Context.— Misinterpretation of high-grade squamous intraepithelial lesion (HSIL) is an important problem in daily practice and in the College of American Pathologists (CAP) PAP Proficiency Test (PAP-PT). Objective.— To investigate factors related to misinterpretation of HSIL through responses in a proficiency test versus an educational environment. Design.— We retrospectively evaluated 28 000 responses in the PAP Education Program (PAP-Edu) and 59 140 responses in PAP-PT from 1147 field-validated HSIL slides from 2007 to 2014. The related factors, such as program types, preparation types, participant types, and program years, were evaluated. Results.— Overall, 4.0% (2379 of 59 140) of responses for HSIL slides from PAP-PT were misinterpreted as either low-grade squamous intraepithelial lesion (LSIL) or negative, significantly more than those from PAP-Edu (3.2%; 898 of 28 000). However, the false-negative rate (misinterpreted as negative) was 0.9% (519 of 59 140) for PAP-PT, lower than that for PAP-Edu (1.0%; 266 of 28 000). The misinterpretation rates in PAP-PT trended down with time. Misinterpretation rates did not vary significantly by preparation methods. The misinterpretation rate for HSIL in the pathologists' responses was lower than that in cytotechnologists' responses in PAP-PT. More HSIL was misinterpreted as LSIL than as benign in both programs. Cytotechnologists interpreted HSIL as LSIL twice as much as pathologists. The most common false-negative misinterpretations were negative for intraepithelial lesion or malignancy and reparative change. Conclusions.— The higher LSIL misinterpretation rate by cytotechnologists may be related to the differences in reporting responsibilities and proficiency test grading criteria. The trend of gradually decreasing misinterpretation rates of a reference diagnosis of HSIL in the PAP-PT program may be related to higher test-taking confidence and better preparation through educational programs. The fact that pathologists performed better than cytotechnologists in PAP-PT, but not in PAP-Edu, may reflect a heightened approach and attentiveness in the test-taking environment for pathologists.

PurposeThis paper examines reasons of pragmatic functions' misinterpretation of three types of Facebook hashtags: long, short and multiple mixed hashtags.Design/methodology/approachFocus group interviews of 15 English language learners, who are also active users of Facebook and hashtags, are conducted. Thematic analysis is performed through the software Nvivo for arriving at reliable findings.FindingsThe findings show that unknown vocabulary in contents and lowercasing in long and short hashtags are major factors that cause misinterpretation. Also, the symbol of underscore and appropriate usage of upper and lowercasing of alphabets should be adopted in hashtag writing for the improvement in communication and successful conveyance of the intended meaning.Originality/valueThis paper contributes to the online hashtag writing style by finding reasons for the misinterpretations of different types of hashtags. Hashtags have been developed for adequate communication (Livingstone, 2012), but in Pakistan, hashtags are practiced as a “fashionable” trend and thus result in misinterpretation and inadequate communication among readers even of the same background (Tariq and Sarfraz, 2018). Moreover, this study focuses on the trend of hashtagging that is common among university students and particularly second-language learners because they are active users of Facebook and adopt the latest trends quickly.

The thrust of my note (Barr 2014), to which Ken McGoogan was responding (McGoogan 2014), was that in discovering Rae Strait in the spring of 1854 John Rae did not discover the final link in the northwest passage, since a substantial section of that particular variant of the passage some 240 km in length (namely Franklin Strait and Larsen Sound) lying further north, had not yet been discovered. McGoogan has wrongly concluded that I must therefore support the notion that Sir John Franklin discovered the passage. This is an unwarranted assumption. I do not subscribe to this belief; in this, at least McGoogan and I are in agreement. As David Buisseret, the editor of the The Oxford companion to world exploration has elegantly defined it, geographical discovery is ‘the process by which one or more people leave their society and venture to another part of the world [. . .] then return in order to explain what they have seen’ (Buisseret 2007, I: xxiii). Neither Franklin nor any of his officers and men returned.

