La ligne directrice employée vise à confronter de façon permanente, progressive et itérative, modèle simulé et système réel dans un objectif de dimensionnement de partie opérative, de validation de modèle et d’algorithmes de commande en tirant partie d’outils logiciels de contrôle/commande temps-réel, de synthèse de code et de simulation multiphysique.

Résumé Cet article a pour objet de montrer l’importance pour la pratique éducative d’être guidée par un cadre théorique global et cohérent. Le modèle théorique présenté s’inspire de la théorie générale des systèmes et donne à la fois une analyse et une synthèse du système éducatif. Le modèle définit les composantes universelles et essentielles de tout système d’enseignement. Ces composantes du système éducatif doivent converger vers un même but, soit l’apprentissage individualisé. Le modèle théorique présenté est surtout heuristique, cherchant à définir les principes fondamentaux d’un enseignement qui répond aux besoins éducatifs de tous les apprenants.

Contexte. L’incertitude est inhérente à l’exercice de la médecine générale. Face aux plaintes des patients, le médecin généraliste (MG) peut se sentir démuni, indécis, anxieux, et doit trouver un moyen d’intégrer cette incertitude dans sa prise de décision. Cette composante a été étudiée dans diverses études qualitatives. Objectif. Apporter une analyse compréhensive de l’incertitude dans la prise de décision en soins premiers, à partir d’une métasynthèse qualitative. Méthodes. Analyse secondaire des études qualitatives : 1) revue de la littérature avec sélection des études qualitatives selon des critères d’inclusion et de qualité ; 2) identification de thèmes clés et « translation réciproque » des différents thèmes entre les études pour aboutir à une synthèse globale, avec une nouvelle interprétation ; 3) triangulation de deux chercheurs ; 4) protocole ENTREQ et guide eMERge pour la rédaction. Résultats. Dans la synthèse des 57 articles inclus, la prise de décision médicale en situation d’incertitude a pu être comprise comme : 1) une décision qui s’appuie sur le modèle de l’approche centrée sur le patient, par une relation de confiance et en s’adaptant au patient dans sa complexité ; 2) une décision guidée par un sentiment viscéral – le « gut-feeling » – visant à écarter le danger ; 3) une décision devant être partagée, de même que l’incertitude qui l’accompagne ; 4) une décision contextualisée, influencée par la notion de pression ; 5) une situation conditionnant le ressenti des MG vis-à-vis des expériences (manque de confiance, anxiété, stress). Conclusion. Cette synthèse originale précise comment la décision médicale partagée avec le patient, son entourage et les autres professionnels, ainsi que l’approche centrée sur le patient, permettent au MG de mieux tolérer son incertitude. Elle ne disparaît pas avec l’expérience, mais la façon de l’appréhender peut s’améliorer par une posture réflexive et en lien avec les valeurs du médecin. Elle est une compétence intrinsèque complexe du métier de MG.

Les travaux présentés dans cette synthèse portent essentiellement sur l'analyse de scènes à partir de caméras mobiles avec pour application immédiate l'apport d'une vision par ordinateur efficace dans les systèmes autonomes. Ils sont le fruit d'une décennie de recherches menées d'abord à l'INRETS (actuellement IFSTTAR : Institut français des sciences et technologies des transports, de l'aménagement et des réseaux ) puis à l'Université Paris Sud XI (Institut d'Électronique Fondamentale). L'idée initiale est que l'autonomie d'un système implique, ne serait-ce que pour raisons énergétiques, une faible variété d'opérateurs de perception, dont les algorithmes de vision. Les "primitives" extraites des images seront intrinsèquement robustes et stables vis-à-vis de perturbations variées. Elles doivent de plus anticiper, voire faciliter, un processus de décision à divers niveaux voulu systématique. Les lignes de niveaux répondent parfaitement à ces contraintes : on vérifie sans peine leur robustesse et leur abondance dans une image suggère et alimente un processus de décision cumulatif (manipulant un objet de décision unique : l'histogramme généralisé en espace de vote). Nos efforts se sont alors concentrés sur deux aspects : 1) le premier concerne la définition d'une méthodologie cohérente dans laquelle un processus primaire d'extraction de lignes de niveaux est enrichi afin de permettre la construction de primitives plus complexes guidée par un modèle de déformation de l'image. Le nombre de composants donc la forme des primitives est fonction directe du nombre de variables caractérisant le mouvement (déformation) à déterminer. 2) Le second intéresse une méthode de décision cumulative unifiée permettant de traiter des thèmes applicatifs de complexité croissante. Nos travaux se déclinent alors en trois niveaux de cumul, chacun associé de manière à un stade particulier de l'analyse d'images. Les thèmes applicatifs traités pour illustrer notre démarche sont de complexité croissante : détection et estimation du mouvement en caméra fixe, recalage d'images en caméra mobile (type de mouvement connu et profondeur des objets contrainte) puis estimation générale du mouvement propre et de la structure de la scène en caméras embarquées sur un véhicule mobile. Les résultats obtenus montrent comment un choix de primitives robustes associé à un processus de décision cumulatif permet la réutilisation des opérateurs dans plusieurs secteurs. Les systèmes proposés ont la particularité d'être compacts et cohérents, propriété recherchée dans les applications considérées.

Terpenoids are a large structurally diverse group of natural products with an array of functions in their hosts. The large amount of genomic information from recent sequencing efforts provides opportunities and challenges for the functional assignment of terpene synthases that construct the carbon skeletons of these compounds. Inferring function from the sequence and/or structure of these enzymes is not trivial because of the large number of possible reaction channels and products. We tackle this problem by developing an algorithm to enumerate possible carbocations derived from the farnesyl cation, the first reactive intermediate of the substrate, and evaluating their steric and electrostatic compatibility with the active site. The homology model of a putative pentalenene synthase (Uniprot: B5GLM7) from Streptomyces clavuligerus was used in an automated computational workflow for product prediction. Surprisingly, the workflow predicted a linear triquinane scaffold as the top product skeleton for B5GLM7. Biochemical characterization of B5GLM7 reveals the major product as (5S,7S,10R,11S)-cucumene, a sesquiterpene with a linear triquinane scaffold. To our knowledge, this is the first documentation of a terpene synthase involved in the synthesis of a linear triquinane. The success of our prediction for B5GLM7 suggests that this approach can be used to facilitate the functional assignment of novel terpene synthases.

