Pełny tekst źródłaThis work studies the psychical troubles of mentally handicapped adolescents. It refers to the attachment theory and psychoanalysis. The hypothesis is that mentally handicapped adolescents have an insecure attachment. The method used is an experimental one. The data analysis includes the results stemming from the experimental method and the analysis of the clinical data coming from a psychoanalytical approach of the material. All our results support our hypothesis. The adolescents assessed have an unresolved attachment. Their mental functioning attests the traumatism of the announcement of the mental handicap on their family. Mental handicap is looked on, by their psyche, as an evidence of the destructivity of aggressiveness. It appears either a crime, for which the handicapped adolescent must be punished or a punishment, evidence of a prior culpability that cannot be worked over. The reality of the handicap appears a hindrance to the fantastical working over of aggressiveness

Pełny tekst źródłaThis thesis proposes to investigate the psychological disability construction, based on three axes: historical, social and institutional. Studying ITEP (Pedagogic and Educative Therapeutic Institute) from Gironde, we interrogated the assignation process of the teenagers in these facilities and their backgrounds (social, familial, etc) that leads them to be designed as psychologically disabled people (2005 law). Consequently, when the collective attention is focused on the consequences of being handicapped to compensate, we want to study the origins of psychological disability that conduct them – or not – to be oriented in an ITEP. First, we realized an historical diagnosis. We studied five organisations managing ITEP by interviewing their directors, in order to compare the historical beginning of psychological disabilities and the development of Gironde organisations. The second axe consists of a social characterisation of ITEP teenagers by studying social data in all 20 Gironde ITEP (a pool of 965 teenagers). The study of the complex arrangement of ITEP teenager characteristics show that being psychologically disabled is affected by family social origins, showing a first assignation form. The third analytic axis is an ethnographic observation about how the Multidisciplinary Evaluation Team (EPE) operates in Gironde MDPH during a year. The results of this orientation procedure show how psychological disabilities is built between psychic disorder and social-family difficulties. Linking these three analytic parts allows for a better understanding about how the psychological disability is constructed, all invisible steps of the assignation processes and what legitimates them. In addition to having difficulties living an ordinary life, all conditions are combined to prevent these psychologically disabled teenagers and their families to have a space dedicated to social representation

Pełny tekst źródłaChildren with intellectual disabilities experience major inequality in the field of oral health. Opposition behaviors are very common. The care is then carried out sometimes in difficult conditions. This work examines the ethical issues faced by the dentist in the refusal of care in children intellectual disabilities. The objectives are: (1) to expose elements of contextualisation (2) to identify the practices of dental care in a national and international contexte and to initiate the philosophical reflection (3) to focus on the opinions and perceptions of practitioners to explain these practices and to identify areas of ethical tension (4) to propose a synthesis and a discussion. Our research is based on two studies, each of which is directed at dentists who are specialists in special care dentistry in children. A deductive approach was conducted to study practitioner profiles according to their practices and the opinions they have of these practices (online quantitative survey) and analyzed by MFA (Multiple Factor Analysis). An inductive approach based on focus groups was conducted to study the perception and opinion of practitioners. The thematic content analysis made it possible to understand the motivations and difficulties encountered. The results showed that most dentists are part of a teleological approach when they have to choose between respect for autonomy and beneficence. Nevertheless, if these constrained decisions are impregnated with a deontological scruple, this guarantees their ethical character. It would be necessary to develop education and the ethics of the discussion to reconcile the dual technical and emotional requirements of care

Pełny tekst źródłaThe handicap psychique (psychological handicap) is a French expression which has imposed itself recently. It is the subject of various meanings. We place it in the very broad context of vulnerabilities and in the more restricted situations of handicap. While it is commonly accepted that psychological handicap is a social consequence of a mental disease, we show that people with handicap of psychological origin (HOPO) have no proven mental disorder and do not undergo any psychiatric treatment. We rely on the experience of a plan called Médiation logement. In this “Housing mediation” clinical psychologists meet people who are reclusive in their homes, or who exhibit behaviour or neighbourhood disturbances. Those people have no psychiatric history and we show that they share a common feature: a discrete incapacitating syndrome of psychological origin (DISOPO). This leads us to give a new definition of psychological handicap. Then we propose a clinical reading tool that would allow a team to make an early diagnosis of this DISOPO that appears in a person with a singular fragility when faced with personal and / or social traumatic of stressful event