The criminal justice system struggles to remain current with modern technology, and the result is the conviction of innocent victims due to the misinterpretation of digital forensic data. It is common for criminal trials to rely on digital evidence and not uncommon the guilty acquitted based on digital evidence. The research contained in this report covered some of the factors contributing to the misinterpretation of digital forensic data. Among the contributing factors are lack of professional and ethical standards governing digital forensic investigators, inadequate training, outdated equipment, and lack of standardized protocols for analyzing digital evidence. All are problems leading to evidentiary issues, and improper jury decisions based on shaky digital evidence (Sloan, 2015).

Research conducted for this report also provides recommendations to minimize the risk of digital forensic data misinterpretation. Recommendations included new technology training and establishment of formal ethics training and requirements for all investigators. Other recommendations included adopting the use of behavior analysis and knowledge management during investigations. Results from a 2015 survey to identify the biggest challenges facing digital forensic investigators also included investigator training, new technology and the need to provide digital technology training for prosecutors.

State misinterpretation has been identified as a causal factor in several accidents where humans were operating complex systems in dynamic domains. The concept of state misinterpretation, although undefined, is characterised by its unobservability, and its relative infrequency. These features make gathering data about state misinterpretation difficult. It was therefore decided to use archive incident report data; the NASA Aviation Safety Reporting System database was used. A definition of state misinterpretation was formulated and translated into database queries to retrieve relevant incident reports for a homogeneous set of expert pilots over a fixed time period. These reports were encoded using the Cognitive Reliability and Error Analysis Method suitably adapted to analysis of aviation incidents. Each report was categorised by the type of state misinterpretation, and a taxonomy of these types was developed. Those types which occurred more than 20 times were analysed at three levels of abstraction. First, a concordance of individual actions showed that communication failures, missed observations and distractions were the most common causal factors. Second, the sequences of possible causal actions showed that some sequences are common across different types of error and state misinterpretation. Third, the causal trees for each state misinterpretation type were quantitatively compared. The lack of measured similarity between the trees suggests that the types in the taxonomy are distinct. Most of the analysed incidents were preventable by better management of flight crew actions. Two particular sequences of actions dominated the results. The first is where the flight crew missed an observation when they were distracted by a competing task. The second is where a communication failure between the flight crew and air traffic control occurred. Some suggestions are offered about how flight crews can better manage their actions to prevent the occurrence of some types of state misinterpretation, thereby reducing incident numbers.

This thesis investigates how Old English poets understood the processes of signification and interpretation through analysis of depictions of poor interpreters and the use of 'sign terms' such as tacen and beacen in the longer Old English poems. The first chapter deals with the Beowulf Manuscript, the second and third chapters consider Elene and Andreas within the network of related poems found in the Vercelli Book and the begin- ning of the Exeter Book, the fourth chapter is on the Junius Manuscript, and the conclusion looks at the use of the 'bright sign' motif across all four major poetic codices. I suggest that there is a 'heroic sign-bearing interpreter' character-type which several of the poems utilize or ironically invert, and that poor interpretation is nearly always asso- ciated with hesitation, which often resembles acedia. I also argue that there is greater nuance in the poems' depictions of modes of understanding than has previously been acknowledged: Eve in Genesis B does not stand for the senses which subvert the mind, but rather models the limits of rational thought as a means of understanding God, and Elene does not depict a simple opposition of letter and spirit, but a threefold mental pro- cess of learning about the Cross with analogues in exegesis and Augustine's Trinity of the Soul. Finally, I argue that there is a 'bright sign' motif which functions within a brightness-sign-covenant concept cluster, whose evocation as a traditional poetic unit is not identical to the denotation and connotation of its constituent parts. These strands of inquiry taken together demonstrate how Old English poems invest signs with significance by tapping into a specifically poetic network of allusion.

