Flame retardant chemicals were used as topical finishes on cotton and cotton blended fabric. Comparison of flame resistance and durability of non-bromine/non-antimony flame retardants were explored based on flame resistance testing and physical testing results. Three 100% cotton fabrics and 3 cotton blended fabrics were used. Twenty one different commercially available flame retardants were used as topical finishes on all fabric types. AbstractPeripheral T cell lymphomas comprise a heterogeneous group of rare diseases, representing 10–15% of all non-Hodgkin lymphomas (NHLs). Upfront treatment for peripheral T cell lymphoma (pTNHL) includes CHOP-like (cyclophosphamide, adriamycin, vincristine, prednisone) multiagent chemotherapy with or without etoposide, followed by stem cell transplantation as consolidation in responsive fit patients. This approach induces durable long-term remission in approximately 40% of cases; early refractoriness during induction occurs in approximately 25% of patients, with the remaining patients typically relapsing within 24 months. With the exception of patients with anaplastic large cell lymphomas who are eligible to receive brentuximab vedotin, there is no standard of care in the relapse setting. In patients not eligible to receive high-dose chemotherapy followed by allogeneic stem cell transplantation, the prognosis is dismal.

AbstractOrthopaedic and dental implant treatments have allowed to enhance the quality of life of millions of patients. Total hip/knee arthroplasty is a surgical replacement of the hip/knee joint with an artificial prosthesis. The aim of joint replacement surgery is to relieve pain improve function, often for sufferers of osteoarthritis, which affects around a third of people aged over fifty. Nowadays, total hip and knee replacement (THR) surgeries are considered routine procedures with generally excellent outcomes. Given the increasing life expectancy of the world population, however, many patients will require revision or removal of the artificial joint during their lifetime. The most common cause of failure of hip and knee replacements is mechanical instability secondary to wear of the articulating components. Thus, tribological and biomechanical aspects of joint arthroplasty are of specific interest in addressing the needs of younger, more active patients. The most significant improvements in the longevity of artificial joints have been achieved through the introduction of more wear resistant bearing surfaces. These innovations, however, brought about new tribocorrosion phenomena, such as fretting corrosion at the modular junctions of hip implants. Stiffness mismatch between the prosthesis components, non-physiological stress transfer and uneven implant-bone stress distribution are all involved in premature failure of hip arthroplasty. The development of more durable hip and knee prostheses requires a comprehensive understanding of biomechanics and tribocorrosion of implant materials. Some of these insights can also be applied to the design and development of dental implants.

AbstractIndolent non-Hodgkin lymphoma (iNHL), including follicular (FL) and marginal zone (MZL) lymphoma, now enjoy durable disease control with first-line immunochemotherapy, with a median overall survival (OS) of over 15 years in most series (Kahl and Yang 2016). However, iNHL is still widely considered incurable in most cases, and disease history remains characterized by a relapsing and remitting course, with each remission period shorter than the previous one, and OS and progression-free survival (PFS) decrease with each subsequent line of conventional therapy (Batlevi et al. 2020). Patients with unmet needs include approximately 20% of FL patients who experience disease progression within 24 months (POD24) after initial chemoimmunotherapy (with a 5-year OS of 48% (Casulo et al. 2015)—although it remains unclear how much this worse outcome is driven by misdiagnosed transformed follicular lymphoma (Freeman et al. 2019)) and those who fail multiple regimens (5-year PFS of 23%) (Rivas-Delgado et al. 2019) have double refractory disease (Gopal et al. 2017) or experience relapse after autologous stem cell transplantation (ASCT) (Sesques et al. 2020). Although promising results were obtained with an immunomodulatory regimen combining anti-C20 Moab and lenalidomide (Leonard et al. 2019; Morschhauser et al. 2019), most current approved therapies do not overcome incremental disease resistance, resulting in multiple lines of treatment with cumulative toxicity over a patient’s lifetime. The autologous anti-CD19 chimeric antigen receptor T cell (CAR-T) therapies tisa-cel and axi-cel, which are now approved for patients with relapsed/refractory (r/r) large B cell lymphoma (LBCL), have also been tested in iNHL, with promising results.

AbstractThe chapter is focused on collaborative processes through which the functions and spatial hierarchies of public or public use areas are redefined. The field of action is: on the one hand the urban metabolism, interpreted as a study of the life cycle of the city, including wastescapes; on the other, collaborative processes, aimed at defining the uses of tailored, place-based, and collective services. In this sense, the research moves from the analysis of places born for public use, but abandoned over time or never actually completed; disused places waiting to reenter the urban metabolism. Among those, there are also Italian “planning standards,” publicly designed in compliance with the quantities defined by law, and often partially used or not properly managed. The proposal of new uses and services for these contexts is based on criteria of flexibility, not fixed once and for all, not predetermined in time, but in progress in order to overcome the limits of the implementation of policies and programs of the past. These integrated processes can activate a dialogue between public institutions, privates, local associations and citizens’ groups. The research also intends to cross-reference the issue of spatial inequalities in access to spaces and services, with the evolution of the public actor from provider to service enabler, in a wider redefinition of welfare and welfare spaces concept, as an effect of global economic and financial crisis. The question needs non-sectoral responses, which take into account environmental, social, spatial issues. Welfare can no longer be provided as a self-sufficiency device: contextual services, for everyone, can be realized by recapitalizing wastescapes, co-creating “planning standards” through the recovery of degraded local contexts, collectively investing in the use and care of public, and open services. The paper will focus on: (a) the case of the former NATO area in Naples (in Bagnoli neighborhood) which is the subject of a Plan for urban renewal, adopted by the Municipality of Naples in 2020. The area, owned by a public company whose purpose is the assistance of children in the disadvantaged segment (Fondazione Campania Welfare), has been redesigned as a public facility on a metropolitan scale, within a public consultation process between the ownership, the Municipality of Naples and several local stakeholders (third sector organizations, citizens, cultural associations, etc.). As an effect of this collaborative process, the reuse of the area started before the adoption of the Plan; (b) the case of Horizon2020 research REPAiR in which the issue of circular economy applied to the recovery of wastescapes for public purposes has been investigated in living labs, working on waste perception and awareness as key factors for regenerating wastelands. The co-creation process partly resumed a strategy foreseen in 2013 by the Campania Region in the Plan of waste prevention, for the implementation of Integrated Centres for the reuse of durable goods, originally excluded by the Regional Waste Law.