Background Sesquiterpenes are 15-carbon terpenes synthesized by sesquiterpene synthases using farnesyl diphosphate (FPP) as a substrate. Recently, a sesquiterpene synthase gene that encodes a 65 kDa protein was isolated from the aromatic plant Persicaria minor. Here, we report the expression, purification and characterization of recombinant P. minor sesquiterpene synthase protein (PmSTS). Insights into the catalytic active site were further provided by structural analysis guided by multiple sequence alignment. Methods The enzyme was purified in two steps using affinity and size exclusion chromatography. Enzyme assays were performed using the malachite green assay and enzymatic product was identified using gas chromatography-mass spectrometry (GC-MS) analysis. Sequence analysis of PmSTS was performed using multiple sequence alignment (MSA) against plant sesquiterpene synthase sequences. The homology model of PmSTS was generated using I-TASSER server. Results Our findings suggest that the recombinant PmSTS is mainly expressed as inclusion bodies and soluble aggregate in the E. coli protein expression system. However, addition of 15% (v/v) glycerol to the protein purification buffer and removal of N-terminal 24 amino acids of PmSTS helped to produce homogenous recombinant protein. Enzyme assay showed that recombinant PmSTS is active and specific to the C15 substrate FPP. The optimal temperature and pH for the recombinant PmSTS are 30 °C and pH 8.0, respectively. The GC-MS analysis further showed that PmSTS produces β-sesquiphellandrene as a major product and β-farnesene as a minor product. MSA analysis revealed that PmSTS adopts a modified conserved metal binding motif (NSE/DTE motif). Structural analysis suggests that PmSTS may binds to its substrate similarly to other plant sesquiterpene synthases. Discussion The study has revealed that homogenous PmSTS protein can be obtained with the addition of glycerol in the protein buffer. The N-terminal truncation dramatically improved the homogeneity of PmSTS during protein purification, suggesting that the disordered N-terminal region may have caused the formation of soluble aggregate. We further show that the removal of the N-terminus disordered region of PmSTS does not affect the product specificity. The optimal temperature, optimal pH, Km and kcat values of PmSTS suggests that PmSTS shares similar enzyme characteristics with other plant sesquiterpene synthases. The discovery of an altered conserved metal binding motif in PmSTS through MSA analysis shows that the NSE/DTE motif commonly found in terpene synthases is able to accommodate certain level of plasticity to accept variant amino acids. Finally, the homology structure of PmSTS that allows good fitting of substrate analog into the catalytic active site suggests that PmSTS may adopt a sesquiterpene biosynthesis mechanism similar to other plant sesquiterpene synthases.

Rat Nap57 and its yeast homologue Cbf5p are pseudouridine synthases involved in rRNA biogenesis, localized in the nucleolus. These proteins, together with H/ACA class of snoRNAs compose snoRNP particles, in which snoRNA guides the synthase to direct site-specific pseudouridylation of rRNA. In this paper we present an Arabidopsis thaliana protein that is highly homologous to Cbf5p (72% identity and 85% homology) and NAP57 (67% identity and 81% homology). Moreover, the plant protein has conserved structural motifs that are characteristic features of pseudouridine synthases of the TruB class. We have named the cloned and characterized protein AtNAP57 (Arabidopsis thaliana homologue of NAP57). AtNAP57 is a 565 amino-acid protein and its calculated molecular mass is 63 kDa. The protein is encoded by a single copy gene located on chromosome 3 of the A. thaliana genome. Interestingly, the AtNAP57 gene does not contain any introns. Mutations in the human DKC1 gene encoding dyskerin (human homologue of yeast Cbf5p and rat NAP57) cause dyskeratosis congenita a rare inherited bone marrow failure syndrome characterized by abnormal skin pigmentation, nail dystrophy and mucosal leukoplakia.

Afin d'améliorer la sélectivité de la radiothérapie dans le cadre du traitement de certains cancers, nous proposons l'utilisation de nanoparticules radiosensibilisantes fonctionnalisées pour le suivi par imagerie médicale. Ces objets, constitués d'un coeur d'or recouvert d'une couche organique, permettent de combiner imagerie multimodale et activité thérapeutique télécommandée, ouvrant ainsi la voie vers la thérapie guidée par imagerie. L'effet radiosensibilisant pourra être généré au moment opportun, déterminé à partir de l'imagerie. L'étude de biodistribution par imagerie TEMP/CT chez des souris a montré une libre circulation des nanoparticules et une élimination rénale avec une présence modérée des nanoparticules dans le foie qui n'est, cependant, pas due à un phénomène d'opsonisation. L'étude par 1RM du cerveau de rats portant un gliosarcome a indiqué une accumulation significative des nanoparticules Au@TADOTAGA-Ge au sein de la tumeur. Enfin le co-marquage des nanoparticules d'or par Gcr et 64Cu2+ (complexées par les ligands macrocycliques) a permis de suivre ces objets, chez le même animal, après injection intraveineuse, à la Ibis par TEP et 1RM dont la combinaison bien que très recherchée est rarement mise en oeuvre. Le caractère radiosensibilisant de la suspension de nanoparticule Au@TADOTAGA a été confirmé par le suivi de croissance tumorale après traitement par irradiation MRT 15 minutes après l'injection des nanoparticules par voie intratumorale. Le gain thérapeutique de la mise en oeuvre de ce traitement a ensuite été validé par une irradiation MR'F 24 heures après l'injection intraveineuse des nanoparticules à des rats portant un gliosarcome<br>The original properties of nanoparticles make them extremely attractive in the field of oncology. In fast, gold nanoparticles coated by macrocyclic ligands allow imaging and therapy with only one object. Therefore, multifunctional platforms are very promising for image-guided therapy, winch constitutes an important step towards personalization of treatment. This consists of stimulating the therapeutic activity of the nanoparticles when their accumulation is high within the tumor zone and low in healthy tissues. A higher selectivity of the treatment is therefore expected. Biodistribution study by SPECT/CT has shown free circulation, renal elimination and a moderate retention by the liver of the nanoparticles. However, this retention is not due to the opsonisation processus. The MRI study of rats' brain carrying a gliosarcoma has shown an accumulation of nanoparticles Au@FADOTAGA-Gd in the tumor. Moreover, the co-labeling of these nanoparticles by Ge and 64Cts2+was successfully performed. As a result, PET/MRI images, a much researched combination but rarely achieved, were acquired on the same animal alter intravenous injection of the co-labeled nanoparticles. The radiosensitizing character of nanoparticles Au@TADOTAGA was confirmed by the follow up of tumor growth alter a treatment by MRT (microbeam irradiation) 15 minutes after intratumoral injection of nanoparticles. The therapeutic gain of this treatment has been validated by MRT 24 hours after intravenous injection of nanoparticles to rats carrying gliosarcoma (radioresistant tumor in radiosensitive organ)

