Les ruminants ont la faculté de synthétiser toutes les vitamines B et K dans leur rumen ; par ailleurs, le pâturage leur fournit en abondance des carotènes (pro-vitamines A) et de la vitamine E alors que l’ensoleillement direct ou par l’intermédiaire du foin assure un bon approvisionnement en vitamine D. Traditionnellement, les risques de déficiences se limitaient principalement à la vitamine A en fin d’hiver. Mais à présent, l’intensification des productions animales, l’évolution des modes d’élevage et d’alimentation, les plus grandes exigences concernant la santé et la qualité des productions, imposent de prévoir des supplémentations étendues relatives à l’ensemble des vitamines liposolubles et même à certaines vitamines B en tenant compte d’indications particulières telles que fertilité (A), immunité (A, E), prévention des rétentions placentaires et des myodystrophies (E et Se), des hypocalcémies puerpérales (D), de la Nécrose du cortex cérébral lors d’acidose ruminale (Bl). stimulation de la microflore digestive et lutte contre la cétose (PP).

Resumen
Après un rappel de l’origine digestive et métabolique des acides gras et des composés vitaminiques du lait, cet article relate l’effet des principaux facteurs alimentaires sur leur concentration. L’herbe pâturée, par rapport à des rations hivernales, diminue les teneurs du lait en acides gras saturés au profit des acides gras trans et polyinsaturés dont le C18:3n-3 et le CLA-c9t11. Les supplémentations des rations des vaches laitières en lin ou en colza augmentent les teneurs du lait en acides gras trans et légèrement en CLA-c9t11, à court terme et aussi à long terme. Le lin produit des laits plus riches en C18:3n-3 et plus pauvres en C18:1-c9 que le colza. Des réponses différentes existent en fonction de l’interaction entre la nature des fourrages, la teneur de la ration en amidon et le type d’oléagineux. Les tannins ou les huiles essentielles ont peu d’influence sur la composition en acides gras du lait. Hormis la vitamine B12 ,l’ensemble des vitamines dans le lait peut provenir des fourrages ou du concentré et/ou du complément minéral contenant les vitamines A, E et D3. Le ruminant peut également synthétiser les vitamines A, C, B3, et D à partir de précurseurs apportés dans la ration. Les vitamines B et K sont synthétisées par les bactéries ruminales. Les facteurs nutritionnels influençant les concentrations en vitamines dans le lait sont peu documentés, à l’exception de la vitamine A. L’alimentation est néanmoins un levier important permettant d’augmenter la qualité nutritionnelle du lait.

Resumen
Le manque et/ou la vétusté de l’information sur les vitamines chez la truie reproductrice est en décalage avec la sophistication des techniques d’élevage et des progrès génétiques considérables des performances de reproduction au cours des dernières décennies chez ces animaux. De plus, l’information disponible selon les vitamines est hétérogène. Tout cela est à l’origine d’un empirisme qui est bien illustré par la variabilité considérable des recommandations de différents organismes privés ou publics. Il ne s’agit plus, aujourd’hui, de prévenir les carences en vitamines dont le risque est nul en production animale mais plutôt de déterminer les niveaux optima pour la productivité des élevages. C’est un défi pour les prochaines années d’autant plus grand que les critères de productivité des élevages d’aujourd’hui sont en constante évolution, bien au-delà de la seule prolificité (survie, vigueur et robustesse des porcelets). Le rôle de certaines vitamines vis-à-vis de l’immunologie de la reproduction, la capacité antioxydante et la compétence immunitaire pourra contribuer à l’amélioration de ces nouveaux critères des performances de reproduction. En outre, il ne faut pas négliger la perception publique généralement positive envers les suppléments vitaminiques.

