Proper treatment of the two-wall fractured orbit is still controversial. Specifically, there is no consensus on the issue of the necessity of medial orbital wall repair. With anatomically critical structures at risk during the surgical approach, surgeons’ view on the necessity of medial orbital wall repair often is restricted and an aesthetically disturbing enophthalmos is more likely to be accepted. Therefore, treatment options range from leaving the medial wall without repair to reconstruction with autogenous tissue or alloplastic materials, which can lead to moderate to severe side effects.

The article presents the method of transnasal surgical treatment of patients with a fracture of the medial wall of the orbit using the Volkov elevator as a repositioning tool and an iodine tampon as a fixation material, which allows efficient fixation of the medial orbit wall fragments and its soft tissue components, allowing to reduce the trauma of surgical intervention, to restore the eyeball position, and to eliminate the cause of emphysema and dynamic correction of postoperative diplopia in one-time operation.

Background: Orbital medial wall fractures, which are more common than inferior wall fractures, have traditionally been difficult to diagnose with conventional radiography. As the fracture extends deep into the bony orbit, accurately visualizing internal structures becomes challenging, increasing the risk of optic nerve compression. In a previous study, the author introduced a technique for treating medial orbital wall fractures using a combined transethmoidal and transcaruncular approach. In this study, the authors hypothesized that the use of surgical navigation could enhance precision, safet

The surgical treatment of isolated medial orbital wall fractures is still a much-debated topic in the literature due to the choice of many surgical accesses. The main options are represented by transcutaneous versus endonasal endoscopic approaches. Our study aims to clarify the role of ethmoidectomy in the pure endoscopic endonasal reduction of medial orbital wall fractures, evaluating the immediate postoperative outcome and its long-term stability. A total of 31 patients affected by isolated medial orbital wall fracture, treated only by endoscopic approach, were included in the study and divi

ABSTRACT An orbital blowout fracture is a fracture of any wall of the orbit, including the medial, lateral, posterior, the floor, or the roof. The most common injury site is the floor or the medial wall. Any blow to the face or eye, especially the orbital rim, can cause this injury. Vision threatening complications like permanent diplopia and cosmetically unacceptable enopthalmos make this an important injury to be identified quickly. Here, we present two atypical cases of orbital blowout fractures with retained nasoorbital foreign bodies where mechanism of injury was different but resulted in

The fracture of the medial orbital wall is relatively common in orbital trauma. Titanium mesh is possibly the actual standard material for orbital wall reconstruction. When the floor of the orbit and the medial wall are simultaneously affected, one larger mesh gives better results than two independent meshes that need to be fixated independently. However, large meshes need a wider surgical field. To gain sufficient exposure to the medial and inferior orbital walls simultaneously, we present an approach that combines the transconjunctival and transcaruncular incisions, detaching if needed the i

Orbital and periorbital injury can occur with localized trauma to the eye or in the setting of multiple trauma associated with injury to other vital organs. In general, the amount of ocular, soft tissue, and bony damage is related to the amount, duration, and direction of force applied to the orbit and face. The function of the orbit is to protect and accommodate the globe in order to maximise its function. The bony orbits are paired four-sided conical cavities within the skull each comprising seven bones – ranging from the paper thin ethmoid and lacrimal plate medially to the buttress thick zygoma laterally. The conical shape consists of an apex posteriorly and a base anteriorly forming the outer margin. The medial wall and floor begin to blend towards the apex forming a posteromedial bulge as the orbit takes on a three-walled pyramidal structure. The walls are lined by periosteum (periorbita), w

The paired orbital cavities are formed by the facial bones and serve as sockets for the eyes. The orbital bones and the structures contained within the orbit (connective tissue, fat, nerves, vessels) act to support, protect, and maximize the function of the eye. In form, the orbit is roughly a quadrilateral pyramid with rounded angles and resembles a pear. Its volume in the average individual is 30 cc, of which the eyeball contributes about 7.5 cc (range: 6.9–9.0 cc). There are four surfaces: the roof, floor, lateral wall, and medial wall.
