The article presents the results of research on a specific recreation space – holiday plots. It is an attempt to identify the conditions for the development of holiday settlements, using the example of the gmina of Stężyca situated in the centre of the Kashubian Lake District. The authors indicate the conditions related to the location of holiday plots, as well as the motivations and expectations of their owners. As a result of the study, it was concluded that natural and landscape assets, as well as temporal and spatial accessibility determined the development of the holiday settlement in the gmina of Stężyca. Equally significant factors are family ties and sentiment towards their place of birth.

Streszczenie:
Introduction The aim of this study was to examine and interrogate outcomes in trauma laparotomy in a South African trauma centre to determine whether systematic factors were associated with any discrepancies in outcome. Methods This was a retrospective review of a prospectively entered trauma registry undertaken at the Pietermaritzburg Metropolitan Trauma Service, Pietermaritzburg, South Africa. The service has developed a hybrid electronic medical record system (HEMR) where clinical data were captured in real time, which were incorporated this into a database. Results During the period from December 2012 to July 2016, 562 patients underwent emergency laparotomy for trauma and the time and date of surgery was recorded in the HEMR. The mean age of all patients was 29.5 years. There were 256 operations during the weekend or over a public holiday, with a mortality of 8% (n = 21) compared with 306 during the week (mortality of 10%, n = 31). This difference was not statistically significant (P = 0.237). A total of 327 operations were performed at night (18:00 – 08:00) and 235 operations were performed during the day (08:00-18:00 Hours). This was a significant difference in mortality (10% (33) vs 7 % (16), P=0.013) These differences persisted if weekends and public holidays were separated out from normal working days. A total of 188 operations were performed on a week night, with a mortality of 11% (n = 20) and 121 operations were performed during a week day, with a mortality of 8% (n = 10). There were 139 operations on a weekend or public holiday night, with a mortality of 9% (n = 13). A total of 114 operations were performed on a weekend or public holiday day with a mortality of 7% (n = 8). A total of 208 procedures were performed with an consultant present. Of these, 32 patients (15%) died. A total of 368 procedures were performed without a consultant present and 8 (2%) died. Conclusions This study demonstrated a discrepancy in outcome for trauma laparotomy, depending on whether the operation was performed at night or during the day. The reasons for this are unclear, although the lack of consultant presence at night in comparison to during the day appears to be implicated.

Streszczenie:
In this paper, I explore the impacts of holiday rentals in the historic centre of Barcelona. The intention is to contribute towards a conceptualisation of this unexplored phenomenon with the aim of better understanding why it represents the new gentrification battlefront in several tourist destinations. I suggest that the rhetoric of the sharing economy conceals the fact that holiday rentals are actually a new business opportunity for investors, tourist companies and individual landlords and, for this reason, long-term residents represent a barrier to capital accumulation. I show that there is an increasing conversion of housing into accommodation for visitors and that such conversion involves different forms of displacement. Importantly, when residents move out, the only buyers tend to be tourist investors. In such a context, I suggest that the growth of vacation flats produces conditions that solely enable the reproduction of further accommodation for visitors, rather than for long-term residential use. I call this process ‘collective displacement’, that is to say, a substitution of residential life by tourism. Ultimately, throughout this paper I suggest the importance of undertaking critical research relevant to those experiencing urban inequalities. Documenting and producing data about the way in which displacement takes place can be a crucial political tool for those who are fighting for staying put.

School holiday enrichment program annually organized by PERMATApintar National Gifted Centre was to uncover hidden talents in science and mathematics. Students in this program were grouped into different courses. In order to run the Introduction to Cryptology course for primary school students, a new module has been developed. This module has been implemented for the first time during the school holiday program in December 2017. Thus, this study aimed at determining mathematics achievement of 34 Malaysian primary school students in Introduction to Cryptology course. An examination consisting of a set of questions on mathematical concepts was being given to each of the students during the final day of the program. Student scores were being evaluated to assess their achievement. Findings indicate that 73.6% of students scored between 80 and 100 in the examination. Students have good achievement in the concept of integers but they have deficiency in the concept of matrices. Hence, it can be concluded that the students indeed have special ability in doing mathematics. Since matrices were new to Malaysian primary school students, thus interesting activities relating to matrices will be included in the module to intensify students’ motivation in mathematics.

Climate change and rising temperatures lead to air-conditioning proliferation in summertime in Mediterranean regions. This study links urban morphology to microclimate. It claims that we can learn from holiday housing morphologies designed with an additive approach during the “Glorious Thirty” (1946-75) French coastal development. It is based on a morphological analysis of four case studies, with on the one hand re-drawing and site visiting, on the other hand assessment of environmental performance through key parameters: Absolute Rugosity, Compactness Ratio, Building Density, Mineralization, Sky View Factor (SVF) and H/W Ratio. Compared to literature reference values of a traditional courtyard morphology, the case studies are less compact and with lower H/W Ratio (higher SVF), but they are less mineral than an historical medieval city centre. This research contributes to the search for semi-collective alternatives (for example additive morphologies) to individual housing in peri-urban areas, with a high environmental performance in summertime.

