Der Beitrag stellt das Entwicklungsprojekt ‹Reuchlin digital› (RD) am Museum Johannes Reuchlin (MJR) in Pforzheim vor, welches im Schnittfeld von kultureller Bildung (in schulischen und ausserschulischen Kontexten), Medienpädagogik und der Lehramtsausbildung verortet ist. Johannes Reuchlin (Humanist und Zeitgenosse der Medienrevolution des Buchdrucks) setzte sich für Toleranz und die Erkundung alles Fremden ein. Seine Botschaften stehen leitmotivisch über dem Projekt. RD ist als praxisbezogenes Entwicklungs- und Modellprojekt mit evaluativen Elementen angelegt. Der Schwerpunkt des Beitrags soll auf der Darstellung theoretisch-konzeptioneller Überlegungen sowie wichtiger Zwischenergebnisse liegen. RD wurde vom Kulturamt Pforzheim initiiert (Förderpartner: Innovationsfonds BW, Arbeitsstelle literarische Museen BW) mit Kooperationspartnern verschiedener Institutionen (PH Ludwigsburg, Medien- und Filmgesellschaft BW, Schulen verschiedener Schularten). Ein zentrales Anliegen von RD ist es, durch die aktiv-produktive, kreative und kritisch-reflexive Nutzung digitaler Medien und insbesondere durch die Vermittlung visueller, auditiver und audiovisueller Ausdrucksformen neue Lernwege zu eröffnen und einen ausserschulischen Lernort unter Einbezug kultureller, ästhetischer und medialer Aspekte handlungsorientiert zu erschliessen.

Brain health may be threatened by aneurysm ruptures, and early recognition of these vascular malformations allows for neuroradiological intervention. Neurointerventional procedures are usually performed with femoral artery access. In patients with severe anatomical complexity of the supra-aortic vessels, however, treatment by this approach could be hindered or impossible. Flow-diverter stent deployment is an effective and safe treatment for large, wide necked intracranial aneurysms, but it requires a complete and firm stability of the coaxial system to achieve a correct and precise deployment of the device. We present the first reported Italian case of a patient with an intracranial aneurysm which was treated with Flow-diverter stent (DERIVO®; AcandisGmbH & Co. KG; Pforzheim; Germany) by direct common carotid artery puncture due to severe tortuosity of supra-aortic trunks.

Im Beitrag entwickeln wir einen kritischen Blick auf die Geographie der Wahlergebnisse der Alternative für Deutschland (AfD) bei den Bundestagswahlen 2017. Wir hinterfragen Erklärungsmuster, die in einem starren Stadt-Land-Gegensatz verhaftet bleiben und die komplexe Prozesshaftigkeit der Urbanisierung ignorieren. Dagegen gehen wir mit Henri Lefebvre und Theodor W. Adorno vom Urbanen und Ruralen als sozialen Verhältnissen aus, die sich im übergeordneten Prozess der Urbanisierung in dialektischer Weise scheiden sowie räumlich im Spannungsverhältnis von Zentrum und Peripherie materialisieren. Beispielhaft illustrieren wir diesen Prozess in der Diskussion von drei unterschiedlichen Orten, an denen die AfD bei den Bundestagswahlen besonders erfolgreich war: dem Landkreis Vorpommern-Greifswald als Fall einer umfassenden Peripherisierung, dem Quartier Pforzheim-Haidach als peripheres Zentrum und dem Stadtteil Mannheim-Schönau als zentrale Peripherie. Der Beitrag versucht damit eine räumliche Perspektive auf aktuelle Erfolge des Rechtspopulismus zu entwickeln wie auch Stadt-Land-Verhältnisse konzeptionell neu zu erfassen.

AbstractThe preconception, pregnancy and immediate postpartum and newborn periods are times for mothers and their offspring when they are especially vulnerable to major stressors – those that are sudden and unexpected and those that are chronic. Their adverse effects can transcend generations. Stressors can include natural disasters or political stressors such as conflict and/or migration. Considerable evidence has accumulated demonstrating the adverse effects of natural disasters on pregnancy outcomes and developmental trajectories. However, beyond tracking outcomes, the time has arrived for gathering more information related to identifying mechanisms, predicting risk and developing stress-reducing and resilience-building interventions to improve outcomes. Further, we need to learn how to encapsulate both the quantitative and qualitative information available and share it with communities and authorities to mitigate the adverse developmental effects of future disasters, conflicts and migrations. This article briefly reviews prenatal maternal stress and identifies three contemporary situations (wildfire in Fort McMurray, Alberta, Canada; hurricane Harvey in Houston, USA and transgenerational and migrant stress in Pforzheim, Germany) where current studies are being established by Canadian investigators to test an intervention. The experiences from these efforts are related along with attempts to involve communities in the studies and share the new knowledge to plan for future disasters or tragedies.

