Patients with ALS require multidisciplinary support during their course. Pathology causes many disabilities and disabilities. This complex management can lead to situations of course failure through absence, cessation or difficulties of management, however the causes of these breaks are not known. The city hospital coordination network SLA Île-de-France has a textual coordination base, on which the needs and demands of patients are described throughout their journey. To this end, we propose to analyze this basis in order to extract knowledge and describe the patient pathways. For this we use a semantic annotation tool that uses as a resource an ontology of the domain that we have created OntoParon. OntoParon is a modular ontology composed of a medical module, a socio-environmental module, a coordination module, a core module and a consolidation module. These ontological modules make it possible to take into account the dimensions involved in patients' journeys. Defined concepts, reporting on important themes such as the exhaustion of the caregiver or even the presence of social problems were created to detect the difficulties encountered. The annotation of 931 patient files has shown that not all patients have the same needs or the same demands, some themes are expressed differently depending on the age, the pathology or the lifestyle of the patients

The Attention Deficit Hyperactivity Disorder (ADHD) is a neurobiological condition that affects 3-7% of the pediatric population. Has a characteristically persistent pattern of inattention, hyperactivity and / or impulsivity, more frequent and severe than that typically observed in individuals at an equivalent level of development. ADHD is associated with losses in several contexts, including academic performance, school behavior, peer relationships and family functioning, directly influencing the quality of life of sufferers. This study aimed to evaluate the quality of life of children with ADHD combined subtype, using the scale PedsQLTM. We evaluated children aged eight to twelve years, compared with a group without the disorder and relating to the perception of their parents or caregivers. In total 88 children participated in the study, in all cases their parents or caregivers answered the quality of life. Of these 45 were study group and 43 in the control group. There was no statistical difference between groups in gender, family income, parents\' marital status, father\'s and mother\'s education, showing similarity in both groups. The results indicated that the self-reported, the group with ADHD had scores lower than the control group in all domains assessed. The difference was significant for the social aspect (p = 0,010), school activity (p <0,001), psychosocial health (p <0,001) and overall quality of life (p = 0,002). The physical functioning domain and emotional aspect did not differ significantly between groups, p = 0,841, p = 0,070, respectively. In the perceptions of parents / carers, the children with ADHD had lower scores in all domains. In this case there was a significant difference in all domains - physical functioning (p <0,001), emotional (p <0,001), social functioning (p <0,001), school activity (p <0,001), psychosocial health (p <0,001) and overall quality of life (p <0,001). The relationship between self-reported child and parents report indicated that there is greater agreement among the group with ADHD, except in school activity. This suggests that the child with the disorder and their father has the perception of functional limitation that the disease brings. Patients with ADHD showed impairment in multiple domains of quality of life, especially those related to psychosocial factors, indicating that the behavioral issues, social and school have great interference in the wellbeing of these children. It is important to undertake studies to verify the quality of life in children with ADHD in our population, which can generate the most appropriate means of intervention aiming to provide and measure the scope of the wellbeing of these children

Ce constat éclaire de façon nouvelle la relation qui se construit entre les différents « temps » du mode indicatif. En effet, la prise en compte d'une opposition supplémentaire conduit à réinterroger la prédominance de la temporalité dans les analyses des « temps » de l'indicatif au profit d'une déconstruction de la combinaison des valeurs. Cette analyse permet alors de rendre compte de la diversité des effets de sens tout en évitant l'atomisation des explications temporelles, modales, aspectuelles au gré des occurrences.
La première partie de la thèse s'articule autour de la relation entre cette analyse morphologique et les différentes analyses formelles du futur et du conditionnel qui ont pu être menées auparavant.
Dans un second temps, se nourrissant de la confrontation avec les analyses conceptuelles existantes de l'infinitif, du passé simple, de l'imparfait, du futur et du conditionnel, elle explore le versant sémantique de l'hypothèse de l'existence d'un morphème ±R qui se combine avec un autre morphème a/ai. Hypothèse qui oblige, d'une part, à caractériser la valeur sémantique du morphème dénoté par ±R et c'est le concept ±potentiel qui lui sera associé, d'autre part, à redéfinir l'opposition imparfait/passé simple puisque c'est cette même opposition qui se retrouve dans le couple futur/conditionnel. C'est le trait ± défini qui sera proposé.
La prise en compte de la combinaison de ces deux traits sémantiques permet de déjouer la complexité d'énoncés dont la richesse n'a d'égal que la variété.