The Natural Medicine Centre was located on the territory of the former Stanisławow Voivodeship (currently Ivano-Frankivsk Oblast, Ukraine). The centre , active in 1893-1939, was one of backbone of Polish cultural life in the Borderlands. As a popular holiday destination for the intelligentsia, it became an ideal platform for stimulating literary and artistic creativity. The preserved text was created by prominent writers, poets and artists, such as: Kazimierz Wierzyński, Wit Tarnawski, Stefan Norblin, Alina Kwiecińska, Wiktor Podoski, Wojciech Zaleski or prof. In addition, the text brings us closer to fuller knowledge about what entertainment the elite provided themselves during the stay in health resorts. The retrieved manuscript undoubtedly inspires further research into the intellectual contribution and cultural achievements of the medical centre in Kosow for the development of the native culture.

On 3 September 2002, the Spanish national centre of epidemiology (Centro Nacional de Epidemiología - CNE) was alerted to a high number of gastroenteritis cases in Spanish tourists who had travelled to a hotel in Punta Cana on different days during august 2002. Entamoeba hystolitica cysts have been visualised by microscopy in the stools of several patients that sought medical attention in the Dominican Republic. The CNE informed the health authorities in the Dominican Republic and conducted in conjunction an epidemiological investigation. A descriptive study of the 76 initial cases estimated a mean illness duration of 5.1+2.9 days and a exposure period of 3.6+2.2 days. Following a retrospective cohort study, the attack rate was found to be 32.4%. Consequently, 216 (95% CI=114.75-317.25) spanish tourists had probably developed the illness. Stool samples were collected in Spain from untreated patients who still felt unwell. None of the samples were positive for E.hystolitica. On 10 September, a hygiene inspection was undertaken at the hotel. Samples of the ice and meals served at the buffet that day, yielded coliform bacteria. Consumption of water from the resort water system was the only risk factor associated with the symptoms (RR= 3.55; 95% CI =1.13-10.99). To avoid similar outbreaks occurring again at the hotel, it is essential to regularly monitor the water quality and to improve food handling hygiene standards. Basic food hygiene training for food handlers should be mandatory. An international guideline for the management foodborne and waterborne outbreaks among tourists in holiday resorts should be drawn up, involving all competent authorities of both destination and tourist origin countries.

Streszczenie:
Abstract Abstract 4413 INTRODUCTION: Red Blood Cells (RBCs) are stored for up to 42 days during which time multiple changes to the cells and media occur. This is collectively referred to as the “storage lesion.” Despite concern over the age of blood (AoB) transfused and the clinical impact of the storage lesion, there are no published data on the impact of donor clinic closures (DCCs) on the AoB transfused. In Canada, DCCs occur Sundays and statutory holidays and the Canadian Blood Services (CBS) can turn around products at the earliest within 24–48 hours. Thus regular DCCs could potentially affect the AoB transfused. METHODS: Data on all transfusions at London Health Sciences Centre (LHSC) between January 2006 and December 2008 were obtained. On average, LHSC issues approximately 25 000 RBC transfusions per year. Data included AoB and blood group for the transfused product, patient demographics, and admitting medical service. Dates for DCCs were obtained from CBS. Outcome measures included mean and variability of AoB transfused by day, month and year. Secondary outcome measures were the AoB transfused in relation to DCCs, and AoB transfused in various patient groups. Statistical analysis of categorical variables was analyzed with the Fisher's exact test and interval measures with the Student's t-test. RESULTS: A total of 67 449 transfusions were analyzed. The average AoB transfused was 20.5 days (d), with a range of 1 to 42 days. Greatest variation between days of the week was Tuesday and Saturday (20.7 vs 19.4d, p<0.001). Comparing Tuesday (2d post-DCC) and Sunday revealed significantly older blood issued on Tuesday (20.7 vs 19.5d, p<0.001). By month, the oldest blood was transfused in January (22.2d), the freshest in September (18.4d), with a difference of 3.8d (p<0.001). The year 2006 had the oldest AoB, 2007 the freshest (21.3 vs. 18.7d, D=2.6d, p<0.001). Analysis of the 5 days before and after holidays found that older blood was issued post-DCC for all holidays save New Year's Day. The oldest AoB by blood group was AB (31.3d), followed by B (25.8d), A (19.4d) and O (18.3d). Of the medical services, the newest transfusions were used by general surgery (19.2d), ICU (19.2d), and the ER (19.7d). CONCLUSION: The average AoB transfused was 20.5 days. Analysis of data by day of the week demonstrated similar AoB values. However, we did find AoB 2 days after regular DCCs was older than the days prior, as well as after all holiday DCCs except New Year's, likely due to two DCCs directly before it (Christmas Day and December 26). Analysis of AoB by blood group found the oldest blood to be AB, followed by B, A and O, corresponding with the national prevalence of ABO groups. One would expect a rarer group to have a smaller product pool from which to draw. Services using the freshest blood were surgery, ICU and ER, likely due to greater use of O units in resuscitation. Despite a trend of older RBCs being transfused in the period following DCCs the minor absolute differences observed are unlikely to represent a clinically significant impact for the recipient. Disclosures: No relevant conflicts of interest to declare.