(1) Background: To assess the clinical safety and efficacy of a new piezoelectric instrument for orthodontic clean-up; (2) Methods: An in-vitro comparative study on 75 teeth extracted for orthodontic reasons compared the tested method (Treatment 1) with two other procedures: One step finisher and polisher (Inverted cone One gloss Shofu Dental, Kyoto, Japan) (Treatment 2) and twelve-fluted tungsten carbide bur (123-603-00, Dentaurum, Pforzheim, Germany) and Sof-Lex discs Pop-On XT Kit (3M ESPE) (Treatment 3), with n:25 samples in each group. Clinical safety (enamel volume loss) and effectiveness (residual adhesive volume) were assessed using the structured light 3D scanner Atos Compact Scan (GOM GmbH) together with the support of Atos Professional software. The surfaces were scanned three times to assess: (i) the volume of the residual adhesive (RAV) after bracket removal; (ii) the volume of the relative residual adhesive (dAV) after the clean-up procedure; (iii) volume of the enamel loss (EVL); (3) Results: The mean RAV (mm3) was 0.239 ± 0.337; 0.069 ± 0.124, 0.120 ± 0.193 and the mean EVL (mm3) was 0.1870 ± 0.177, 0.187 ± 0.299 and 0.290 ± 0.205, for treatment 1, 2 and 3, respectively. The distribution was asymmetrical between groups in both cases; (4) Conclusions: The tested instrument proved to be effective and safe for post-orthodontic clean-up. With the increasing use of invisible aligners, the possibility of using an ergonomic and fast instrument is of benefit to both patient and practitioner.

ABSTRACT Objective: To compare two groups of subjects at the peak of the pubertal growth period treated with the Functional Mandibular Advancer (FMA; Forestadent, Pforzheim, Germany) appliance using either single-step or stepwise mandibular advancement. Materials and Methods: This study was conducted on 34 Class II division 1 malocclusion subjects at or just before the peak phase of pubertal growth as assessed by hand-wrist radiographs. Subjects were assigned to two groups of mandibular advancement, using matched randomization. Both groups were treated with the FMA. While the mandible was advanced to a super Class I molar relation in the single-step advancement group (SSG), patients in the stepwise mandibular advancement group (SWG) had a 4-mm initial bite advancement and subsequent 2-mm advancements at bimonthly intervals. The material consisted of lateral cephalograms taken before treatment and after 10 months of FMA treatment. Data were analyzed by means paired t-tests and an independent t-test. Results: There were statistically significant changes in SNB, Pg horizontal, ANB, Co-Gn, and Co-Go measurements in both groups (P < .001); these changes were greater in the SWG with the exception of Co-Go (P < .05). While significant differences were found in U1-SN, IMPA, L6 horizontal, overjet, and overbite appraisals in each group (P < .001), these changes were comparable (P > .05). Conclusion: Because of the higher rates of sagittal mandibular skeletal changes, FMA using stepwise advancement of the mandible might be the appliance of choice for treating Class II division 1 malocclusions.

Background and purposeStroke recurrence is high in patients with symptomatic intracranial stenosis despite best medical treatment. Based on evidence from past studies using previous stent generations, elective intracranial stenting (eICS) is considered in a minority of patients. This study aims to report on experience performing eICS with a novel device combination.MethodsWe retrospectively reviewed data from three high volume stroke centers and analyzed patients that were treated with eICS for symptomatic intracranial stenosis using the Acclino (flex) stent and the NeuroSpeed balloon catheter (Acandis GmbH, Pforzheim, Germany). Study endpoints were periprocedural rates of stroke regardless of territory or death at discharge and at the time of follow-up after eICS. Safety evaluation included asymptomatic and symptomatic intracranial hemorrhage, serious adverse events related to the intervention, and evaluation of stent patency at the time of follow-up.ResultsThe median age of patients that met the inclusion criteria (n=76) was 69 years. Target vessels were located in the anterior circulation in 55.3% (42/76) of patients. The periprocedural stroke rate was 6.5% (fatal stroke 2.6%; non-fatal stroke 3.9%) at discharge after eICS. Asymptomatic intracranial hemorrhage was observed in 5.2% (4/76) of patients. Follow-up DSA revealed in-stent restenosis of 25% (15/60), and percutaneous transluminal angioplasty was performed again in 11.6% (7/60) of patients.ConclusionStenting for symptomatic intracranial stenosis with the Acclino (flex)/NeuroSpeed balloon catheter seemed to be safe and reinforces eICS as an endovascular therapy option for secondary stroke prevention. Future studies are warranted to confirm these findings and investigate antithrombotic strategies and in-stent restenosis to minimize periprocedural complications and guarantee long term stent patency.

