Resumen
The very nature of detention means that those subjected to it are dependent on detaining authorities to provide protection and to refrain from human rights abuses, including torture and ill treatment. Recognising this, the Optional Protocol to the UN Convention against Torture (OPCAT) obliges signatories to establish detention monitoring bodies through National Preventative Mechanisms (NPMs). Whilst some regions have received considerable attention in terms of CAT and OPCAT compliance, this has not generally been so for the region incorporating Russia, Nordic and Baltic countries.

Resumen
Torture has long been denounced by the international community; the need to protect citizens from abuse at the hands of the state is a principle enshrined in international law. One area where abuse is common is within the correctional system and as a result, there is a need for oversight in places of detention. The Optional Protocol to the UN’s Convention Against Torture (OPCAT) is an international human rights instrument that acts as a preventive measure to monitor all places of detention through regular visits. Supportive of the OPCAT since its adoption, Canada has considered signature/ratification since 2002 but has yet to commit. The purpose of this study is to identify factors that have led to a delay in Canada becoming State Party to the OPCAT despite adherence to the principles that this instrument embodies. A policy analysis framework was utilized to conduct stakeholder interviews and review government documents. The concept of agenda-setting received special attention and content analysis of media reports and a review of government legislative activity were conducted to provide insight into the prevalence of the issue on the public and political agendas. The author argues that while there are real challenges that policymakers must overcome, the absence of political leadership has resulted in stagnation in the decision-making process. As a result, the issue has disappeared from both the public and political agenda. In order for progress to be made, political will must be created and the impetus to act (‘re-setting the agenda’) must come from civil society in the absence of government engagement on this issue.

I attempt to analyze South Africa's legal position pertaining to torture, in relation to the international legal framework. Since it has been established that torture and cruel inhuman and degrading treatment (CIDT) usually occur in situations where persons are deprived of personal liberty, I examine legislation, policies and practices applicable to specific places of detention, such as correctional centres, police custody, repatriation centers, mental health care facilities and child and youth care centers. I establish that although South Africa has ratified the UNCAT and is a signatory to the OPCAT, our legal system greatly lacks in structure and in mechanisms of enforcement, as far as the absolute prohibition and the prevention of torture and other forms of cruel and degrading treatment or punishment are concerned. I submit that South Africa has a special duty to eradicate torture, since many of its citizens and several of its political leaders are actually victims of torture, who suffered severe ill treatment under the apartheid regime. I argue that the South African legal system is sufficiently capable of adopting a zero-tolerance policy toward torture and to incorporate this with the general stance against crime. In many respects, South Africa is an example to other African countries and should strongly condemn all forms of human rights violations, especially torture, since acts of torture are often perpetrated by public officials who abuse their positions of authority. I conclude by making submissions and recommendations for law reform, in light of the obstacles encountered within a South African context.

Through changes in the code of statutes, SOSFS 2009:27 (M), opioid addicts are excluded since March 1 2010 from possibilities to be accepted for substitution treatment. Opiate addicts are solitary admitted for substitution treatment from that date. Opioid addicts are excluded admission for treatment regardless of the fact that they fulfil the ICD-10 diagnosis F.11.2, i.e. opioid/opiate addictive criteria. The alteration in the statutes was carried out in reference to the fact that evidence for this kind of treatment intended for opioid addicts was missing. Both groups i.e. opiate – and opioid addicts, are nevertheless experienced in clinical work , to have extensive problems with addiction, health, social situation along with the risk of premature death.     The purpose with this study is to identify what differences and/or similarities there are in background varieties and the severity of the problems  between the groups. This is conducted with the use of a population (n=127) with comparable background material, e.g. collected Addictions Severe Index- interviews, scientifically survey and compare background factors and the severity of the problems.      The main discovery in this study is that one can demonstrate great similarities between the groups regarding background as well as the severity of the problems. It is therefore of great interest, on a individual as well as a social oriented level, that pursued studies regarding diagnostic safety and on processes in substitution program are needed to generate further knowledge as a foundation for development of future care and changes in the code of statutes.

Title: Consequences of diversion – a qualitative interview study regarding illicit buprenorphine use. In year 1999 buprenorphine was introduced in Sweden and is used in treatment for opioid dependence. An issue concerning substitution treatment programmes is the occurrence of diversion and the following illegal sails of buprenorphine on a black market. The aim of this study was to explore how people who have used illicitly obtained buprenorphine describes such abuse. The study was conducted using qualitative method, where a total of five individuals were interviewed about past experiences of having used illicitly acquired buprenorphine. The findings indicate that polydrug use and intake by injection or through the nose are common amongst illicit users of buprenorphine. Buprenorphine was described as a possible substance of abuse. Moreover, it is discussed whether availability and attitudes concerning buprenorphine as a cleaner and/or safer substance than other drugs can contribute to increased abuse of opiates through buprenorphine amongst drug users who have not previously been in contact with opiates. The authors argue that further research is needed to explore the extent of diversion from domestic substitution treatment programs, and to widen the understanding of how buprenorphine function as a drug of abuse.