Les anticoagulants anti-vitamines K (AVK) sont des inhibiteurs du mécanisme de régénération de la vitamine K. Celle-ci est impliquée dans l'activation post-traductionnelle par gamma-carboxylation de nombreuses protéines, et notamment de quatre protéines essentielles à la coagulation. L'apport alimentaire en vitamine K n'étant pas suffisant pour remplacer le cycle de régénération, un cycle efficace de régénération de la vitamine K est nécessaire. Les AVK bloquant ce cycle, la gammacarboxylation des facteurs de la coagulation n'est plus assurée, ainsi, la production de facteurs de la coagulation utilisables est fortement diminuée, ce qui affecte, par conséquent, le potentiel de la coagulation sanguine. Une inhibition importante du cycle de régénération peut entraîner la mort par hémorragies, c'est dans ce but que sont utilisés les AVK dans la lutte contre les rongeurs. Depuis une trentaine d'années, il a été remarqué que les rats femelles étaient moins sensibles aux AVK que les rats mâles. Plusieurs hypothèses pourraient expliquer cette différence : (i) une activité microsomale responsable de la régénération de la vitamine K plus efficace chez les femelles; (ii) une élimination plus rapide des AVK par les rats femelles; (iii) des concentrations basales et des demi-vies des facteurs de la coagulation vitamine K dépendants plus élevées chez les femelles. Le but principal de ce travail est de déterminer l'origine de la différence de sensibilité des rats femelles aux AVK.Les résultats mettent en évidence des différences significatives entre les mâles et les femelles concernant les niveaux basaux et les cinétiques d'évolution certains facteurs de la coagulation, pouvant expliquer la plus faible sensibilité des femelles aux AVK
Vitamin K antagonists (VKA) are inhibitors of the recycling mechanism of vitamin K. Vitamin K is involved in the post-translational activation by gamma-carboxylation of vitamin K dependant proteins, and in particular four proteins essential to coagulation. Food intake of vitamin K is insufficient to substitute the recycling mechanism, an efficient cycle is necessary. If VKA block the recycling mechanism, gamma-caboxylation of vitamin K dependant clotting factors becomes is not done, thus the production of the active form of these clotting factors is diminished. Consequently, the blood coagulability is affected. An important inhibition of the recycling mechanism can kill by haemorrhage, it’s with this aim that VKA are used in rodent management. Over the past three decades, a slight resistance to VKA has been observed in female rats beside male rats. Some hypothesis can explain this difference: (i) a more efficient activity of female microsomes which are responsible of the vitamin K regeneration; (ii) a faster elimination of VKA in female rats; (iii) basal concentrations and half-lives of vitamin K dependant clotting factors are higher in female rats. The main purpose of this work is to identify the origin of the resistance of female rats to VKA.Our results pinpoint significant differencies between females and males concerning the basal levels and the evolution kinetics of some vitamin K dependant clotting factors. This might explain the weaker sensibility of female rats to VKA