Introduction: Acute bacterial pneumonia is a common and potentially fatal disease where early recognition and treatment are crucial. Increasing medical literature suggests worse outcomes in patients admitted for medical and surgical conditions during the weekend. Little is known about this effect in patients with acute bacterial pneumonia. Objective: The aim of this study was to evaluate the impact of weekend and holiday hospital admission on the outcomes of acute bacterial pneumonia.Material and Methods: Retrospective analysis of adult patients (> 18 years) with acute bacterial pneumonia collected from a tertiary referral center database. Length of stay, total cost, admission to intensive care unit, development of sepsis and organ failure, and mortality were compared between patients admitted on a weekday and patients admitted during a weekend or holiday. Results: We analyzed 53 854 hospital admissions from 42 512 patients (median age 84.0 years, range 18 - 118 years), corresponding to 30 554 admissions during weekdays, 21 222 at weekends and 2078 during public holidays. Weekend and holiday admission was not associated with increased costs, length of stay, intensive care unit admission, development of sepsis, organ failure, and mortality. Conclusion: A weekend/holiday effect in acute bacterial pneumonia was not evident in our series.

Background Surgical scheduling is complicated by both naturally occurring and human-induced variability in the demand for surgical services. Surgical demand time series are decomposed into periodic, lagged, and linear trends with frequent occurrences of nonconstant variations in mean and variance. The authors used time series methods to model surgical demand time series in order to improve the scheduling of scarce surgical resources. Methods With institutional approval, the authors studied 47,752 surgeries undertaken at a large academic medical center. They initially extracted periodic information from the time series using two frequency domain techniques: the harmonic F test and the multitaper test. They subsequently extracted lagged (correlated) behavior using a seasonal autoregressive integrated moving average model. Finally, they used moving variance filters on the residuals to identify variance in the time series that coincided with major US holidays. Results Linear terms such as periodic cycles, trends, and daily and weekly lags explained 80% of the variance in the raw time series. In the residuals, the authors used moving variance filters to detect nonlinear variance artifacts that correlated with surgical activities on specific US holidays. Conclusions After extracting linear terms, the remaining variance was attributable to a combination of nonlinear and unexplained random events. The authors used the term holiday variance to describe a specific nonlinear disturbance in surgical demand attributable to statutory US holidays. Resolving these holiday variances may assist in management and scheduling of scarce surgical personnel and resources.

Increasingly, foreign contacts are a daily fact of life for many companies. It is therefore remarkable that little to no empirical research has been conducted into the language policy applied by such organizations. Companies with transnational contacts are generally assumed to use English throughout the world as a “lingua franca,” yet whether this is actually the case is questionable. In a multicultural Europe that is striving to unite, there may well be other languages that could qualify as the international language of choice. Research is needed to investigate which language or languages are chosen, by whom the choice is made and why, in certain situations, speakers switch to different languages. This article focuses on the report of an empirical study addressing these aspects. A case study of a Dutch parent company that runs a holiday centre in Germany is presented in order to analyse micro-level interactions which can only be understood by considering the language choice as a result of the way in which the environment is “enacted.” For this analysis, use was made of an interview conducted with the Dutch head of the human resources department, along with transcripts of audio-recorded conversations, and also documents such as the mission statement.

From counterculture to subculture to the ubiquity of every black-clad wannabe vampire hanging around the centre of Western cities, Goth has transcended a musical style to become a part of everyday leisure and popular culture. The music’s cultural terrain has been extensively mapped in the first decade of this century. In this article, we examine the phenomenon of the Whitby Goth Weekend, a modern Goth music festival, which has contributed to (and has been altered by) the heritage-tourism marketing of Whitby as the holiday resort of Dracula (the place where Bram Stoker imagined the Vampire Count arriving one dark and stormy night). We examine marketing literature and websites that sell Whitby as a spooky town, and suggest that this strategy has driven the success of the Goth festival. We explore the development of the festival and the politics of its ownership, and its increasing visibility as a mainstream tourist destination for those who want to dress up for the weekend. By interviewing Goths from the north of England, we suggest that the mainstreaming of the festival has led to it becoming less attractive to those more established, older Goths who see the subculture’s authenticity as being rooted in the post-punk era, and who believe that Goth subculture should be something one lives full-time.

Background Previous studies have shown that there is a peak in sexually transmissible infection (STI) cases and sexual activities around summer, but there has been no study examining whether kissing also follows a similar seasonal pattern. The aim of this study was to examine the seasonal patterns of kissing and sex partners among gay, bisexual and other men who have sex with men (MSM). Methods: A short cross-sectional study was conducted among MSM attending the Melbourne Sexual Health Centre between March 2016 and February 2017. Participants were asked to report the number of kissing-only, sex-only and kissing-with-sex male partners in the last 3 months. The mean number of male partners was calculated and stratified by Australia’s seasons. The seasonal trend in the number of partners was assessed by negative binomial regression models. Results: In total, 4391 MSM were included in the analysis. The number of kissing-only and sex-only partners increased significantly from autumn to summer among MSM in Melbourne (Ptrend <0.001). MSM reported the highest number of male partners for kissing-only (mean: 4.91; 95% confidence intervals (CI): 4.78–5.04) and sex-only (mean: 1.91; 95% CI: 1.83–1.99) around summer compared with other seasons. However, the number of kissing-with-sex partners remained stable across seasons. Conclusions: The study data suggest that there is a peak in kissing-only and sex-only partners among MSM around summer and holiday seasons.