Exner's comprehensive system, to a group of patients whose disability derives both from a physical trauma (exogenous) and from an internal somatic dysfunction (endogenous), shows on one side difficult mental working a frequent depression index, a strange living of human experience, and a sense of rupture. On the other side, we observe, a more furious internal rage for exogenous group, difficulty to integrate the emotional aspects, a more frequent use of abstraction and the loss of self-esteem, always for the exogenous. These general results will enable us to discuss the concept of dissociation. We after focus on motion mental images. Bett's shorter questionnaire mentions the failure of some patients confro9nted with kinesthetic solicitation. Besides the mental motion representation may have a defensive value. The results of Rorschach tests suggested in this connection: (i) the absence of "m" responses in inversely proportional to the intensity of anxiety,(ii) the "m"< in so far as they represent a process of devitalisation, can support the processes of perceptive apprehension. Furthermore, when we examine the variable "traumatic dream", chosen for its pathognomonic character, on realizes always through the Rorschach test, that the dream of anguish thoroughly confirmed by this perceptive-projective test, is not free for mental working. This result preserves all its importance in the psychotherapic perspectives; perspectives which will be evaluated in a second study

Pełny tekst źródłaThis thesis is based on multiple observations that paint a picture of a collection of restrictions on social participation for people with handicaps related to mental illness. In the absence of a legal context which encourages the social participation of these individuals, documentary evidence shows that there are roadblocks in various domains, such as domestic, social, economic, professional, political, cultural and artistic domains. In this context, and in line with international terminologies, our thesis aims to define the conditions which enable support groups for individuals with mental handicaps (known in France as Groupes d’Entraide Mutuelle, or GEM for short) to strengthen the social participation of their members. We aim to measure the strength of this involvement both on an individual and a collective level. After an overview of the concepts of participation and social participation, we will propose a theoretical model that covers the process and results of social participation, which we will compare and contrast to what we observe in the context of our research. This research is comprised of open-ended questionnaires administered face-to-face with members of GEMs, fragmented ethnographic observations in twenty GEMs, and extensive participant observation in two GEMs. The analysis of the data, which was in large part qualitative, points to different forms of participation on the individual and collective level. With these forms in mind, we identify various conditions necessary for the emergence of social participation, linked both with the individual characteristics of members and the specific contexts of each GEM, whether they be spatial, organisational, relational and/or professional.Our analysis leads us to understand the individual process of social participation as an experience, whereas at the collective level, the process is a publicization of a set of problems and points of view which transforms each of the actors involved. In terms of results, we identify three levels of social participation as well as the emergence of different types of expertise. In conclusion, we modify our original theoretical model and propose a new transactional model of the process of social participation and its results

Pełny tekst źródłaWork is a key-factor of recovery for people with severe mental illness. Various programs exist to support them to gain and maintain competitive employment. In France, the rate of employment remains low for this population and only scarce researches are developed in this field, although some French practices, such as the Ç ESAT de transition È (transitional workshops) of the MESSIDOR association, appear to be performing well. Theses transitional workshops support people with psychiatric disability to gain a competitive employment on the regular labor market while working in a sheltered activity. The goal of this research was to describe the Ç ESAT de transition È program, comparing it to international evidence-based practices, such as supported-employment programs, and to define how this program can facilitate a recovery process for people with a severe mental illness.The first part of this research is intended to describe the program with a mixed method (qualitative then quantitative) in order to compare it with international evidence-based practices and to describe the roles, tasks and competencies of the two professionals supervising people with a severe mental illness, counselors and supervisors, and to define how they contribute to a recovery process for them. A second part of the research is dedicated to clients of this program, with a longitudinal study (n=160) of clinical and psychosocial variables, at the beginning of the program, and in order to document predictive factors of their job tenure in the program, of their positive evolution to work integration on the regular labor market and of their recovery.These Ç ESAT de transition È are an hybrid form of a sheltered workshop, social enterprise and supported-employment program for people with a severe mental illness. In a workplace close to the context of regular labor market, thanks to a subtle relational posture, the two professionals supervising the workers help them to develop a new positive identity, self-esteem as a worker and hope in the future, which are key-factors of a recovery process. Clients have a positive perception of their situation in this program, on clinical and psychosocial variables, and this perception is stable on the 9 months follow-up of the study. They confirm that self-esteem as a worker, counselors and supervisors are key-factors to ensure their job tenure in the program, their positive evolution to work integration on the regular labor market and their recovery. Practical implications and recommendations from this research are the type of support that should be developed in France for people a with severe mental illness to help their work integration, as well as the type of training and supervision that should be offered to the professionals supporting them