Resumen
La vitamine D (D3 ou cholécalciférol du règne animal et D2 ou ergostérol du règne végétal) est une hormone pléiotrope qui possède de nombreux effets biologiques incluant la régulation du métabolisme du calcium et du phosphate. Chez l’Homme, ce composé est synthétisé au niveau cutané sous forme inactive. On décrit ainsi le métabolisme de la vitamine D qui conduit à la production de métabolites actifs (par les vitamine D 25- et 1α-hydroxylases codées par les gènes CYP2R1 et CYP27B1) et à leur dégradation par la vitamine D 24-hydroxylase (gène CYP24A1). L’expression des vitamine 1α- et 24-hydroxylases est finement et inversement régulée afin de maintenir l’homéostasie phosphocalcique, grâce à plusieurs boucles de rétrocontrôle impliquant entre autres la forme 1,25-dihydroxylée de la vitamine D et son récepteur VDR, la calcémie et la parathormone, la phosphatémie et le FGF23. La carence en vitamine D et les défauts de son activation sont associés à un phénotype de rachitisme, tandis que les excès en vitamine D sont associés à un phénotype d’hypercalcémie-hypercalciurie par intoxication (surdosage) ou hypersensibilité à la vitamine D (excès d’activation ou défaut de dégradation).L’objectif de ce travail de thèse est d’identifier des causes génétiques de dérégulation du métabolisme de la vitamine D et de préciser leurs mécanismes physiopathologiques par une description précise du phénotype associé. Pour ce faire, nous avons utilisé de façon conjointe les outils de la génétique (séquençage nouvelle génération et Sanger) et de la biochimie (dosage des métabolites) dans une cohorte de patients recrutés grâce au centre de référence maladies rares du métabolisme du calcium et du phosphate.Ce travail a permis de préciser le rôle de deux gènes dans les maladies liées à la dérégulation métabolisme de la vitamine D, CYP2R1 et CYP24A1, par la mise en évidence de mutations perte de fonction chez des patients avec un phénotype de rachitisme à 25-hydroxyvitamine D basse et d’hypersensibilité à la vitamine D respectivement. Notre étude a permis aussi de préciser le phénotype de ces affections. Dans la cohorte des patients étudiés, l’identification de mutations de gènes impactant le métabolisme du phosphate (SLC34A1 et SLC34A3), souligne l’intérêt de l’étude des facteurs régulateurs des activités vitamine D 1α- et 24-hydroxylases.Aucune variation significative dans les régions promotrices proximales de CYP27B1 et CYP24A1 n’a été identifiée. Le peu de connaissances sur l’ensemble des éléments régulateurs chez l’Homme n’a pas permis d’approfondir notre étude. L’identification et l’étude de ces éléments régulateurs distaux permettra de déterminer leur implication dans les maladies rares du métabolisme de la vitamine D
The vitamin D (D3 or cholecalciferol from animal kingdom and D2 or ergosterol from plan kingdom) is a pleiotropic hormone who has numerous biological effects including the regulation of calcium and phosphate metabolism. In humans, this compound is synthetized in skin in an inactive form. Thus, we call vitamin D metabolism the biological process which leads to the production of active metabolites (by enzymes 25- and 1α-hydroxylases encoded by CYP2R1 and CYP27B1 genes) and its degradation by vitamin D 24-hydroxylase (gene CYP24A1). The expression of 1α- and 24-hydroxylases is tightly and inversely regulated to maintain calcium and phosphate homeostasis, thanks to several feedback loops including 1,25-dihydroxyvitamin D and its receptor VDR, serum calcium and parathormone, serum phosphate and FGF23. Vitamin D deficiency and vitamin D activation deficiency are associated with rickets, while vitamin D excess are associated with hypercalcemia-hypercalciuria due to vitamin D intoxication (overdose) or hypersensitivity to vitamin D (activation excess or degradation deficiency).Our aim is to identify genetic causes of vitamin D metabolism deregulation and to specify pathophysiologic mechanisms describing phenotype. Thus, we jointly used the tools of genetics (next-generation and Sanger sequencing) and biochemistry (vitamin D metabolites assay) in a cohort of human patients ascertained thanks to the national center for rare diseases of calcium and phosphate metabolism.This work allowed us to specify the role of two genes in diseases of vitamin D metabolism, CYP2R1 and CYP24A1, showing loss of function mutations in patients with rickets and low 25-hydroxyvitamin D and hypersensitivity to vitamin D, respectively. Our study brought new phenotypic elements in these affections. In our cohort of patients, the identification of mutations leading to phosphate deregulation (in SLC34A1 and SLC34A3) highlights the putative role of regulators of vitamin D 1α- and 24-hydroxylases activities in pathophysiology.No significant variation have been identified in the proximal promoting regions of CYP27B1 and CYP24A1. We could not go further considering the lack of knowledge in regulating regions and factors in humans. Identifying distal regulators will allow to study their implication in rare diseases of vitamin D metabolism

