Background: Clinical handover between emergency medical services (EMS) and the hospital trauma team can be subject to errors that may negatively affect patient care. Thus far, there has been limited evaluation of the quality of EMS handover. As such, we sought to characterize handover practices from EMS to the trauma team, identify areas for improvement, and determine if there is a need for standardization of current handover practices. Aim Statement: Identify areas for improvement in handover from EMS to the trauma team, specifically examining handover content, structure, and discordances between different team members regarding handover expectations. Measures & Design: Data were prospectively collected over a nine week period by a trained observer at Canada's largest level one trauma centre. A randomized scheduled was used to capture a representative breadth of handovers. Data collected included outcome measures such as duration of handover, structure of the handover, and information shared, process measures such as questions and interruptions from the trauma team, and perceptions of the handover from nurses, trauma team leaders (TTLs) and EMS according to a bidirectional Likert scale. Evaluation/Results: Of 410 trauma team activations, 79 verbal handovers were observed. Information was often missing regarding airway (present 22%), breathing (54%), medications (59%), and allergies (54%). Handover structure lacked consistency beyond the order of identification and mechanism of injury. Only 28% of handovers had a dedicated question and answer period. Of all questions asked, 35% were questioning previously given information. EMS returned to categories of information unprompted in 84% of handovers. The majority of handovers (61%) involved parallel conversations between team members while EMS was speaking, which was associated with a greater number of interrupting questions from the trauma team (3.15 vs. 1.82, p =.001). There was a statistically significant disparity between the self-evaluation of EMS handovers and the perceived quality determined by nurses and trauma team leaders. Discussion/Impact: At our trauma centre, we have identified the need for handover standardization due to poor information content, a lack of structure and active listening, significant information repetition, and discordant expectations between EMS, nurses, and TTLs. We intend to use our results to guide the development of a co-constructed framework integrating the perspectives of all team members on the trauma team.

A handover strategy is required to ensure connection continuity for cellular network users due to user mobility. Handovers on the same frequency (intra-frequency) and handovers on other frequencies (inter-frequencies) are conceivable in the Intra Radio Access Technology network. The network must be able to give the best quality and optimal bandwidth so that consumers can keep their connection. Most cells have more than one bandwidth channel. A test was performed in this study to determine the influence of priority handover on intra-frequency LTE handover on signal strength and throughput. This study is based on actual measurement findings from the deployed 4G LTE network transceiver device, which contains several carriers for each eNodeB. TEMS Pocket, a mobile gadget, was used to collect data for measurements. For analysis purposes, the parameters RSRP, throughput, frequency band, serving cell identification or PCI (Physical Cell Identity), and event handover are employed. In the inter-frequency LTE handover, there are two priority handovers: handovers from lower bands to higher bands (high priority) and handovers from higher bands to lower bands (low priority). When a low-priority handover happens, the RSRP value and throughput increase, while a high-priority handover causes the RSRP value to decrease.

BackgroundStructured handover can reduce communication breakdowns and potential medical errors. In our emergency department (ED) we identified a safety risk due to variation in quality and content of overnight handovers between physicians.AimOur goal was to develop and implement a standardised ED-specific handover tool using quality improvement (QI) methodology. We aimed to increase the proportion of patients having adequate handover information conveyed at overnight shift change from a baseline of 50%–75% in 4 months.MethodsWe used published best practices, stakeholder input and local data to develop a tool customised for intershift ED handovers. Implementation methods included education, cognitive aids, policy change and plan-do-study-act cycles informed by end-user feedback. We monitored progress using direct observation convenience sampling.MeasuresOur outcome measure was proportion of adequate patient handovers (defined as >50% of handover components communicated per patient) per overnight handover session. Tool utilisation characteristics were used for process measurement, and time metrics for balancing measures. We report changes using statistical process control charts and descriptive statistics.ResultsWe observed 49 overnight handover sessions from 2017 to 2019, evaluating handovers of 850 patients. Our improvement target was met in 10 months (median=76.1%) and proportion of adequate handovers continued to improve to median=83.0% at the postimprovement audit. Written communication of handover information increased from a median of 19.2% to 68.7%. Handover time increased by median=31 s per patient. End-users subjectively reported improved communication quality and value for resident education.ConclusionsWe achieved sustained improvements in the amount of information communicated during physician ED handovers using established QI methodologies. Engaging stakeholders in handover tool customisation for local context was an important success factor. We believe this approach can be easily adopted by any ED.

