This study was aimed at checking out a possible connection between detrusor primary lesions and clinical and urodynamic data in patients with cervico-prostatic obstruction. 40 patients were evaluated, of whom 32 had symptomatic benign prostatic hyperplasia and 8 were controls. A biopsy of the right side wall of the bladder was carried out. The lesions found (muscular hypertropy, fibroelastosis, alteration of the nerve fibres) in relation to their gravity, correlated to the presumable date when symptoms started and to three data from urodynamic evaluation (significant residual p.m., degree of obstruction according to Schafer's chart, presence of detrusor-motor instability). The interference of physiological ageing on results is not underestimated, but this correlation exists. However, the lack of fine quantitative evaluations of follow-up on the possible reversibility of the same lesions after removal of the obstruction, make it impossible at this stage to state how close the correlation is.

A steady increase in the number of young sportsmen predetermines the relevance of studying the nature and severity of cardiovascular system remodeling in young athletes, including the development of arterial hypertension (AH) and left ventricular myocardial hypertrophy (LVHM).Objective. To study the prevalence and nature of arterial hypertension (AH) and its associations with signs of cardiovascular remodeling in young athletes taking into account pathogenetic relationship and genetic determination.Characteristics of children and methods. The study included 80 young athletes (including 42 boys, average age 14,5±1,54 years), divided into 4 groups depending on the kind of sports (1 – cyclic, 2 – game, 3 – complex coordination, 4 – power). We used the following research methods: “office” (i.e. during visit to the doctor) measurement of blood pressure, electrocardiography, echocardiography, color Doppler scanning of the neck vessels, daily monitoring of blood pressure, bicycle ergometry, genetic testing using the original panel with 448 markers.Results. An increase in blood pressure at rest was detected in 2,5% of children engaged in power sports, an insufficient decrease at night – in 16,3% of children, and an excessive reaction of blood pressure to dosed physical activity – in 25% of children. These changes correlated with signs of sports remodeling, in particular with the formation of left ventricular myocardial hypertrophy, which was diagnosed in 7–12,5% of athletes. According to the results of genetic testing, pronounced polymorphism in the NO synthase gene at three loci (rs10918594, rs12143842, rs16847548) was associated with a reorganization of the cardiovascular system in 45% of athletes.Conclusion. Violation of the circadian profile of blood pressure and excessive reaction to physical activity demonstrate 7,5–25% of young athletes, especially those involved in complex coordination and power sports. These changes were associated with the severity of sports remodeling and determined by a genetic predisposition.

Aim. The aim of this report is to identify variables determining the effectiveness of small football games at subsequent stages of training. Material and Methods. The study material comprised the results of tests conducted among 42 football players who were assigned to 3 categories: youngsters, younger juniors and seniors. The scope of the study included basic somatic features, selected motor skills tests and specific coordination tests of motor skills recommended for football players. Detailed correlation matrices were calculated using Spearman’s signed-rank test between all the analysed variables and the overall game perforance index (OPI). For each training group, morpho-functional profiles were calculated taking variables from previous analyses into account. Variables forming the structure of these models were normalised on the T scale. Results. 11 statistically significant Spearman’s signed-rank correlation coefficients were obtained between game performance indices and the analysed variables. In the 3 training groups of football players, considerable diversity of the internal structure of morpho-functional models was noted. In the group of juniors and seniors, differentiation between features was 1.2 of standard deviation. Conclusions. In terms of fitness, the influence of circulatory-respiratory endurance and speed ability on the indicator of overall performance for the seniors’ game was noted. In turn, taking the neuro-functional side of the players’ motor ability into account, correlations with the effectiveness of the game were mainly found within the ability of balance, kinesthetic differentiation and motor adaptation. Such regularities mainly refer to the youngsters and younger juniors.

(1) Objective: To use optical coherence tomography angiography (OCTA) and microperimetry (MP) to evaluate the correlation between retinal structure and function in patients with idiopathic, full-thickness macular holes (FTMHs) (2) Methods: This prospective, observational study included 11 eyes of 10 patients with FTMHs evaluated before surgery using OCTA and MP. MP sensitivity maps were superimposed and registered on slabs corresponding to superficial capillary plexus (SCP) and deep capillary plexus (DCP) on OCTA, and on the outer plexiform layer (OPL) and the Henle fiber layer (HFL) complex in en face OCT. On these maps, mean retinal sensitivity was calculated at 2° and 4°, all centered on the FTMH. Cystic cavity extension was assessed on the slab corresponding to the OPL + HFL complex in en face OCT and DCP in OCTA using the Image J software (Version 1.49v; National Institutes of Health, Bethesda, MD, USA); (3) Results: Absolute scotomas were observed corresponding to the FTMH. Additionally, rings of relative scotoma in the perilesional area were detected and correlated to the cystic spaces on en face OCT and OCTA. There was a significant correlation between reduced retinal sensitivity at 2° and 4° diameters around the FTMH and the extension of cystic areas (p < 0.01). There was a significant correlation between the extension of cystic cavities and BCVA (p < 0.01). (4) Conclusions: Morpho-functional analysis of FTMH using OCTA and MP, and the correlation between vascular abnormalities and impaired retinal sensitivity, may provide new, useful information. This integrated evaluation of FTMH may be useful to determine the function–structure correlation before and after vitreoretinal surgery, in order to gain a better understanding of the functional consequences induced by the morphological alterations, assessing outcomes in a more objective way, and potentially adding new surgical prognostic factors.