Les vitamines D et K sont des micronutriments liposolubles qui participent au bon fonctionnement de l’organisme. Elles jouent des rôles clés dans la prévention de trouble de l'hémostase et de la coagulation, des pathologies osseuses, métaboliques et cardiovasculaires. Cependant, même si ces vitamines sont apportées en quantités suffisantes par notre alimentation, leurs effets bénéfiques sont étroitement conditionnés par leur biodisponibilité. Or, mieux connaitre les mécanismes d’absorption permettrait d’appréhender leur biodisponibilité. Nous avons tout d’abord montré que l’absorption de la vitamine K implique des transporteurs du cholestérol, SR-B1 et CD36. Nous avons également montré que l’entérocyte est non seulement capable d’effluer les vitamines D et K néo-absorbées mais également d’excréter ces vitamines du compartiment sanguin vers la lumière intestinale. Ce phénomène bien connu pour le cholestérol (excrétion transintestinale du cholestérol) implique des transporteurs communs, dont ABCB1 et ABCG5/G8. Dans un second temps, dans le cadre de la relance de la consommation des légumineuses, nous avons mis en évidence que la présence de légumineuses dans un repas limite la biodisponibilité de ces vitamines. En effet, les fibres, phytates, saponines et tanins diminuent leur bioaccessibilité et/ou leur captage. La méthode de cuisson des légumineuses, en affectant leur composition nutritionnelle, peut moduler l’incorporation des vitamines D et K au sein des micelles mixtes et donc affecter leur biodisponibilité. Ces données soulignent ainsi le fait que les légumineuses doivent être cuites de manière appropriée et consommés dans des repas riches en micronutriments
Vitamin D and K are fat-soluble micronutrients that participate to the proper functioning of the organism. They are essential to prevent bleeding, bone, metabolic and cardiovascular disorders. However, even if those vitamins are provided in sufficient quantities in our diet, their health effects are closely linked to their bioavailability. A better knowledge of their absorption mechanisms would help to optimize their bioavailability.Firstly, we showed that vitamin K absorption involves the cholesterol transporters SR-B1 and CD36. We also showed that enterocytes can not only efflux newly absorbed vitamins D and K but also excrete vitamin D and K from the blood compartment to the intestinal lumen. This phenomenon of transintestinal excretioninvolves the cholesterol membrane transporters ABCB1 and ABCG5/G8.Secondly, we showed that the presence of pulses within a meal limits vitamin D and K bioavailability. Indeed, fibers, phytates, saponins and tannins can decrease bioaccessibility and/or uptake of vitamin K. By modulating the nutritional profile of pulses, the cooking method can impact on fat-soluble vitamin transfer to mixed micelles, and in turn affect their bioavailability. These data suggest that pulses must be cooked in an appropriate manner and consumed in micronutrient-rich meals.Keywords: vitamin D, vitamin K, bioaccessibility, intestinal absorption, pulses

Resumen
In this work, molecular dynamics simulation (MD) was used to study the encapsulation of fat-soluble vitamins D3 (vD3) and E (a-TOC) into cucubit[7]uril (CB[7]) in an aqueous solution. Cucurbiturils is a class of macrocyclic molecules largely used as carrier and controlled release agent in order to improve the solubility and chemoprotective of drugs. Along 50 ns of MD trajectory, the vitamins formed a stable complex with CB[7] without significantly altering its structure. Moreover, the second solvation shell of the CB[7] was not disrupted by the inclusion of the vitamins. The solvation enthalpy was ~ –173.0 kcal/mol for both complexes and –177.6 kcal/mol for the isolated CB[7], suggesting that the vitamin@CB[7] complexes are soluble in water. The binding free energy indicates that CB[7] can act as carrier agent for these vitamins, with values of –17.54 and –23.76 kcal/mol for vD3@CB[7] and a-TOC@CB[7], respectively. Finally, herein we highlight that CB[7] can be a new host to be used for vitamin delivery in biological systems.

Resumen
The self-assembly of supramolecular complexes between DNA and lipophilic vitamins, e.g. vitamin D group was examined, in the absence and presence of different cations, to the first time. A novel method for preparing DNA-lipophilic vitamins nanostructured complexes in the existence of divalent cations is introduced. The nanostructure of the supramolecular complexes ranged from beads-on-strings to compact globules and liquid crystalline forms depending on the vitamin nature, cations concentration and incubation time. A nucleation mechanism and flower-like aggregates are proposed as an initial state for complex formation. The biomedical applications of the functional nanostructured supramolecular complexes will be discussed in details.

