized into themes and-overarching value dimensions. Four dimensions of value were uncovered, namely a Physical, Social, Utilitarian and Hedonic Dimension. The analysis of the correlation of these themes and dimensions leads the development of a number of propositions regarding their relationships.

The research enquiry using a sample of 20 NHS organisations with similar revenue and population characteristic profiles sets out to identify the information needs of the NHS General Managers and in doing so highlight the information that they need to meet their organisations' key success factors. The research identifies through the enquiry process the General Managers' information needs and describes the categories of those needs, the pressures and influences of the General Managers' working environment on those needs and the links to the influences that have been reflected in their information needs. In particular the external influence of the Department of Health and the business environment has not only changed the General Managers' information needs but affected the balance of power between the stakeholders, which in turn has affected those information needs. These organisational and environmental changes, health policies, stakeholder demands, and changes in the balance of power between the stakeholders has resulted in a change in the way in which the General Managers work with information, which has in turn increased the need for more accurate, timely and complex information. The research analyses the General Managers' understanding of their roles in the General Manager/patient/doctor relationship and analyses the potential areas of conflict arising when the patients' interests clash with the market-led (business) interests of the healthcare organisations and the diverse needs of the Provider, Purchaser and Regional Executive General Managers. These problems together with the effects of organisational resistance, organisational culture clash and system requirements and its effect on the information needs of the General Managers were examined for associated links with the difficulties that the General Managers experience in defining their information requirements. The research provides an understanding of the links between a market-led healthcare environment, the General Managers' information needs, and their attitude towards information as well as an understanding as to whether the patient, a key stakeholder, has benefited in the healthcare empowerment stakes as a result of the re-delineation of the General Managers' information needs and the impact upon their decision making. The information needs identified as a result of the research have shown them to have become business orientated with financial targets as a key measure and clinical performance (outcomes) increasingly being seen as the other key factor as an indication of success for the organisation. The hypothesis (Null Hypothesis) of the research asserts that it is not possible to link a market-led healthcare environment, the General Managers' information needs, their attitudes and behaviour towards information, and patient empowerment in such a way as to develop a model of information needs that is common across the Purchaser, Provider and the NHS Executive organisations. However, the research has developed as a first step, a series of outline models of information needs that will lead to a more complex and common model of information needs across the General Managers' organisational groups that will allow, when assessed against key success factors, a judgmental view of the ability of both the General Managers and their organisations to deliver their aims and objectives.

Stroke is a medical event that has serious consequences for the survivor, his family, and society. Stroke is the third leading cause of death in the United States, and two-thirds of stroke survivors are permanently disabled with one-half of these survivors considered severely impaired. The economic burden from stroke is estimated to be $30 billion annually in health care costs and lost productivity.This study described the needs of 35 caregivers of stroke survivors within 24 hours of discharge to home from an acute care "safety net" teaching hospital in Indiana and how well those needs were met. Two subscales, (a) the Need for Information and (b) Patient Care Needs, of the Home Caregiver Need Survey were used. The theoretical framework for this study was the Neuman Systems Model.The stroke survivors were over 50 years old (77.1 %), half were African-American and half were Caucasian, and about a quarter were minimally impaired (25.8%) and less than a quarter were very impaired (22.9%) cognitively and/or physically upon discharge to home. Their caregivers were their children (48.6%) or spouses (28.6%), half of whom worked full-time outside the home. Most (85.7%) caregivers were female, in good health, and with no experience (81.8%) in caring for patients at home.

This study aimed to explore and develop a fresh understanding of the community service needs of persons with spinal injuries. Different concepts of need were explored, which required multiple methodologies including a review of government policies and legislation, analysis of demographic, injury related and service usage data obtained from a survey, and analysis of interviews with persons with spinal injuries and their carers. Normative need was interpreted by government and service providers in terms of functional impairment and economic disadvantage, which did not reflect the individual needs of persons with spinal injuries. Neither were services for persons with spinal injuries provided to individuals according to the criteria of their felt or expressed need for services, their perceptions of the intrinsic importance of services, nor on the basis of redressing comparative inequities in service distribution. Persons with spinal injuries’ need for community services could only be understood on the basis of the contribution services made to the realisation of their plans of life, that is, to be ordinary. In order to parallel plans of ordinariness, services need to be offered in partnership with, or under the control of the person with spinal injuries, foster an essential relationship between the provider and the person with spinal injuries, and not inappropriately assess and judge the life plans of persons with spinal injuries based upon providers’ perceptions of difference.