Objective To evaluate the prevalence of bisphos-phonate use without a drug holiday and to assess the success of an interdisciplinary approach to manage and discontinue bisphosphonate therapy. Setting The study was completed at one rural family medicine center. Practice Description The practice employs two fulltime pharmacists. Clinical pharmacists' responsibilities include chronic care management as well as collaboration with the team during interdisciplinary clinics. Clinical pharmacists frequently collaborate with other professionals on medication evaluations and quality improvement projects. Practice Innovation The pharmacy team and medical resident collaborated to determine appropriateness of bisphosphonate use. This was a two-phase evaluation. In the first phase, therapies were evaluated based on duration and consistency with guideline recommendations based on a retrospective chart review. In the second phase, the pharmacy and medicine team determined if therapy warranted further continuation or if a drug holiday was needed. The team reached out to providers proactively and provided patient and provider education on discontinuing therapy. Main Outcome Measurements Patient demographics, bisphosphonate regimen and duration of prescription, frequency of drug holidays, and success rate of therapy discontinuation. Results Bisphosphonates were prescribed for an average of 4.2 years with 56 patients prescribed therapy for >5 years. Twenty-one of the 56 patients had a history of a drug holiday, and 13 of the 35 remaining patients had therapy discontinued or a drug holiday was initiated based on the team's recommendation. Patients were more likely to be prescribed therapy for >5 years if they were older and had more provider appointments.

In this article I examine a negotiating strategy observed in telephone calls made by (prospective) clients to the Latin American call centre operation of a multinational company specialised in holiday time-shares. Through this strategy, which I have termed ‘fabricated ignorance’, the (prospective) clients show an unawareness of how the system works in order to gain access to information, services, or benefits that they are not, in theory, entitled to. They do so, among other things, by formulating pre-sequences designed to address a gap in knowledge as a way of dealing with the possibility of their requests being rejected. Essentially, the callers approach the interactions displaying only partial knowledge of the system and manage the conversations in such a way that the agents will be induced to have a false notion of what is going on. I contend that service operationalization, which positions the (prospective) clients as information-disadvantaged relative to the agents, coupled with unfair commercial practices leads them to pursue ways of counterbalancing such imbalances providing thus fertile ground for the emergence of this strategy. Fabricated ignorance is a (prospective) client’s way of sizing up opportunities. Sizing up entails a participant’s assessment of where the interaction is leading, an estimation of the extent to which is conducive to meeting the participant’s goals and the steps that might be needed to achieve them. One avenue for achieving this aim is judging the moment in the encounter when it might be potentially more convenient to make their move and to act out an uninformed stance.

Some people, in order to succeed in the tourist business, emphasize today the importance of regulating and implementing certain standards, as if to avert the doubt in the quality of satisfying tourist requirements. On the other side, the development trend towards "experiencing type" of tourism is emphasized, where emotional value of the tourist stay is at the centre of attention, i.e. a holiday with a purpose. Taking into consideration all difficulties and disproportions, necessary changes are the condition for a newtourist future in Croatia, However, it is necessary then to find a balance between "old" values and the modem way of doing business, with a right concept of development. The author investigates in this sense the necessity of accepting that invisible thread between the traditional and the modem, and their constant permation. He pleads for a creative application of foreign experience and not for a simple copying. Croatia needs positive ideals but without neglecting the possibilities of its own creative spirit and without interrupting the connections with its own tradition. Globalization as a process is a great challenge for Croatian tourism offering chances but also bringing dangers. Development and innovative changes in the field of tourist facilities should, therefore, be directed towards the co-existence of modem rationality and traditional hospitality, discovering and encouraging specific identity in order to offer readily our own values and the richness of our own tradition. Consequently, all these elements should be put into balance as a harmony of the new and the old in shaping tourist facilities and their functioning.

Winter sports tourism is a key economic factor in many mountain regions worldwide. Research to date has concentrated on economic and environmental questions related to winter sports and winter outdoor activities, with the impacts of climate change and adaptation strategies being the dominant fields of discussion. An analysis of German winter holidays indicates that winter sports are only one of several product types. The fact that the majority of German travellers prefer long-haul warm weather destinations in the winter raises the question of triggering factors for winter holiday trips. A qualitative study based on a Germany-wide online discussion forum analyses all aspects of winter trips. It centres on people’s everyday life during the winter and their linked emotions, travel motives, destination choices and general expectations, in particular with relation to trips to Alpine destinations. The results show that the grey winter weather and factors such as cold, humidity, lack of sun and dull atmosphere are the dominant factors in the desire to escape on a holiday trip. Non-skiers expect snow-covered landscapes and an environment conducive to relaxation-related activities and are therefore likely to be looking for sustainable winter products. This supports Alpine destination managers who are developing sustainable winter products in addition to winter sports in order to become more competitive in the non-skier market.

Background: Health experts recommend daily step goals of 10,000 steps for adults and 12,000 steps for youths to achieve a healthy active living. This article reports the findings of a Canadian family project to investigate whether the recommended daily step goals are achievable in a real life setting, and suggests ways to increase the daily steps to meet the goal. The family project also provides an example to encourage more Canadians to conduct family projects on healthy living. Methods: This is a pilot feasibility study. A Canadian family was recruited for the study, with 4 volunteers (father, mother, son and daughter). Each volunteer was asked to wear a pedometer and to record daily steps for three time periods of each day during a 2-month period. Both minimal routine steps, and additional steps from special non-routine activities, were recorded at work, school and home. Results: The mean number of daily steps from routine minimal daily activities for the family was 6685 steps in a day (16 hr, approx 400 steps/hr). There was thus a mean deficit of 4315 steps per day, or approximately 30,000 steps per week, from the goal (10,000 steps for adults; 12,000 steps for youths). Special activities that were found to effectively increase the steps above the routine level include: walking at brisk pace, grocery shopping, window shopping in a mall, going to an entertainment centre, and attending parties (such as to celebrate the holiday season and birthdays). Discussion: To increase our daily steps to meet the daily step goal, a new culture is recommended: “get off the chair”. By definition, sitting on a chair precludes the opportunity to walk. We encourage people to get off the chair, to go shopping, and to go partying, as a practical and fun way to increase the daily steps. This paper is a call for increased physical activity to meet the daily step goal.