Pode-se observar uma carencia de vitaminas e minerais frequente em pacientes que realizaram bariatrica, ocasionada pela restricao do duodeno e jejuno como tambem ao contato limitado do alimento. Objetivo: Analisar a deficiencia de micronutrientes relacionados a estetica em mulheres que realizaram cirurgia bariatrica e metabolica em uma clinica que tem como foco o tratamento cirurgico da obesidade. Metodos: A pesquisa realizada em 2019 teve carater quantitativo e descritivo, a amostra foi composta por 21 mulheres que ja realizaram cirurgia bariatrica com prontuarios que continham niveis sericos de zinco, ferro, ferritina, vitamina D e B12 e apresentaram sinais clinicos de queda de cabelo, tontura e fraqueza. Resultados: Foram analisados 21 prontuarios de mulheres que realizaram a cirurgia. No pre operatorio, 95,23% ja apresentavam deficiencia de micronutrientes, sendo a vitamina D em maior prevalencia. No pos operatorio, o mesmo numero de mulheres continuaram apresentando deficiencias sendo 52,38% vitamina D, o zinco com 19,04%, vitamina B12 e ferritina 9,52%. Conclusao: O acompanhamento nutricional antes mesmo da cirurgia, bem como a longo prazo juntamente com a suplementacao de vitaminas e necessaria e essencial para prevenir essas deficiencias. A maioria das mulheres analisadas referem queda de cabelo, tontura e fraqueza. A deficiencia de micronutrientes nesses pacientes pode desencadear um desequilibrio no organismo por serem essenciais em varios processos metabolicos, bem como afetar a auto estima por isso um acompanhamento nutricional e recomendado antes, durante e depois da cirurgia.,

Few measurements have been made of vitamin K in neonatal tissues. Using an assay based on HPLC with dual-electrode electrochemical detection we have investigated the vitamin K status of the newborn from analyses of paired cord-maternal plasma samples and liver samples obtained at post-mortem. For vitamin K1 (K1) the median value in cord plasma (16 pg/ml, range 4-45 pg/ml) in 20 babies was some 30 fold lower than that in maternal plasma (median 0.47 ng/ml , range 0.14-2.42 ng/ml). This is the highest maternal-cord gradient of all the fat-soluble vitamins and together with the lack of correlation between cord and maternal values suggests that K1 does not rapidly equilibrate across the placenta. Hepatic neonatal-adult differences in K1 levels were less marked being about 5 fold lower at birth (median 1.2 ng/g, range 0.1-8.8 ng/g, n = 22) than in adults (median 5.4 ng/g, range 1.1-21.3 ng/g, n = 32). No relationship was found between hepatic K1 and gestational age and relatively high levels (1-2 ng/g) were detected at 10-12 weeks gestation. Post mortem livers obtained after intramuscular K1 prophylaxis at birth (0.5-1.0 mg) had K1 levels which were raised dramatically (1000 to 5000 fold after 24-48 h) and which remained raised for at least one week. A preliminary assessment of the contribution of vitamins K1 (menaquinones, MKs) to vitamin K1 status revealed undetectable levels in fetal or neonatal livers until about 14 days post-partum. This was in marked contrast to adults in whom MKs 7-10 accounted for the majority of liver vitamin K (75-97% on a molar basis). In adult plasma MKs were present at much lower levels than K1; the low circulating levels and poor1placental transport would explain our inability to detect MKs in newborn livers. When expressed as total vitamin K (K1 and MKs) we concluded that the newborn may have only about 2% of adult hepatic concentrations; this relative deficit of MKs may be responsible for the high susceptibility of the newborn to vitamin K deficiency.

High intensity pulsed UV Light is a non-thermal treatment used in sanitization of fruits and vegetables. In this work, we have applied high intensity pulsed UV light as a pretreatment for convective air-drying evaluating the benefits of the pretreatment to the drying process and to the nutritional quality of the dried product. Mangoes were subjected to pulses of UV light. The pretreated samples were further dried in a convective oven-drier until 90% of the initial water content has been removed. Drying kinetics, water apparent diffusivity, vitamin B, vitamin C content and total carotenoids content were analyzed. Pulsed UV light showed to be an interesting pretreatment for mangoes given the higher nutritional content of the dried product. Keywords: mango; drying; ultraviolet; vitamins; kinetics

