This study aimed to assess the relationship between radiation dose and changes in the irradiated myocardial F-18 fluorodeoxyglucose (FDG) uptake after radiotherapy (RT) in breast cancer patients. The data of 55 patients with left and 48 patients with right breast cancer who underwent curative surgical resection and adjuvant three-dimensional conformal RT and staging (PET1), post-adjuvant chemotherapy (PET2), post-RT (PET3), and surveillance (PET4) FDG positron emission tomography/computed tomography (PET/CT) were retrospectively reviewed. The median interval between PET1 and curative surgical resection, between the end of adjuvant chemotherapy and PET2, between the end of RT and PET3, and between the end of RT and PET4 were five days, 13 days, 132 days, and 353 days, respectively. The myocardial-to-blood pool uptake ratio was measured in all patients. For patients with left breast cancer, the 30 Gy- (30 Gy) and 47.5 Gy-irradiated myocardium-to-low-irradiated myocardium (47.5 Gy) FDG uptake ratios were additionally measured. There were no differences in the myocardial-to-blood pool uptake ratios between left and right breast cancer on all PET scans. For left breast cancer, higher 30 Gy and 47.5 Gy uptake ratios were observed on PET3 than on PET1 and PET2. Both uptake ratios decreased on PET4 compared to PET3, but, were still higher compared to PET1. On PET3 and PET4, the 47.5 Gy were higher than the 30 Gy uptake ratios, while there were no differences between them on PET1 and PET2. Although the whole myocardium FDG uptake showed no significant change, the irradiated myocardium FDG uptake significantly increased after RT and was related to radiation dose to the myocardium in breast cancer patients. These results might be an imaging evidence that supports the increased risk of heart disease after RT in patients with left breast cancer.

Abstract Background: Positron emission tomography / computed tomography with fluorodeoxyglucose (FDG-PET/CT) is widely accepted for staging and for end treatment assessment in patients with DLBCL, and has demonstrated to have prognostic impact when used to evaluate early response to chemotherapy (CT) in this group of patients. In the last years, many changes in the way of interpreting PET/CT have been proposed, both for interim and for end of treatment (Barrington SG, J Clin Oncol 2014;32:3048-3058) We present preliminary data of the first Spanish centralized PET/CT review in a Phase 2 randomized trial in young patients with poor prognosis diffuse large B-cell lymphoma (DLBCL), which compares 6 cycles of RCHOP versus 6 cycles of a modified RCHOP regimen, Bortezomid-R CAP (BRCAP). ClinicalTrials.gov Identifier: NCT01848132. The main objective is to evaluate concordance between reviewers in interim and final PET, in order to stablish the best criteria for PET/CT assessment. PET2 ability to predict PET4 result is also analyzed. Methods: A blinded, prospective, centralized review in real time of PET/CT images was realized by the GELTAMO PET network. For each patient, images of basal (PET0), interim PET2 and PET4 and final PET after completion of chemotherapy (PET 6) were sent to a central platform, and then analysed by the review panel composed of seven expert nuclear medicine physicians. PET2 and PET4 were interpreted visually based on Deauville criteria (considering scores 4 and 5 as positive), and also semiquantitavely (considering a positive PET2 when ΔSUVmax≤66% and a positive PET4 when ΔSUVmax≤70%). PET6 was interpreted following Deauville criteria. Final result of every PET/CT was defined as positive or negative by the central review panel. When discordance between visual and semiquantitative analysis was found in interim PET, semiquantitative method was determinant of final result. A positive PET4 result determined dropped out from trial. Concordance between all readers was analysed using Cohen's kappa coefficient. Results: Of the first 76 patients that underwent PET/CT, 64 patients completed PET0, PET2 and PET4; 34 patients underwent PET0, PET2, PET4 and PET6. In the central review, 44/64 patients (69%) were PET2(-) and 20/64 (31%) were PET2(+); 44/64 (69%) were PET4(-) and 20/64 (31%) PET4(+); 21/34 (62%) were PET6(-) and 13/34 (38%) were PET6(+). We found 43 patients with negative PET2 and PET4, 19 patients with positive PET2 and PET4, 1 patient with positive PET2 and negative PET4, and 1 patient with negative PET2 and positive PET4. PET2 result was predictive of PET4 (p<0.001). Concordance between reviewers for PET2 using visual assessment was good (median kappa=0,74) and very good using semiquantitative analysis (median kappa: 0,83). Concordance between reviewers for PET4 using visual assessment was good (kappa=0,72) and very good when semiquantitative analysis was used (median kappa=0,87). For PET6, concordance between readers was moderate (kappa=0.45). Conclusion: In this homogeneus group of patients, a centralized semiquantitative analysis of interim PET after 2 and 4 cycles of chemotherapy improves concordance between readers in comparison with visual analysis. When semiquantitative method is used, PET2 is predictive of PET4 result. In