Abstract BACKGROUND Most brain tumour patients report clinically significant fatigue. Here, we aimed to explore patients’ views of the experience, acceptability and usefulness of participating in a trial of novel lifestyle interventions for fatigue. METHODS Qualitative sub-study within the ‘BT-LIFE’ multi-centre phase II RCT (submitted separately). Fatigued adult primary brain tumour patients in receipt of one of the trial interventions (‘Health Coaching’: eight coaching sessions targeting lifestyle behaviours; plus or minus ‘Activation Coaching’: two additional interviews targeting motivation to change) took part in a semi-structured interview following completion of the interventions. A realist approach to the ‘framework’ method was used to analyse verbatim transcripts, with inductive and deductive codes assigned to the realist domains of Context-Mechanism-Outcome. RESULTS Twenty themes and 53 sub-themes were derived from the data. Patients’ (n= 21) understanding of and engagement with the interventions were influenced by their expectations, attitudes to life, and experience of living with a brain tumour. Behaviour change was supported by goal-setting, monitoring using a weekly diary, the motivational ‘push’ by coaches, and family/ social support where available. Barriers to engagement included technical diary difficulties, time limitations including work and holiday schedules, and interference from life events. Most patients described beneficial changes in health behaviours, self-efficacy, and general health and wellbeing. About half indicated actual improvement in fatigue levels; others reported no direct change but they could now cope better with fatigue. A minority experienced no change or worsening fatigue. CONCLUSION Most fatigued brain tumour patients were able to make positive changes after lifestyle coaching, despite physical and cognitive impairments. However, their experiences were varied. Those for whom the interventions were a good ‘fit’ with their pre-existing outlook, lifestyle, and physical and emotional capabilities, appeared to achieve most benefit. These qualitative findings will inform further work addressing the disabling symptom of brain tumour-related fatigue.

e13093 Background: Recently a novel clinical trial generation based on a therapeutic strategy driven by early treatment response assessment to therapy by functional imaging was conceived and a central review of the images became necessary. We describe hear the application of a new web-based tool for imaging exchange and online review of PET scan (WIDEN) conceived for the prospective multicenter clinical trial HD0607. Methods: In the HD 0607 clinical trial 367 patients affected by advanced-stage Hodgkin Lymphoma (HL), are treated with 2 ABVD courses and a PET scan performed afterwards (PET-2). PET-2 positive patients shift to an aggressive treatment, PET-2 negative patients continue with ABVD. PET-2 along with the baseline scan (PET-0) is uploaded to a dedicated web site thanks to WIDEN and hence distributed to six expert reviewers. The latter report the scans within 72 hours from the upload and determine the choice whether intensify or not the treatment intensity (NCT identifier 00795613). Results: The average (median) time per scan upload and download were 6’39” (1’35”) and 3’26” (1’53”) respectively. The average (median) PET scan size were 133.8 MB (121.5 MB), with a minimum size of 51.2 MB and a maximum size of 469.8 MB. The average (median) time frame between the case files upload by the submitting centre and the case review reporting was 48h 45‘ (40h 31’). 7% of the cases were reviewed among the forth and the fifth days. 3% cases were reviewed after the fifth days. In all cases of a review time spanning over more than three days the delay depended on the presence of a week-end of other holiday time. Concordance rate among reviewers measured with Krippendorff,s alpha was 0.799. Conclusions: WIDEN proved to be an effective tool for medical imaging exchange and review. Data security, simplicity, low cost, feasibility and prompt scan review were demonstrated. Its applicability in any clinical trial in which imaging is decisional for treatment modulation is warranted

Abstract. In many cities around the world people are exposed to elevated levels of air pollution. Often local air quality is not well known due to the sparseness of official monitoring networks or unrealistic assumptions being made in urban-air-quality models. Low-cost sensor technology, which has become available in recent years, has the potential to provide complementary information. Unfortunately, an integrated interpretation of urban air pollution based on different sources is not straightforward because of the localized nature of air pollution and the large uncertainties associated with measurements of low-cost sensors. This study presents a practical approach to producing high-spatiotemporal-resolution maps of urban air pollution capable of assimilating air quality data from heterogeneous data streams. It offers a two-step solution: (1) building a versatile air quality model, driven by an open-source atmospheric-dispersion model and emission proxies from open-data sources, and (2) a practical spatial-interpolation scheme, capable of assimilating observations with different accuracies. The methodology, called Retina, has been applied and evaluated for nitrogen dioxide (NO2) in Amsterdam, the Netherlands, during the summer of 2016. The assimilation of reference measurements results in hourly maps with a typical accuracy (defined as the ratio between the root mean square error and the mean of the observations) of 39 % within 2 km of an observation location and 53 % at larger distances. When low-cost measurements of the Urban AirQ campaign are included, the maps reveal more detailed concentration patterns in areas which are undersampled by the official network. It is shown that during the summer holiday period, NO2 concentrations drop about 10 %. The reduction is less in the historic city centre, while strongest reductions are found around the access ways to the tunnel connecting the northern and the southern part of the city, which was closed for maintenance. The changing concentration patterns indicate how traffic flow is redirected to other main roads. Overall, it is shown that Retina can be applied for an enhanced understanding of reference measurements and as a framework to integrate low-cost measurements next to reference measurements in order to get better localized information in urban areas.