This study aims to analyze the application of the SBAR method of handover management innovation in improving effective communication. The method used is a case study that starts from the process of identification, analysis, problem priority setting, plan of action (POA), implementation and evaluation. Data collection was done by interview, observation and survey methods. The results of the priority problems were analyzed using fishbone analysis. The results showed that the SBAR method of handover management innovation influenced the team leader's ability to carry out handovers. The evaluation results show that 75% of the team leaders can properly fill out handover forms and submit handovers using the SBAR method. In conclusion, the SBAR method of handover management innovation can be used to improve handover capabilities to improve effective communication. Communication failures in information transfer during intraoperative handover can pose a risk to patient safety incidents. One of the factors influencing the success of intraoperative handover is the existence of a structured handover checklist. This study aimed to explain the implementation of a structured intraoperative handover checklist to improve information transfer during intraoperative handovers. The design employed was a literature review. Articles were selected from databases including ScienceDirect, Scopus, PubMed, and Springer Link. The criteria for article selection included publication between 2017 and 2022, full text in English, and the use of keywords such as 'intraoperative handover,' 'handover checklist,' and 'information transfer.' The PRISMA flowchart stages were utilized to filter appropriate articles. The critical assessment of article quality employed The Joanna Briggs Critical Appraisal Tools 2020 and Mix Methods Appraisal Tool 2018. The results of the review of 8 articles indicated that a lack of good coordination during intraoperative handovers often occurs due to the absence of structured guidelines regarding handover content. The application of an intraoperative handover checklist has been shown to increase the completeness of data transfers and reduce the percentage of information omission during the handover process. The implementation of structured intraoperative handovers has a positive influence on improving the quality of communication between surgical teams and reducing incomplete information conveyed. A structured intraoperative handover checklist is needed as a guide to reduce the number of errors in information transfer, thus enhancing patient safety.

ObjectivesTo investigate residents’ and relatives’ views and experiences of handovers in care homes. This paper reports residents’ and relatives’ awareness of handovers, knowledge of and views on handover practices and purpose, and views on handover effectiveness. Outcomes, safety and satisfaction in clinical settings are influenced by shift handovers. Despite this link with quality, residents’ increasing support needs and the provision of 24 hours care in care homes for older people, little is known about handovers in these settings from a resident and visiting relative perspective.SettingFive purposively sampled care homes for older people in South East England.ParticipantsHome managers (n=5), residents (n=16) relatives of residents (n=10) were interviewed; residents (n=15) and their interactions with staff were observed during handover periods. Participation was voluntary and subject to consent. Residents were identified by managers as having mental capacity to take a decision about participation which was then assessed. An ethnographic approach to data collection was taken, preceded by an evidence review.ResultsShift handovers were largely invisible processes to participating residents and relatives, many of whom had given little thought to handover practice, logistics or effectiveness prior to study participation. Their awareness and understanding of handovers, handover practices, and handover purpose and effectiveness varied. There appeared to be an underlying assumption that administrative procedures in care homes would operate without input from residents or relatives. A small number of residents, however, were highly aware of the routine of handovers and the implications of this for the timing of and response to their requests for care or support.ConclusionsThe care home setting and perspectives of the effectiveness of handovers may influence awareness of, knowledge of and levels of interest in involvement in handovers.