the same way, visual analysis of PET6 using Deauville criteria seems to have a worse concordance between readers, but more patients need to be analyzed to confirm this result. Fluorine-18 fluorodeoxyglucose (FDG) positron emission tomography (PET)/computed tomography (CT) after one cycle of standard chemotherapy in patients with diffuse large B cell lymphoma (DLBCL) was assessed. Prospectively enrolled 51 patients had four PET/CT studies using the same protocol and system: at baseline and after one, three, and six cycles of chemotherapy (PET0, PET1, PET3, PET6). The PET1 and PET6 Deauville five-point score (D5PS) agreed in 60.8%, while PET3 and PET6 D5PS agreed in 90.2%. The absolute and percent changes of peak standard uptake value corrected for lean body mass (SULpeak) compared to baseline were significantly different between PET1 and PET3 (p = 0.001, p < 0.001) and PET1 and PET6 (p = 0.002, p = 0.001), but not between PET3 and PET6 (p = 0.276, p = 0.181). The absolute SULpeak from PET1 predicted treatment failure with accuracy of 78.4% (area under the curve 0.73, p = 0.023). D5PS, SULpeak, and metabolic tumor volume (MTV) were not statistically different between responders versus non-responders, or the one year disease-free versus relapse groups. D5PS and PERCIST responses showed 100% agreement at end-of-therapy. In conclusion, the responses after three and six cycles of therapy showed high degree of agreement. D5PS or MTV after one cycle of chemotherapy could not predict response or one-year disease-free status, but the SULpeak from PET1 was associated with response to first line therapy in DLBCL. Deauville and PERCIST criteria show high concordance.

Semiconductor power devices made from silicon carbide (SiC) reached a level of technology enabling their widespread use in power converters. Two different approaches to implementation of modern traction converters in electric multiple units (EMU) have been presented in recent years: (i) 3.3-kV SiC MOSFET-based three-level PWM inverter with regenerative braking and (ii) 6.5-kV IGBT-based fourquadrant power electronic traction transformer (PETT). The former has successfully reached optimized dimensions and efficiency but still requires a bulky line frequency transformer for multisystem applications. The latter characterizes inherent galvanic isolation from AC traction, which is realized by cascaded system of power electronic cells containing medium frequency transformers (MFT). The downsizing of the 6.5-kV IGBT-based cells is, however, problematic. The present paper proposes a different approach, that involves the use of a fast switching 1.2-kV SiC MOSFETS. The SiC-based PETT proposed in the paper is dedicated first for the DC traction. For multi-system application the input voltage of the proposed PETT can be adjusted using weight-optimized adjusting autotransformer. Thanks to utilization of fast-switching SiCbased power modules the weight and size of the power electronic cells can be optimized in a convenient way.

Radio frequency oscillations up to several hundreds of MHz were observed during turn-off the high-current conducting IGBTs. They are described in this work as Transient Avalanche oscillations. Other than PETT oscillations that happen in the tail current phase, this oscillation appears during the rise of collector-emitter voltage VCE and during the fall of the collectoremitter current ICE. The turn-off process of IGBTs are investigated with the help of measurements, the circuit- and device-simulations to reveal the working mechanisms of transient high-frequency oscillations. For the first time the Transient Avalanche Oscillation is successfully reproduced with computer simulation, which allows the further investigation on the influences of individual parameters. The participation of IMPATT mechanism in Transient Avalanche oscillation is demonstrated. The interaction between IMPATT mechanism und PETT mechanism during the setting up of depletion region in IGBTs was investigated. Measures to suppress such oscillation are discussed on the levels of chip design, module design and the driver concept. Supplying electron current during critical phase is found to be an effctive method to avoid the dynamic avalanche and the TA-oscillation
Radio Frequenz Oszillationen bis zu mehreren hundert MHz wurden während des Abschaltens von Hochstrom leitenden IGBTs beobachtet. Sie werden in dieser Arbeit als Transient Avalanche Oscillationen beschrieben. Anders als PETT Oszillationen, die in der Tail-Strom Phase vorkommen, erscheint diese Oszillation während des Anstiegs der Kollektor-Emitter-Spannung VCE und des Fallens des Kollektor-Emitter Stroms ICE. In dieser Arbeit werden die Abschaltvorgänge von IGBTs mit Hilfe von Messungen, Schaltungs- bzw. Device-Simulationen untersucht, um die Wirkungsmechanismen der transient erscheinenden hochfrequenten Oszillationen zu erklären. Es ist erstmals gelungen solche Transient Avalanche Oszillationen mit der Simulation zu reproduzieren und anschließend die Einflüsse einzelner Parameter zu untersuchen. Die Beteiligung des IMPATT Mechanismus an der Transient Avalanche Oszillation wurde nachgewiesen. Die Interaktion von IMPATT und PETT während Aufbau der Raumladungszone im IGBT wurde untersucht. Maßnahmen zur Unterdrückung der Oszillation werden auf den Ebenen von Chip-Design, Modulauslegung und der Treiberschaltung diskutiert. Das Zuführen des Elektrostromes während der kritischen Phase wird als ein effective Methode zur Vermeidung der TA-Oscillation nachgewiesen

