In France, the decree published 16 June 1975 ordering the application of the provisions of the labour legislation to public health service staff, with reference to the law of 16 July 1971 "providing for the organisation of continuous professional training under the aegis of the permanent education programme", serves more to require the hospital system to rethink its policy than to actually provide a framework for the organisation of a permanent training system. Permanent education of hospital staff is a varied and complex organisation. Like all organisations in which men work and evolve, it involves the setting up of a specific educational system: adult education, within another system: the health system. It can therefore be looked upon as an open system, in other words as a system related to its environment. How do these two systems co-exist in hospitals? How can they develop together, and what are their objectives? The hospital has become another training institution, in which educational and health care problems appear interconnected and overlapping. It is no longer reasonable to expect progress in the health care and social administration sectors of our hospital system without genuine democratic growth in the training of the adults who work there. Analysing the functioning of this institution (permanent training) within an institution (hospitals), striving to gain some understanding of its present state and future developments - these are preoccupations which anybody involved in training in this environment feels instinctively. Describing these preoccupations and trying to draw some communicable insight from them is not a neutral activity - either for the person involved (the training specialist-researcher), or for the object of his study (the system he observes). Such an approach fits into an ongoing heuristic and epistemological pattern which poses the question of the permanent training programme's resources, in terms of both objectives and methods, confronted with the psychological and social laws which govern the development of the working adult

Previously called ward sister because the activity is widely feminine and marked by the presence of nuns to this post, the nurse managers exercise an activity that exists in hospitals from their origin: the organization and supervision of care services. They can also practice in nurses' schools as master trainers. However, the missions and the status of these supervisors evolved a lot, because of medical advances and technology of care activity that generate an increasing specialization of tasks, but also political and organizational changes. If all nurse managers were first worked as a "caregiver", they are now carrying out duties related primarily to management. In addition, they must be trained in an executive’s school for a year and now belong to a unified category which comprises three sectors, together twelve allied trades. Nurse managers are currently still in France from the health paramedical care, care remains the heart of their business. We analyze in this research what is played in the passage of a trade to a nursing management function in construction skills. It is indeed moving from a culture where priority is the patient to a manager posture, accountant of a balanced budget-related activity. This survey presents four situations of change in french hospitals and health clinics'professionnal techniques. These situations take place in inhouse training and developement interventions aimed at change, considering the money invested in them. Two method are used to identify a communicative structure, as well as to build a communicative analysis based on the four situations : - the method of phenomeno-structural analysis, inspired by levis strauss's structural analysis and phenomenology by husserl, modeled by alex mucchielli. Thanks to this method, a communication structure, result of the play played by the actors, has been brought out from situations of change. The logice behind the play is to make the change impossible. - the method of communicative analysis we have created using the six communicative process that take place in situation of communication,as identified by alex mucchielli. This method has helped us understand the different communicative phenomena which make change impossible. This survey has demonstrated that communicative analysis allows to model situations and, therefore, to compare their architecture. The analysis refines the comprehension of these phenomena and their possible treatment. We have come to the conclusion that current training and development techniques nourish the play of impossible change implicitly played by the actors. We suggest new training and development tehniques that take into account the elements of the situation and use the information and communication science concepts and tools render change possible with the actors

We have described the outcome and the changes in this training course (associations, unions, definitions, programmes, training schools) and we have studied the social features and the career choices by teachers. The education of nurses goes back to a permanent of submission to the medical and administrative authority in hospitals. It is regularly subjected to operating changes in order to influence the recruiting processes of students and their motivations to start a career as a teacher or a nurse. The patient's handling activity is often considered by caregivers to be an undervalue task. In addition, each year it generates health damage among professionals. In response to this disaster of training courses are proposed. Mostly guided by a safe approach, professionals are not very receptive. Training are too often based on a masterful, demonstrative pedagogy centered on learning techniques and their application. This thesis reflects an iterative design approach to "reasoned handling care" training (RHC). The research is based on empirical data collected in the design and deployment of training sessions. An innovative simulation methodology has been implemented not to train the "right gesture" but to help caregivers towards the construction of a suitable gesture which becomes a path of emancipation for the professional and respect for the autonomy and well-being of the beneficiary. The handling activity is thus approached as a problem-solving activity based on the experience of the participants, seeking to bring out their embedded knowledge, working on opportunities for collaboration between the patient and the caregiver. The trainer does not demonstrate, she has a guiding role. The proposed solutions are discussed within the collective during the test of the scenario by the designers, during the simulation that are played in front of an audience of learners or during the debriefing. Debates on professional practices, observed or reported, can thus be engaged offering professionals new perspectives to understand handling in a different way. It becomes a source of emancipation, development for the caregiver and a possibility of autonomy, well-being for the beneficiary

This research is based on a five-year experience (2003-2008) in a gastroenterology department. The author of this thesis has felt the need to request some caregivers having voluntarily participated in a think tank which gathered them regularly in the department; these moments allowed them to express their hardships, their questioning, their doubts, even their suffering. Many years later, the nurse and PhD student who actively participated wanted to know why this experience had motivated some of the caregivers. It was important to meet them ten years later to identify with hindsight the benefits which they had possibly gained from these educational informal times. Thanks to the collected narratives, a hypothesis became obvious: to take care of others, the caregivers has no recourse but to talk, surrounded by his peers. This is the position of the learner which emerges through these learning narratives realized into group practice. By making their choice, these learners have access to four dimensions that characterize the subject : he is capable, sensitive, socially situated and able to lead a reflexion. By the emergence of this enigmatic learning subject within think tank, these caregivers make a criticism of their own knowledge, question themselves and so participate in a process of professional and personal self-growth both for themselves and others

The aim of this work is to analyse the historical process of the hospital nursing professionalization in Portugal by defining its original characteristics, despite the French and British influences. Several factors contributed to the development of nursing as a profession especially the continuous changes in the Portuguese political and religious situation, the progress in medicine and hospitals' improvements. This. Analysis starts by describing the Portuguese historical background, underlining the role played by several religious orders and institutions such as the Misericordias in the care of the sick when nursing becomes a profession. The development of nursing as a profession requires first a training, then the organisation of a restricted group, socially recognized and with a unique identity. The existing original characteristics are revealed by examining the nursing schools from their creation until their standardisation in Portugal, pointing out their organisation as well as the training they provided. The profile and the results of the students of two private nursing schools (managed by Oporto's and Braga'sMisericordias) will be analysed in detail. We will study the evolution of the nursing profession until its regulation and the creation of a system controlling its practice. The nursing associations and their publications will also be taken into account since they contributed to exchange knowledge and to reinforce the group identity. Finally, the analysis of nursing practice at the Hospital GeraI de Santo Antonio (Oporto) enables us to understand how this practice evolved and what changed for the nursing professionals