La radiothérapie est aujourd'hui l'un des principaux traitements mis en œuvre dans le domaine de la lutte contre le cancer, malgré son manque de sélectivité dans la destruction des tissus. Afin d'augmenter la spécificité de la réponse tumorale à l'irradiation, l'accumulation au sein des tumeurs de nanoparticules radiosensibilisantes, capables d'augmenter les effets d'un traitement par irradiation, a été proposée. Le présent travail de thèse visait à synthétiser et à caractériser des nanoparticules d'or multifonctionnelles (Au@DTDTPA) pouvant être suivies in vivo par imagerie X, IRM et scintigraphie y et à évaluer leur capacité à être utilisées comme agents radiosensibilisants dans le cadre de la radiothérapie guidée par imagerie. L'étude de la biodistribution chez le rat sain des nanoparticules Au@DTDTPA réalisée par imagerie X, IRM et scintigraphie y a montré qu'elles circulent librement dans le flux sanguin sans être détectées par le système immunitaire et qu'elles sont excrétées par l'urine via la filtration rénale. Chez le rat porteur d'une tumeur cérébrale, les nanoparticules s'accumulent de façon transitoire dans la zone tumorale avant d'être éliminées par la voie urinaire. Le caractère radiosensibilisant des nanoparticules Au@DTDTPA a été démontré in vitro, en quantifiant l'augmentation du nombre de dommages infligés à l'ADN après irradiation de cellules de glioblastome humain U87 marquées par les nanoparticules par rapport aux cellules non marquées, à l'aide du test des comètes. La capacité de ces nanoparticules à augmenter les effets d'une irradiation a ensuite été démontrée in vivo chez des rats porteurs d'une tumeur osseuse agressive (ostéosarcome). L'injection intratumorale de nanoparticules Au@DTDTPA réalisée avant l'irradiation de la tumeur a permis de prolonger la survie de rats et de différer la réapparition de la tumeur par rapport aux groupes contrôles. Le suivi par IRM de l'accumulation des nanoparticules Au@DTDTPA au sein du site tumoral après leur injection intraveineuse chez des rats porteurs d'une tumeur cérébrale (gliosarcome 9L) a permis de mettre en œuvre l'irradiation par microfaisceaux X lorsque la répartition en nanoparticules était favorable. La survie des rats traités par cette méthode a été prolongée de façon importante par rapport aux groupes contrôles. Ce travail a révélé le potentiel des nanoparticules Au@DTDTPA pour la radiothérapie guidée par imagerie, qui repose à la fois sur la possibilité de les suivre par différentes techniques d'imagerie médicale (imagerie X, IRM, scintigraphie y) et sur la capacité d'exercer un effet radiosensibilisant après absorption de photons X<br>Radiation therapy is nowadays one of the most common treatments in the fight against cancer, in spite of its lack of selectivity in tissue destruction. A better selectivity is expected to be achieved when radiosensitizing nanoparticles are used for enhancing the dose effects of the ionizing radiations. The aim of this PhD work is to synthesize and to characterize multifunctional gold nanoparticles (Au@DTDTPA), detectable in vivo through various techniques such as by X-ray imaging, MRI and y-ray scintigraphy, as well as evaluating their capability to be used as image-guided radiosensitizing agents to treat tumors. The biodistribution of the Au@DTDTPA nanoparticles in healthy rats is monitored by X-ray imaging, MRI and y-imaging and shows that they freely circulate in the blood flow without being detected by the immune system and that they are excreted by the urine through renal filtration. In the case of cerebral tumor bearing rats, the gold nanoparticles accumulate temporarily in the tumoral zone before being eliminated by urine. The radiosensitizing behavior of the Au@DTDTPA nanoparticles was demonstrated in vitro using the comet assay, by quantifying the increase of DNA strand breaks caused to nanoparticle-labeled U87 human glioblastoma cells, compared to the unlabeled cells. The capability of these nanoparticles to increase the effects of ionizing radiations was then shown in vivo with rats bearing aggressive bone tumors (osteosarcoma). When an intratumoral injection of Au@DTDTPA nanoparticles was performed before the tumor irradiation, the rats' median survival time was prolonged and the tumor reappearance was differed, compared to the control groups. The MRI monitoring of the accumulation of the Au@DTDTPA nanoparticles on the tumoral site of gliosarcome 9L bearing rats after intravenous injection permitted the irradiation by X-ray microbeams when the nanoparticles distribution was favorable. The median survival time of nanoparticle treated rats was indeed extended compared to the control groups. This work therefore showed the possibility to use Au@DTDTPA nanoparticles combining imagery and therapy features as image-guided radiosensitizing agents to treat tumors. The increase of the retention time of Au@DTDTPA nanoparticles within the tumor site could prolong threated rats’ lifespan

Dans le domaine infographie, l'animation par ordinateur désigne notre capacité à faire se mouvoir, via un ensemble de programmes ou d'algorithmes, des représentations géométriques virtuelles d'objets le plus souvent réels. Si les jeux vidéos ou le champ des effets spéciaux sont les premiers domaines d'application, le développement des technologies de l'information dans notre vie de tous les jours a ouvert la voie à un grand nombre d'autres utilisations. L'animation à proprement parler nécessite de spécifier le comportement dans l'espace et le temps de l'objet considéré. Il existe un grand nombre de méthodes permettant de réaliser cette spécification. Parmi celles-ci, la catégorie des méthodes dites "basées données" permettent d'obtenir un réalisme important en s'appuyant sur des exemples capturés du monde réel. A ces méthodes sont associés trois problèmes fondamentaux : - un problème d'acquisition : quels sont les bons descripteurs du mouvement ? Comment, et via quel médium, les capter du monde réel ? - un problème de généralisation : comment généraliser l'information obtenue à partir de quelques exemples ? Comment en caractériser leur portée ? - un problème de contrôle : par quel biais introduire cette information dans une boucle d'animation ? Peut on la modifier pour l'adapter à des cadres différents de ceux de la captation ? Pris ensemble, ces problèmes permettent d'établir des schémas d'Analyse/Synthèse, où l'information peut être capturée grâce à une formalisation (un modèle) numérique du phénomène considéré, puis utilisée à son tour dans la définition même du modèle. Ce document présente des travaux de recherche dans cette direction menés par moi et mon équipe depuis 2004. Deux domaines d'application "phare" sont considérés : l'animation de personnages virtuels, et plus particulièrement des personnages doués de la faculté de communiquer en langue des signes, et la simulation de foules de gens guidée par les données. Dans les deux cas est montré comment les données peuvent interagir avec les modèles numériques. Nous tentons même de mettre en avant l'émergence de cercles vertueux où l'on s'aperçoit que de meilleures données entrainent de meilleurs modèles, qui à leur tour autorisent la captation de nouvelles données et ainsi de suite. Ainsi cette thématique ouvre un large panel de problèmes évoluant de l'apprentissage à la simulation numérique.