AbstractEven after a decade of use, CAR-T cell therapy for non-Hodgkin lymphoma (NHL) is still evolving, and disease control is now the main concern in the majority of experienced centres. Indeed, despite highly appealing objective response (OR) rates in refractory patients, the long-term overall survival (OS) of this population has only slightly improved. Pivotal studies have suggested that 2-year OS rates do not surpass 30%, even though results improve when complete response (CR) is achieved within the first 3 months after treatment (Wang et al. 2020; Schuster et al. 2019; Neelapu et al. 2017). Although achieving this exceptionally high level of OR is praiseworthy, similar improvements have not been made regarding OS, and current OS probabilities are not satisfactory. Of course, there are multiple reasons for this; a substantial proportion of patients either do not achieve an initial response or experience progression very soon after treatment, with poor OS (Chow et al. 2019). Both populations present with disease burden or aggressive cancer prior to CAR-T cell therapy, possibly having been referred too late in the course of treatment or waited too long before CAR-T cells were processed for them. Both of these issues have potential solutions, such as more widely publicizing the efficacy of CAR-T cells, which may increase referrals at an earlier stage, and developing methods, which are already being heavily investigated, for shortening the manufacturing process (Rafiq et al. 2020). In the latter case, the use of allogeneic lymphocytes could allow for already prepared cells to be readily used when needed and would most likely be the most efficient strategy as long as the risk of graft-versus host disease is offset (Graham and Jozwik 2018). Thus, achieving CR is a crucial step in increasing OS, as patients with partial response (PR) or stable disease (SD) present with lower OS, while currently, recurrence appears to be rare when CR is maintained for more than 6 months (Komanduri 2021). However, the disease will likely recur in more than half of patients in the months following treatment, possibly due to issues such as the poor persistence of CAR-T cells (which may not be as crucial as once thought for acute lymphoblastic leukaemia (Komanduri 2021)) or the loss of target antigen expression (which has been regularly documented (Rafiq et al. 2020)). Both of these mechanisms could potentially be used to develop methods that reduce recurrence after CAR-T cell therapy. In fact, the most popular approaches currently being investigated are attempting to either use two CAR-T cell types that each target different antigens or to create CAR-T cell constructs that target either multiple antigens or an antigen other than CD19 (Shah et al. 2020). The concomitant infusion of CAR-T cells with targeted therapies is also being explored in other B-cell malignancies and appears to both increase the CR rate and decrease recurrence (Gauthier et al. 2020). When recurrence does occur, patient OS is rather dismal, and the best remaining option would most likely be inclusion in a clinical trial. If this option is not available, salvage therapy may be attempted, although cytotoxic treatments are extremely limited given that most diseases have been refractory to numerous lines of treatment prior to immunotherapy. A few case reports and studies with a small patient population receiving anti-PD-1 antibodies, ibrutinib, or ImiDs have been reported with largely anecdotal supporting evidence (Byrne et al. 2019). However, even in the case of a new objective response (OR), the subsequent risk of recurrence is substantial and may invite further consolidation with allogeneic haematopoietic stem cell transplantation (Byrne et al. 2019), which has already been performed in patients treated for acute lymphoblastic leukaemia (Hay et al. 2019). However, the efficacy of this strategy remains to be validated in NHL patients in clinical trials. Further supporting evidence, although limited, has recently been reported concerning an additional treatment with CAR-T cells inducing an OR. Of the 21 NHL patients included in the study, the OR rate after the second infusion was 52% (CR, n = 4; PR, n = 7), with some durable responses inviting further investigations (Gauthier et al. 2021). Overall, with such poor outcomes after recurrence, current efforts are also focused on predicting the patients most likely to experience disease progression and that are potential candidates for preemptive consolidation therapy, although there is no doubt that patients who do not achieve a rapid CR should be the first candidates. Additionally, immune monitoring should encompass not only CAR-T cell survival but also the detection of circulating tumour DNA (Komanduri 2021) because this could aid in detecting subclinical recurrence and in deciding whether consolidation or maintenance therapy should be administered. However, currently, all these approaches are highly speculative and require further clinical study.

This chapter tackles autocracies, including the twelve democratic transitions that do not satisfy the strict criteria for the paths. Several are near-misses that fit the underlying logic of minimal power loss from democratization, while others are true outliers. It discusses commonalities across the outliers and examines whether other crises should be added to the paths but argue none add much explanatory power. The chapter provides an overview of all cases of non-democratization, explaining many cases of stability. Specifically, most non-democratizing countries lack both a recent shock and a durable ruling party, making their stability expected and many had shocks or competitive parties that failed to democratize.
