Streszczenie:
While non-headache, non-oral craniofacial neuralgia is relatively rare in incidence and prevalence, it can result in debilitating pain. Understanding the relevant anatomy of peripheral branches of nerves, natural history, clinical presentation, and management strategies will help the clinician better diagnose and treat craniofacial neuralgias. This article will review the nerves responsible for neuropathic pain in periorbital, periauricular, and occipital regions, distinct from idiopathic trigeminal neuralgia. Two patients with occipital neuralgia due to severe arthropathy of the C1–2 facet joint were treated using atlantoaxial fusion with transarticular screws without decompression of the C-2 nerve root. Both patients experienced immediate postoperative relief of occipital neuralgia. The resultant motion elimination at C1–2 eradicated not only the movement-evoked pain, but also the paroxysms of true occipital neuralgia occurring at rest. A possible pathophysiological explanation for this improvement is presented in the context of the ignition theory of neuralgic pain. This represents the first repo

We report a very rare case in which a patient believed to have auriculotemporal neuralgia due to the repeated recurrence of paroxysmal stabbing pain in the preauricular temporal region for four years developed occipital neuralgia, which finally improved with decompression of the greater occipital nerve (GON). The pain of occipital neuralgia has been suggested to be referred to the frontoorbital (V1) region through trigeminocervical interneuronal connections in the trigeminal spinal nucleus. However, the reports of such cases are very rare. In occipital neuralgia, the pain referred to the ipsil

Background: The clinical application of pulsed radiofrequency (PRF) by interventional pain physicians for a variety of chronic pain syndromes, including occipital neuralgia, is growing. As a minimally invasive percutaneous technique with none to minimal neurodestruction and a favorable side effect profile, use of PRF as an interventional neuromodulatory chronic pain treatment is appealing. According to the International Headache Society, occipital neuralgia is classified as a cranial neuralgia. It is characterized by paroxysmal pain in the distribution of the greater, lesser, or third occipital nerve. The pain is typically located in the upper posterior cervical region and radiates to the vertex of the scalp, and it may be reproduced upon palpation of the involved nerve. The diagnosis of occipital neuralgia can definitively be made if the patients achieves pain relief after a local anesthetic nerve block. Occipital neuralgia is described as unilateral or bilateral paroxysmal, stabbing pain in the occipital distribution that often is associated with tenderness and occasionally is associated with dysesthesia. Typically, patients are treated conservatively with pharmacologic agents (e.g., NSAIDs, muscle relaxants, antiepileptics); however, conservative treatment has a high failure rate, often resulting in an interventional procedure (e.g., local nerve block, botulinum toxin injection) or surgery (e.g., ganglionectomy, neurectomy, or neuromodulation). Context: Occipital neuralgia is characterized as uni or bilateral pain in the topography of the occipital nerve. The condition may be harrowing or acute and worsens with digital pressure on the back of the neck. The pain appears in the distribution of the greater (GON), minor (LON), and third (TON) occipital nerves. Besides that, it may radiate to the retro-orbital region and angle of the mandible, and it can be associated with paresthesias. A less common cause is direct head trauma, and it is believed that the mechanism for its development is the injury or entrapment of the nerve fibers due t