The present study had an overarching objective of seeking information to better understand the service needs of children entering foster care who are universally enrolled into behavioral health services at the time of their home removal. The first specific aim was to examine how various case attributes (i.e., aspects of a child’s background, personal and familial characteristics, and CPS involvement) related to behavioral health needs at entry, represented by global functioning ratings and service intensity scores. A second specific aim was to explore the existence of natural groupings of foster youth to examine if meaningful groupings occur in relation to demographic and risk and factors. Finally, this study sought to evaluate service needs of subgroups of foster children who are typically under-referred for services. Data were collected from records of 327 school-age youth who were automatically enrolled into treatment from one community-based outpatient behavioral health provider at the time of home removal. The results of this study showed that the average school-age child entering foster care is performing in the at-risk range of global functioning (mean CGAS score of 69.23) and requires at least short-term behavioral health intervention services and/or recovery monitoring (mean CASII Composite Score of 13.1). With respect to severity of impaired functioning, the results indicated that racial background, medical history, speech problems, school grade level (e.g., <1 year, K-3,...9-12), psychological stability, and reasons for removal significantly contributed to scores of global functioning. With regard to the intensity of service needs, the findings suggested gender, historical involvement with child welfare services, foster placement, simultaneous removal of all children, history of sexualized behaviors, and global functioning predicted level of care composite scores. Subgroups of foster youth that are typically under-referred and/or under-utilize services (i.e., preschool age children, children who did not experience physical or sexual abuse, and ethnic minorities) generally did not differ in terms of functionality or service intensity ratings from their counterpart groups; the only exception being that Caucasian/Non-Hispanic youth demonstrated lower average ratings on global functioning than children who identify with other racial backgrounds. The data showed inadequate fit for examining latent classes among youth entering foster care.

This thesis presents a critical ethnography of decision making with respect to the assessment of health care needs in the UK health system. Theories of need, justice and rights are reviewed in relation to structural changes to the National Health Service, together with the different theoretical approaches underpinning health policy based on human needs. The research on which this thesis is based focuses on a case study of an independent review of renal services in London, concentrating on the needs assessment work of the review group set up by the government and the decision making debates this review group engaged in. The methods used are based on a participatory, critical ethnography. The review process is evaluated critically by relating the technical knowledge produced by the group to a theoretical framework for assessing needs and by using a Habermasian perspective to investigate the ways in which the language of need is used to legitimise the agendas of various vested interests. This work is linked with an analysis of quasi-markets in the health service to explore the capacity that the technical discourses of markets and contracting have for reinforcing the ideological distortions identified in the analysis of the group's debates concerning need. Finally, by linking an analysis based on a case study of renal services to theoretical understandings of health care needs and health policy, a general critique of the UK health system is constructed.

Introduction: Nearly 60,000 children receive emergency services each year due to accidental medication ingestion. Since families regularly receive verbal anticipatory guidance about locking up medications during pediatric well child visits, primary care clinics are an opportune place to distribute lock boxes to families who need them. ETSU Pediatrics is an interprofessional outpatient clinic that serves a population with many social, medical, and psychological needs. ETSU Pediatrics partnered with a local non-profit organization to provide medication lock boxes free to charge to families who need one. However, it was not known whether more families would endorse a need for a lock box in response to a systematic effort to screen for this need. We hypothesized that more medication lock boxes would be administered if families were given a written screener asking explicitly about their need for a lock box. Method:ETSU Pediatrics added the question “To increase child safety, is your household in need of a lockbox for medications?” to an existing social determinants of health screener that is given annually to each patient during their well child visit. Social work students imbedded in the clinic gave out lock boxes to families who endorsed a need. Frequency counts of the number of medication lock boxes given out were compared 6 months before and after the initiation of the screener. Results: Out of the 2,018 well child visits that occurred in the 6 months after screener initiation, 5.5% (111) of families endorsed a need for a medication lock box. Of those 106 were given a lock box. This was a substantial increase in demand for lock boxes compared to the 6 months prior to screener initiation in which only 16 lock boxes were given out. Conclusions: Systematic screening for medication lock box need resulted in more lock boxes being administered to families over a 6-month period, compared to care as usual. An embedded interprofessional student team helped facilitate this process. These results show promise for other healthcare organizations aimed at reducing accidental medication ingestions in children.

