He exhibited fever, back pain, and fatigue, leading to magnetic resonance imaging (MRI) of the brain and orbit, which suggested lymphoma. Castleman's disease is a group of atypical lymphoproliferative disorders. Extranodal involvement is rare. We report a case of a 39-year male who had left painless proptosis for three months. Examination revealed a firm mass in the left inferior orbit, displacing the globe superiorly. MRI showed an irregular mass causing enlargement of the left recti muscles.
