Many cases of cutaneous vasculitis are druginduced with histology revealing leukocytoclastic vasculitis (LCV). We present a case of levofloxacin- associated LCV successfully treated with prednisone and cessation of the offending drug. Although case reports describe a link between LCV and older fluoroquinolones, such as ciprofloxacin and ofloxacin, recent reports have implicated the newer fluoroquinolone levofloxacin. Over-the-counter products rarely cause unwanted reactions in the oral cavity. Oral reactions to these agents are not specific and might present with various clinical oral findings. Detailed medical history is a key to the proper diagnosis of these lesions and fortunately other diagnostic procedures are rarely needed. Fixed drug eruptions (FDE) are cutaneous adverse drug reaction characterized by well demarcated erythematous plaques which on removal of the offending agent resolves with residual hyperpigmentation patches at the site. FDE to nitroimidazoles and fluoroquinolones have nevertheless been infrequently reported. Awareness about the adverse reaction to the fluoroquinolone-nitroimidazole combination drug and also the likelihood of recurrence with same or similar drugs and the possible cross reaction is eminent.
