AbstractThis chapter provides an overview of Taiwan’s National Health Insurance (NHI) system. In 1995, major social insurance programmes, such as labour insurance, government employee health insurance and farmers’ insurance, were merged and enlarged to form the NHI to deliver universal health coverage. Since its inception, the payment system of the NHI is the fee-for-service method. Moreover, most of the health care is provided by private sectors, and there are no restrictions on patients seeking medical care. Owing to the high medical accessibility, the volume of outpatient services is high, and the National Health Insurance Administration (NHIA) has to develop various measures to maintain its financial stability. Several strategies have been implemented by the NHIA for health equity, and the NHI MediCloud system, the NHI card and ‘My Health Bank’ were provided to ensure patients’ safety and enhance healthcare quality.

AbstractTax-financed public health insurance programs are a newer, but increasingly important form of social policy across developing countries, including India. With the scope of stemming the flow of people into poverty, public health insurance focuses on the vulnerable and provides them an avenue to seek quality health care without incurring exorbitant costs. Although enrollment in the program and its effectiveness in reducing out-of-pocket health expenditures remain low, it is expected that the importance of health insurance will increasingly become recognized everywhere. As the demand for health care increases, the key to its effectiveness could reside in the prioritizing of health as an important policy goal—recognizing health as a citizenship “right,” increasing budgetary allocation for health, improving the quality of health care infrastructure, and putting into place effective regulations to check unscrupulous practices by private health care providers—with the scope of equitable access to universal health care and overall improved health outcomes.

AbstractThrough a case study of the Employees’ State Insurance Act of 1948, this chapter examines the historical evolution of a type of welfare schemes in India that made entitlements conditional on specific forms of employment. Global trends in social policy had influenced debates on a social insurance for Indian workers since the 1920s. Transformations of Indian industry, World War II, the post-war crisis and postcolonial economic planning then created conditions for legislation. Just when the international welfare discourse, Indian contributions included, converged on social welfare as a universal citizen right, the regulatory content of the health insurance scheme devised for India diverged from this normative consensus: “Employees’ State Insurance” remained strictly employment-based but also generated horizons of expectation that continue to inform labour struggles.

This policy brief answers three main questions with respect to the no-user-fee policy adopted across public health centres in Jamaica: (i) Has the policy improved health status among working age adults? (ii) Has the policy influenced labour market dynamics? (iii) Has the policy had differential effects by age groups? Evidence suggests that the policy improved overall health status, as the likelihood of suffering illnesses associated with inability to carry out normal activities decreased by 28.6 percent. The introduction of mandatory health insurance in developing countries is often questioned because of the difficulties in enrolling self-employed workers, who represent a segment of the labor force that is increasing in size. Partial population coverage, coupled with adverse selection effects, raises serious concerns about the financial viability of social health insurance schemes. Despite these difficulties, an increasing number of developing countries have implemented social health insurance schemes with the aim of attaining universal health coverage and improving access to health services.