AimsThis project was undertaken in the Tyrone and Fermanagh Hospital, an inpatient Psychiatric Unit in Omagh, Northern Ireland. It was recognised that the previous method of medical handover via anonymised WhatsApp messages had several issues including patient data on personal phones, over-reliance on phone apps which have the potential to crash and short messages with limited details were included. The aim of our project was to improve patient safety by improving communication between medical staff members.MethodsThe previous method of medical handover was via text message on messaging platform WhatsApp. A message was sent the morning of handover, to the WhatsApp group.We continued to use a digital platform, but used Microsoft word, and Western Trust email addresses to record and send the handover. At the start of this project, we allowed free space recording, resulting in variation in the handovers.We agreed a minimum number of details to be included to ensure quality of handover, and audited the word documents, to assess the adherence to this.This change, still does not require face to face handover. There have been pros and cons to the change, which will be discussed in this presentation.We reviewed four months’ worth of handovers. They were reviewed for specific elements of essential handover criteria. The areas included Patient Name, H&C, detained or voluntary, admission or review, presenting complaint, patient's history, risks, physical issues, and handover to specific person.It should be noted that the doctors involved were aware of the changes made, and standards being introduced, and therefore were aware that they would be auditing their own handovers. This might have created bias in the subsequent handovers.We then analysed each month to see what percentage of handovers had been concordant with the standards. These data were then represented in graphs, as we will show.ResultsWe identified areas which were performed well when completing handover. The areas which were consistently 100% included the date of handover and patient name or initials. Outstanding jobs were performed in 97.5% of handovers.Areas for improvement identified when collecting results were the status of admission (i.e. voluntary or detained), the main patient risks and use of the document password.100% of the shifts used the new digitalised format for handover using Microsoft word and trust email system.ConclusionWe reviewed the results of the data, which highlighted areas for improvementWe hope to implement a standard performa for handovers, reducing the chance of key information being missed, thereby improving patient safety. We aim to collect data following this 2nd intervention in the next rotation and continue to examine handover processes using PDSA cycles.

Abstract Background High-quality, shift-to-shift handovers by residents are critical to ensuring to patient safety. The 2011 Accreditation Council for Graduate Medical Education duty hour requirements have increased the number of handovers occurring daily, necessitating new approaches to this challenge. Research suggests standardized approaches, electronic systems, and education programs can improve the handover process. Methods We conducted a 2-phase, observational study comparing an electronic handover system (experimental) in one clinical setting to a standard card-based system (control) at a second site. Outcome data included an objective assessment of the completeness and accuracy of handovers, and resident assessment of the handover systems. In phase 1, data were recorded at both sites and not shared with residents. In phase 2, data from the experimental system were used to provide standardized feedback to residents on the quality of their handovers. Results A total of 3184 individual patient sign-outs were evaluated during the 11-month period. Following introduction of a feedback intervention in the experimental arm, errors were present in only 5.2% of handovers, compared with 16.1% of controls (P < .001), and 67% of the 38 residents responding reported they perceived the experimental system as facilitating better patient care. Conclusion Regular, real-time feedback through an electronic handover system can improve the accuracy and completeness of handovers in patient care.

(1) Background: we compare a new SBAR based electronic handover tool versus a paper-based checklist for handover in a pediatric intensive care unit (PICU). (2) Methods: this is a randomized, observational study of 40 electronic vs. 40 paper checklist handovers after pediatric cardiac surgery, with a 48 items checklist for comparison of reporting frequencies and notification of disturbances and noise. PICU staff satisfaction was evaluated by a 12-item questionnaire. (3) Results: in 14 out of 40 cases, there were problems with data processing (incomplete or no data processing). Some item groups (e.g., hemodynamics) were consistently reported at higher frequencies than other groups. Items not specifically asked for did not get reported. Some items, automatically processed in the SBAR handover page, did not get reported. Many handovers suffered a noisy and distracting atmosphere. There was no difference in staff satisfaction between the two handover approaches. Nurses were highly unsatisfied with the general approach by which the handover was performed. (4) Conclusions: human error appears to be a main factor for unreliable data processing. Software is still too complicated, and multitasking is a stressful and error prone event. Handover is a complex task with many factors required for a successful completion.