Pełny tekst źródłaHaving a child with a disability confronts the parent with multiple difficulties. This thesis focuses on the psychopathological topics (parental distress and burnout) as well as the personal resources of parents of children with a disorder or handicap. Study 1 aims to understand, thanks to quantitative and qualitative approaches, the difficulties encountered in everyday life and in the moments initially dedicated to rest. Even when the child with a disability is an adult, the chronicity of difficulties and the time-consuming tasks have consequences on the personal life of the parent. The wishes for support are expressed (eg. home help) that can relieve the daily load. Study 2 focuses on a qualitative analysis of the experiences of parents of adolescents and adults with disabilities. Analyze of 14 interviews make it possible to identify changes (difficulties and resources) within different areas. The particularity of this study shows that parents maintain their parenting role despite the fatigue generated. Furthermore, they report concerns about the future of their child. Study 3 shows three clusters of parents of adults with mental disorders based on their level of parental distress and their participation in support groups. The clusters differed in the sense of loneliness, optimism and unconditional self-acceptance. The type of parental support would depend on their level of distress. Study 4 aims to study the various aspects related to the organization of family tasks and care of the child in connection with parental burnout. Of the 386 mothers of children with a disorder, 30% are exhausted and have more personal and family-related tasks than non-exhausted mothers

This research is supported by the clinical and theoretical articulation of psychoanalytical allegiance. The general assumption is as follows: in the setting of a psychotherapeutic caretaking, a mentally handicapped adolescent is able to produce a psychic elaboration of his handicap, which I name “work on the handicap”. The setting of the psychotherapeutic caretaking leads the mentally handicapped adolescent to build some representations of his own handicap. This research is supported by three psychotherapeutic care takings of mentally handicapped adolescents. Equally rich and complex, the clinical material coming from the three clinical cases demonstrates the carrying out of a work on the handicap, based on a co-construction between the therapist and the subject. The work on the handicap therefore could be the result of a psychic elaboration combining the work of mourning, the transformation of thought and representability. If the work on the handicap turns out to be painful in many regards for the mentally handicapped adolescent subject, it remains essential for his psychic construction

The earlier is the stroke, and the more numerous the body parts impaired, the lesser are the mentalization capacities - capacities of regression, projection and fantasy. They are related to the familly investment. When the results of the projective tests are very poor and not correlated with the result in school, there exist and essential depression - i. E. Not mentalized - due to the type of disease, to the length of the treatment, and or to the absence of family support. Pełny tekst źródłaWhen we hear about a crime or an act of violence, the first questions we ask ourselves concern generally the author and the victim then the act which was committed. With regard to the high rates of victimization of the disable people, it seems relevant and important to investigate how disability is apprehended in the field of justice. This thesis is situated at the intersection of three different domains that are social psychology, handicap and criminology. Its main purpose is to examine the influence of disability in the face of an unlawful situation that the disabled person is the author or the victim of the offense. Five research combining descriptive and experimental approaches have been made. The first two studies focus on the analysis of court records (domestic violence and thefts with violence), with the aim comparison of different factors (individual, family, social and legal) between the situations with and without disabilities. Three experimental studies were conducted to determine the effect of the presence of a disability and the type of disability (physical, intellectual, mental) on the attribution of responsibility of the protagonists, what is felt when an offense is committed and on possible actions. The results indicate a relative similarity factors between the judicial archives whether or not the presence of a disability. Those of the experimental studies show an effect of the presence of a disability as well as the type of disability on rates of responsibility and on what is felt. The implication of these results is discussed from a theoretical and practical perspective