L’utilisation de dispositifs de synthèse d’ouverture présente des avantages indéniables autant pour réaliser des systèmes récepteurs que pour réaliser des systèmes émetteurs à haute résolution angulaire. Dans ce type d’instruments, les chemins optiques des différents bras interférométriques du réseau doivent être contrôlés avec une grande précision afin que le dispositif puisse fonctionner en mode cohérent. Ce manuscrit présente la mise en oeuvre en laboratoire de deux dispositifs de synthèse d’ouverture dont la finalité est d’être utilisée en réception pour le premier et en émission pour le second. La première étude a consisté à étudier et à mettre en place expérimentalement un dispositif de métrologie externe d’une antenne synthétique destinée à l’observation d’objets stellaires. Ce projet se place dans le cadre d’un instrument spatialisable et met à profit les techniques fibrées disponibles au laboratoire. Cette étude est particulièrement focalisée sur le contrôle de la différence de marche entre la source et le foyer des télescopes. Cette fonction est assurée entre autre à l’aide d’un asservissement de la position des différentes ouvertures du dispositif dans le plan d’onde de l’objet observé en utilisant une méthode interférométrique. Le dispositif expérimental de laboratoire constitué de 2 télescopes séparés de 2,5 mètres a permis d’obtenir des franges d’interférence avec des contrastes variant entre 85 %et 92 %. Le principe de positionnement des ouvertures dans le plan d’onde de la source scientifique a été validé expérimentalement dans cette étude. La seconde partie du travail a consisté à réaliser l’étude théorique et la mise en oeuvre expérimentale d’un dispositif émetteur (Antenne Synthétique) constitué d’un réseau de lentilles alimentées par des fibres optiques. Un réseau de deux lentilles a été mis en place et a permis dans un premier temps de démontrer expérimentalement la possibilité de réaliser un pointage précis, sur une plage de 138 μrad par un contrôle précis du tilt et du piston de chaque onde élémentaire. Dans un second temps l’obtention d’un signal d’erreur destiné à l’asservissement du chemin optique, sur les différents bras de l’antenne, a été réalisé. Les résultats obtenus montrent que les fluctuations résiduelles de différences de marche resteraient inférieures à λ/32 en boucle ouverte ce qui est inférieure à λ/24 erreur à partir de laquelle nous pouvons constater une dégradation notable de la figure de diffraction de l’antenne<br>The use of synthesize aperture setup offers undeniable advantages moreover for realizing high angular resolution receiving as for transmitting setup. In this kind of instrument, the optical path on the different interferometric arms of the array must be controlled with high accuracy in order to allow the setup to work in coherent mode. This manuscript presents the laboratory implementation of two synthesize aperture setup which the goal is to be used; as a receiver for the first and as a transmitter for the second. The first study has consisting in implementing an external metrology setup of a synthesize antenna destined to observed stellar objects. This project takes place within the framework of a spatial instrument using fibred technique available in our laboratory. This study is particularly focused on the control of the path difference between the source and the telescope focus. This function is achieved among other by the servo control of the position of the different aperture of the setup in the wave plane of the observed object by using an interferometric method. The experimental setup composed of two apertures spaced of two meters allows obtaining interference fringes with contrasts varying between 85% and 92 %. The principle of positioning of the apertures in the wave plane of the scientific source has been validated in this study. The second study has consisting in realizing a transmitting synthesize antenna composed of a lenses array fed by optical fibre. In a first time, a two lenses array has been experimentally implemented and has shown the possibility to realize the pointing of the antenna with several steps in a range of 138 μ rad. In a second time, an error signal destined to servo control the optical path difference between the two arms of the antenna has been obtained. The results on the error signal shows that the optical path residual fluctuation will stay lower than λ/32 in the case of open loop. This fluctuation stays lower than λ/24 error from which we can notice a notable deterioration of the diffraction pattern of the antenna

Dans la recherche de nouveaux traitements des maladies broncho-pulmonaires comme l'asthme et la broncho-pneumopathie chronique obstructive, les inhibiteurs de PDE4 sont des cibles intéressantes. Dans cette voie, notre laboratoire s'intéresse à la synthèse d'une nouvelle famille d'inhibiteurs à base pyridazinone. Pour cela, cette thèse couple la modélisation moléculaire (docking : développement méthodologique et application), la synthèse organique ainsi que des tests biologiques.Dans un premier temps, le développement du logiciel d'amarrage moléculaire AlgoGen a été poursuivi. AlgoGen (créé initialement à l'université de Lorraine en 2009) est un logiciel qui couple un algorithme génétique pour la recherche conformationnelle à une évaluation de l'énergie protéine-ligand à un niveau quantique semi-empirique alors que les autres logiciels existant effectuent ce calcul à un niveau classique en général. Le calcul d'une énergie à un niveau quantique est très coûteux en temps. C'est pourquoi, nous avons apporté de nombreuses modifications à ce logiciel afin d'accroître son efficacité dans la recherche conformationnelle. Ce logiciel a ensuite été utilisé sur un jeu de 22 dimères (typiques des reconnaissances moléculaires biologiques) et à huit systèmes protéine-ligand.Dans un deuxième temps, huit inhibiteurs ont été synthétisés et testés in vitro sur la cible PDE4. Pour compléter ce volet expérimental, une étude de structure-activité a été effectuée grâce au docking moléculaire (AlgoGen, Autodock, Glide) pour rationaliser les activités mesurées (IC50). Pour terminer, des pharmaco-modulations guidées par docking ont été réalisées afin de proposer de nouveaux inhibiteurs de plus grandes affinités avec la protéine PDE4D<br>For the research of new treatment of bronchopulmonary diseases such as asthma and chronic obstructive pulmonary disease (COPD), the PDE4 inhibitors are an attractive target. Our laboratory is interested in a new PDE4 inhibitors family based on the pyridazinone pattern. For this purpose, this thesis couples molecular modeling (docking: methodological development and application), organic synthesis and biological tests.First, the development of the molecular docking software AlgoGen was continued. AlgoGen (initially created at university of Lorraine in 2009) is a program which couples a genetic algorithm for the conformational research and a protein-ligand energy evaluation at the quantum semi-empirical level while other software do this evaluation at a classical level. Quantum energy calculations are very time consuming. That is the reason why some modifications have been made to improve its efficiency for the conformational search. This software was then used for calculations on a set of 22 dimers (typical in biological molecular recognition) as well on 8 ligand-protein complexes.Secondly, eight inhibitors were synthesized and tested in vitro on the PDE4 target. To complete the experimental part, a structure-activity relationship study was carried out through a molecular docking to rationalize the measured activity (IC50). Finally, pharmaco-modulations guided by docking were made to propose new inhibitors with more affinity with the protein

