This would dramatically reduce drug costs, with positive consequences not only for treatment, but also on the social level. Drugs would become much more affordable, hence curing and improving the lives of more people. This hypothetical, best-case scenario should point scientists to move towards the direction of attempting to make scientific endeavors for the social benefit. Systems biology is a discipline that does indeed move towards that direction. Of course, as most things in life, the use of systems biology could be used in a dual manner, that is, for social benefit or profit.

Microbial Fuel Cells (MFC) are the main topic of this chapter. Different types of electrochemical devices are presented and their typical power output is compared with other energy sources, providing a framework for the uses and applications of MFC technology. Following an historical approach of how this technology came to be, a more detailed description of some aspects of a typical microbial fuel cell is then brought forward. The energy harvesting concept, its use on low power wireless systems and maximum power point tracking (MPPT) techniques are presented and described. Wastewater treatment plants are a kind of infrastructure where this technology could be applied with a major success to power wireless sensing networks. An experimental setup, develop to improve the use of MFC in waste water treatment plants is presented. This chapter also provides a review on research trends for microbial fuel cells and maximum power point tracking algorithms, therefore, pointing current researches on this technology.

This chapter introduces the use of middleware tools in applications for healthcare and social services, focusing on solutions designed for Interactive Digital Television (IDTV), and discusses its implications to modern roles for easing patient and caretaker relations. The authors explore the capacities of such solutions to assist the patients in their personal needs, optimizing the time and tasks of the caretaker. They discuss the limits of the use of Internet in satisfying the needs of communication of elderly and other patients, and then they propose new roles for caretaking based on this new reality. The authors also present an architecture that allows the remote use and control of electronic devices via IDTV set-top boxes for the middleware Ginga. They show applications based on this architecture that promote healthcare and social services, as a set of infrared lights coupled to a hat that can be used as input for impaired patients, and how an accelerometer can be used with IDTV applications for treatment and entertainment. Before concluding, the authors point out research directions on the topics discussed in this chapter.

We have previously seen how the Dirac equation for one particle requires some rather special consideration and interpretation in order to arrive at a form that is able to treat electrons and positrons on an equal footing. These problems persist also when we go to systems with more than one electron. One might think that the extension to several electrons should not introduce dramatic changes. After all, we noted that even the one-electron problem must be viewed as a many-electron (and -positron) system in order to arrive at a consistent description. The problem with introducing more electrons is that electron–electron interactions that were previously small—for the one-electron case typically arising from vacuum polarization and self-interaction—now occur to the same order as the kinetic energy and the interaction with the potential. So while a perturbative approach such as QED can use the solutions of the one-electron Dirac equations as a very good starting approximation to a more accurate description of the full system, the same would not work for a system with more electrons because it would mean neglecting interactions of the same magnitude as the zeroth-order energy. For applications to quantum chemistry, the treatment of the entire electron–electron interaction as a perturbation would be hopelessly impractical, as it is even in manyelectron relativistic atomic structure calculations. The technique for dealing with this problem is well known from nonrelativistic calculations on many-electron systems. One-particle basis sets are developed by considering the behavior of the single electron in the mean field of all the other electrons, and while this neglects a smaller part of the interaction energy, the electron correlation, it provides a suitable starting point for further variational or perturbational treatments to recover more of the electron–electron interaction. It is only natural to pursue the same approach for the relativistic case. Thus one may proceed to construct a mean-field method that can be used as a basis for the perturbation theory of QED.