Pełny tekst źródłaEquitherapy, or quite other activity referring to introduction of the horse in a device of care, seems to arouse more and more the interest of the professionals in medical, paramedical and social sectors, although these practices are not regulated, and although few rigorous researches are led on their efficiency. In a one step, the aim of this research is to draw-up the first overview of the indications, practices and perspectives of the equitherapy. It’s based on an investigation sent to 600 French children’s mental healthcare institutions. Institutions proposing this type of activity (N=103) were invited to inform a questionnaire to determine the emphasis on the equine mediation in the psychiatric care and to describe their conception of this specific approach. In a second step, we observed 23 children with multiple mental disorders (pervasive developmental disorders, conduct and emotional disorders, mental retardation) in sessions of equine mediation, over a period of one to three years. They was the subject of an evaluation of their adaptive skills, using two questionnaires especially designed for the parents and the educational team, inspired by the Adaptive Behavior Scale (ABS), adapted by Magerotte in 1978. The identification of patterns of change allows us to note significant progress in almost of children, in most areas. These results help us to increase our understanding of the indications and the specific focus of the frame of intervention. By considering the appreciable amount of establishments proposing this type of care, and profits reported by the professionals, this research attempts to highlight the status of these practices, and their development

All forms of physical and mental handicaps affecting the intrinsic value of the human being. These forms mainly belong to two groups: on the one hand deformity and blindness, and on the other hand madness. From which a two-part inventory looking into nearly three centuries of literary history: epic, archaic poetry, tragedy of the vth century and ancient comedy. The theme is approached from a literary point of view rather than a historical one, by the study of the vocabulary selected by the authors, the dramatic-function of the taint in the scheme of works and the embodiment of the different forms of handicaps in great characters stemming from archaic mythology. That literary point of view is completed by the comparison between poetic fiction and the pathological reality of illness, by refering to the medical works of antiquity, mainly hippocratic treatises. Thus great recurrent subjects can be brought out: the loss of human dignity, affecting the core of human abilities such as standing and autonomous motion, the retrogression towards the monstrosity of the primitive period of the mankind through physical decay and the resurgence of beast in human being

The WHO’s classifications of handicap (ICH) and function (ICF) have changed our understanding of disability. The Disability Discrimination law of 11th February 2005 introduced the concepts of activity and participation. G-MAP is an assessment tool that looks at the limitations of activity and participation by measuring the interaction between individual and environmental dimensions (facilitators/obstacles) that contribute to the handicap. The main factors that help modify the attitudes of professionals and students, are the nature of their work, training and being around people with disabilities in their daily lives. The evolution and development of an organisation sometimes negatively impacts on the participation of employees with a disability. Adapted software ergonomics seems to be a promising way forward, appearing to change the attitudes towards disabled employees. The use of specific work supporting activities for the professionals of an establishment that employs disabled people (ESAT) to develop an understanding of cognitive and behavioural disorders facilitates good employee relationships within the company. The socialised and political vision of disability engendered by the law of February 2005 has considerably improved the image of disabled people in the general population. This change has to be facilitated, but it should not be detrimental to the awareness of underlying health problems. Pełny tekst źródłaMeeting people with intellectual or psychic disabilities is a source of unease for 'normal' people. Until recently, these persons were assigned to institutional places and housing. But at the same time that these placements were being made, theses persons were also gradually invited to find a place within the ideal of the emancipated and empowered individual. It the meeting of these two traditions and their uses that produce what now appears to be the diversity of trajectories that occur in the space of the medical/social bureaucracy. This thesis based on an investigation of their intersections in sheltered work establishments during the period from 1975 to 2005, explores the effects of the encounters between these traditions and their practical uses. The first section presents the mental disabilities in relation to the obligations of social interactions in daily life. In a second stage these infirmities of sign are presented in relation to the historical transformations of these expectations and in relation to the devices established to support these disabilities. The second section, based on interviews of disabled persons and an examination of archives, offers typical segments of these trajectories as viewed by these persons. The third section examines the share of professional mediations in the construction of these intersections. In conclusion, these investigations lead to infer that the practices resulting of this ideal of emancipation, whose current directional words would be accountability, supportive community living and coordinated social work, could lead to new moral obligations and… to new practical uses of theses obligations

