Resumo:
Multi drug resistant (MDR) and Extended Spectrum Beta Lactamase (ESBLs) of Pseudomonas aeruginosawere detected. Pseudomonas aeruginosa is a bacterium responsible for severe infections in burn’s units,plasmid DNA analysis and encoded many types of genes responsible for beta-lactamases. To determine thetype of genes responsible for beta-lactam broad spectrum in P. aeruginosa strains isolated from 100 swabsof burn’s units environment, using a molecular methods (PCR) by primers specific to ESBLs (bla ) genesoxacillin hydrolyzing capabilities OXA-10, OXA-4 and Vietnam Extended-Spectrum β-Lactame VEB-1.The results revealed that 15 strains were isolated from burn units environment. All of 15 (100%) werepositive OXA-10 and only one (6.6%) for OXA-4 while the other gene VEB-1 was found in 6 (40%) isolates.تم

β-Lactams disrupt bacterial cell wall synthesis, and these agents are the most widely used antibiotics. One of the principle mechanisms by which bacteria resist the action of β-lactams is by producing β-lactamases, enzymes that degrade β-lactams. In Gram-negative bacteria, production of β-lactamases is often induced in response to the antibiotic-associated damage to the cell wall. Here, we have identified a previously unidentified mechanism that governs β-lactamase production. In the Gram-negative enteric pathogenVibrio parahaemolyticus, we found a histidine kinase/response regulator pair (VbrK/VbrR) that controls expression of a β-lactamase. Mutants lacking either VbrK or VbrR do not produce the β-lactamase and are no longer resistant to β-lactam antibiotics. Notably, VbrK autophosphorylation is activated by β-lactam antibiotics, but not by other lactams. However, single amino acid substitutions in the putative periplasmic binding pocket of VbrK leads its phosphorylation in response to both β-lactam and other lactams, suggesting that this kinase is a β-lactam receptor that can directly detect β-lactam antibiotics instead of detecting the damage to cell wall resulting from β-lactams. In strong support of this idea, we found that purified periplasmic sensor domain of VbrK binds penicillin, and that such binding is critical for VbrK autophosphorylation and β-lactamase production. Direct recognition of β-lactam antibiotics by a histidine kinase receptor may represent an evolutionarily favorable mechanism to defend against β-lactam antibiotics.