A study of cross-impact degree and morpho-functional correlation characteristics of primary open angle glaucoma and age-related macular degeneration was performed at 25 clinical sites in Russia and some CIS countries from January through May 2013. The study final protocol included 87 patients (108 eyes) divided into two groups, mean age of patients was 66.6 ± 5.65 years (min 55; max 81). Intragroup and intergroup variance analysis did not establish any significant difference in the macular area between the «glaucoma» group and the «glaucoma + AMD» group depending on the disease stage. The findings suggest that macular degeneration develops subsequent to a preexisting glaucomatous state.

The aim of this study was to identify new disease-related circulating miRNAs with high diagnostic accuracy for breast cancer (BC) and to correlate their deregulation with the morpho-functional characteristics of the tumour, as assessed in vivo by positron emission tomography/magnetic resonance (PET/MR) imaging. A total of 77 untreated female BC patients underwent same-day PET/MR and blood collection, and 78 healthy donors were recruited as negative controls. The expression profile of 84 human miRNAs was screened by using miRNA PCR arrays and validated by real-time PCR. The validated miRNAs were correlated with the quantitative imaging parameters extracted from the primary BC samples. Circulating miR-125b-5p and miR-143-3p were upregulated in BC plasma and able to discriminate BC patients from healthy subjects (miR-125-5p area under the receiver operating characteristic ROC curve (AUC) = 0.85 and miR-143-3p AUC = 0.80). Circulating CA15-3, a soluble form of the transmembrane glycoprotein Mucin 1 (MUC-1) that is upregulated in epithelial cancer cells of different origins, was combined with miR-125b-5p and improved the diagnostic accuracy from 70% (CA15-3 alone) to 89% (CA15-3 plus miR-125b-5p). MiR-143-3p showed a strong and significant correlation with the stage of the disease, apparent diffusion coefficient (ADCmean), reverse efflux volume transfer constant (Kepmean) and maximum standardized uptake value (SUVmax), and it might represent a biomarker of tumour aggressiveness. Similarly, miR-125b-5p was correlated with stage and grade 2 but inversely correlated with the forward volume transfer constant (Ktransmean) and proliferation index (Ki67), suggesting a potential role as a biomarker of a relatively more favourable prognosis. In situ hybridization (ISH) experiments revealed that miR-143-3p was expressed in endothelial tumour cells, miR-125-5p in cancer-associated fibroblasts, and neither in epithelial tumour cells. Our results suggested that miR-125-5p and miR-143-3p are potential biomarkers for the risk stratification of BC, and Kaplan-Maier plots confirmed this hypothesis. In addition, the combined use of miR-125-b-5p and CA15-3 enhanced the diagnostic accuracy up to 89%. This is the first study that correlates circulating miRNAs with in vivo quantified tumour biology through PET/MR biomarkers. This integration elucidates the link between the plasmatic increase in these two potential circulating biomarkers and the biology of untreated BC. In conclusion, while miR-143-3b and miR-125b-5p provide valuable information for prognosis, a combination of miR-125b-5p with the tumour marker CA15-3 improves sensitivity for BC detection, which warrants consideration by further validation studies.

Correlation of structural and functional data might lead to better identification of hormone-dependent tumors. Sixty breast cancer specimens, sent to the biochemistry laboratory for estrogen receptor (ER) analysis, were studied here by a combined morpho-functional approach. Histological examination of needle biopsies on frozen tissue blocks showed that 12 cases (10%) were free of tumor cells; these cases mostly proved ER negative. On the other 48 cases, an immunocytochemical reaction was performed on the biopsy sections with a monoclonal antibody directed against p 29, an estrogen receptor related antigen. The staining values for p 29 and the biochemical ER findings were significantly correlated. A combined histological, immunocytochemical study seems to offer advantages in the selection of patients for hormonal therapy.

AbstractFrom ancient times, the principal medical cultures tried to classify individuals according to their morpho-functional characteristics in order to study and understand their tendencies in terms of health and sickness. In the last two centuries, in Europe and America, a number of academic schools emerged whose theoretical concepts and ideas were important theroretically but had relatively little practical application. By contrast, in France and Italy, there was the joining of biotypological schools and homoeopathy, which permitted the correlation between biotypes and their relevant homoeopathic remedies. This article outlines the evolution of biotypological thinking and the results of the above said merger.

