In the modern therapy of arterial hypertension (AH) along with thiazide diuretics b-blockers, calcium antagonists, angiotensin converting enzyme inhibitors and angiotensin receptor blockers are considered for initial and maintenance of antihypertensive therapy in the form of mono - or combination therapy. Five of these classes of drugs have comparable antihypertensive effectiveness and proven impact on prognosis in accordance with the results of randomized controlled clinical trials. The most widely used in clinical practice for the treatment of patients with AH received thiazide diuretics. Th

The recommendation for first-line therapy for hypertension remains a beta blocker or diuretic given in a low dosage. A target blood pressure of less than 140/90 mm Hg is achieved in about 50 percent of patients treated with monotherapy; two or more agents from different pharmacological classes are often needed to achieve adequate blood pressure control. Single-dose combination antihypertension therapyis an important option that combines efficacy of blood pressure reduction and a low side effect profilewith convenient once-daily dosing to enhance compliance. High blood pressure (BP) is thr major independent risk factor for cardiovascular disease and the associated high mortality in this country. However, there is convincing evidence for the forecast improvement with regular use of antihypertensive drugs, while achievement of blood pressure control (target level) is a crucial factor in reducing the risk of fa

Background and Objective: Guidelines recognized the dual combination in initial antihypertensive therapy. Studies demonstrated low-dose quadruple combination antihypertensive was superior to monotherapy. However, whether low-dose quadruple therapy is better than dual combination is unknown. Methods: A randomized blinded crossover trial was conducted to compare the efficacy and safety of low-dose quadruple antihypertensives (irbesartan 75 mg + metoprolol 23·75 mg + amlodipine 2·5 mg + indapamide 1·25 mg) with standard-dose dual antihypertensives (irbesartan 150 mg + amlodipine 5 mg) in the init

In the article, with regard to current clinical recommendations, the place of combined antihypertensive therapy, especially triple drugs regimens, is considered in the treatment of patients with arterial hypertension. Special focus is given to the body of evidence for the efficacy of valsartan and amlodipine, as the reference drug of angiotensin II receptor blockers and calcium channel blockers, respectively. Not only their high antihypertensive properties are demonstrated, but also a favorable effect on target-organ protection and prognosis is described. In particular, the possibilities of va

Control over blood pressure is an existing problem in cardiology and does not exceed 30 % for Europeans, despite prevalence and availability of effective antihypertensive drugs. Low adherence to treatment is one of the reasons of low control of blood pressure. Implementation of the combined antihypertensive drugs with the high fixed doses allows to simplify therapy in subjects receiving combination therapy and to increase the adherence in high risk patients receiving drugs separately.

The study addresses antihypertensive effects of fixed low-dose perindopril-indapamide combination therapy in the elderly hypertensive patients. Combinations with metoprolol were also studied. Is was shown that antihypertensive effect can be observed ay second week of treatment and is dose-dependent. Besides, additional administration of metoprolol 50 mg per day was not associated with further blood pressure decrease but resulted in significant heart rate reduction.