Ce travail de thèse s’est focalisé sur la formation de liaison carbone-oxygène et carbone-carbone via des réactions à économie d’atomes catalysées par des complexes de scandium. Après une introduction générale sur le scandium et ses propriétés physico-chimiques, une description détaillée du triflate de scandium (Sc(OTf)₃) met en lumière ses propriétés acides de Lewis et son utilisation en catalyse dans diverses transformations de formation de liaisons carbone-carbone telles les réactions d'aldolisation ou de Michael. Dans le premier chapitre, la cyclisation d'acides δ/ε-alcénoïques et des δ-alcénamides N-protégés catalysée par Sc(OTf)₃ en conditions thermiques ou par activation micro-ondes a été développée pour préparer, pour la première fois, des γ/δ-lactones et des γ/δ-lactames. Au cours de cette étude, la diversité moléculaire qui peut être atteinte par l’utilisation du triflate de scandium en catalyse a pu être mise en évidence. Il a aussi été montré que la sélectivité de la transformation des acides δ/ε-alcénoïques est directement corrélée aux motifs de substitution de la chaîne du squelette alkyle et de la partie alcénique. Dans la plupart des cas, la formation de γ- ou de δ-lactones est observée avec de bons rendements et de hautes régiosélectivités. Toutefois, lorsque le squelette des acides δ/ε-alkénoïques porte des groupements phényles riches en électrons en position géminale, une cyclisation de type Friedel-Crafts entre l’un de ces deux groupements et l’oléfine se produit exclusivement avec des rendements élevés. La réaction d'hydrofonctionnalisation des δ-alcénamides N-protégés catalysée par Sc(OTf)₃ procède via une cyclisation O-sélective originale et rarement rapportée, soit totale soit de manière préférentielle. Dans le second chapitre, de nouveaux complexes de scandium β-dicétiminates de structures bien définies ont été synthétisés et caractérisés notamment par spectroscopie par diffraction des rayon-X. Le potentiel de ces complexes comme catalyseurs a été étudié dans la réaction d’alkylation C(sp²)-H des anilines avec des dérivés styrèniques et des dérivés α-méthylstyrèniques. Au départ des dérivés styréniques, une réaction d’alkylation ortho-sélective conduisant au produit ramifié est observée avec de bons à d’excellents rendements. Elle tolère non seulement une large gamme de dérivés aniliniques mais est également compatible avec certains alcènes non-activés tels que le norbornène et le 1,3-cyclohexadiène. De plus, la réaction peut être menée à l'échelle du gramme avec une faible charge catalytique sans changement notable de l'activité catalytique. Au départ des dérivés α-méthylstyrènes, des produits d’alkylation ramifiés substitués en position para ont été obtenus avec de bons rendements. Enfin, des informations sur le mécanisme ont été collectées à l’aide d’études cinétiques, d’expériences de marquage au deutérium, de synthèses d’intermédiaires réactionnels et d’expériences de contrôle ou compétitives. Des premiers essais de développement d’une version asymétrique de cette transformation ont aussi été menés par l’emploi de complexes de scandium chiraux énantiopurs
This PhD work focused on the formation of carbon-oxygen and carbon-carbon bonds via atom-economical reactions catalysed by scandium complexes. After a general introduction to scandium and its physico-chemical properties, a detailed description of scandium triflate (Sc(OTf)₃) highlights its Lewis acid properties and its use as a catalyst in various carbon-carbon bond-forming transformations such as aldol or Michael reactions. In the first chapter, the cyclisation of N-protected δ/ε-alkenoic acids and δ-alkenamides catalysed by Sc(OTf)₃ under thermal conditions or microwave activation was developed to prepare, for the first time, γ/δ-lactones and γ/δ-lactams. In the course of this study, the molecular diversity that can be achieved by using scandium triflate in catalysis was highlighted. It was also shown that the selectivity of the transformation of δ/ε-alkenoic acids is directly correlated to the substitution patterns of the alkyl backbone chain and the alkene moiety. In most cases, the formation of γ- or δ-lactones is observed with good yields and high regioselectivities. However, when the δ/ε-alkenoic acid backbone bears electron-rich phenyl groups in the geminal position, Friedel-Crafts-type cyclisation between one of these two groups and the olefin occurs exclusively in high yields. The Sc(OTf)₃-catalysed hydrofunctionalisation reaction of N-protected δ-alkenamides proceeds preferentially or exclusively via an original and rarely reported O-selective cyclisation. In the second chapter, novel well-defined β-diketiminate scandium complexes were synthesized and characterized in particular by X-ray diffraction spectroscopy. The potential of these complexes as catalysts was investigated in the C(sp²)-H alkylation reaction of anilines with styrene and α-methylstyrene derivatives. Starting from styrenes derivatives, an ortho-selective alkylation reaction leading to the branched product is observed with good-to-excellent yields. The reaction tolerates a wide range of aniline derivatives and is compatible with certain non-activated alkenes such as norbornene and 1,3-cyclohexadiene. Furthermore, the reaction can be carried out on a gram scale with a low catalyst loading without any noticeable change in catalytic activity. Starting from α-methylstyrene derivatives, branched alkylation products substituted at the para-position were obtained in good yields. Finally, mechanistic information was gathered using kinetic studies, deuterium labelling experiments, synthesis of reaction intermediates and control or competitive experiments. Une des facons de lutter contre la resistance bacterienne est de modifier le cycle -lactame. La modelisation moleculaire laisse a penser que les composes du type -lactame peuvent remplacer les -lactamines dans l'activite qu'elles exercent sur les plp, proteines liant la penicilline, impliquees dans la formation de la paroi des bacteries et particulierement dans la derniere etape de la synthese du peptidoglycane. La synthese de composes du type -lactame possedant des groupements electroattracteurs lies a l'azote du cycle lactame ainsi qu'a la fonction amine de la chaine laterale a constitue l'objectif de ce travail. Cette fonctionnalisation des molecules permet d'accroitre la reactivite des composes et en particulier celle de la liaison c-n du cycle vis a vis de l'acylation par les plp, ce qui se traduit par une meilleure activite antibacterienne. Dans ce contexte, des pyrazolidinones monocycliques notamment n-sulfonees, d'une structure voisine de l'aztreoname, ont ete synthetisees. Des pyrrolidinones ont egalement ete realisees et leur activite biologique a ete comparee avec celle de la lactivicine et d'analogues decrits dans la litterature. La liaison amide du cycle -lactame a aussi ete activee par une deuxieme fonction carbonyle, ce qui permet d'obtenir des composes du type imides susceptibles de posseder une activite antibacterienne accrue. Ces differentes molecules ont ete testees sur les bacteries staphylococcus aureus, escherichia coli et micrococcus luteus. Certains de ces composes ont une activite biologique moderee ou interessante ce qui suggere le developpement de travaux ulterieurs.