Resumen
This paper describes the summer research experiences of a high-school chemistry and biology teacher and a middle-school science teacher at the Milwaukee School of Engineering (MSOE). The first project involved researching B Complex Vitamins at MSOE’s Center for Biomolecular Modeling, developing molecular models using rapid prototyping technology (Z Corp 3D Printer) and creating curriculum modules for teaching the role of B Complex Vitamins to chemistry students in public high schools. A Javascript/HTML for interactive and dynamic presentation for understanding of thiamine (Vitamin B1) via web was written and implemented with Jmol software. A visual inspection of the family of Vitamin B Complex created and the curriculum modules developed during this project provide exciting and effective learning avenues for students in biology and chemistry classrooms. The second project dealt with the study of bioheat transfer and its simulation using MATLAB, and creation of a curriculum module that illustrates heat transfer principles reinforced by implementing the simulation. The teachers enjoyed the interaction with their advisors and the positive environment for their professional development. Details of their projects and experiences along with the evaluation of the program are presented in this paper. The teachers were pleased to be involved in connecting physics, biology, engineering and math into real projects that will motivate the students in their classes to pursue careers in STEM fields.

Resumen
An increased incidence of bleeding episodes due to hypopro-thrombinaemia has been associated with several cephalosporins especially those which contain an N-methyl-thio-tetrazole (NMTT) side chain. To study the etiology of cephalosporin-induced hypo-prothrombinaemia in the clinical situation we have investigated the ability of different cephalosporins to alter the metabolism of vitamin K and the relationship between hypoprothrombinaemia and vitamin K status as assessed from plasma levels of vitamin K. Cephalosporins containing an NMTT side chain (latamoxef, cefmenoxime, cefoperazone, cefotetan, cefamandole) or related structure (cefozolin) all caused the transient plasma appearance of vitamink1 2,3-epoxide in response to a 10 mg intravenous dose of vitamin ; those without NMTT (cefotaxime and cefoxitin) did not. The plasma accumulation of vitamin k1 2,3-epoxide was qualitatively similar to, but quantitatively less than, that produced by the oral anticoagulant phenprccoumon. In 36 patients eating normally, the median endogenous plasma vitamin k1 (370 pg/ml) was not significantly different from that in healthy, fasting subjects (372 pg/ml) and clotting tests remained consistently normal for all antibiotics tested. In 22 patients on total parenteral nutrition the median plasma vitamin k1. (223 pg/ml) was significantly lower than normal (p < 0.01) with 61patients having levels below the normal range (< 150 pg/ml) but normal clotting before starting antibiotic therapy. All 7 parenterally-fed patients treated with latamoxef developed hypoprothrombinaemia (as shown by prothrombin time, PIVKA-II and protein C measurements) within 4 days whereas 12 patients treated with cefotaxime or cefoxitin did not. Latamoxef-associated hypoprothrombinaemia was readily reversible by 1 mg of vitamin k1 given intravenously but hypoprothrombinaemia and sub-normal plasma vitamin k1 could recur within 2-3 days. The data suggest that NMTT-cephalosporins are inhibitors of hepatic vitamin K epoxide reductase and that a lowered vitamin K status predisposes to hypoprothrombinaemia.

INTRODUÇÃO: As doenças autoimunes representam reações imunes exacerbadas direcionadas a autoantígenos, devido à ausência de mecanismos destinados a autotolerância. Ao longo das últimas décadas, a prevalência destas patologias apresenta elevadas proporções principalmente em mulheres, devido a condições hormonais. OBJETIVOS: Determinar a relação entre a concentração de vitamina D e o desenvolvimento de doenças autoimunes. MATERIAIS E MÉTODOS: Trata-se de uma revisão de literatura realizada a partir do banco de dados PubMed, entre os anos de 2019 e 2021, através dos descritores Vitamin D Deficiency e Autoimmune Diseases. RESULTADO: Os pacientes com doenças autoimunes geralmente possuem a hipovitaminose D. As células do sistema imunológico expressam o receptor de vitamina D, desse modo, o composto lipossolúvel descrito exerce a modulação do sistema imune, possui ação antiinflamatória e contribui para o aumento de linfócitos T regulatórios, capazes de limitar a resposta imune. A vitamina D quando introduzida via ingestão de alimentos de origem animal, torna-se susceptível a consecutivas reações químicas de hidroxilação no organismo humano, presentes tanto a nível renal como hepático. Cabe ressaltar também, que a forma ativa designada como calcitriol, suprime a ação da resposta Th1, associada a interferon gama e interleucinas, induz a ação de linfócitos auxiliares 2, além disso, contempla características antioxidante, assim como antifibrótica. CONCLUSÃO: Estudos estabelecem relação direta entre insuficiência de vitamina D e progressão de doenças autoimunes. A adesão de níveis séricos adequados desta substância através da suplementação ou exposição solar, consiste em um recurso com baixo custo de saúde pública e acessível para tratar ou prevenir as doenças autoimunes.

