Abstract Background Prophylactic epidural morphine administration after unintentional dural puncture with a large-bore needle has been shown to decrease the incidence of post–dural puncture headache. The authors hypothesized that prophylactic administration of intrathecal morphine would decrease the incidence of post–dural puncture headache and/or need for epidural blood patch after unintentional dural puncture. Methods Parturients with an intrathecal catheter in situ after unintentional dural puncture with a 17-g Tuohy needle during intended epidural catheter placement for labor analgesia wer

Background: Pain management is often inadequately addressed, especially in a mobile surgical camp set up. We try to look at an easier and effective method, i.e. intrathecal morphine for pain relief and it’s practicability in a camp set up.Method: 89 patients under going vaginal hysterectomy under spinal anesthesia were studied. They were randomly allocated to those receiving bupivacaine and intrathecal morphine (n=61) and another group receiving bupivacaine only (n=28). Their need for post operative analgesia and untoward effects like nausea/ vomiting, pruritis and respiratory depression were

Background: Government of Nepal has been conducting Cesarean section under “Safe Motherhood” program all over country. The purpose of this study was to evaluate the efficacy and safety of intrathecal morphine for post cesarean analgesia under spinal anesthesia. Methods: A total of 300 parturients posted for Cesarean section under spinal anesthesia were divided into two groups of 150 each in this prospective randomized case-control study. Morphine group received 0.15 mg of intrathecal morphine mixed in 12 mg of 0.5% bupivacaine heavy while control group received 12 mg of 0.5% bupivacaine heavy

Background This series investigated the quality of analgesia and the incidence and severity of side effects of intrathecal morphine for post-cesarean analgesia administered over a dose range of 0.0-0.5 mg. Methods ONE hundred eight term parturients undergoing cesarean delivery at term and given spinal anesthesia were randomized to receive a single dose of intrathecal morphine (0.0, 0.025, 0.05, 0.075, 0.1, 0.2, 0.3, 0.4, or 0.5 mg). Background and Objectives: Morphine has been used spinally for postoperative pain management for long time. However post-operative nausea, vomiting and pruritus are some of the common adverse effects of it.The incidence of pruritus being 20-100%, the prevention of pruritus remains a major challenge. This study was conducted tocompare the efficacy of chlorpheniramine maleate versus cetirizine in prevention of post-operative pruritus in patients receiving intrathecal morphine for lower segment caesarean section. Postdural puncture headache (PDPH) may follow either deliberate or unintentional (accidental) penetration of the interdigitating meninges, the dura and arachnoid mater. It is one of the most common and clinically important complications of regional anaesthesia and analgesia in the obstetric population. The headache develops as a consequence of cerebrospinal fluid loss, low intracranial pressure and cerebrovascular changes in the upright position and can prove debilitating. The diagnosis is clinical, making thorough assessment and regular review all the more important, to revise treatment plans

Streszczenie:
Pain management after a surgical intervention is one of the fundamental pillars for optimal patient recovery. In obstetric patients, this management may affect the mother and the newborn. The gold standard for analgesic management is the use of intrathecal morphine due to its long-lasting effect; however, adverse effects related to the use of opioids are evidenced, whether administered intrathecally or systemically in case of contraindication to the neuraxial approach or if a long-acting opioid is not available. Cesarean sections have been associated with moderate-to-severe postoperative pain.

Advances in obstetric regional anesthesia, particularly ultrasound-guided techniques, have significantly improved pain relief and safety during childbirth. This chapter explores the latest developments in ultrasound-assisted central neuraxial anesthesia and fascial blocks for post-cesarean analgesia. The use of ultrasound guidance in neuraxial procedures has been extensively studied, demonstrating improved success rates, reduced complications, and increased patient satisfaction. It enhances the accuracy of identifying lumbar intervertebral spaces and facilitates needle insertion, resulting in
