Mucoepidermoid carcinoma (MEC) can be rarely found as a primarily intraosseous lesion and mistaken for other intraosseous or odontogenic pathology. Radicular cysts are the most common odontogenic cystic lesions of jaws, these are inflammatory in origin. These cysts result from the proliferation of cell rests of Malassez in the periodontal ligament as a consequence of inflammation, following pulpal necrosis of a nonvital tooth. A case of periapical infection resulting in unilateral maxillary sinusitis and cellulitis of the ipsilateral lower eyelid is presented. The sole symptom was right orbital swelling. The possible pathway for the spread of this type of infection predisposing factors and possible complications are reviewed.