Pełny tekst źródłaIntroduction: Adolescence is the age of transformations and changes in balance between the inside and the outside. The program "My Roof and I!" aims at exploring the theme of “the home environment”, as a marker of identity, through the setting up of a program of therapeutic mediation, based on the assumption of the existence of a link between the appropriation of one’s living space and the construction of identity. Method - 65 teenagers with a mental disability took part in the study, being taken care of in a Medical Special Needs Educational Institute presenting a mental disability (average age: 16.58; S.D.: +/-1.55) with an experimental group (n=32) who followed a therapeutic mediation program consisting of 6 sessions and a control group (n=33). The following investigation tools were used: the Toulousian Self-Esteem Scale (E.T.E.S), the Big Five Questionnaire for Children (B.F.Q.-C) and the Multidimensional Anxiety Scale for Children (M.A.S.C), following a test-retest protocol. This quantitative approach is completed by a qualitative approach, focused on sociodemographic, diagnostic aspects and the therapeutic process in workshops. Results – The study includes 3 parts: 1. Singular typology of teenagers in a Medical Special Needs Educational Institute; 2. Development and assessment of an art therapy program from a comparative perspective. Among the main results, two therapeutic working axes can be seen: the somato-emotional axis and the psycho-social axis; 3.Clinical assessment of the workshop process and creative work made by the teenagers. Conclusion - The program of art therapy enables a whole set of dimensions of teenage identity to be improved. The teenagers who benefited from the program gradually appropriate spaces which help them fix their identity. This approach should be developed within the institutions caring for teenagers presenting a mental disability

Pełny tekst źródłaThe Paris Clubhouse is a psychosocial rehabilitation service co-managed by members, users of the service, and staff. It is a recovery oriented non-medicalized place, aiming at the social and professional rehabilitation of people living with mental health problems.We realized an exploratory study (over 18 months), with both a qualitative and quantitative approach, on a sample of 30 subjects, all members of the Paris Clubhouse. The aim of the study was to evaluate the evolution of several variables, aiming to prove the Clubhouse model’s efficiency (quantitative approach): vocational integration, psychosocial skills, quality of life, satisfaction of life, self-esteem, personal efficiency, social support, symptomatology and number of hospitalizations. The other aim was to describe the place of the Clubhouse in the members’ lives and recoveries, through research interviews (qualitative approach).The results confirm that the Paris Clubhouse efficiency with the increase of vocational integration, psychosocial skills, physical quality of life, feeling of global personal efficiency and the decrease of anxious symptomatology, tobacco consumption, antisocial behavior and number of hospitalizations. Furthermore, during the interviews, 87% of members quote at least one characteristic of the Clubhouse as a factor that allows the recovery and improvement of mental health. These results attest to the relevance, in France, of a service such as the Clubhouse to accompany people in their recovery, on a different model from what is usually proposed in France

Pełny tekst źródłaAn orphan disease discovered in the last decade of the 20th century, 22ql 1 syndrome causes progressive disharmony in affected children, beyond the physical troubles that result from the deletion of chromosome 22. Still today, the progressive prognosis of this syndrome remains; little-known and uncertain. This pathology, which creates a specific way of "being to the world", "being in the world", "being for the world", does not only affect the body and its functions, but also the psychological and cognitive functions. This is the main line of our research: to pinpoint the original psychological behaviours of children suffering from 22qll, who, far displaying patterns of deficiency, display the possibilities of both subjectivation and symbolisation. The trauma that the handicap causes to parents, and the fact that one of the family's children suffers from it, affects the place of each member within the family System. Each has a personal path with his own temporality and modalities, time for the family group, time for the handicapped child who has suffered early trauma, but also time in the generational chain, centred round feelings of shame and guilt. The forming of a more comprehensive view of certain of aspects of their lives, of the family set-up and of projective tests, will enable the identification of some of the consequences and stakes concerning the psychological transmission of a family when confronted with this orphan disease, thus showing how these children integrate this painful singularity into their personalities and libidinal economy, while remaining within universal human psychological processes

It highlights the organizational and strategic conditions for success of this type of organization in order to refine the model to better suit the needs of the people concerned and their families