Severe remodeling processes may occur in the heart due to both genetic and non-genetic diseases. Structural remodeling, such as collagen deposition (fibrosis) and cellular misalignment, can affect electrical conduction at different orders of magnitude and, eventually, lead to arrhythmias. In this scenario, arrhythmogenic cardiomyopathy (ACM) is an inherited heart disease that involves ventricular dysfunction, arrhythmias, and localized replacement of contractile fibers with fibrofatty scar tissue. Unfortunately, nowadays, redicting the impact of fine structural alterations on the electrical disfunction in entire organs is challenging, due to the inefficacy of standard imaging methods in performing high-resolution three-dimensional reconstructions in massive tissues. In this work, we developed a new full-optical correlative approach to quantify and integrate the electrical dysfunctions with three-dimensional structural reconstructions of entire hearts, both in controls and in a mouse model of ACM. We combined optical mapping of the action potential propagation (APP) with advances in tissue clearing and light-sheet microscopy techniques. First, we employed an optical platform to map and analyze the APP in Langendorff-perfused hearts. Then, we optimized the SHIELD procedure for the clearing of cardiac tissue, thus converting the previously electrically characterized samples into well-preserved and fully-transparent specimens. A high-throughput light-sheet microscope has been developed allowing the reconstruction of the whole mouse heart with a micrometric resolution allowing fine quantification of myocytes alignment and fibrosis deposition across the organ. Finally, we developed a software pipeline that employs high-resolution 3D images to analyze and co-register APP maps with the 3D anatomy, contractile fibers disarray, and fibrosis deposition on each heart. We found that although fiber disarray is not involved in this remodeling process, the accumulation of fibrotic patches found in the ACM mouse model can deform the propagating wavefront potentially facilitating the formation of reentry circuits. We believe that this promising methodological framework will allow clarifying the involvement of fine structural alterations in the electrical dysfunctions, thus enabling a unified investigation of the structural causes that lead to electrical and mechanical alterations after the tissue remodeling.

Introduction : Les rétinopathies pigmentaires (RP) sont caractérisées par une grande hétérogénéité clinique et génétique rendant difficile la fiabilité du pronostic évolutif. Les nouvelles techniques d’évaluation laissent envisager la découverte de nouveaux marqueurs prédictifs palliant à ce problème. Matériel et méthode : Les patients sont majeurs et atteints de RP type bâtonnets-cônes. Le projet de thèse comprend l’étude de la qualité de vie (QDV), l’identification de nouveaux marqueurs prédictifs et l’examination du traitement par optogénétique. Résultats : 1) Un diamètre du champ visuel inférieur à 20° et une acuité visuelle inférieure à 0.3 représentent un seuil fonctionnel de dégradation de la QDV et de l’état émotionnel du patient. 2) L’intérêt des examens ophtalmologiques varie selon le stade de progression de la maladie. L’OCT est un examen révélé important à chaque stade de la RP. Le fond d’œil autofluorescent n’est pertinent que lorsqu’un anneau hyper-autofluorescent (HAF) est présent. 3) L’Optique Adaptative (OA), un potentiel nouveau marqueur innovant mais non-standardisé, révèle une diminution de la densité en cônes dans toutes les zones de l’anneau HAF. 4) Le traitement par optogénétique ne peut être proposé qu’à une minorité de patients avec une RP avancée ayant encore des cônes résiduels centraux. Conclusion : Les examens de suivi anatomo-fonctionnels à préconiser varient selon l’évolution de la RP. L’OA fournit de riches espoirs pour le suivi des patients RP par la visualisation des structures fines de la rétine. Les données de QDV et d’état émotionnel, sont indispensables pour mieux appréhender la maladie et avérer de l’efficacité d’un traitement
Introduction: Retinitis pigmentosa (RP) are characterized by a great clinical and genetic heterogeneity making difficult any reliable evolutionary prognosis. New assessment techniques presage discovering new predictive markers overcoming this problem. Materials and Methods: RP patients are over 18 years old and have been diagnosed with rod-cone dystrophy. This thesis studies the impacts on quality of life (QOL), new predictive markers and optogenetics treatments. Results: 1) A diameter of less than 20° visual fields combined with a visual acuity of less than 0.3 set a functional level of degradation of QOL. 2) The benefits of ophthalmic examinations at follow-up RPs vary with stages of the disease. However the OCT exam has been proven beneficial throughout all PR stages; the autofluorescent funduscopy is only relevant when a hyper-autofluorescent (HAF) ring is present in the retina. 3) Adaptive Optics (AO), a new innovative marker not yet standardized, reveals a decrease in the cones density in all areas of the HAF ring. 4) Optogenetics treatments, based on a neuromodulation method, can only be offered to a minority of patients with advanced RP still having central residual cones. Conclusion: The anatomical and functional follow-up examinations to advocate to RP patients vary according to the stages of RP. OA provides rich hopes for monitoring RP patients by viewing the fine structures of the retina. Besides, data on QOL and emotional states are essential to better understand the RP disease and prove the effectiveness of a treatment