Abstract Background It is unknown whether technological advancement of stent-retriever devices influences typical observational indicators of safety or effectiveness. Methods Observational retrospective study of APERIO® (AP) vs. new generation APERIO® Hybrid (APH) (Acandis®, Pforzheim, Germany) stent-retriever device (01/2019–09/2020) for mechanical thrombectomy (MT) in large vessel occlusion (LVO) stroke. Primary effectiveness endpoint was successful recanalization eTICI (expanded Thrombolysis In Cerebral Ischemia) ≥ 2b67, primary safety endpoint was occurrence of hemorrhagic complications after MT. Secondary outcome measures were time from groin puncture to first pass and successful reperfusion, and the total number of passes needed to achieve the final recanalization result. Results A total of 298 patients with LVO stroke who were treated by MT matched the inclusion criteria: 148 patients (49.7%) treated with AP vs. 150 patients (50.3%) treated with new generation APH. Successful recanalization was not statistically different between both groups: 75.7% for AP vs. 79.3% for APH; p = 0.450. Postinterventional hemorrhagic complications and particularly subarachnoid hemorrhage as the entity possibly associated with stent-retriever device type was significantly less frequent in the group treated with the APH: 29.7% for AP and 16.0% for APH; p = 0.005; however, rates of symptomatic hemorrhage with clinical deterioration and in domo mortality were not statistically different. Neither the median number of stent-retriever passages needed to achieve final recanalization, time from groin puncture to first pass, time from groin puncture to final recanalization nor the number of cases in which successful recanalization could only be achieved by using a different stent-retriever as bail-out device differed between both groups. Conclusion In the specific example of the APERIO® stent-retriever device, we observed that further technological developments of the new generation device were not associated with disadvantages with respect to typical observational indicators of safety or effectiveness.

Abstract Background Endoscopic sphincterotomy (EST) carries several risks (e. g., pancreatitis and bleeding). The risk of bleeding is increased in patients with a compromised coagulation system, often due to antithromboembolic therapy. Recent guidelines caution to perform endoscopic procedures that carry a high risk of bleeding in these patients. However, data to support current recommendations are scarce, and EST frequently has to be performed as an emergency procedure. Therefore, it was the aim of our retrospective study to evaluate the rate of procedural bleeding in patients undergoing EST in our endoscopy unit while on antithromboembolic therapy. Methods Between March 2005 and August 2015, 1798 consecutive patients underwent EST at HELIOS Hospital in Pforzheim, Germany. Concomitant therapy with anticoagulants and/or antiplatelet agents was noted, and bleeding following sphincterotomy was recorded. Results We observed 54 bleeding events in 1482 patients (3.6 %) without and 20 events in 316 patients (6.3 %) with antithromboembolic therapy. Bleeding was recorded in 7 out of 123 patients (5.7 %) taking aspirin, in one out of 34 patients (2.9 %) taking clopidogrel, and in 12 out of 209 patients under heparin (5.7 %). Compared to controls, no statistically significant increase in the bleeding rate was seen. However, we observed an association between a lower physical health score and increased bleeding rate. If precut was necessary for biliary tract access, the bleeding rate increased significantly (p < 0.01). Conclusion Bleeding following EST is neither increased in patients taking clopidogrel and/or aspirin or heparin and rarely requires transfusion of packed red blood cells nor does it lead to an increased mortality. However, bleeding following EST seems to occur more frequently in patients with a compromised health status or following precut of the papilla.