Dizertační práce se zabývá návrhem nového algoritmu řízení handoveru v rámci protokolu Mobile IPv6, který umožní nasazení tohoto protokolu v leteckých datových sítích. Existující algoritmy řízení handoveru sice dosahují dostatečné výkonnosti v konvenčních pozemních bezdrátových sítích disponujích velkou šířkou pásma a nízkou latencí, jako jsou WiFi nebo UMTS, ale jak ukazuje tato práce, nasazení těchto algoritmů prostředí leteckých datových sítí nepřináší očekávané výhody. Analýza ukazuje, že v úzkopásmových leteckých sítích trpí tyto algoritmy řízení handoveru velkou latencí a způsobují značnou režii. Nový algoritmus řízení handoveru v MIPv6 navržený v této práci je založený na jednoduché myšlence: ''Já jsem letadlo, já vím, kam letím!'' To znamená, že pohyb letadla není náhodný, ale vysoce předvídatelný. Díky tomu je možno předvídat handovery mezi přístupovými sítěmi podél očekávané trajektorie letadla a vykonat nezbytné operace pro přípravu handoverů již na zemi, kde je letadlo připojeno k širokopásmové síti letiště. Tato dizertační práce dále uvádí porovnání existujících algoritmů řízení handoveru s nově navrženým pomocí analytické metody ohodnocení handoveru. Díky tomu je možno kvantifikovat výhody, které nový algoritmus přináší a taktéž popsat slabiny algoritmů existujících.

Relevant and quality-assured geographic data is critical information in several steps of the building process. There are many methods for quality-assuring geographic data during data collection, and in the building process, but in order for the data to be useful throughout the life cycle of a facility it is important that the data maintains reliability and quality even after the handover from a building project. The handover of geographic data from building projects has, by municipal management units, been described as inefficient. When designers do not follow the client's requirements the data must be handled manually, which is both time consuming and a possible risk for the quality of the data. In a time that place great demands on efficiency, quality and sustainability, it is not justifiable to manually digitizing data that has previously already been digital, furthermore there is also a risk that the human factor and interpretations affect the data quality. There are factors in the handover process that needs to be prevented in order to achieve an unbroken information supply throughout the life cycle of a facility. In this thesis, a qualitative study has been conducted with the purpose to identify what factors in the handover process prevent quality-assured handover of geographic data from building projects to management organizations. The findings of the study should also result in recommendations of how the City of Stockholm and other municipalities in general can prevent obstacles in the handover process to assure that geographic data maintains its quality throughout the handover. To identify barriers in the handover of geographic data in building projects, the data collection in this study was done through several interviews with project managers, designers and system managers. The results show that important factors for a successful, quality-assured handover of geographic data are related requirements and communication. Final recommendations in order to assure that the geographic data maintains its quality throughout the handover are: early involvement of the management, clear and early communicated requirements, improved communication to achieve a more holistic view, and automated processes that could replace manual work and check data against requirements.

An important application of Machine Learning (ML) in mobile networks is to predict if a user device has coverage on a frequency other than the current serving frequency. This is a use-case called Secondary Carrier Prediction (SCP). In this thesis we deduce if data across different cells and frequencies can be successfully combined when learning this task, thus reducing the number of models that require training. Aggregation of data involves several challenges, such as different prevalences and varying amounts of available data, but more importantly the possibility of achieving synergies in training by exploiting recurring patterns in data. By using an experimental setup in which models are trained and validated on aggregated datasets it is shown that synergies in fact can be achieved through aggregation. The scalability of this task is improved so that the number of models can be reduced with a factor as large as the number of cells times the number of frequencies, while maintaining similar or improved prediction performance.
Prediktion av täckning på sekundära frekvenser är en signifikant tillämpning av maskininlärning inom mobila nätverk. I den här avhandlingen utreds möjligheten att träna modeller på aggregationer av data, med följden att antalet modeller blir färre. Olika klassbalanser och varierande tillgång på data är utmaningar som uppstår vid aggregation, men även möjligheten att uppnå synergier genom att utnyttja återkommande mönster i datat. Med en experimentell uppställning där modeller tränas och valideras på aggregerade dataset visas att synergier kan uppnås genom aggregation. Skalbarheten på denna tillämpning förbättras till den grad att antalet modeller kan reduceras med en faktor lika stor som antalet celler gånger antalet frekvenser, med likvärdig eller förbättrad prediktionsprestanda.

With the upcoming of the fifth generation of cellular networks there are several perfor- mance requirements that need to be satisfied. This thesis focuses on the topic of mobility which allows users to move through the network using the concept of handovers to switch between base stations. However, the thesis also keeps the energy efficiency in mind and investigates a strategy of reducing the energy consumption. Moving across base stations will inevitably lead to some handover failures, a goal of the system developers is to reduce these, but there exists a tradeoff between too early and too late handover failures. This thesis investigates two approaches of lowering the number of failures by letting the net- work self-optimize parameters in the handover procedure based upon the tradeoff. The first approach includes a parameter adaption to an estimated velocity of the users and the second one making a parameter more granular. Simulating different scenarios in a detailed network simulator shows performance gain by adapting handover parameters to the esti- mated velocity, but gathering more data regarding failures is needed before generalizing the conclusions.