Actuellement, la majorité des convertisseurs embarqués dans des trains circulant sous une caténaire alternative est composée d’un transformateur basse fréquence, puis de redresseurs,alimentant des moteurs de traction via des onduleurs de traction. Les inconvénients majeurs de ces structures sont un volume et une masse embarqués importants, dus au transformateur fonctionnant en basse fréquence. Le rendement est également mauvais, à cause des contraintes de volume et de masse. Grâce aux développements des semiconducteurs haute tension et forte puissance et des transformateurs moyenne fréquence, i.e. de l’ordre de quelques kilohertz, de nouvelles topologies de convertisseurs embarqués, appelées transformateurs électroniques, sont à l’étude. Si plusieurs topologies ont déjà été étudiées dans la littérature, elles n’ont jamais été comparées. L’objectif principal de cette thèse est donc de proposer une méthodologie de dimensionnement des différentes topologies de transformateurs électroniques, afin de pouvoir les comparer. Un état de l’art des différentes structures proposées dans la littérature est présenté dans le premier chapitre de ce mémoire. Le chapitre 2 est consacré à la comparaison de structures indirectes. Pour cela, une méthodologie, permettant d’optimiser le dimensionnement de chaque structure afin de maximiser son rendement sous des contraintes de masse et de volume, a été développée. Elle est ensuite appliquée sur des topologies utilisant des MOSFET SiC, contrairement aux structures à IGBT Si développées dans la littérature. L’inductance magnétisante est considérée afin d’assurer un fonctionnement en commutation douce, et ainsi limiter les pertes. Un troisième chapitre propose un filtre actif innovant, intégré aux DC-DC du convertisseur. Celui-ci a pour but de réduire le volume du condensateur de filtrage des bus intermédiaires, et ainsi le volume total du convertisseur, sans dégrader la fiabilité intrinsèque de celui-ci. Son fonctionnement et son impact sur les pertes du DC-DC y sont étudiés. Enfin, le dernier chapitre est dédié à l’étude des interactions entre le convertisseur embarqué et l’infrastructure ferroviaire. Pour cela, des modèles d’infrastructure 25 kV-50 Hz ont été réalisés. Ceux ci comportent notamment un circuit original modélisant l’effet de peau dans la caténaire. Des résonances à certaines fréquences, caractéristiques de la géométrie du réseau et de la position du train sur celui-ci, ont été mises en évidence dans l’impédance vue par le train. Ces modèles ont aussi été implémentés dans un simulateur numérique, pour alimenter une maquette petite échelle de convertisseur. Ce type de test n’a, a priori, jamais été réalisé sur un transformateur électronique. Une conclusion générale et des perspectives sur les travaux présentés concluent ce mémoire
Current on-board converters, running on AC catenaries, are mainly composed by a low frequency transformer, then rectifiers, supplying traction motors through three-phase inverters. Due to volume and mass constraints on the converter, the efficiency of the transformer is limited. Moreover, this transformer is quite bulky and heavy. Thanks to the development of high voltage and high power semiconductors, such as Si IGBTs or SiC MOSFETs, and of medium frequency transformer, i.e. operating at a few kilohertz, new topologies of on-board converters, named Power Electronic Traction Transformer (PETT), are studied. Though several structures have been studied in the literature, they have never been compared. The main objective of this thesis is, thus, to develop a methodology to size PETT topologies, in order to compare them. In the first chapter, a state of the art of the PETT structures proposed in literature is presented. The second chapter is dedicated to the comparison of indirect topologies. A methodology, optimising the sizing of each structure to maximise its efficiency under mass and volume constraints, is developed. It is applied on topologies using SiC MOSFETs, contrary to Si IGBT structures developed in the literature. The magnetizing inductance is also considered to insure soft switching and reduce the losses. In the third chapter, an novel active filter, included in the DC-DCs of the converter, is proposed. The aim is to reduce the volume of the filtering capacitors on the intermediate buses, and thus, of the entire converter, without impacting the intrinsic reliability of the converter. Its impact on the losses of the DC-DC is studied. The last chapter deals with the interactions between the on-board converter and the infrastructure. Thus, the 25 kV-50 Hz railway network is modeled. It includes a novel circuit, modelling the skin effect in the catenary. Some resonances, dependant on the sector geometry and the train position, are highlighted