Les virus de l'ordre des Bunyavirales constituent un problème de santé publique en raison du manque d’options thérapeutiques et de leur répartition mondiale. La protéine L gouvernant le système de réplication virale possède une activité endonucléase responsable du mécanisme du vol-de-coiffe permettant la transcription. Cette enzyme clé a été identifiée comme une cible thérapeutique de choix pour développer des antiviraux. Son mécanisme d’hydrolyse de l’ARN médié par les ions Mg2+ permet de développer des inhibiteurs chélateurs de métaux de type dicéto-acide (DCA). La synthèse guidée par la cible biologique est une méthode puissante qui met au centre la cible thérapeutique qui assemble ses propres inhibiteurs en son site-actif. Nous décrivons l’utilisation des endonucléases de Bunyavirales comme « moule de réaction » pour la génération de leurs propres inhibiteurs chélateurs de métaux. Une librairie de 25 composés N-triazolyl-DCA (NT-DCA), C-triazolyl-DCA (CT-DCA) et C-triazolyl-DHP (CT-DHP) a été générée par des voies de synthèses nouvelles par chimie click. Les composés ont été caractérisés biophysiquement par Thermophorèse à Micro-échelle (MST) et par Fluorimétrie Différentielle à Balayage (DSF) et leur pouvoir inhibiteur a été évalué in vitro et dans des cellules infectées. De plus, la synthèse d’un ligand fluorescéine-DCA a été conduite pour développer un test par compétition par Polarisation de Fluorescence (FP) pour tester l’activité des composés. Des ligands à haute affinité possédant une forte activité in vitro et en cellules infectées ont été identifiés. Dans le contexte de la pandémie de COVID-19, la librairie a été repositionnée sur l'exonucléase du SARS-CoV-2<br>With a worldwide repartition and limited therapeutic options Bunyavirales viruses represent a major public health issue. The replication machinery of these viruses is governed by the intricate L-protein that displays an endonuclease activity responsible for the cap-snatching mechanism that allows viral transcription. This key enzyme was identified as a promising target to develop antivirals. Its catalytic mechanism of RNA hydrolysis mediated by Mg2+ ions enables the development of diketo acid (DKA) metal-chelating inhibitors. Target-Guided-Synthesis (TGS) is a powerful method that directly involves the therapeutic target that assembles its own inhibitors in its active site like LEGOs®. Herein, we describe the use of Bunyavirales endonucleases as reaction vessels for the In Situ generation of metal-chelating inhibitors. A library of 25 N-triazolyl-DKA (NT-DKA), C-triazolyl-DKA (CT-DKA) and C-triazolyl-DHP (CT-DHP) compounds was generated with new and original synthesis pathways via click chemistry. The compounds were biophysically characterized by Microscale Thermophoresis (MST) and Differential Scanning Fluorimetry (DSF) and their inhibitory potency was evaluated in vitro and in infected cells culture. Moreover, a fluorescein-DKA ligand was synthesized for the development of a competition Fluorescence Polarization (FP) assay to determine the activity of the compounds. High-affinity ligands that demonstrated a potent efficiency in vitro and in cellula were identified. In the context of the COVID-19 pandemic, the compounds library was repositioned against SARS-CoV-2 exonuclease which opened new possibilities for the development of metal-chelating Coronavirus inhibitors

Le développement de nouveaux antibiotiques est un enjeu majeur de santé publique. Etant donné, le fort potentiel des aminoglycosides en tant qu’antibiotique, ces composés ont attisé l’intérêt de plusieurs groupes de recherche. Cependant, leur usage est encore très limité, malgré leur ancienneté, du fait de leur toxicité et du développement toujours croissant des mécanismes de résistances aux aminoglycosides. Afin de mieux appréhender les problèmes inhérents à leur utilisation, il est crucial de mieux comprendre leur action sur les différentes cibles cellulaires, et d’étudier leur interaction avec leur cible moléculaire (ARN et protéine). En plus de leur pouvoir antibiotique, les aminoglycosides sont également des ligands universels pour des ARN, capables d’interagir spécifiquement avec notamment les ARN du VIH-1 suivants : DIS, TAR, RRE. L’élaboration d’aminoglycosides modifiés présente un énorme avantage car le domaine d’application, et en conséquence les retombées, sont grandes. Néanmoins, la complexité structurale de ces molécules est un frein majeur, la fonctionnalisation chimiosélective est indispensable mais malheureusement peu décrite dans la littérature. Dans le cadre de ce travail, nous avons développé deux types d’approches pour cibler le DIS et/ou le site A du ribosome bactérien. La première originale, mais risquée se base sur le concept de click in situ. La seconde approche est traditionnelle et est basée sur la fonctionnalisation sélective de certaines positions clés des aminoglycosides<br>The development of new antibiotics is a major public health issue. Given the high potential of aminoglycosides as antibiotics, these compounds have aroused great interest in many research groups. However, despite their maturity, their use is still limited because of their toxicity and the increasing development of resistance mechanisms to aminoglycosides. To better understand the problems inherent to their use, it is crucial to understand their action a cellular level, and to study the interactions with their molecular targets (RNA and protein). In addition to their antibiotic power, aminoglycosides are also universal ligands for several RNAs, capable of specific interactions with RNAs of HIV-1: DIS, TAR and RRE. The elaboration of modified aminoglycosides presents a huge advantage because the domain of application, and therefore the benefits, are important. Nevertheless, the structural complexity of these molecules is a major constraint, chemoselective functionalization is essential but unfortunately poorly described in the literature.In this work, we developed two approaches to target the DIS and/or the A site of the bacterialribosome. The first one, unique but challenging is based on the concept of in situ click chemistry. The second approach is conventional and is based on the selective functionalization of some keypositions of aminoglycosides

Les matériaux photoluminescents ont un rôle important dans les applications liées à la photonique, l'optoélectronique ou l'éclairage. Leur combinaison avec des polymères conduit à des matériaux fonctionnels polyvalents faciles à mettre en forme. Dans ce contexte, des matériaux hybrides composés d'une matrice organique contenant des clusters métalliques octaédriques fortement luminescents ont été préparés. Ces luminophores présentent plusieurs avantages : une forte luminescence dans le rouge et le proche-IR, un rendement quantique élevé, un large décalage de Stokes et aucun effet de photoblanchiment ou de photoscintillement. Comme matrice hôte, le PMMA et le PDMS ont été utilisés en raison de leurs propriétés optiques, mécaniques et leur stabilité thermique. Cependant, une condition nécessaire pour réaliser les matériaux hybrides est d'introduire de manière homogène l'émetteur inorganique dans le polymère organique afin d'éviter les phénomènes de ségrégations. Pour cela, plusieurs approches d'intégration ont été développées. Les analyses spectroscopiques effectuées sur ces hybrides ont montré leur grand potentiel d’applications. Ceux-ci ont été utilisés pour concevoir un matériau imprimable à la lumière, sans encre et auto-effaçable, lorsque qu’il est combiné à un fluorophore organique ; ainsi que pour l’élaboration de guides d’ondes 1D et 2D. Les guides 1D peuvent être appliqués en photonique intégrée, tandis que les guides 2D combinés à des cristaux liquides conduisent à un nouveau matériau PDLC (Polymer Dispersed Liquid Crystal) applicable pour des concentrateurs solaires luminescents<br>Photoluminescent materials are playing a major role in applications related to photonics, optoelectronics or lighting. Combining them with polymers allows the design of easy-to-shape-functional materials with enhanced application versatility. Related to this, hybrid materials composed of an organic polymer matrix containing strongly luminescent octahedral metal clusters were prepared. Octahedral metal clusters have several advantages: a deep luminescence in the red and the near-IR region, a high quantum yield, a large Stokes shift and no photobleaching or photoblinking effects. As organic host matrices, PMMA and PDMS were used because of their optical and mechanical properties in addition to their thermal stability. However, a prerequisite for realizing hybrid materials is to homogeneously introduce the ceramic inorganic emitter in the organic polymer to avoid segregation-phase phenomena. For this purpose, several integration approaches were developed to prepare new hybrid materials. Meanwhile, spectroscopic analysis were performed on these hybrids demonstrating their huge potential. Actually, these hybrids were used to design a self-erasable inkless light-imprinting material when the hybrid is combined with an organic fluorophore, and 1D- and 2D-waveguides. The 1D-waveguides can be applied for integrated photonics while 2D-waveguides combined with liquid-crystals lead to a new luminescent PDLC (Polymer Dispersed Liquid Crystal) material for Luminescent Solar Concentrator technology