Dho offre des routines qui permettent d'acquérir des ressources en lecture ou en écriture de façon cohérente et transparente pour l'utilisateur. Nous avons modélisé le cycle de vie de Dho par un automate d'état fini puis, constaté expérimentalement, que notre approche produit un recouvrement entre le calcul de l'application et le contrôle de la donnée. Les expériences ont été menées en mode simulé en utilisant la libraire GRAS de SimGrid puis, en exploitant un environnement réel sur la plate-forme Grid'5000. Par la théorie des files d'attente, la stabilité du modèle a été démontrée dans un contexte centralisé. L'algorithme distribué d'exclusion mutuelle de Naimi et Tréhel a été enrichi pour offrir les fonctionnalités suivantes: (1) Permettre la connexion et la déconnexion des processus (ADEMLE), (2) admettre les locks partagés (AEMLEP) et enfin (3) associer les deux propriétés dans un algorithme récapitulatif (ADEMLEP). Les propriétés de sûreté et de vivacité ont été démontrées théoriquement. Le système peer-to-peer proposé combine nos algorithmes étendus et le modèle originel Dho. Les gestionnaires de verrou et de ressource opèrent et interagissent mutuellement dans une architecture à trois niveaux. Suite à l'étude expérimentale du système sous-jacent menée sur Grid'5000, et des résultats obtenus, nous avons démontré la performance et la stabilité du modèle Dho face à une multitude de paramètres

We improved, the distributed algorithm for mutual exclusion (of Naimi and Trehel), by introducing following features: (1) Allowing the mobility of processes (ADEMLE), (2) introducing shared locks (AEMLEP) and finally (3) merging both properties cited above into an algorithm summarising (ADEMLEP). We proved the properties, safety and liveliness, theoretically for all extended algorithms. The proposed peer-to-peer system combines our extended algorithms and original Data Handover model. Lock and resource managers operate and interact each other in an architecture based on three levels. Following the experimental study of the underlying system on Grid'5000, and the results obtained, we have proved the performance and stability of the model Dho over a multitude of parameters

With increasing demands on network reliability and speed, network suppliers need to effectivize their communications algorithms. Frequency measurements are a core part of mobile network communications, increasing their effectiveness would increase the effectiveness of many network processes such as handovers, load balancing, and carrier aggregation. This study examines the possibility of using supervised learning to predict the signal of inter-frequency measurements by investigating various learning algorithms and pre-processing techniques. We found that random forests have the highest predictive performance on this data set, at 90.7\% accuracy. In addition, we have shown that undersampling and varying the discriminator are effective techniques for increasing the performance on the positive class on frequencies where the negative class is prevalent. Finally, we present hybrid algorithms in which the learning algorithm for each model depends on attributes of the training data set. These algorithms perform at a much higher efficiency in terms of memory and run-time without heavily sacrificing predictive performance.

With advancements in the mobile network architecture, from the Fourth Generation to the Fifth Generation, a vast number of new use cases becomes available. Many use cases require cloud-based services, where a service is deployed close to the user. For a user to communicate with a service, it connects to the mobile network base station, Fifth Generation Core network and then to the service. When the user changes physical location, the mobile network and the service must apply mobility techniques. This is to prevent tromboned traffic and provide low latency between user and service. When a handover occurs, so that a user’s attachment point to the mobile network is changed from the one base station to another and the User Plane Function changes, the cloud-based service may have to seamlessly move from one cloud to another as well. In this thesis, a Service mobility framework is proposed and implemented, which enables service live migration between edge clouds and it provides simple RESTful APIs. The evaluation of the framework shows that the proposed implementation adds low delays to the total migration time and the service downtime is also shown to be low in the case of video streaming with no service interruption.