The article presents an analysis of the colonial exhibition of 1931 in the context of the metamorphosis of the colonial idea in France. After the First World War, the difficulties in managing the colonies were increasingly felt in France. The French political class hoped to give new vitality to the national consciousness, which was threatened by various social-revolutionary and anti-colonial movements, through the reform of colonial policy. The colonial exhibition of 1931 became the apogee of imperial propaganda in the metropolis and a symbol of unity between the Third Republic with its colonies. Its success was associated with the extent to which the colonial idea penetrated French society and with the stabilization of the mother country's relations with her colonies between the two world wars. The colonial discourse of the 1931 exhibition was an apology for republican centrism expressed through the firm positioning of racial superiority, the demonstration of the validity of the ideals of progress inevitably brought about by colonization, and the dominance of French values. The author demonstrates that the new political situation that developed after the Great War contributed to the achievement of colonial consolidation, on the part of the majority of parties and, mainly, through the deployment of the state propaganda machine. The colonies and the colonial question marked the outlines, the brushstrokes, as it were, of a national union. This union between the national and the colonial, the nation and the empire, was twofold. Between the two world wars, national and colonial issues became logically interlinked and interdependent. The author concludes that the 1931 exhibition propagated the idea of the imperial order through the display and presentation of idealized indigenous cultures represented by a variety of artifacts, fine arts, and architecture. The 1931 exhibition became a general imperial holiday, and was intended to serve the unity between the imperial centre and the colonies. It became an important tool of imperial construction, a fairly effective means of broadcasting the official imperial ideology, and a metaphor for the colonial republic, which embodied the cultural, social, and mental characteristics of the imperial nation; its hidden meaning was directed against the growing ideas of colonial nationalism and resistance.

Purpose The paper aims to explore the array of individual experiences (initial reactions and impact and lasting impressions) relating to Obersalzberg, an authentic site of National Socialist history that had originally been Adolf Hitler’s holiday residence in the 1930s and later became an important Government centre during the Second World War. Obersalzberg is considered a “site of the perpetrators” because here war crimes were planned on a major scale but carried out elsewhere. Design/methodology/approach The study uses innovative methodological approaches to capture initial reactions and lasting impressions of the visit. These include extensive, qualitative interviews with visitors (n = 20), and to clarify the long-term effects of the visit, phone interviews were conducted with selected visitors (n = 22) three months after the visit. The aim of the study is to increase the existing pool of knowledge on the quality of experience of visitors to National Socialist sites. Findings Overall assessment of the exhibition and the analysis of individual sections of it and individual objects clearly showed that a visit to Obersalzberg is an emotional experience for many people that “gets under the skin” and can possibly never be forgotten. Although the impact of the exhibition is different for each individual, various modes of confrontation with the exhibition contents are discernible in the visitors. Although these are naturally also dependent upon the degree of knowledge and the motives for the visit, spontaneous reactions certainly also occur – depending on the intensity of the examination of the topic of National Socialism. The four reactions to the events can be designated as modes of rationalization, memory, empathy and overwhelming distress. Originality/value This paper sheds new light on the reaction of visitors to this kind of exhibition. Visitors find themselves thinking over what they have seen, and together with their overall assessment of the exhibition, visitors go into detail about many particularly moving topics; they are stunned about the world view of those in power at that time and the resulting events; they report on surprising, new knowledge about Obersalzberg; many times, they express empathy toward the evicted residents and experience the bunker as a formidable complex. Their past experiences are often linked to the present, even if the quality and intensity of the experience is different for each individual.

The article presents the results of research of the specifics of second homes as a consequence of accelerated tourism development in the Municipality of Neum in Bosnia and Herzegovina. This is a phenomenon which, primarily due to insufficient data, has not been scientifically researched in more detail. Based on secondary data collected from scant relevant sources, the authors analysed the available data regarding the number of holiday homes/apartments as well as the structural characteristics of holiday home owners. The phenomenon of second homes was analysed from the point of view of its development in Neum, as the only urban-type settlement within the municipality, and in rural settlements in its hinterland. The research showed the expansion of second homes from the narrow coastal zone into the interior and the concentration of holiday home owners in neighbouring municipalities and larger urban centres of Bosnia and Herzegovina, as well as in nearby areas of Dalmatia. The process of internationalisation was also analysed, as a large number of second homes are owned by foreign citizens, primarily those from neighbouring Croatia.