There has been a phenomenal growth in distance education since it's early days as correspondence study. Computers and the tools that go along with them, help to bridge the gap between physically separated people. Computers allow for resource sharing, decentralization, and limitless opportunities for instantaneous communication and transfer of information around the world. A variation of the traditional distance learner is the on-line learner. The on-line learner is a student on or near campus who chooses to take a class on-line versus the traditional classroom method. Questions are generating regarding why these students choose to take these on-line courses when they have they have access to the physical classroom. Since these students are familiar with an on-line setting, questions are also generating regarding the availability of on-line services. The participants included in this study were on-line learners enrolled in 13 totally on-line classes offered at a large, public, research university located in a mid-Atlantic state. The purpose of this study was to examine what motivates students to take on-line courses and to assess their interest in receiving other academic and student services on-line. Data were collected through a survey designed specifically for this study.

Care ethics is primarily about responding to needs. Yet, surprisingly, attempts to apply the ethics of care in the domain of law have paid almost no attention to the concept of legal needs. This study fills that gap by systematically defining legal needs. It does this by revising current understandings of legal need through a unified conceptual framework for the philosophy of needs and a comparative analysis of legal action, and its major alternatives in dispute resolution and prevention. The conception of legal need that results is both more sensitive to preventative functions of the law and opens the door to a much wider range of policy options beyond legal aid. Legal needs are found to be a special case of institutional needs, i.e. needs that cannot be satisfied without an institution. I argue that the existence of institutional needs means institutions, rather than any particular actor within them, can be caregivers, but not all conceptions of the ethics of care are compatible with this kind of need. Joan Tronto’s conception of care is found to be the most accommodating and is used as a framework for a series of policy recommendations to move us towards a caring legal system.

A longitudinal study of 43 families referred to a Clinical Genetic Service was undertaken to ascertain the needs and expectations of the service, from the client's perspective. Previous studies have mainly focussed on changes in knowledge or reproductive patterns as outcomes for genetic counselling. Clients were interviewed before contact with the genetic service, after the consultation, and six months later. At each interview, psychological questionnaires to assess Need for Cognitive Closure (Webster & Kruglanski, 1994), anxiety (Spielberger et aI, 1970) and the impact of the genetic condition on the family (Horowitz et aI, 1979) were used. Statistical analyses of these tools revealed that the Need for Closure is a stable entity, and that genetic counselling does not significantly influence anxiety in the client. A grounded theory approach was used to analyse the data, and the theory presented includes the client's need for certainty, the client's prior lay knowledge, and assimilation of scientific information into the family knowledge base. Clients had little prior knowledge of genetics, but had constructed lay explanations for the inheritance of the condition in their family. Scientific information was tested against the family history for validity, and where there was apparent conflict, the scientific explanation was sometimes rejected. Results of this study indicate that the majority of families are not seeking information for reproductive decision-making, but as a; means of obtaining certainty, via a diagnosis, a prognosis, or a test result. Clients defined the most important outcomes as alterations in their psychological ability to deal with the situation in their family. In this cohort, certainty was seen as helpful in enabling the client to cope, and failure to obtain certainty influenced the outcome adversely. Implications for clinical practice include the need to address the client's need for certainty, to explore the family's lay explanations as part of the genetic counselling process, and to relate explanations directly to the family experience.

There is an on-going debate among health professionals, policy-makers and politicians, as to the optimal way of delivering sexual health services to young people. There is as yet, no consensus on their best patterns of organisation or configuration. This study uses qualitative and quantitative research methods, to explore both the views of young people accessing sexual health services, expressed through in-depth interview, and variations in client satisfaction with different characteristics of service delivery, expressed through completion of a questionnaire. The key research questions are:  How does young people’s satisfaction with sexual health services vary with the age-dedication of the service; that is, whether it serves young people only, or all ages?  How does young people’s satisfaction with sexual health services vary with the integration of the service; that is, whether family planning and genito-urinary services are offered separately, or together?  How does young people’s satisfaction with sexual health services vary with the location of the service; that is, in community or hospital based services? In the qualitative component, in-depth interviews were conducted with 25 young people recruited from a purposively selected sample of young people’s services. In the survey, a total sample of 1166 was achieved. Of these, 36% were attending an integrated contraceptive and STI service and 64% were attending a more traditional ‘separate’ service. 48% attended a service dedicated to young people and 52% an all-age service. 50% attended a hospital-based service and 50% a service located in the community. Of the total sample, 22% were male and 78% female. The analysis has been done not on a comparison of services in their entirety, but on a comparison of key features of their organisation, that is, whether they are provided separately as contraceptive and STI sessions or services, or whether these aspects of sexual health provision are integrated in sessions or services (integration); on whether they are run exclusively for young people or for all ages (dedication); and on whether they are located in the community or in a hospital setting (location). Recommendations are made for future service development and delivery and implications for policy are discussed.

