Pełny tekst źródłaChronic kidney disease (CKD) is a serious public health problem due to the constant increase of its incidence. In particular, CKD patients are at high risk of developing cardiovascular disease further the accumulation of molecules called uremic toxins. The purpose of this thesis was to evaluate risk markers of CKD complications in a cohort of patients at different CKD stages by evaluating some uremic toxins and some non-invasive techniques to determine their potential associations with vascular and biochemical parameters, global and cardiovascular mortality. In this study, we demonstrated that the kappa and lambda free light chain, sclerostin and β2 microglobulin (ß2M) levels were increased in early CKD stage and culminated in dialysis patients. Free light chain and sclerostin levels were associated with inflammation markers but not with vascular parameters. These levels appeared to be associated with mortality, but this association disappeared after adjustment for other factors. On the other side, we have demonstrated that the ß2M levels were independently associated with overall and cardiovascular mortality in our cohort and cardiovascular events in pre-dialysis patients. In addition, we demonstrated that the addition of vascular calcification scores (especially the coronary artery calcification score) to TRFs appears to improve CV risk assessment in a CKD population.This work identified several important markers of CKD complications, as ß2M and measurement of vascular calcification scores. The addition of these two markers in routine practice could improve the prediction of cardiovascular risk and mortality in this population