Bacterial endocarditis is a serious infectious disease, the treatment of which is traditionally performed with the patient hospitalized and receiving intravenous medication. When the etiological agent is Streptococcus, progression is generally less aggressive. Thus, the possibility of outpatient treatment becomes an attractive option from the social and economic standpoint. In the literature, this type of treatment is known as outpatient parenteral antimicrobial therapy (OPAT). Objective: The aim of the present study was to demonstrate that outpatient treatment is safe and effective in cases of streptococcic endocarditis. Patients and Methods: Six patients with bacterial endocarditis, diagnosed using the modified Duke University criteria, were followed up between January 2006 and November 2008. Four patients were female (66.6%) and two were male (33.4%). Mean age was 47.8 years, ranging from 32 to 65 years. All patients were eligible for outpatient treatment and agreed to take part in the study, having been either partially or wholly treated in an outpatient regimen. xiii Imaging and laboratory exams were performed at the beginning and end of treatment and when otherwise deemed necessary. Results: All patients progressed with no complications and with the complete resolution of the infection. The results of the laboratory and imaging exams are presented and discussed. Conclusion: Streptococcic bacterial endocarditis may be safely and effectively treated in an outpatient regimen for selected patients with no concomitant aggravating factors.
A Endocardite bacteriana é uma grave doença infecciosa cujo tratamento é tradicionalmente feito com o paciente internado, recebendo medicação intravenosa. As endocardites cujo agente etiológico é o Streptococcus costumam ter evolução menos agressiva. Assim, a possibilidade de tratamento ambulatorial passa a ser atraente tanto do ponto de vista social como econômico. Esse tipo de tratamento é conhecido na literatura pela sigla OPAT (Outpatient Parenteral Antimicrobial Therapy) Objetivo: O objetivo deste estudo foi demonstrar que, em casos de endocardite estreptocócica bem selecionados, o tratamento ambulatorial é seguro e eficaz. Casuística e Método: Foram acompanhados seis pacientes com endocardite bacteriana por Streptococcus, diagnosticados pelos critérios modificados da Universidade de Duke, no período de janeiro de 2006 a novembro de 2008. Quatro pacientes eram do sexo feminino (66,6%) e dois eram do sexo masculino (33.4%). A idade média foi de 47,8 anos, variando de 32 a 65 anos. xi Todos os pacientes que eram elegíveis para o tratamento ambulatorial concordaram em fazer parte do estudo tendo sido tratados parcialmente ou integralmente em regime ambulatorial. Os exames de imagem e laboratoriais eram feitos no início e ao final do tratamento ou de acordo com a necessidade Resultado: Todos evoluíram sem complicações e com resolução completa do quadro infeccioso. São apresentados e comentados os resultados evolutivos dos exames laboratoriais e de imagem realizados. Conclusão: A endocardite bacteriana Estreptocócica pode ser tratada de forma segura em regime ambulatorial em pacientes selecionados, nos quais não existe concomitância de fatores agravantes, com conseqüente benefício para o paciente.

No Brasil, o transporte rodoviario e responsavel pela movimentacao de mais de 60% de toda a carga que trafega no territorio nacional, existindo cerca de 130 mil empresas de transporte de cargas no Brasil com mais de 1,6 milhoes de veiculos que oferecem servicos, segundo a Agencia Nacional de Transportes Terrestres. A funcionalidade dos pavimentos de fato se torna util, diante do conforto e seguranca, porem o principal comparativo na opcao de uma analise e a parte economica, pois a preferencia para esta opcao e bastante analisada. O comparativo entre pavimentos nao trata, evidentemente, de procurar a simples substituicao dos pavimentos betuminosos pelos de concreto, ignorando toda uma tecnologia desenvolvida durante muitos anos, trata-se de apresentar uma excelente opcao de pavimentacao e de expandir o uso de concreto como pavimento rodoviario. Os pavimentos rigidos apresentam melhores resultados, no aspecto de seguranca, conforto e sustentabilidade, que sao bens nao mensuraveis financeiramente como satisfacao, bem-estar, futuro do planeta, preservacao da vida.,

Resumen
This paper explores the meaning and distribution of obratno, one of the Russian repetitive and restitutive morphemes. We identify three essential characteristics of obratno: obligatoriness of the restitutive reading, narrow scope with respect of indefinites, and incompatibility with eventuality descriptions that entail a result state in the sense of [Kratzer 2000]. We argue that like garden-variety repetitive and restitutive morphemes (e.g., Russian opjat’), obratno denotes a partial identity function with a presupposition. Unlike such morphemes, however, the presuppositional content of obratno involves a return to the same state in which an entity had been before. We capture this characteristic relying on [Landman’s 2008] notion of crosstemporal identity of eventualities and the derivative notion of a cross-temporal substate. This makes the repetitive reading of obratno unavailable, forces identity of the holders of a state, deriving the narrow scope effect, and guarantees that obratno is only compatible with target state descriptions.