Distance learning is a fast-growing method of delivery in higher education. The literature about distance learning is rich with information about learning and course design. However, there is little information about the non-academic support services that universities can and should provide for distance students. This study addresses that gap in the literature by exploring the non-academic needs of distance learners. Students and faculty members involved with distance learning at four-year institutions in the Fall of 2000 were asked to identify what, if any, needs distance learners have outside the classroom. Specifically, an original web-based survey was designed to identify 34 possible non-academic needs. Respondents were asked to rate the importance of those needs in the success of distance learners, the accessibility of services to meet those needs, and when the needs present themselves to distance learners. Findings indicate that the non-academic needs of distance learners are very similar to those of campus-based students. The most important needs are basic: information about the institution and program, a way to purchase books, a contact person at the institution, and academic advising. More traditional developmental needs such as leadership development, experience with diversity, and career services were important to respondents pursuing Bachelors degrees. Services to meet the needs are currently not easily accessible. Most needs present themselves throughout a studentâ s academic career. Institutions involved with distance learning should consider addressing basic needs first. If an institution targets a large number of students pursuing Bachelor degrees at a distance, they might also add services to meet more traditional developmental needs.

Breast milk is acknowledged and recommended as the best food for babies by the World Health Organization. However, globally the proportion of mothers who breastfeed is still low and this is particularly the case in Thailand. The aim of this study is to improve women’s ability to breastfeed. A two phase study was designed. Phase I was to better understand women’s experiences and breastfeeding support needs, which formed the development of the ‘Breastfeeding Support Package’. The package, which consisted of the tool and leaflets, was to be administered by the nurses with the postpartum women. Phase II aimed to explore mother’s and nurses’ experiences following the use of the package. A pragmatic qualitative approach was used throughout. A government hospital in Bangkok, Thailand, was the setting for this research. Purposive sampling was used to recruit the participants for both phases of the study. In Phase I, semi-structured interviews of 17 mothers who visited the Family Planning Clinical, post-delivery, were undertaken. Thematic analysis was used to examine the data. For phase II, three groups of participants were involved: five participants from phase 1, five postpartum women and five nurses working on the postpartum ward. Telephone interviews were used to assess face validity to the tool while semi-structured interviews were used to investigate mothers’ and nurses’ experiences regarding the use of the package. Three themes that contributed to women’s experience of breastfeeding were ‘knowledge and attitude towards breastfeeding’, ‘practicality’ and ‘support’. Three themes that contributed to ‘women’s needs of breastfeeding support’ were ‘knowledge’, ‘attitude’, and ‘practice’. The use of the package could open a ‘platform to communication’ regarding women’s needs and act as a ‘platform to support’ for nurses. The postpartum women were able to better communicate their needs regarding breastfeeding support with the nurses, as well as receiving the support they required. The packaged worked well with present conditions and environment at the postpartum ward of the hospital.

My research takes a social work perspective to investigate the concept of the child with ‘additional needs'. This concept arose out of the Labour Government's programme ‘Every Child Matters' (HM Government, 2003) which proposed that children's needs for support should be picked up at an earlier point by an integrated Children's Services consisting of social care, health and education. This would stop them from ‘falling through the net' of services. A focus on ‘additional needs' should mean that children in distress are helped at an early stage before problems became critical, improving the ‘well-being' of children and their families. The research has traced the cases of twelve children with ‘additional needs' through their contacts with Children's Services, using an interactionist methodology to interrogate the meaning-making between respondents. Further, following Hacking (2004), a Foucauldian approach to discourse allowed me investigate the discourses which shape formal diagnosis and categorization. Focusing on the ways that the child is positioned and perceived has allowed me to address the question of whose ‘need' is prioritized when the child enters the professional gaze. In doing so it has examined the role of formal and informal labels in constructing the child, the emotional content that goes into creating the ‘meaning-labels' of the child, and the ways that failures in knowing about the child affect the ways that a child becomes pictured. It concludes that in the shifting practices that make up Children's Services, the child with additional needs can become lost in the complex interaction between adult needs and emotions. The informal ‘meaning-labels' which arise out of this complexity often identify the child as carrying a ‘spoiled identity'. This can be carried through into practice with the child, including the processes of formal diagnosis and categorization. Adult emotions need to be managed better if children are to get fitting and timely help to allow them to thrive.