Hypochondriasis is a disorder that may affect ten percent of all individuals seeking medical care. This places a great burden on the health care resources that are currently available. Unfortunately, very few of these individuals come to the attention of mental health professionals. Various models have attempted to conceptualize hypochondriasis. These include the psychiatric model, the psychodynamic model, the social learning and the perceptual or cognitive abnormality model. The perceptual or cognitive abnormality model suggests that individuals who are hypochondriacal misinterpret and/or amplify normal bodily sensations. These processes lead the individuals to believe they are suffering from a serious disease. Few empirical studies have been conducted to confirm this model, and no research has been conducted testing this model using psychophysiological measures to test whether or not these indices are indeed different for non-hypochondriacal persons. Pain is a symptom often reported by hypochondriacs and this is what usually brings them into contact with the health care system. Being able to measure how hypochondriacs react to the experience of pain would give insight into whether or not they react more strongly to pain than do non-hypochondriacal persons. Although the objective measurement of pain has been considered difficult in the past, recent work by researchers using visual analogue scales have shown them to be valid and reliable instruments for measuring both the sensory and affective dimensions of the pain experience. The present study tested the perceptual and cognitive abnormality model of hypochondriasis using painful physical stimuli (heat stimulation and a cold pressor task) to measure subjects' pain tolerance and to rate their experience of pain. Subjects rated their pain experience on both sensory (intensity) and affective (unpleasantness) dimensions using visual analogue scales. The model was also tested using a psychological stressor, a visualization task which incorporated everyday life events. The psychophysiological measure heart rate was continuously recorded to assess subjects' physiological activity to stress. It was hypothesized that hypochondriacal individuals would withdraw their feet from the cold water bath, before being instructed to, at a significantly higher rate than the control group. It was also hypothesized that visual analogue scale ratings of intensity and unpleasantness would be significantly higher for the hypochondriacal group than for the control group for both cold pressor and thermal radiant heat. Further, it was hypothesized that the hypochondriacal group would exhibit increased heart rate, as well as a longer return to baseline time compared to the control group. In general, the data offered little support for the hypotheses used to test the amplification/misinterpretation components of the perceptual and cognitive abnormality model. Methodological problems with the study were discussed and improvements suggested. Also, problems and advantages of the present model were noted.

This practice-led research project explores how humour can be employed to develop a methodology for examining the socio-political dimensions of contemporary art practice. This research aims to identify and elaborate on how using the evasive strategies and elliptical frameworks associated with ideas of the absurd and nonsense can lead to new ways of understanding the nexus between social, political and cultural practices. This is achieved primarily through an examination of the art practices of Marcel Duchamp, Bruce Nauman, and Martin Kippenberger. These artists contextualise this research because in different ways they all engage with humour as a device to critique conventional notions of how art can be read or understood. Using these strategies the project aims to demonstrate new ways for considering how visual art can use humour to creatively and critically investigate the relationships between art and the social.

Diagnosis of the functional severity of an epicardial coronary stenosis using parameters like Fractional Flow Reserve, FFR (ratio of distal to proximal pressure of a stenotic region), might be affected in the presence of an additional downstream stenosis. In order to assess this effect, we have performed an in-vitro experiment which is used to validate a computational study. Three combinations of serial stenoses were tested: 80%-64%, 80%-80% and 80%-90% area stenosis (AS). The physiological mean hyperemic flow (flow at maximal arterial dilatation) values were obtained using an in-vitro experimental set-up. These flow rates were used as steady flow inputs by time-averaging the spatially averaged flow pulse over two cardiac cycles for the computational study. FFR values were calculated at hyperemic flow using both the experimental and numerical pressure data. As the downstream severity increased from 64% AS to 80% AS, hyperemic coronary flow decreased from 136.4 ml/min to 126.4 ml/min. Flow decreased further to 90.7 ml/min with a downstream severity of 90% AS. FFR of the intermediate stenosis increased from 0.76 to 0.79 and further to 0.88 as the downstream stenosis increased from 64% to 80% with a final severity of 90% AS. Similarly, numerically obtained FFR values increased to 0.83, 0.80 and 0.92 for the corresponding cases indicating an error within 7% of the experimental values. These results indicate that the presence of a downstream stenosis might lead to a clinical misinterpretation of the upstream stenosis severity.