AbstractMeasuring the rate at which the universe expands at a given time–the ‘Hubble constant’– has been a topic of controversy since the first measure of its expansion by Edwin Hubble in the 1920s. As early as the 1970s, Sandage and de Vaucouleurs have been arguing about the adequate methodology for such a measurement. Should astronomers focus only on their best indicators, e.g., the Cepheids, and improve the precision of this measurement based on a unique object to the best possible? Or should they “spread the risks”, i.e., multiply the indicators and methodologies before averaging over their results? Is a robust agreement across several uncertain measures, as is currently argued to defend the existence of a ‘Hubble crisis’ more telling than a single 1% precision measurement? This controversy, I argue, stems from a misconception of what managing the uncertainties associated with such experimental measurements require. Astrophysical measurements, such as the measure of the Hubble constant, require a methodology that permits both to reduce the known uncertainties and to track the unknown unknowns. Based on the lessons drawn from the so-called Hubble crisis, I sketch a methodological guide for identifying, quantifying and reducing uncertainties in astrophysical measurements, hoping that such a guide can not only help to re-frame the current Hubble tension, but serve as a starting point for future fruitful discussions between astrophysicists, astronomers and philosophers.

AbstractIt is generally accepted that science sometimes involves reasoning with analogies. Often, this simply means that analogies between disparate objects of study might be used as heuristics to guide theory development. Contemporary black hole physics, however, deploys analogical reasoning in a way that seems to overreach this traditional heuristic role. In this chapter, I describe two distinct pieces of analogical reasoning that are quite central to the contemporary study of black holes. The first underpins arguments for the existence of astrophysical Hawking radiation, and the second underpins arguments for black holes being ‘genuinely’ thermodynamical in nature. I argue that while these are distinct analogical arguments, they depend on one another in an interesting way: the success of the second analogical argument presupposes the success of the first. This induces a tension for those who wish to take black hole thermodynamics seriously, but who are sceptical of the evidence provided for astrophysical Hawking radiation by the results of analogue gravity. I consider three ways to resolve this tension, and show that each fails.

AbstractDomestic dogsand cats have evolved differentially in some aspects of nutrition, metabolism, chemical sensing, and feedingbehavior. The dogs have adapted to omnivorous dietscontaining taurine-abundant meat and starch-rich plant ingredients. By contrast, domestic catsmust consumeanimal-sourced foodsfor survival, growth, and development. Both dogsand catssynthesize vitamin C and many amino acids (AAs, such as alanine, asparagine, aspartate, glutamate, glutamine, glycine, proline, and serine), but have a limited ability to form de novo arginineand vitamin D3. Compared with dogs, cats have greater endogenousnitrogen losses and higher dietary requirements for AAs (particularly arginine, taurine, and tyrosine), B-complex vitamins (niacin, thiamin, folate, and biotin), and choline; exhibit greater rates of gluconeogenesis; are less sensitive to AA imbalances and antagonism; are more capable of concentrating urine through renal reabsorption of water; and cannot tolerate high levels of dietary starch due to limited pancreatic α-amylase activity. In addition, dogs can form sufficient taurinefrom cysteine(for most breeds); arachidonic acidfrom linoleic acid; eicosapentaenoic acid and docosahexaenoic acid from α-linolenic acid; all-trans-retinol from β-carotene; and niacinfrom tryptophan. These synthetic pathways, however, are either absent or limited in all cats due to (a) no or low activities of key enzymes (including pyrroline-5-carboxylate synthase, cysteinedioxygenase, ∆6-desaturase, β-carotene dioxygenase, and quinolinate phosphoribosyltransferase) and (b) diversion of intermediates to other metabolic pathways. Dogs can thrive on one large meal daily, select high-fat over low-fat diets, and consume sweet substances. By contrast, cats eat more frequently during light and dark periods, select high-protein over low-protein diets, refuse dryfood, enjoy a consistent diet, and cannot taste sweetness. This knowledge guides the feeding and care of dogsand cats, as well as the manufacturing of their foods. As abundant sources of essentialnutrients, animal-derivedfoodstuffs play important roles in optimizing the growth, development, and health of the companionanimals.

The increasing use of online discussion forum (ODF) as an online computer-mediated communication (CMC) system that allows students to construct knowledge, collaborate and engage with new knowledge has resulted in a growing body of research that aimed at finding ways to enhance student learning outcomes using this platform. After an intensive web search was conducted using Google Scholar, seven previous systematic reviews that studied research trends in ODF were found. These studies have conducted systematic reviews on many different trends, factors and conditions to enhance student learning outcomes in ODF. The aim of the present study was to gather, analyze, and synthesize the seven previous systematic reviews in identifying holistic trends, factors, and conditions that have been reviewed and reported by them towards understanding the successful enhancement of student learning outcomes in this platform. The potential combinations of trends, factors, and conditions that emerged from the syntheses can act as a more holistic predetermined trends, factors and conditions for future improvement on the systematic review research. This study identified and categorized the trends, factors and conditions emerged under three different categories (i.e. general current trends, factors influencing ODF learning outcome (independent variables), and the achievement of ODF learning outcomes (dependent variables)). First and foremost, this study found that the general current trends reported on the ODF research area were based on the research site by region, academic context and academic discipline, forum platform, number of papers per year and research papers per journal, research design, type of analysis, and analysis framework. Next, in terms of the factors and conditions influencing the learning outcome of ODF, previous systematic reviews have reported their findings in terms of (i) stating the purpose in learning activity in ODF, (ii) protocol used in ODF, (iii) pedagogical strategies used in ODF, (iv) assessment used in ODF, (v) moderation used in ODF, (vi) student characteristics in ODF, and (vii) technology issues in ODF. Finally, in terms of the factors and conditions influencing the achievement of ODF learning outcome, they have reported their findings in terms of (i) thinking skills, (ii) achievement, (iii) social construction of knowledge, (iv) communication skills, and (v) others. The findings can help guide novice researchers in finding research gap from previous ODF studies and create opportunity that include forming an important reference base for a more holistic future systematic review in ODF field.