AbstractSerum lactate levels are considered a biomarker of tissue hypoxia. In sepsis or septic shock patients, as suggested by The Surviving Sepsis Campaign, early lactate clearance-directed therapy is associated with decreased mortality; thus, serum lactate levels should be assessed. Monitoring a patient’s vital parameters and repetitive blood analysis may have deleterious effects on the patient and also bring an economic burden. Machine learning and trend analysis are gaining importance to overcome these issues. In this context, we aimed to investigate if a machine learning approach can predict lactate trends from non-invasive parameters of patients with sepsis. This retrospective study analyzed adult sepsis patients in the Medical Information Mart for Intensive Care IV (MIMIC-IV) dataset. Inclusion criteria were two or more lactate tests within 6 h of diagnosis, an ICU stay of at least 24 h, and a change of ≥1 mmol/liter in lactate level. Naïve Bayes, J48 Decision Tree, Logistic Regression, Random Forest, and Logistic Model Tree (LMT) classifiers were evaluated for lactate trend prediction. LMT algorithm outperformed other classifiers (AUC = 0.803; AUPRC = 0.921). J48 decision tree performed worse than the other methods when predicting constant trend. LMT algorithm with four features (heart rate, oxygen saturation, initial lactate, and time interval variables) achieved 0.80 in terms of AUC (AUPRC = 0.921). We can say that machine learning models that employ logistic regression architectures, i.e., LMT algorithm achieved good results in lactate trend prediction tasks, and it can be effectively used to assess the state of the patient, whether it is stable or improving.

Adult lactase deficiency – the inability to effectively digest lactose in adulthood – causes digestive discomfort and may be associated with the elimination of a whole category of dairy products from the diet. The objective of the study was to assess the prevalence of the gene associated with lactose deficiency for a sample of 24 thousand people living in Russia. It turned out that among the studied population groups, the frequency of occurrence of the GG genotype in rs4988235 was higher than the average in European populations. In particular, the prevalence of the genotype of lactase deficiency in the group of Eastern Slavs was 42.8% (95% CI: 42.1–43.4%). The regional prevalence of lactase deficiency was also investigated depending on the place of residence.

β-Lactam antibiotics have been extensively employed in bacterial treatment ever since penicillin's groundbreaking discovery. Despite the proliferation of antibiotics in the pharmaceutical sector today, bacteria often evolve defense mechanisms. Chief among these is the production of β-lactamase enzymes, which degrade β-lactam antibiotics, representing a prevalent form of antibiotic resistance. Additionally, these antibiotics exhibit limited biodegradability, with only 20% breaking down naturally. Hence, finding effective methods to mitigate the presence of β-lactam antibiotics is crucial in combating antibiotic pollution.

Occupational compression sleeves are commonly used to relieve pain in the upper and lower extremities. Anecdotal data agree on the benefits of using compression sleeves while performing physical work. The purpose of this study was to assess the effects of wearing compression sleeves on the upper limbs on blood lactate concentration while completing a physically demanding activity. This study included six college-age participants from the University of Texas at El Paso. Anthropometric measurements were taken before the beginning of the study. Afterward, each participant’s basal capillary blood lactate level was established by averaging lactate concentration from three consecutive days. Capillary blood samples were taken from the participant’s earlobe, and lactate concentration (mmol/L) was assessed using the Lactate Plus Lactate Analyzer (Nova Biomedical). After obtaining all the baseline measurements, each participant completed six simulated work sessions, one with sleeves and another without sleeves, at an intensity of 25 Watts, 50 Watts, and 75 Watts a cadence of 60 RPM, and 10 minutes of duration. All participants rested for at least 72 hours between sessions to prevent muscular fatigue effects of lactate concentrations. The use of compression sleeves showed an average reduction of lactate concentration of 0.2 mmol/L. This study shows the potential benefits of wearing compression sleeves while performing activities involving the upper limbs by helping prevent lactate accumulation, a common indicator of muscular effort.