Aim: The objective of this study was to measure and compare the in vitro performance of active and passive self-ligating brackets in orthodontic rotation by means of an electromagnetic field simulation. Methods: The study sample consisted of 32 mandibular right central incisor brackets (n=8), slot 0.022", that were divided into the following groups: 1) BioQuick® (Forestadent, Pforzheim, Germany) active brackets; 2) In-Ovation®R (Dentsply-GAC, Central Islip, New York, USA) active brackets; 3) Damon-Q® (Ormco, Orange, California, USA) passive brackets, and 4) Smartclip® (3M, Monrovia, California, USA) passive brackets. The orthodontic wire used was CuNiTi round section 0.016", thermoactivated at 35o C (ORMCO-Orthodontics Glendora, California, USA). The experiment was performed in a simulator machine, composed of two fixed lateral axes and a movable central axis, which simulated the dental rotation. Qualitative analysis (n = 4) was performed using SEM. After the descriptive and exploratory analysis, the yield and grade data were submitted to one-way analysis of variance (ANOVA) followed by the Tukey test, considering the level of significance of 5%. Results: In-Ovation®R brackets showed significantly higher yield than BioQuick® and Damon-Q®. Damon-Q® brackets presented a significantly lower mean value than In-Ovation R and Smartclip®. BioQuick® did not differ significantly from Damon-Q® and Smartclip®. In-Ovation®R did not differ significantly from Smartclip®. Conclusion: In the present study, it was observed that there is a difference in rotational control in the different self-ligating brackets tested being the best rotational control was the In-Ovation R® group (active), followed in descending order by the groups Smartclip® (passive), Bioquick® (active) and Damon Q® (passive).

Context: Whole-body vibration (WBV) has shown many positive effects on the human body in rehabilitation and clinical settings in which vibration has been used to elicit muscle contractions in spastic and paretic muscles. Objective: The purpose of this study was to investigate whether WBV exercise (WBVe) differently modulates the cortical activity associated with motor and prefrontal function based on its frequency. Methods: A total of 18 healthy male adults (mean age: 25.3 [2.4] y) participated in this study and performed WBVe (Galileo Advanced plus; Novotec Medical, Pforzheim, Germany) under 3 different vibration frequency conditions (4-mm amplitude with 10-, 20-, and 27-Hz frequencies) and a control condition (0-mm amplitude with 0-Hz frequency). Each condition consisted of 2 alternating tasks (squatting and standing) every 30 seconds for 5 repetitions. All subjects performed the 4 conditions in a randomized order. Main Outcome Measure: Cortical activation during WBVe was measured by relative changes in oxygenated hemoglobin concentration over the primary motor cortex, premotor cortex, supplementary motor area, and prefrontal and somatosensory cortices using functional near-infrared spectroscopy. Results: Oxygenated hemoglobin concentration was higher during the 27-Hz vibration condition than the control and 10-Hz vibration conditions. Specifically, these changes were pronounced in the bilateral primary motor cortex (P < .05) and right prefrontal cortex (P < .05). In contrast, no significant changes in oxygenated hemoglobin concentration were observed in any of the cortical areas during the 10-Hz vibration condition compared with the control condition. Conclusion: This study provides evidence that the motor network and prefrontal cortical areas of healthy adult males can be activated by 27-Hz WBVe. However, WBVe at lower frequencies did not induce significant changes in cortical activation.

The aim was to identify the influence of conventional polymeric resin based cement (RC), hybrid polymer modified glass ionomer (RMGIC) and Zinc phosphate cement (ZPC) on the pull out strength of the customized zirconia post in premolars. Access cavity and root canals were performed in sixty premolar teeth with the standardized crown down technique (ProTaper Universal, Dentsply). Post space impressions were scanned, and the pre-sintered Zenostar Zr Translucent blanks (Weiland Dental, Pforzheim) were milled with the Opera-system to form the post. All prepared specimens were divided equally in three groups based on the cement type employed for luting as follows: group A: ZPC; group B (GC Fuji PLUS Capsule): RMGIC; group C (and RC (3M RelyX ARC). Ten specimens in each group were thermocycled (TC) at 5 and 55 °C in distilled water baths (40,000 cycles). Pull out bond strength was assessed using a universal testing machine at 0.5 mm/min. The means and standard deviations were compared using ANOVA and Tukey Kramer multiple comparisons tests. A significant difference among the cement groups as well as between TC and non-thermocycled (NTC) groups (p < 0.05) was observed. The highest tensile stress was demonstrated among group C (Resin, 69.89 ± 4.81 (NTC), 64.06 ± 4.36 (TC)) with the least in group A, (zinc phosphate, 43.66 ± 5.02 (NTC), 37.70 ± 5.10 (TC)) for both groups. Group A presented with 100% adhesive bond failures, followed by 80% in group C and 70% in group B, respectively. A similar outcome was observed in the TC group for the cement; however, unlike the NTC group, the TC group showed more cohesive failures compared to the NTC mixed failure. Dual cure polymer based cement demonstrated higher bond strength and efficient adhesive bonding of the customized Zr post with root dentine compared to zinc phosphate (non-polymeric) and RMGIC (hybrid polymer). Thermocycling compromised Zr post adhesive bonding to root dentin.
