ABSTRACT Background: A good bowel preparation for colonoscopy is the most important factor that has an impact on visualization and outcome of the procedure. Aim: The primary aim of the study was to assess the quality of bowel preparation as reported by the Boston Bowel Preparation Score (BBPS). The secondary aim of the study was to analyze the patient feedback on ease of administration, palatability, and side effects with the bowel preparation. Materials and Methods: Patients undergoing colonoscopy between March 2018 and September 2018 were enrolled in the study. Patients <18 years of age, those with the previous history of colorectal surgery, emergency procedures in an unprepared colon, and those not willing to participate were excluded from the study. Colon preparation of the patient was decided by senior consultants. A predesigned pro forma that included demography, indication for the procedure, preparation details, dietary recommendations the previous day if any, side effects, and patient’s comfort to preparation was completed by two-independent observers. The BBPS was used to assess the bowel preparation. A score of <5 was deemed inadequate. P < 0.05 was considered statistically significant. Results: The study cohort consisted of 141 patients, of which 78 were male (55.3%). Eighty (56.7%) patients received oral sulfate-based preparation and 61(43.4%) polyethylene glycol-based preparation. Nearly one third of cases reported the solution to be non palatable. 15.4% respondents reported nausea, vomiting and bloating as the major side effects of the preparation. The median duration of colonoscopy was 25 min (8–45 min). One hundred and eighteen patients (83.6%) had a BBPS score of ≥5. Sulfate preparation resulted in better bowel cleansing (P = 0.01). Age, gender, and dosing schedule of preparation, including bedtime dosing of stimulant laxative, did not alter the BBPS score. Conclusion: Sulfate- and polyethylene glycol-based preparations are commonly used for cleansing the colon. Bowel cleansing was adequate in most patients and sulfate-based yielded better bowel cleansing. Nearly 15.4% of patients reported side effects to these preparations.

Resumo:
The aim of this master thesis project was to develop methods for sample preparation and analysis by LC-MS suitable for global metabolomics of human plasma samples. In this thesis six different methods was tested, based on precipitation, solid phase extraction (SPE), and ultrafiltration. The methods differ in their mechanisms of action, but the end goal is the same, to remove the proteins and other high molecular weight compounds from the samples and to retain as many metabolites as possible in a reproducible manner. The LC-MS analysis were performed on a C18 and a HILIC type column using electrospray ionization (ESI) with both positive and negative ionization to cover as much of the metabolome as possible. The MarkerLynx software was then used to extract features from the chromatograms. The coefficient of variation (CV) was calculated and the features with CV > 30 % were removed. The features were then used to compare the methods with each other in order to see whether any of the methods yielded the same capture of features. The largest amount of features was detected for precipitation with MeOH when using a C18 type column. For HILIC type columns precipitation with MeOH with a small addition of H3PO4 resulted in most unique features detected. The ionization mode was found to be less important, compared whit the choice of column, but more features was detected using positive mode than negative mode.

