Texto completoThe aim of this study is to describe the possible consequences, in terms of mental health and quality of life of patients suffering burns requiring hospitalization. In addition, we attempt to identify some of the mechanisms involved in the appearance of distress among these patients. To do so, a review of the literature was performed and also, demographic data of the participants, nature of the injury and its treatment, medical history and use of toxic substances during the six months after the time of injury, were collected using a semi-structured interview created ad-hoc specifically for this project. In addition, several assessment measurements of personality, anxiety, depression, and posttraumatic stress symptoms, general health status and quality of life were administered. This doctoral thesis confirms the presence of psychopathology secondary to burns and points to subjective variables, such as personality or those related to the creation of traumatic memories, as risk factors. These results may increase understanding of the psychosocial effects due to serious burn injuries on their victims and their relatives, and identify certain variables of vulnerability that could help to prevent more torpid evolution with respect to mental and emotional symptoms.

Texto completoIntroducción: El paciente gran quemado (PGQ) presenta en los días posteriores al traumatismo térmico, una alteración de la coagulación de origen multifactorial. Varios son los estudios realizados sobre coagulación en el PGQ, sin embargo, el Factor XIII (FXIII), encargado de convertir los monómeros de fibrina en un coágulo estable y relacionado con el proceso de cicatrización de las heridas, no se ha estudiado hasta la actualidad. Los déficits de FXIII no se detectan con las pruebas de coagulación habituales, por lo que aunque estas fueran normales el PGQ podría llegar a quirófano en un situación de coagulopatía inadvertida. Dado que el PGQ presenta un proceso de cicatrización extensa, el consumo de este Factor podría estar justificado. Justificación científica: El PGQ se somete a una cirugía con alto riesgo de sangrado para el desbridamiento de las quemaduras, a la cual podría estar llegando en una situación de coagulopatía inadvertida por déficit de FXIII. Este estudio nos permitirá saber si los niveles de Factor XIII en el PGQ se encuentran dentro de los rangos de normalidad en el momento de la intervención quirúrgica y si nuestro paciente se encuentra en riesgo de sufrir una coagulopatía. Hipótesis: El paciente gran quemado sufre un déficit adquirido de Factor XIII. Objetivos: El objetivo principal de este estudio es determinar si el paciente gran quemado presenta un déficit de Factor XIII en el momento del acto quirúrgico. Objetivos secundarios: Describir la cinética del FXIII en los 30 primeros días tras el trauma térmico. Relacionar los niveles de FXIII con el consumo intraoperatorio de hemoderivados así como con la cicatrización. Relacionar el consumo de FXIII con la TBSA. Ver si hay relación entre el MCF del ROTEM y los niveles de FXIII. Describir la cinética de diferentes factores y cofactores implicados en la coagulación, anticoagulación y fibrinolisis, así como diferentes test de coagulación durante los 30 días tras el trauma térmico. Relacionar los parámetros estudiados con la morbimortalidad. Diseño del estudio: Se trata de un estudio piloto observacional, prospectivo unicéntrico, de la cinética del FXIII, así como de otros factores y cofactores de la coagulación del PGQ durante los 30 primeros días tras el trauma térmico en una muestra de 21 pacientes grandes quemados. Resultados: El FXIII presenta una caída significativa de 75.10 puntos porcentuales desde el trauma térmico hasta el preoperatorio y de 87.70 puntos hasta las 24 horas postoperatorias, llegando a la cirugía con una mediana de FXIII antigénico del 32%. La mayor parte de factores sufren un descenso significativo en niveles plasmáticos a las 24 horas del trauma térmico. Conclusiones: El PGQ experimenta una disminución de los niveles plasmáticos de FXIII significativa desde el momento del ingreso hasta 24 horas tras la cirugía, encontrándose en niveles infranormales en el momento de la intervención quirúrgica que no se pudieron detectar mediante otros tests de coagulación. El descenso de la mayoría de los factores a las 24 horas parece estar asociado con la dilución por la fluidoterapia intensiva de la resucitación.
Introduction: In the days after thermal trauma, the patient with severe burn (MBP) injury presents a multifactorial alteration of coagulation. There are several studies carried out on coagulation in MBP, however, Factor XIII (FXIII), responsible for converting fibrin monomers into a stable clot and related to the wound healing process, has not been studied to date . FXIII deficits are not detected with the usual coagulation tests, so even if these were normal, the MBP could reach the operating room in a situation of inadvertent coagulopathy. Given that MBP has an extensive healing process, the consumption of this Factor could be justified. Scientific justification: The PGQ undergoes a surgery with a high risk of bleeding for the debridement of burns, to which it could be arriving in a situation of inadvertent coagulopathy due to FXIII deficiency. This study will allow us to know if the levels of Factor XIII in the MBP are within normal ranges at the time of surgery and if our patient is at risk of coagulopathy. Hypothesis: The large burn patient suffers from an acquired deficiency of Factor XIII. Objectives: The main objective of this study is to determine if the severely burned patient has a Factor XIII deficiency at the time of surgery. Secondary objectives: To describe the kinetics of FXIII in the first 30 days after thermal trauma. Relate the levels of FXIII with the intraoperative consumption of blood products as well as with healing. Relate the consumption of FXIII with the burned surface area. See if there is a relationship between the ROTEM MCF and FXIII levels. Describe the kinetics of different factors and cofactors involved in coagulation, anticoagulation and fibrinolysis, as well as different coagulation tests during the 30 days after thermal trauma. Relate the parameters studied with morbidity and mortality. Study design: This is an observational, prospective single-center pilot study of FXIII kinetics, as well as other factors and cofactors of MBP coagulation during the first 30 days after thermal trauma in a sample of 21 large burn patients. Results: The FXIII shows a significant drop of 75.10 percentage points from the thermal trauma to the preoperative period and of 87.70 points until the 24 postoperative hours, reaching the surgery with a median antigenic FXIII of 32%. Most factors suffer a significant decrease in plasma levels 24 hours after thermal trauma. Conclusions: MBP experiences a significant decrease in plasma levels of FXIII from the time of admission to 24 hours after surgery, being at infranormal levels at the time of surgery that could not be detected by other coagulation tests. The decrease in most factors at 24 hours seems to be associated with dilution by intensive fluid therapy of resuscitation.