Paediatric OCD is the disorder, in child psychiatry, whose clinical picture most closely resembles its adult counterpart. Despite a relative diversity, the symptom pool is remarkably finite, and very similar to that seen in older individuals. Prevalence, comorbidity, and response to behavioural and drug treatment also appear similar across the lifespan. For tic disorders, there is continuity between child and adult presentations, but the disease is much more prone to resolve spontaneously, or to be less disruptive in adulthood. Both OCD and tics occur more often in males than in females, and are likely to be linked to an array of neurobiological abnormalities, many of which remain to be understood. Invaluable benefits can now be obtained from available behavioural and pharmacological treatments, but complete remission remains uncertain and long-term management may be required. Thus, the treatment of OCD and tics in children and adolescents remains a clinical challenge. It requires careful assessment of the targeted symptoms and, in many cases, comorbidity; attention to the quality of the child's functioning at home and with peers; use of specific CBT interventions, which are not readily available (or accessible) in all communities; patience and caution in the choice and adjustment of medication; and vigilance in watching potential side effects. Given the possible chronicity of OCD and/or tic disorders, and their changing patterns in severity and impact over the childhood and adolescent years, optimal treatment generally requires a long-term ongoing relationship with the child and family. Current conceptualizations of OCD and tic disorders have been shaped by advances in systems neuroscience and functional in vivo neuroimaging. Continued success in these areas should lead to the targetting of specific brain circuits for more intensive research. This should include testing novel pharmacological agents, tracking treatment response using neuroimaging techniques, and possibly investigating circuit-based therapies using deep-brain stimulation for refractory cases. The identification of the PANDAS subgroup of patients, with an abrupt onset and dramatic exacerbations, certainly brings new insights into the pathophysiology of OCD and tic disorders, and may lead to new assessment and treatment strategies. The increasing evidence for susceptibility genes in OCD and tic disorders will also doubtless point to new therapeutic directions. Furthermore, it is likely that many of the empirical findings used in research on paediatric OCD and tic disorders will be relevant to a better understanding of both normal development, and other disorders of childhood onset.

These payment practices are undergoing scrutiny and many changes have already begun to transform treatment policies. Recognizing a turning point for the engagement of third-party payers and an impetus for progressive payment reform, Shatterproof, a national nonprofit organization, partnered with health insurers to advance the substance use disorder treatment system in the United States and developed eight principles of care. This chapter describes this work and changes to payment models to better support patient needs, community and public health, and the interests of private insurers and health care providers.

Since several years scientists are carrying on research about innovative systems, based on robotics, mechatronics devices and IT tools – especially the graphical ones – to support patients in rehabilitation programs for rescuing from various brain damages due to adverse events as stroke. Training sessions with a combination of robot and virtual reality (VR) lead to better rehabilitation outcomes than using only a robot and evidence from the field proved the importance of the use of VR interfaces in rehabilitation. The main aim of such a kind of environments is to monitor, motivate and drive the patient during the rehabilitation sessions. These systems mainly provide motor guidance and multimedia communication channels also measuring patients' performance and other observable variables. The approach implemented is usually based on gaming, where the patient has the key role to perform certain tasks or movements for controlling the game in the correct way and exercise the injured part. According to daily experience from physiotherapists, different aspects related to the physical and self-perceived patients' state revealed to have a fundamental role in influencing the rehabilitation session. Indeed, the treatment result depends not only on motor skills but also on patient's personal behavior and feelings that are not directly investigable, observable and measurable from outside. In other words this kind of elements can only be assessed by subjective measurements (as questions, interviews, narratives) revealing the point of view of the patient. Moreover, the emotional state has implications at different levels: on one hand, it is important to evaluate if the single rehabilitation session affects the emotional state of the patient, on the other, if the performance was influenced by this state. Some answers of a questionnaire administrated to post-stroke patients in a previous study underlined also the need for the patient to focus his/her attention on the trained body portion and the relevance of a visual feedback on movements to increase self-awareness on the action performed, avoiding any possible distraction derived from other kind of tasks and related visual/auditory stimuli. Patient-centered models of care are actually becoming increasingly common among rehabilitation clinical settings. They help to focus the therapy on improving the treatment of those deficiencies that most influence the quality of life of the patient. Another important aspect is represented by the relationship with the caregiver that in virtually assisted rehabilitation would not be direct anymore and will need to be mediated without completely loosing natural interaction. According to this patient centered vision, and in order to reduce possible side effects related to semi-automatic rehabilitation systems, it has been studied and developed a system which has not the aim of merely entertain the patient but to focalize the rehabilitation on him/her as a proactive character aware of what is happening and of the quality of the work performed.

Information provided by these tools was generally collected via paper records. These records were then physically moved (generally in a very inefficient way) from one place to another in order to exchange information. In many situations, these records represented the most critical element in the health system due to misunderstanding, errors, and loss of information caused by their use. Nevertheless, its main drawback was the difficulty, or even the impossibility, to retrieve information from paper records when there were needs in the future and/or outside the hospital. This situation was largely inefficient, especially from the patient’s point of view. Indeed, he or she had to spend a lot of time moving to and from the hospital and supporting a great deal of stress to supply time and again the same information (often in an incomplete, vague, or erroneous manner), and eventually suffering for errors and/or unavailability of information previously stored in other records.