Protein C (PC) and Factor VII (F VII) are both vitamino-K dependent factors with similar short half-life. Differential behaviour of both factors in their two expressions was assessed on :- 16 resolutive liver toxicity due to reinforced chemotherapy,- 7 specific liver toxicity of Asparaginase or Bisanthrene- 6 veno-occlusive disease complicating an allogenic bone marrow transplantation- 5 cirrhotic liver insufficiencies- 10 patients submitted to stabilised treatment by oral anticoagulant- 3 families of congenital deficiencies in F VII.In case of congenital deficiency in F VII, both expressions of this factor were parallely decreased and PC was normal. During anti-vitamin K therapy, functional levels of PC and F VII were parallely decreased, more profondly than antigenic dosages. In case of severe liver insufficiencies both expressions of both factors were parallely decreased. Evoluated V0D behaved as severe liver insufficiency. Onset of therapy by asparaginase and bisanthrene, resolutive toxicity due to reinforced chemotherapy and early phase of the V0D were characterized by a decrease of the functional activities of these factors before molecular decrease and despite a systematic preventive treatment by injection of vitamin K. Often one of these factors was more affected than the other (in these cases : PC more often than F VII). The ratio activity/antigenicity of these factors proved to be an early and sensitive marker of liver involvement. Changes occured earlier than modifications in prealbumin or in liver enzymes. Considerations of these tests to follow liver functions and specially to assess early diagnosis of VOD must be considered.

Fatal vitamin K deficiency haemorrhage has been observed in breast fed babies. Though the incidence of vitamin K deficiency haemorrhage seems to be low in exclusively breastfed babies in Germany, subclinical vitamin K deficiency is by far more common as demonstrated in recent studies. Vitamin K concentrations in human milk are lower than in cow's milk and infant formula, however, nothing is known about the factors determinating the vitamin K1 concentrations in human milk. Vitamin K1 concentrations in human milk were studied during the first five weeks of lactation with respect to a) stage of lactation, b) interindividual differences, c) relationship of vitamin K1 to other lipids, and d) influence of oral supplements of vitamin K1 given to the mother. Milk samples from 9 mothers were collected on day 1,3,5,22,29 and 36 of lactation using standarized techniques.a) Vitamin K1 concentrations in colostral milk, day 1-5 (median 1,8 ng/ml) were significanctly higher than in mature milk, day 22-36 (median 1,1 ng/ml) (Wilcoxon U-Test p< 0,01). These changes during the course of lactation must be considered for estimation of the vitamin K supply in breastfed babies.b) Vitamin K concentrations both for colostral and mature milk were found to vary widely: colostral milk 0,6-4,4 ng/ml, mature milk 0,4 - 2,8 ng/ml.c) For colostral milk regression analyses revealed good correlations of vitamin K1 to cholesterol but none to total lipid and phospholipids, whereas no correlation to either lipid was observed for mature milk. Cholesterol appears to have a role in vitamin K1 secretion into colostral milk.d) Vitamin K1 concentrations of maternal milk were influenced by oral supplements given to the mother. Even with a dose of 100 μ vitamin K1 (similar to the dose which may be ingested with a meal) a twofold increase of the vitamin K1. content of breast milk was observed. These data suggest that mutritional factors may influence the vitamin concentration in human milk. Vitamin K supplements for breastfeeding mothers on vitamin K1 poor diets could improve the vitamin K supply of these babies.
