Background Endoscopic sinus surgery is performed for many reasons, most commonly for chronic rhinosinusitis refractory to medical treatment. A paradoxical middle turbinate is an anatomic variant that can hinder endoscopic access to the sinuses. No publication has addressed how to surgically treat a paradoxical middle turbinate. Method We present a basic endoscopic surgical approach to conservatively resect a paradoxical middle turbinate in order to improve access to the middle meatus and the sinuses while preserving support and function. Aim: Computed tomography (CT) provides an accurate assessment of sinonasal anatomy and osseocartilaginous pathologies in patients complaining of sinonasal symptoms. Although it is not a routine practice, CT is frequently used in preoperative screening of patients planned for septoplasty and septorhinoplasty. In this study, anatomical variations and ethmoid lateral lamella depths in preoperative CT images of septoplasty and septorhinoplasty operations performed in our clinic were analyzed retrospectively and discussed with current literature. Material and Method: Septorhinoplasty and sept

Results confirmed the middle turbinate to be a useful landmark in performing extensive sinus surgery. The most frequent change due to chronic inflammation seems to be polypous degeneration followed by hyperplastic mucosa. Anatomical variations, being paradoxically bent turbinate and concha bullosa, are not seen frequently.

The anatomical variations of sinonasal cavity are well documented in literature [1-3]. The sinonasal cavity is usually partially or fully imaged in cone beam CT (CBCT) exams taken for the teeth and jaws. So, oral and maxillofacial radiologists need to be familiar with the regional anatomy of the sinonasal complex (Figure 1). However, some anatomical variations can predispose to sinus disease or complicate sinus surgery. Therefore, solid understanding of the imaging anatomy of the sinonasal cavity is of paramount importance in order to eliminate or minimize surgical complications [4, 5]. CBCT s