Cette note stratégique résume les considérations clés, en mars 2021, inhérentes au contexte social, politique et économique de la flambée épidémique de maladie à virus Ébola (MVE) survenue dans la préfecture de N’Zérékoré, en Guinée. La flambée épidémique a été déclarée le 14 février 2021, deux semaines après le décès du premier cas connu, une agente technique de santé (ATS) originaire de Gouécké. La ville de Gouécké est située à 40 km au nord de N’Zérékoré par la Route Nationale 2 goudronnée. L’agente de santé avait reçu des soins dans un centre de santé à Gouécké, ainsi que dans une clinique et auprès d'un guérisseur traditionnel à N’Zérékoré. Elle est décédée à N’Zérékoré le 28 janvier. Lorsqu’ils sont tombés malades, les proches du premier cas connu se sont rendus à l’hôpital régional de N’Zérékoré, où ils ont transmis la MVE à des membres du personnel soignant. Bien que le risque de transmission dans les zones rurales de la souspréfecture de Gouécké soit élevé, jusqu'à présent, la majorité des cas ont été signalés dans le milieu urbain de N’Zérékoré, qui fait l'objet de cette note stratégique. À la date de la rédaction (22 mars), le nombre total de cas s'élève à 18 (14 cas confirmés, 4 cas potentiels), avec 9 décès et 6 guérisons. Le dernier nouveau cas a été signalé le 4 mars.

The purpose of this document is to provide readers with the coding protocol that authors used to code 36 research syntheses (including meta-analyses, evidence-based reviews, and quantitative systematic reviews) focused on mathematics interventions for students with learning disabilities (LD), mathematics learning disabilities (MLD), and mathematics difficulties (MD). The purpose of the systematic review of mathematics intervention syntheses was to identify patterns and gaps in content areas, instructional strategies, effect sizes, and definitions of LD, MLD, and MD. We searched the literature for research syntheses published between 2000 and 2020 and used rigorous inclusion criteria in our literature review process. We evaluated 36 syntheses that included 836 studies with 32,495 participants. We coded each synthesis for variables across seven categories including: publication codes (authors, year, journal), inclusion and exclusion criteria, content area focus, instructional strategy focus, sample size, methodological information, and results. The mean interrater reliability across all codes using this coding protocol was 90.3%. Although each synthesis stated a focus on LD, MLD, or MD, very few students with LD or MLD were included, and authors’ operational definitions of disability and risk varied. Syntheses predominantly focused on word problem solving, fractions, computer- assisted learning, and schema-based instruction. Syntheses reported wide variation in effectiveness, content areas, and instructional strategies. Finally, our results indicate the majority of syntheses report achievement outcomes, but very few syntheses report on other outcomes (e.g., social validity, strategy use). We discuss how the results of this comprehensive review can guide researchers in expanding the knowledge base on mathematics interventions. The systematic review that results from this coding process is accepted for publication and in press at Learning Disabilities Research and Practice.

Remote sensing using Synthetic Aperture Radar (SAR) offers powerful methods for monitoring ground deformation from both natural and anthropogenic sources. Advanced analysis techniques such as Differential Interferometric Synthetic Aperture Radar (DInSAR), change detection, and Speckle Offset Tracking (SPO) provide sensitive measures of ground movement. With both the RADARSAT-2 and RADARSAT Constellation Mission (RCM) SAR satellites, Canada has access to a significant catalogue of SAR data. To make use of this data, the Canada Centre for Mapping and Earth Observation (CCMEO) has developed an automated system for generating standard and advanced deformation products from SAR data using both DInSAR and SPO methods. This document provides a user guide for this automated processing system.

The Global Meeting on Postabortion Care: Advances and Challenges in Operations Research was held at the Population Council’s New York office on January 19–21, 1998, to review progress made by three regional operations research (OR) projects in improving delivery of postabortion care. The resources of the three OR contracts were pooled, and, in collaboration with Ipas and the Ebert Program, approximately 65 researchers, senior program managers, and government officials from Africa, Latin America, the Middle East, and Southeast Asia were convened. Three objectives guided the planning of the meeting: review progress in the development of OR methodologies pertaining to postabortion care, identify elements in the design of new postabortion care services, and suggest future areas for postabortion care OR. The meeting was organized around the presentation of 20 papers distributed to all participants several weeks earlier. Discussants presented critical syntheses of the papers and indicated directions for future OR studies. This report presents a summary overview of the principal themes and key points drawn from the papers and discussions, organized according to the theme of each session.

As of April 19, 2021, South Africa has recorded 1.56 million COVID-19 cases and almost 54,000 deaths - more than any other country on the African continent. The country has begun the national rollout of the Johnson & Johnson (J&J) COVID-19 vaccine, with over 292 thousand doses administered it aims to achieve herd immunity by vaccinating at least 67 percent of its population (around 40 million people) by the end of 2021. The government suspended its initial rollout of the AstraZeneca (AZ) vaccine due to concerns over its effectiveness, particularly against the new B.1.351 variant, which accounts for 90% of the infections in South Africa. The J&J vaccine was put on temporary hold in April due to concerns about rare clotting disorders. Although data show that expected acceptance of COVID-19 vaccines is relatively high, the suspension of two vaccines in South Africa, where fear of infection is decreasing, will likely influence public reactions. Understanding how individuals and population groups perceive and make sense of COVID-19 vaccines is critical to inform the design and implementation of risk communication and community engagement (RCCE) strategies, and guide interventions aiming to promote and sustain acceptance of COVID-19 vaccines, while encouraging compliance with other COVID-19 preventive measures. This review syntheses community perceptions of COVID-19 vaccines in South Africa to inform RCCE strategies and policies and provides examples of successful practice. It draws on multiple secondary data sources: scientific literature, qualitative and quantitative studies, grey literature, and mainstream and social media. The review was supported by consultation with four local expert key informants from different fields. It is part of the Social Science in Humanitarian Action Platform (SSHAP) series on social science considerations relating to COVID-19 vaccines. It was written for SSHAP by Tamara Roldan de Jong and Anthrologica on request of the UNICEF South Africa Country Office. Caribou (Rangifer tarandus) are an integral part of the ecological and cultural fabric of northwest Alaska. Western Arctic Herd (WAH) caribou roam over this entire region, including all 5 Arctic Network (ARCN) Inventory and Monitoring Program?s National Park units. Conservation of healthy caribou populations is specifically mentioned within the enabling legislation (Alaska National Interested Lands Conservation Act or ANILCA) for 3 of these park units and is of importance to subsistence hunters. Caribou are, by far, the most abundant large mammal in northwest Alaska and are famous for their long-distance migrations and large population oscillations. For these reasons, the ARCN parks chose WAH caribou as a Vital Sign for long-term monitoring. This report documents the monitoring results of this Vital Sign during its 14th year (September 2022?August 2023) of implementation. Results from the previous years of monitoring are also included for ease of comparison. Periodic syntheses of these data will be performed and reported on as appropriate. National Park Service (NPS) monitoring of the WAH is done in conjunction and cooperation with the Alaska Department of Fish and Game (ADFG). Thanks to a 2015 data sharing agreement, the report includes data funded by the ADFG dating back to September 2013. That particular year was chosen as it represents when the ADFG started deploying a substantial number of GPS collars on an 8-hour relocation schedule. Monitoring of the herd relies heavily on the use of Global Positioning System (GPS) radio telemetry collars that are capable of transmitting location data to a satellite. Given the extremely remote area that the WAH inhabits, this system provides the most efficient and accurate means to track individual caribou. These data are utilized to monitor the timing and location of migrations, as well as seasonal distributions of WAH caribou. Monitoring movement and the phenology of movement is perhaps the simplest means to track the influences of climate change, natural perturbations, development, and other potential impacts on a species?an analysis of which is outside the scope of this current report. This report also documents the NPS commitment and involvement with the WAH Working Group. The group is composed of important stakeholders including representatives for rural villages, sport hunters, conservationists, hunting guides, hunting transporters, and reindeer herders. In addition, all the agencies charged with managing the WAH, including the ADFG, NPS, US Fish and Wildlife Service (FWS) and Bureau of Land Management (BLM), serve as advisors to the group. Information gathered by the Caribou Vital Sign monitoring program are intended to supplement and complement existing data streams gathered by the other cooperating agencies and will be important in future management decisions.

