Buccal bifurcation cyst (BBC) is a rare inflammatory odontogenic cyst, which commonly affects children in the first decade of life. We report a case of a seven-year-old healthy boy with bilateral BBC, which involved unerupted incomplete permanent mandibular first molars. A review of the literature in English language revealed few similar cases. We reviewed 16 manuscripts of bilateral mandibular BBC, reporting a total of 20 cases since 1970. The clinical features of bilateral mandibular BBC summarized here could assist specialists with an accurate diagnosis and provide patients with optimal management.

Introduction: A buccal bifurcation cyst (BBC) is an uncommon inflammatory odontogenic cyst associated with the permanent mandibular first or second molar in children. Case reports: These reports present two cases of BBC and describes the clinical and radiographic features leading to the diagnosis and the treatment of this lesion. Two patients complained of mandibular buccal swelling around the permanent first molar. The diagnosis of BBC in both cases was based on the clinical and radiographic features. In both cases, only enucleation was performed without extracting the involved tooth. Results: There were no recurrences during follow up. All teeth remained vital and erupted normally. Conclusion: The most appropriate treatment is usually enucleation of the cyst without extraction of the associated tooth. Therefore, knowledge of the distinct features of BBC is important for diagnosis and appropriate treatment.

ABSTRACT Buccal bifurcation cyst is an unusual inflammatory odontogenic cyst of unknown etiology which occurs at the buccal region of the permanent mandibular first molars in children and adolescents aged 6-15 years old. The aim of this manuscript is to present two clinical cases of buccal bifurcation cyst incidentally detected in adolescents referred for tomographic exams (CBCT) as part of the treatment planning for the management of impacted mandibular molars. The first case is very interesting due to the proximity of the lesion to the inferior alveolar canal. In the second case the inferior alveolar canal was not in close proximity to the lesion, but in contact with the mesiobuccal root of the second molar. The diagnosis of Buccal bifurcation cyst is of utmost importance in order to avoid complications during surgical procedures. In addition, the present case reports have highlighted the importance for dental practitioners to be aware of the possibility of a buccal bifurcation cyst being present as an incidental finding in CBCT images, especially in young patients.