När användaren förflyttar sig från en plats till en annan, så måste det mobila nätverket och tjänsten tillämpa rörlighetstekniker, som förflyttning av tjänsten. Förflyttningen är för att förhindra trombonerad trafik och att förse låg latens mellan användare och tjänst. När en överlämning sker, d.v.s att en användares kopplingspunkt till det mobila nätverket ändras, från en basstation till en annan, och att User Plane Function ändras, så kan även den molnbaserade tjänsten förflytta sig sömlöst från ett moln till ett annat. I denna avhandling presenteras ett tjänströrlighetsramverk som möjliggör tjänströrlighet mellan moln och erbjuder enkla RESTfulla API:er. Evaluering av ramverket visar att implementationen bidrar med låga fördröjningar till den totala migrations tiden samt att tjänster med videoströmming har lågt driftstopp utan tjänstavbrott.

Resumen
In this thesis we propose a system for robots to learn through a semisupervised approach from observations, proper handover features for objects that can be applied onto new objects. Using recordings of handovers, features are extracted for the purpose of classifying the objects through unsupervised learning. The results from the classification are used to train a network in a supervised fashion as to properly identify handover class from images. The results of this work show that objects with similar visual features are handed over in similar way and that with a limited amount of data a model can be fitted as to properly predict handover settings for an object that has been previously encountered or not.
I detta examensarbete presenteras ett förslag på ett system för robotar att lära sig på ett autonomt semi-supervised vis egenskaper vid överlämning för olika objekt genom att observera människor, som kan senare även användas till nya objekt. Med hjälp av inspelat material på överlämningar, identifierar vi egenskaper som gör det möjligt att klassificera objekten genom unsupervised learning. Resultaten från denna klassificering kombineras med bilder på objekten som används till att träna ett nätverk på ett supervised vis, som lär sig att förutspå korrekt klass för ett objekt via bilddata. Resultaten från detta arbete visar att objekt som överlämnas på liknande vis även har liknande visuella egenskaper, och med en begränsad mängd med data kan vi träna en modell som med hög träffsäkerhet ger oss inställningarna för överlämningen utav ett objekt vare sig det har påträffats tidigare eller inte.

High Speed Trains (HST) face some of the most stringent requirements for ensuring passenger safety and comfort while operating at very high velocities. HST wireless communication systems therefore similarly require special considerations for network design, technology selection and system implementation. For infrastructure-based wireless communications, a handover occurs whenever a radio transitions from the coverage of the current base station to the next base station. The faster the train moves the shorter the time that it spends under coverage area before a handover is required. For HST this can be as short as 10–20 seconds. Furthermore, a HST moving at 220 mph experiences significant fast and small-scale wireless signal fading due to this velocity, but similarly may incur frequent connectivity losses due to the rapid transit across coverage areas. A route consisting of viaducts, bridges, tunnels or hilly regions drastically increases the need for rapid handovers since a large number of base stations are required to provide coverage and achieve acceptable Quality-of-Service (QoS) in such environments. Due to the relatively fixed amount of time needed to complete a handover, and the possibility of failed handovers, this reduces the usable time under coverage, and thus ensuring an optimal handover strategy selection is vital. Most of the routes span rural and suburban areas, which reinforces the need for a comprehensive network planning strategy, as these areas tend to be underdeveloped for cellular coverage. Given the complexity in available radio spectrum resources, detailed studies are required to aid in this technology selection process. However, with the predicted increase in the demand of railroad network data traffic generated by onboard sensors, onboard control and operation devices, passenger Internet services, etc., it becomes apparent that more resources are needed than are provided by current technologies such as LTE. Thus, including 5G New Radio for Railways (5G-NR2) in the consideration does not only provide railways a more cost-effective licensing option for frequencies, but also an opportunity to select the best possible radio access method for a given HST corridor. In this paper, we focus on the metrics related to handover and how they correlate with different radio access technologies for HST. Our results integrate the uncertainty of environmental factors to provide answers on technology selection driven by specific route, inter-site distances, and available bandwidth. Our wireless communication simulation results are driven by well recognized mathematical models that consider a variety of key parameters. The analysis of the handover performance will offer insights vital to future railroad network planning for multivariate radio access technologies, and to answer crucial questions about the potential for using frequency bands above 6 GHz in HST.