Overview Skin cancer prevention is a component of the new Cancer Plan 2022–27, which guides the work of the Cancer Institute NSW. To lessen the impact of skin cancer on the community, the Cancer Institute NSW works closely with the NSW Skin Cancer Prevention Advisory Committee, comprising governmental and non-governmental organisation representatives, to develop and implement the NSW Skin Cancer Prevention Strategy. Primary Health Networks and primary care providers are seen as important stakeholders in this work. To guide improvements in skin cancer prevention and inform the development of the next NSW Skin Cancer Prevention Strategy, an up-to-date review of the evidence on the effectiveness and feasibility of skin cancer prevention activities in primary care is required. A research team led by the Daffodil Centre, a joint venture between the University of Sydney and Cancer Council NSW, was contracted to undertake an Evidence Check review to address the questions below. Evidence Check questions This Evidence Check aimed to address the following questions: Question 1: What skin cancer primary prevention activities can be effectively administered in primary care settings? As part of this, identify the key components of such messages, strategies, programs or initiatives that have been effectively implemented and their feasibility in the NSW/Australian context. Question 2: What are the main barriers and enablers for primary care providers in delivering skin cancer primary prevention activities within their setting? Summary of methods The research team conducted a detailed analysis of the published and grey literature, based on a comprehensive search. We developed the search strategy in consultation with a medical librarian at the University of Sydney and the Cancer Institute NSW team, and implemented it across the databases Embase, MEDLINE, PsycInfo, Scopus, Cochrane Central and CINAHL. Results were exported and uploaded to Covidence for screening and further selection. The search strategy was designed according to the SPIDER tool for Qualitative and Mixed-Methods Evidence Synthesis, which is a systematic strategy for searching qualitative and mixed-methods research studies. The SPIDER tool facilitates rigour in research by defining key elements of non-quantitative research questions. We included peer-reviewed and grey literature that included skin cancer primary prevention strategies/ interventions/ techniques/ programs within primary care settings, e.g. involving general practitioners and primary care nurses. The literature was limited to publications since 2014, and for studies or programs conducted in Australia, the UK, New Zealand, Canada, Ireland, Western Europe and Scandinavia. We also included relevant systematic reviews and evidence syntheses based on a range of international evidence where also relevant to the Australian context. To address Question 1, about the effectiveness of skin cancer prevention activities in primary care settings, we summarised findings from the Evidence Check according to different skin cancer prevention activities. To address Question 2, about the barriers and enablers of skin cancer prevention activities in primary care settings, we summarised findings according to the Consolidated Framework for Implementation Research (CFIR). The CFIR is a framework for identifying important implementation considerations for novel interventions in healthcare settings and provides a practical guide for systematically assessing potential barriers and facilitators in preparation for implementing a new activity or program. We assessed study quality using the National Health and Medical Research Council (NHMRC) levels of evidence. Key findings We identified 25 peer-reviewed journal articles that met the eligibility criteria and we included these in the Evidence Check. Eight of the studies were conducted in Australia, six in the UK, and the others elsewhere (mainly other European countries). In addition, the grey literature search identified four relevant guidelines, 12 education/training resources, two Cancer Care pathways, two position statements, three reports and five other resources that we included in the Evidence Check. Question 1 (related to effectiveness) We categorised the studies into different types of skin cancer prevention activities: behavioural counselling (n=3); risk assessment and delivering risk-tailored information (n=10); new technologies for early detection and accompanying prevention advice (n=4); and education and training programs for general practitioners (GPs) and primary care nurses regarding skin cancer prevention (n=3). There was good evidence that behavioural counselling interventions can result in a small improvement in sun protection behaviours among adults with fair skin types (defined as ivory or pale skin, light hair and eye colour, freckles, or those who sunburn easily), which would include the majority of Australians. It was found that clinicians play an important role in counselling patients about sun-protective behaviours, and recommended tailoring messages to the age and demographics of target groups (e.g. high-risk groups) to have maximal influence on behaviours. Several web-based melanoma risk prediction tools are now available in Australia, mainly designed for health professionals to identify patients’ risk of a new or subsequent primary melanoma and guide discussions with patients about primary prevention and early detection. Intervention studies have demonstrated that use of these melanoma risk prediction tools is feasible and acceptable to participants in primary care settings, and there is some evidence, including from Australian studies, that using these risk prediction tools to tailor primary prevention and early detection messages can improve sun-related behaviours. Some studies examined novel technologies, such as apps, to support early detection through skin examinations, including a very limited focus on the provision of preventive advice. These novel technologies are still largely in the research domain rather than recommended for routine use but provide a potential future opportunity to incorporate more primary prevention tailored advice. There are a number of online short courses available for primary healthcare professionals specifically focusing on skin cancer prevention. Most education and training programs for GPs and primary care nurses in the field of skin cancer focus on treatment and early detection, though some programs have specifically incorporated primary prevention education and training. A notable example is the Dermoscopy for Victorian General Practice Program, in which 93% of participating GPs reported that they had increased preventive information provided to high-risk patients and during skin examinations. Question 2 (related to barriers and enablers) Key enablers of performing skin cancer prevention activities in primary care settings included: • Easy access and availability of guidelines and point-of-care tools and resources • A fit with existing workflows and systems, so there is minimal disruption to flow of care • Easy-to-understand patient information • Using the waiting room for collection of risk assessment information on an electronic device such as an iPad/tablet where possible • Pairing with early detection activities • Sharing of successful programs across jurisdictions. Key barriers to performing skin cancer prevention activities in primary care settings included: • Unclear requirements and lack of confidence (self-efficacy) about prevention counselling • Limited availability of GP services especially in regional and remote areas • Competing demands, low priority, lack of time • Lack of incentives.