Texto completoThe present research project is Crematto and it came from wanting to disprove the common relationship between healthy foods and high spending. The idea of Crematto is practically sell the idea of an artisan ice cream with exotic fruits that have an unusual flavor in an ice cream and, in addition, have high nutritional properties. The product will be offered to the market at a competitive price with a fast and efficient delivery. The income from the Crematto project will be obtained through the online sale of Artisan Ice Creams and the main costs are resources such as glass containers, fruit, and labor. To successfully develop this project, we established as main key partners the suppliers of fruit and glass containers. Crematto business model is aimed at people between 18 and 35 years old with a Socio-Economic Level A and B, with a modern, sophisticated, and adapted lifestyle, who clearly have good eating habits and some preferences for eco-friendly products. The lack of low-sugar exotic fruit desserts and the increasing demand and environmental responsibility of consumers allowed us to find an extremely attractive and interesting segment. At the Concierge, after posting the flavors that would be offered on social media, different followers of Crematto, today called “Cremattolovers,” began to comment and share the publication. Then we established our mission to provide the best option for a healthy exotic fruit ice cream and vision to be a national brand reference recognized for our ice cream with unconventional flavors. The last price determined per jar of healthy ice cream is 15 soles, which is less than that of the competition of any nature; However, in terms of flavors, we are not ahead, since Crematto has only five. After establishing objectives by area, we decided on our generic strategy, which is Differentiation because its main characteristic is that it is an alternative to ice cream with exotic flavors.

Texto completoSeveral inflammatory mediators have been shown to be increased after burn injury. They may be important in burn pathophysiology, contributing to organ dysfunction and sepsis apparition, and they may also predict outcomes. Moreover, they have been involved in pathophysiology of some special processes, such as inhalation injury or wound healing. Consequently, some biomarkers have been described as potential therapeutic targets. Importantly, as therapeutic interventions may also affect biological processes, biomarkers may be a useful tool to guide some treatments and may also explain why some treatments succeed or fail in improving outcomes. Therefore, investigation into biomarkers in severe burn patients is a key feature of translational medicine in this area of knowledge. Our aim was to analyze whether plasma levels of biomarkers involved in the IL33/ST2 axis might help to predict mortality in burn patients. This is the first study to show the prognostic significance of plasma levels of sST2 after burn injury. Indeed, higher plasma concentrations of sST2 were consistently associated with a higher risk of death, even after adjusting for different potential confounding. Moreover, higher levels of sST2 were also observed in burn patients who developed any infectious complication during their stay in the Burns Unit as well as in patients who presented MODS. In conclusion, the results of this study suggest that plasma sST2 levels predict mortality in burn patients and may be useful to help or guide physicians in the bedside decision-making process during patient management.
