Ultrasound (US) guidance remains a very common, relatively inexpensive, and versatile option for guiding percutaneous interventions. The need for procedural sedation and anesthesia for minimally invasive procedures continues to grow as the number of procedures and interventional applications increase. This trend will continue as the population ages and the complexity and array of imaging-guided interventional procedures expand. This chapter discusses common interventional procedures that are performed under MRI and US guidance and outlines patient issues, procedural techniques, safety, and other clinical considerations that pertain to both interventionalists and anesthesiologists.

Computed tomography (CT) has become a valuable guidance modality for a large number of diagnostic and therapeutic percutaneous interventions. Diagnostic and therapeutic interventions guided by CT are among the most common interventional radiology procedures performed in clinical practice. Common diagnostic CT-guided interventions include tissue biopsies, fluid aspirations, and catheter drainages. Needle biopsies can be used to obtain tissue for cytologic or histologic assessment from almost any anatomical region of the body. Positron emission tomography (PET)/CT–guided diagnostic and therapeutic interventions have emerged recently and are increasing in frequency and number of indications. This chapter discusses common interventional procedures that are performed in both the interventional CT and PET/CT suites and outlines patient issues, procedural techniques, radiation/safety, and other clinical considerations that pertain to both interventionalists and anesthesiologists.

Over the past four decades, a variety of invasive diagnostic and therapeutic procedures have been developed by radiologists. The term ‘interventional radiology’ most appropriately refers to therapeutic procedures performed under imaging guidance. Interventional radiological procedures have virtually replaced several more invasive and hazardous surgical alternatives. Other interventional techniques offer completely new therapeutic options. Some diagnostic radiological procedures are frequently followed by therapeutic manoeuvres. Interventional radiology can make a significant contribution to the palliation of patients with irresectable malignant tumours, as many of the procedures can relieve symptoms without the need for general anaesthesia, a prolonged stay in hospital, or the discomfort associated with recovery from a surgical operation. The vast majority of procedures are performed using local anaesthesia and mild sedation. The emphasis in this chapter is on the indications, contraindications, and likely outcomes, rather than on detailed technical descriptions.

Intracardiac echocardiography is a relatively new echocardiographic technique, essentially used for monitoring of percutaneous atrial septal defect closure procedures and for electrophysiological catheter-based ablations. It is still limited to a few experienced centres, but presents with several advantages, including the safe guidance of percutaneous interventional treatments, avoidance of general anaesthesia, and reduction in radiation exposure. Intracardiac echocardiography is likely to be more and more used, with the development of new catheter interventional procedures. This chapter discusses the principles, methods, and main indications for intracardiac echocardiography in monitoring and guidance of interventional procedures as well as new developments and its cost-benefit ratio.

This chapter discusses the accuracy and safety of lumbar transforaminal or nerve root (periradicular) injections under CT guidance. To maximize procedure accuracy and minimize complications, spinal injections are best performed with image guidance. Though fluoroscopic guidance has been used for decades, greater accuracy and possibly greater safety can be achieved with CT guidance. The radiation exposure is operator dependent, and maintaining the lowest possible dose is critical in optimizing patient care. This is also true for fluoroscopic guidance, for which coning and intermittent exposure are used as well. The future may usher in ultrasound and magnetic resonance imaging guidance without the associated radiation exposure. For interventional practitioners, ensuring patient safety and efficacy goals remains a priority, and needs to evolve with advances in technology.

Imaging is reliant upon the contrast between different body elements, which may be present naturally or may require the introduction of extrinsic contrast agents. Since the 1920s, the use of contrast agents has been refined to enhance the diagnostic potential of multiple imaging modalities. Contrast agents are a vital part of diagnosis and treatment algorithms involving image guidance. Given the wealth of contrast agents on the market, a basic understanding of the various types is critical for budding interventional radiologists who need to use such agents judiciously on a daily basis. Such contrast agents may be administered intravenously, intraarterially, intrathecally, orally, via inhalation, transrectally, or via indwelling tubes or catheters cannulating a specific viscus. In this chapter, we review the key categories, contraindications, and alternatives for such agents, with special attention to their use in an interventional radiology (IR) practice.

The thoracic facet and costotransverse joints are often implicated as the source of thoracic pain, yet definitive diagnostic and treatment guidance is significantly limited. This chapter reviews the anatomy, innervation, and biomechanics of these joints, as well as associated pathology. Definitive innervation of the posterior primary rami has yet to be established, and significant pain pattern overlap between the thoracic facet joint, costotransverse joints, and visceral referral patterns, as well as the limitations of current biomechanics, challenge the clinician’s ability to examine pain of suspected thoracic origin. The use of clinical reasoning in the absence of definitive diagnostic and treatment approaches is necessary to optimize outcomes in patients with pain of suspected thoracic musculoskeletal origin. A progression from noninvasive to minimally invasive to interventional techniques may be warranted based on the patient’s response to treatment.

Percutaneous drainage catheter placement is a frequently performed interventional radiology procedure. One of the common management complications of such catheters is obstruction or clogging of the catheter. Occluded drainage catheters are routinely exchanged over a guidewire under fluoroscopic guidance. At times, however, a guidewire cannot be passed through the catheter obstruction, and exchanging the catheter over a guidewire may become difficult or even impossible without losing access. Because salvaging an obstructed catheter, without risking loss of access, can be difficult, multiple techniques to preserve organ access have been developed. This chapter describes a technique whereby the creation of a new side hole in the catheter is used to re-establish wire access and facilitate catheter exchange. It is frequently useful for exchanging enteric tubes, urologic drainage catheters, biliary drainage catheters, and abscess drains.

This chapter describes the use of an elongated radiopaque gelatin sponge plug for tract occlusion after percutaneous biliary, portal venous, or hepatic venous access or intervention. Often, embolization coils or a gelatin sponge “slurry” is sufficient for hemostasis following liver intervention. This technique offers an inexpensive, temporary, and readily available option for achieving hemostasis following liver interventions. A contrast-soaked Gelfoam plug is loaded into a delivery cylinder that is advanced into the access sheath and positioned and deployed under fluoroscopic guidance. This technique is an alternative option to the use of embolization coils or a gelatin sponge slurry when seeking hepatic parenchymal tract hemostasis following intervention.

Resumen
Safe and successful ultrasound-guided interventional procedures depend on the ability to visualize both anatomical structures of interest and the advancing needle. This chapter describes various strategies for optimizing needle visualization and tip localization during freehand guidance of a needle to a tissue target using ultrasound. Challenges to needle visualization include the poor echogenicity of standard needles at insertion angles steeper than 30–45 degrees to the horizontal, the difficulty of aligning the needle with the ultrasound beam, and manipulation in three dimensions based on two-dimensional visual information. Attention to ergonomics improves probe and needle control and facilitates alignment. Needle echogenicity can be improved by using a shallower angle, where possible, and echogenic needle technology. Hand movements of probe and needle should be kept small and controlled. Indirect cues of needle tip location are also extremely useful and should be utilized routinely.