in the impedance seen by a train. Moreover, the models are implemented in a numerical simulator to supply a small scale mock-up of a PETT. PHIL tests have, a priori, never been carried on a PETT. Introdução: Em março de 2020 a SARS-CoV-2 foi elevada ao status de pandemia. A fim de diminuir a disseminação viral, foi instituído o isolamento domiciliar sobre a população mundial. Tendo início dos casos na China, o Covid-19 distribuiu-se rapidamente pelo globo. Conforme estudos científicos há registros do vírus infectar cães, gatos e ferrets, todavia sem relatos de que se possa ocorrer a transmissão do patógeno presente nos pets aos humanos. Após o isolamento, foi possível observar o aumento de peso em parte da população, dado que conforme pesquisa também foi observado nos animais mantidos como pets no Brasil. Objetivo: Averiguar os impactos do isolamento domiciliar devido a pandemia da Covid-19 sobre a rotina alimentar de animais mantidos como pet no Brasil. Material e Métodos: Foi realizada pesquisa de caráter exploratório, de forma online, via formulário, obtendo 551 respostas de 22 estados e Distrito Federal. Resultados: As respostas obtidas na pesquisa foram referentes ao tipo de animal mantido como pet (cães, gatos, aves, répteis, roedores e outros), mudanças na rotina alimentar e de atividades dos pets anterior e após o isolamento domiciliar, sendo questionado sobre a observação ou não de aumento no escore corporal dos animais. Os dados obtidos após a análise foram que 30% dos pets desenvolveram sobrepeso após a pandemia, podendo relacionar os dados como resultado de maior fornecimento de alimento aos animais, tanto em periodicidade tanto em quantidade em associação a menor quantidade de atividades físicas realizadas com os animais, como atividades de recreação e passeios. Conclusão: Através da análise dos dados, foi constatado que assim como em humanos, percentual dos animais mantidos como pet no Brasil também adquiriram sobrepeso após o isolamento domiciliar. Sendo dado que 30% destes após a pandemia tiveram aumento em seu escore corporal, fato ligado ao maior fornecimento de alimento aos animais pelos tutores, com maior fornecimento de petiscos, gerando uma superalimentação dos pets a qual em conjunto com o menor índice de atividade física realizada com os animais, como a menor periodicidade de passeios e atividades de recreação, levaram ao sobrepeso dos animais mantidos como pet no Brasil.

The outlet distance of large river, Odra in Poland is a place where large area of the terrain is made of peat. Those places are close to the river so the investors would like to use the area for industrial purpose having direct access to the waterways (cheap transport). One of the methods to reinforce the soft soil (peat) is consolidation. In the presented case the surcharging layer is made of fly ashes. The researches have been done at the Technical University of Szczecin that allow to formulate a mathematical description of peat consolidation in case when elasticity modulus of peat increases along with the consolidation process and the permeability coefficient for peat decreases. The method also allows to calculate the terminal settlement of the peat layer in a long time. The method was invented by Z. Meyer. The paper presents the method and the results of practical calculations in the case of peat from area of Szczecin – Skolwin close to Odra River in Poland.

A fundamental study of the influence of polymer surface properties on interactions with cultured human endothelial cells (HEC) may lead to the development of improved i.e. endothelium-lined vascular grafts.We have previously demonstrated that HEC adhere preferably onto moderately wettable polymers.Here we present the results of spreading measurements and scanning- and transmission- electron microscope investigations on the interactions of HEC with moderately wettable tissue culture polyethylene terephthalate (TCPETP) and more hydrophobic PETP. The numbers of adhering HEC on TCPETP were always found to be much higher than the numbers of HEC adhering onto PETP. However, similar adhesion behavior, occurring in both the presence and absence of serum in the incubation medium, was observed at both TCPETP and PETP surfaces during the first 30 minute period after cell seeding. In contrast with PETP, which showed a decrease in cell spreading during further incubation in the presence of serum, cell spreading increased on TCPETP, resulting in a confluent monolayer at two hours. The decrease of spreading on PETP was a result of detachment of cells. On PETP different structured cells, such as cells with detached central portions or cells, which internalized another cell were observed.When HEC were first seeded onto PETP in the absence of serum, no decrease but rather an increase in cell spreading occurred after the addition of serum and during further incubation.HEC spreading on PETP also occurred, when PETP was first precoated with cell adhesion promoting protein(s), such as whole blood, plasma or fibronectin. However, proliferation results indicated that HEC did not form a confluent monolayer after the pretreatment of (TC)PETP with plasma.