A formal analysis of the verbal endings in the indicative mode draws special attention to the combination of two morphemes, [Ø/(±R)] and [Ø/(ai/a)]. Thus, the conditional and future tenses differ from the other indicative tenses in that they share the +R morpheme – and it can be postulated that it is also shared by the infinitive. Furthermore, they differ from one another in the opposition between ai and a, which also differentiates the imperfect tense from the past historic when it is combined with the morpheme –R. This statement sheds light on the relationship between the various indicative “tenses”. Indeed, taking into account a further opposition allows the predominance of temporality in analyses of “tenses” in the indicative to be questioned while focusing on the deconstruction of the value combinations. Thus, the analysis allows the diversity of meanings to be accounted for and at the same time avoids a too large panel of temporal, modal or aspectual explanations according to the context. The first part of the doctoral thesis turns on the relationship between the morphological analysis and the various formal studies that have been undertaken on the future and the conditional tenses. In a second part, in a confrontation with the already existing conceptual studies on the infinitive, the past historic, the imperfect tense, the future and the conditional, we will explore the semantic side of the hypothesis that states that there is a combination of a ±R morpheme with an ai/a morpheme. Such a hypothesis compels us to characterize the semantic value of the ±R morpheme on the one hand – the ± potential concept will be associated with it – and on the other hand, to redefine the opposition between the imperfect tense and the past historic, since it is this very opposition that can be found in the future/conditional pair – the ± definite concept will then be suggested. Taking into account the combination of the two semantic features enables us to undermine complexity in utterances whose richness is only equalled by their diversity

Commercially available liquid embolization agents used in endovascular treatment have many limitations. Polymeric agents as Onyx and cyanoacrylate are available. Ethanol also is a potent sclero-embolic agent. Cyanoacrylates are effective liquid embolic agents, however, their rapid polymerization makes their behaviour unpredictable with possibility of incomplete treatment. These properties render their use challenging.Onyx is easy to use. However, in very small arterial niduses, Onyx, is unable to penetrate deeply. Deep penetration is obtained with ethanol, associated with risk of systemic migration.Poor visualization of ethanol under fluoroscopy is major drawback. Mixing Onyx with ethanol had never been described in the literature till now. In this work, various mixtures have been tested with different concentrations of Onyx 18 and absolute ethanol. Alconyx 25 (75% Onyx 18; 25% ethanol) seems to be a promising product. We proved its ease of injection in vivo and in vitro, its cohesive nature showing no fragmentation or interruption of the injected column as well as its good visualization under fluoroscopy. It was able to penetrate deeply in the arterial bed. The occlusive properties of Alconyx 25 were rated as good as Onyx 18 under high pressure in vitro. Further investigation is needed to better understand the behavior of ethanol in the suspension and its effect on tissues compared to Onyx diluted simply with an equivalent amount of DMSO. Studies on other commercially available concentrations of Onyx would certainly be interesting

A diagnosis of ADHD and treatment with methylphenidate (Ritalin) or atomoxetine (Strattera), combined with cognitive-behavioral therapies (though not always), has emerged in recent decades as the solution that education and health professionals customarily provide to children who are considered not to make adequate progress at school. This “solution” has generated intense debate and produced controversial literature ever since the 1980s when ADHD was included in clinical psychiatric diagnostic manuals, especially in the US, a pioneer in pathologizing this area of children’s and families’ lives. While some maintain that ADHD is an “invented” disease, others point to the “scientific evidence” of its location in the brain and the genetics involved in its presentation. In this study, I aim to explore the social phenomenon of ADHD through ethnographic field- work, specifically by studying a local context comprised by two sites of analysis, both located in Barcelona: a Childhood & Juvenile Mental Health Center (CSMIJ, in Catalan) and my own workplace, a Secondary Education Institute, (IES, in Catalan). I have planned the research to take place in two periods, from two different angles, reflected in the two parts that make up the ethnography. In the first part, in order to better grasp the phenomenon of ADHD in its totality, I approach it as a “global social fact,” using the concepts of “interplay” and “institution” to inquire and describe the processes of construction and institutionalization of the medical classification of ADHD and its management in children, adolescents and young adults at both the CSMIJ (Chapter 1) and the IES (Chapter 2). In the second part (Chapter 3), I approach the personal experience of certains youths who at some point in their lives have been diagnosed with ADHD and medicated. I have listened to their voices and considered what it means to “develop ADHD” by focusing on the processes of subjectivation and internalization of the biomedical taxonomy, as well as what I call the “logics of diagnosis.” The final reflection centers on the configuration of the personhood and identity of these children, adolescents and young adults who have been diagnosed with ADHD and medicated. I understand “identity” as a construction-in-process and in constant dialogue, between institutional interplay[1] and individual biographical particularities. I venture to ask if we could be dealing with “medicalized identities,” and finally, I attempt to explain what the expression “cursant Concerta[2]” (one that I felt charged with meaning on the ground) means to me.
