This study describes the patterns and prevalence of advanced osteoarthritis of the wrist. Bilateral wrist radiographs of 1327 patients over the age of 40 years were reviewed. Osteoarthritis was present in 228 (17%) patients (368 wrists). One wrist was affected in 88 (39%) patients and both wrists in 140 (61%). In 1966, Wilhelm published a technique of denervation of the wrist joint based on his own anatomical studies of 1958. Other authors have published their experience in wrist denervations with similar results, but there are some discrepancies in descriptions of the innervation of the wrist. We have studied the innervation of the wrist by dissection under magnification on 20 cadaver limbs and by histological examination of five human foetal wrists. Twenty-four wrists in 19 patients with rheumatoid arthritis affecting the wrist were treated by arthroscopic synovectomy. Range of motion, subjective pain, wrist function and X-ray changes were recorded preoperatively and at an average of 3.8 years after operation.