THERE ARE SIGNIFICANT CHALLENGES facing the health system that stem from the tendency for decisions to be made by disparate groups focusing more on the needs of institutions rather than the needs of patients.1 As a result, patient care is becoming episodic, with poor communication leading to inefficiencies, errors and adverse outcomes. An ideal health system provides ?quality care that is centred on the patient, communitybased, coordinated, continuous and cost-effective, and utilises clinical information systems?.2 (page 229) We believe health call centres (HCCs) could be instrumental in achieving such aims. In May 1999, the Western Australian Department of Health in conjunction with McKesson Asia Pacific established the Western Australian Health Call Centre (WAHCC).3 Clinical governance is overseen by three medical directors and a specialist nurse, and staff participate in comprehensive, continuous education and quality improvement. Since its inception, the WAHCC has witnessed an increase in demand and serves about 225 000 callers annually. This amounts to 600 callers per day at a cost (in 2004) of about $28 per call ? 0.0018% of the health budget. Seventy per cent of calls are received after hours, particularly on weekends and public holidays. The initial program offered by the WAHCC was HealthDirect. Services have expanded to include programs and pilot projects listed in the Box .

Abstract Abstract 4368 Paroxysmal nocturnal hemoglobinuria (PNH) is a rare, acquired bone marrow disorder characterised by intravascular hemolysis with resultant anemia often leading to transfusion dependence, severe disabling symptoms and, frequently, life threatening thrombosis. Historically the management of PNH was largely supportive, relatively ineffective and resulted in frequent visits to hospital, admissions, an inability to function normally including loss of employment or other daily activities. PNH is a chronic condition and in most patients persists for the remainder of the patient’s life. Eculizumab was first used for PNH in 2002 and has been licensed since 2007. Eculizumab has been reported to improve all symptoms due to hemolysis in PNH as well as preventing the common complications, such as thrombosis and renal failure and normalising survival. However eculizumab has to be given as an intravenous infusion every 2 weeks indefinitely. In the UK PNH is managed in a shared care model between local hematologists and the National PNH Service from two Centres based in St James’s University Hospital, Leeds and Kings College Hospital, London. Here we report the management of patients treated with eculizumab within the PNH National Service. A total of 130 patients have been treated with eculizumab since May 2002 with 120 currently receiving therapy. 5 patients have died and none were directly related to PNH or eculizumab. 99 patients requiring transfusions prior to eculizumab have been on treatment for at least a year and 65 (66%) of these have not required transfusions for at least the last 12 months. The rarity of PNH means that patients frequently have to travel long distances for review and treatment. This leads to major issues both in terms of time commitment and expense. In order to allow patients to lead as normal lives as possible we have developed a service model in which Specialist PNH Clinics are performed regionally by the PNH Centre and in which patients receive eculizumab every 2 weeks in their homes delivered by a homecare nursing team. In the UK, the PNH Service and Healthcare at Home Ltd (www.hah.co.uk) have been working in partnership for over 7 years during the clinical trials of eculizumab and since its license in 2007. The PNH Service manages the prescription and delivery of eculizumab including an education program for the homecare nurses. This innovative home infusion programme ensures the safe administration of eculizumab outside of the hospital environment, leading to enhanced treatment-associated convenience for patients and their families. Each year the home infusion program has grown, now over 3000 infusions are given annually including whilst patients are on holiday, visiting family, at University or in the workplace. A recent patient survey has been conducted from the 2 PNH Centres to assess the patients’ experience of their PNH diagnosis and treatment. 122 patients responded with 70 of these patients receiving treatment with eculizumab and all currently on the home infusion programme. 63 of 68 patients reported the homecare service as excellent or very good compared to 1 reporting it as poor and 66 of the 68 patients preferred to have their treatment at home compared to hospital. The patients main concerns before starting treatment were reduced life expectancy and the requirement for blood transfusions along with fatigue. With eculizumab treatment and the convenience of homecare 30 patients reported being able to return to work. The homecare service is supported by contact between the clinic appointments, 56 of 67 patients having contact with their PNH Specialist Centre by phone or email in addition to the care of the patient’s local hematology team that over 90% of the patients continue to see. The impact of PNH on patients lives before eculizumab treatment was rated and improved from a median of 3 out of 10 (0 = no quality of life; 10 = normal) prior to eculizumab to a median of 8 out of 10 on treatment. In summary, a novel model of provision of care in PNH with Outreach Specialist Clinics, a 24 hour on call service and homecare delivery of eculizumab permits the normalisation of patients’ lives and overcomes most of the hurdles associated with prolonged regular intravenous therapy. This allows patients to benefit fully from eculizumab including reduction in transfusions, the prevention of serious complications, normalisation of quality of life and where appropriate a return to work. Abstract Background The United Kingdom documented a decline of >30% in imported cases of malaria annually between 1996 and 2003; however, there are still approximately 1700 cases and 5–10 deaths each year. Prophylaxis health messages focus on families returning to their country of origin. Methods We reviewed 225 records of patients seen in Cambridge University Hospital Foundation Trust [CUHFT], a tertiary referral center in Cambridge, England. All records of patients seen in CUHFT between 2002–2016 were analyzed in the context of national figures from Public Health England. Results Between 2004–2016, there was no decrease in imported cases of malaria locally or nationally. Plasmodium falciparum remains responsible for most imported infections (66.7%); Plasmodium vivax contributed 15.1%, Plasmodium malariae 4%, and Plasmodium ovale 6.7%; 7.5% (17/225) of patients had an incomplete record. Most cases were reported in people coming from West Africa. Sierra Leone and the Ivory Coast had the highest proportions of travelers being infected at 8 and 7 per 1000, respectively. Visiting family in the country of origin (27.8%) was the commonest reason for travel. However, this was exceeded by the combined numbers traveling for business and holidays (22.5% and 20.1%, respectively). Sixty percent of patients took no prophylaxis. Of those who did, none of the patients finished their chemoprophylaxis regimen. Conclusions Significant numbers of travelers to malarious countries still take no chemoprophylaxis. Health advice about prophylaxis before travel should be targeted not only at those visiting family in their country of origin but also to those traveling for holiday and work.