Pełny tekst źródłaMultiple reforms aiming at modernizing the public utilities and controling in a better way their spending have gradually transformed the running and organization of public health institutions in our country. A complete overhaul according to the terms of the new hospital governance accompanied with the introduction of management tools stemmed from New Public Management has been carried out. This work is about the transformations of care organization in health care facilities. The study of the proceduralization of care practices allowed to bring to the fore the role of Evidence-Based Medicine in the enterprise of standardization in care systems. Effects of these management logics on the different registers of the intersubjective bond have been grasped, bringing out the forms they spread in group dynamics and teams instituted, with in the background the transformations of sociocultural meta-framework. A particular care has been attached to health managers'view considering the specificity of their task which set them at the crossroads of clinical, managerial and administrative dimensions. The analysis of clinical situations allowed to bring out the sight of the medicalization of psychological suffering and the extension of the concept of "psychic handicap". Transformations in the notion of psychic care have been confronted with mental health paradigm ; Michel Foucault's work has enable to show that mental health policy system are part of a mode of neoliberal governmental rationality framework

Pełny tekst źródłaIn the field of child psychopathology, a set of diverse behaviors, like disobedience, theft, lying, and verbal and physical aggression, are grouped under the label “Behaviour and Conduct Disorders”. The question of the origins of the category used to name certain types of transgressions raised questions. The following thesis suggests to follow this nosographic term in its scholarly singularity, its sedimentations, its historical temporality and its inscription in a discursive regime. That is why, the work of Georges Heuyer, a pioneering figure in the structuration of the Child and Young Psychiatry field, occupies a significant position in this thesis. Then, it is in this investigation to seize, as close as possible, the name chain starting from the school, going through MDPH and ending at ITEP. Our interest was focused on the medico-administrative assessment process within multidisciplinary teams at the MDPH. Finally, the conducted analysis highlights the way the classification “behavior and conduct disorders” reverberated on the representations of the designated people

Pełny tekst źródłaLong-term studies about outcome for people who are suffering from schizophrenia showed that those with severe mental illness often recover. This epidemiologic research also pointed out that long-term consequences of severe mental illness are not always an aggravation despite ongoing symptoms. A lot of jurisdictions around the world are now calling for mental health services to become “recovery-oriented”. The aim of this study is to focus on experience of people suffering from severe mental illness and who are not supposed to recover from it, with a narrative approach. Therefore, this research suggests a phenomenological study of the transition time between psychiatric long-term care and community care for these people. In this perspective, the research project is made up of two phases. The first phase, called “early phase”, consists of a series of interviews, before the admission of people in a social housing. The second one, called “later phase”, is a second series of interviews, between one and two years after the effective admission. We have seen that far from the traditional representation of schizophrenia, that implies a long-term psychological degradation, the analyzis of clinical material by the grounded theory method shows a rich psychological experience for people, with a preserved desire and hope for the future. This study also seeks the role of the supportive environment in people recovery. Though the health care strategies paradigm is changing, there is still a gap in France between the latter and field practices. In this way, this research assembles several scientific fields, such as psychological, medical, sociological, philosophical and ethical questions, showing the institutional mechanisms impact on the outcome of people who suffer from schizophrenia, when on a long-term hospitalization

Pełny tekst źródłaThrough an ethnographic approach and based on the theory of sociological interactionism as well as the sociology of experience, we offer a study of social stigmatisation and segregation in school. We also study the reactions of individuals that fall victim to them. We base our work on young people diagnosed as suffering from behaviour disorder and who are admitted in Institut Thérapeutique, Educatif et Pédagogique (Therapeutic, Educational and Pedagogical Institute). We start by unravelling the process that leads to the stigmatisation of some pupils as well as their deviant behaviour and the obvious difficulties they face in school. These deviances are described as mental disorders and disability. Thus we can understand the socio-political issues that explain how the setting aside of students with behaviour disorder is justified by those involved. I.T.E.P. develops a strategy of behaviour normalisation based one two ideas: the necessary relief of psychological suffering responsible for the behaviour disorders these young people suffer from, and “social orthopaedics” as a way to re-educate and re-socialise them. This comprehensive approach to care puts them at risk of social isolation and might see them slip into a situation of liminality that can prove to be difficult to do away with. Despite the labelling and the discrediting care they have to deal with, these young people answer by resorting to tactics that aim at shrugging off the stigma they suffer from and embark on a process of « normification » of their own personality and therefore of their behaviour. They aim is first to convince that this kind of care is not justifiable in their case and secondly that they are ready to go back to an ordinary training structure