This study explores, through a thematic analysis, the messages published in Spanish in the Holiday Celebrations and Small Gatherings [Celebraciones y pequeñas reuniones por las fiestas] section of the Center for Disease Control and Prevention's website (CDC). To analyze these messages, this study used the IDEA model. The conclusions suggest CDC prioritized internalization and action messages over distribution and explanation in this section of the website. The conclusions also show that although the CDC incorporated all elements of the IDEA model in the messages, the information shared was a mere translation of the English version of the Holiday Celebrations and Small Gatherings section of the website, showing thus, that the messages did not seem to be created to appeal to Hispanics intentionally. Suggestions are offered to improve the communications delivered by the CDC to the Hispanic community in the United States.

All 110 patients seen in North East Scotland after contracting malaria from foreign travel were treated in the Regional Infection Unit in Aberdeen. Those patients managed there from January 1980 to March 1991 are described. There were 54 episodes of Plasmodium falciparum malaria (49%) and 26 episodes (23%) of Plasmodium vivax malaria. The remainder had either mixed infection or were diagnosed as malaria on high clinical probability. The majority of the patients were male (80%) and under 40 years of age (84%). Most patients were either Caucasians born in the UK (69%) or native Africans (23%) who were students recently arrived for further education or who had returned from visiting their country of origin for summer holidays. The British residents acquired infection either while on oil related business in West or Central Africa (46%) or after travelling on holiday (30%). The peak incidence of presentation was August and September. 93.5% of patients with falciparum malaria had returned or originated from Africa. 42% with vivax malaria had visited Africa and 27% Papua New Guinea. 70% had been prescribed antimalarial prophylaxis but less than half of these took their medication correctly. The majority of patients with falciparum malaria presented within two weeks of arrival in Britain while patients with vivax malaria presented at varying (but generally longer) intervals, 42% being diagnosed more than three months after exposure. Falciparum infection was more severe although there have been no deaths in the unit from malaria. Our experience seemed of interest and worth reporting because of the number of patients whose infection reflected travel related to the offshore oil industry, which is centred in Aberdeen.

AimTo investigate the practice of melatonin prescribing amongst clinicians in outpatient setting and whether the treatment has been reviewed regularly.MethodData was collected from JAC (Pharmacy Computer System) report detailing patients receiving melatonin from pharmacy, cost centre, quantity, date between 1 st Nov 2014 and Oct 2015. Data on in-patient supply were excluded as the focus of the audit was around patients who are on long term treatment and are seen in outpatient clinics or in the community setting.A sample of patients was randomly selected from the list. Details about consultant, clinical indication, whether melatonin was reviewed, date of clinic, other sedatives patients received, type of melatonin prescribed, were recorded by reviewing clinic letters between Nov 2014 and Dec 2015.ResultsA total of 774 patients received melatonin dispensed from pharmacy on 4 or more occasions between Nov 2014 and Oct 2015. In-patient supplies were excluded. 404 out of 774 patients were audited. The sample size was large enough to represent our overall population.8% of patients were prescribed Circadin 2 mg tablets; 61% of patients were on a combination of unlicensed immediate release (IR) melatonin 2 mg capsules and Circadin; 31% of patients were on unlicensed IR melatonin 2 mg capsules.238 (59%) patients attended follow up clinics and had their melatonin reviewed; 81 (20%) patients attended clinics but melatonin review was not documented; 42 (10%) patients who were on long term melatonin had not attended a clinic appointment for over a year and another 39 (10%) patients who were newly initiated on melatonin had not attended a follow up appointment since treatment started; 1% of patients were not due a clinic review since starting. Out of those who didn’t have their melatonin reviewed for over a year or since treatment had started, 52 (64%) patients failed to attend their clinic appointments.26 (6.4%) patients were recorded to be on other sedatives concurrently in their clinic letters – 23 patients on chloral hydrate and 3 patients on promethazine.6 (1.5%) patients were advised to either stop melatonin or take a holiday break, but out of these, 3 patients continued to receive melatonin supply from the pharmacy.About 35% of patients have been on melatonin for 1 to 2 years and another 34% for 3 to 5 years. The longest treatment duration was over 9 years (17 patients).ConclusionMelatonin is prescribed across many different specialties both within the hospital and in the community with the greatest proportion being prescribed for sleep disorders in Attention Deficit Hyperactivity Disorder. There was a general lack of clinic review regarding efficacy, doses and duration of treatment. In over 60% of cases this was due to patients failing to attend clinic appointments. It is questionable why melatonin is continued to be prescribed without regular treatment review. The concurrent use of other sedatives in some patients is debatable. This information has been presented to prescribers. Another audit is planned in the end of 2016 to determine whether review of treatment has improved and will include an evaluation of melatonin efficacy.