Resumo:
Dans le présent travail, nous avons étendu aux solvants organiques notre méthodologie radiolytique de synthèse de polymères conducteurs (PCs), initialement développée en solutions aqueuses. Dans ce contexte, la polymérisation des PCs a été étudiée par radiolyse gamma dans différents solvants organiques et sous différentes conditions expérimentales. La synthèse radio-induite a, en particulier, été optimisée dans le dichlorométhane grâce à la variation et à l’ajustement de différents paramètres : atmosphère, dose, débit de dose, concentration des monomères, etc. Cette synthèse a ainsi pu mener à la préparation de différents types de polymères conducteurs : poly (3,4-éthylènedioxythiophène), poly (3-thiophène acétique acide) and Poly (3-hexylthiophène). Ces derniers ont été totalement caractérises en solutions ou après dépôt par des techniques analytiques, spectroscopiques et microscopiques complémentaires. Nous avons en particulier démontré la simplicité et la versatilité de la polymérisation radio-induite de TAA que ce soit dans le dichlorométhane ou dans l’eau, et avons mis en évidence quelques différentes notable entre ces deux voies de synthèse. Nous avons, par ailleurs, évalué l’influence de la nature des espèces radiolytiques oxydantes générées dans le dichlorométhane, via la variation de l’atmosphère de travail (N₂, air ou O₂), sur les propriétés des polymères conducteurs radio-synthétises, en particulier dans le cas de P3HT. Parmi les nombreuses propriétés physiques chimiques que nous avons sondées dans le cas de tous nos polymères conducteurs radio- synthétises les propriétés électroniques et électrochimiques ont fait l’objet d’une attention particulière. Nos matériaux ont alors été incorporés au sien de cellules solaires à pérovskite hybrides organiques-inorganique (PSCs) et y ont été utilisés comme matériaux de transport de trous (HTMs). Notre nouvelle stratégie radiolytique de synthèse décrite et étendu dans le présent manuscrit, ouvre sans aucun doute la voie à la préparation de nouveaux PCs nanostructurés, de morphologie contrôlée et aux propriétés augmentées : par exemple grâce à l’utilisation d’une polymérisation en microémulsions ou par le développement d’une copolymérisation raisonnée
The extension of our original radiolytic methodology to the use of organic solvents was an important alternative approach to radiation-induced polymerization of conducting polymers (CPs) in aqueous solutions. The polymerization of CPs was studied by using gamma-radiolysis of several organic solvents under different environmental conditions. The optimization of the synthesis conditions of CPs was then conducted into dichloromethane solvent. After optimization of the synthesis conditions (atmosphere, dose, dose rate, concentration of organic monomers, etc.), the use of dichloromethane radiolysis was successfully employed to synthesize various types of conducting polymers: Poly (3,4-ethylenedioxythiophene) (PEDOT), Poly (3-thiophene acetic acid) (PTAA) and Poly (3-hexylthiophene) (P3HT). The radio-synthesized polymers were fully characterized in solution and after deposition by complementary analytical, spectroscopic and microscopic techniques. Also, the simplicity and versatility of radiation induced polymerization of 3-thiophene acetic acid in dichloromethane and in aqueous solutions was demonstrated. The differences between the two radiolysis routes were highlighted. Furthermore, the influence of generating different oxidizing species under different atmospheres (N₂, air or O₂) upon ɣ-irradiation of dichloromethane solutions containing organic monomers was also studied in particular in case of P3HT. The electronic and electrochemical properties were checked for all radio-synthesized CPs. Accordingly, these polymers were then incorporated in hybrid organic and inorganic perovskite solar cells (PSCs) and used as hole transport materials (HTMs). Our new radiolytic strategy described and extended in this manuscript opens the way for the preparation of new nanostructured CPs with controlled morphology and enhanced properties by using microemulsion polymerization and also for the preparation of processable conjugated materials through copolymerization

Resumo:
Tools that allow cost-effective screening of the susceptibility of cell lines to operating conditions which may apply during full-scale processing are central to the rapid development of robust processes for cell-based therapies. In this study, an ultra scale-down (USD) device has been developed for the characterization of the response of human cell lines to membrane-based processing, using just a small quantity of cells that is often all that is available at the early discovery stage. Key operating conditions investigated were cross-membrane flow rate, cell age prior to processing and cell concentration (viscosity). The impact was evaluated by cell damage on completion of membrane processing as assessed by trypan blue exclusion and release of intracellular lactate dehydrogenase (LDH). Similar insight was gained from both methods and this allowed the extension of the use of the LDH measurements to examine cell damage as it occurs during processing by a combination of LDH appearance in the permeate and mass balancing of the overall operation. The main cell line studied was a clinically relevant human fibroblast. As expected, increased shear rates led to significant increases in rate and extent of cell damage. Cells aged (21°C hold for 24 hours) before processing led to a doubling of the extent of damage. Increased cell concentration from 1x106 to 100x106 cells mL-1 gave no change in the proportion of cells damaged. Preliminary studies showed that increased shear stress also led to morphological changes and the appearance of apoptotic cells post-processing. Two other human cell lines were also tested briefly for cell damage; a neural stem cell line and a prostate cancer cell line. These appear to be less robust than the fibroblasts with, for example ~0%, ~18% and ~42% damage being observed at the lowest shear stress (~44 Pa) conditions for fibroblasts, prostate cells and neural stem cells respectively. The effects of increasing shear rate, age of cells or concentration varied for each of the cell lines studied. Overall, this work suggests how membrane processing may be used for the recovery of human cells for therapy and how USD studies can speed the route to manufacture.

Bakgrund: Barnperspektiv och barns perspektiv är grundläggande för ett barncentrerat förhållningssätt där barnet respekteras och tillåts vara delaktig. Vården ska utformas och genomföras i samråd med barnet och föräldrarna vilket ställer krav på vårdpersonalen att ge individanpassad information utifrån barnets förutsättningar. Syfte: Syftet var att utvärdera implementeringen av bildstöd vid vanligt förekommande procedurer på barnakuten och barnavdelning 64 i Västerås. Syftet var även att undersöka föräldrars uppfattning om bildstödet kan hjälpa barn vid procedurer som insättande av perifer venkateter och provtagning. Metod: Data insamlades genom att 57 föräldrar besvarade en enkät med slutna och öppna frågor. Kvantitativ data analyserades med beskrivande och jämförande statistik och de öppna frågorna analyserades med kvalitativ innehållsanalys. Resultat: Alla barn som planerades genomföra insättning av perifer venkateter eller provtagning erhöll information inför proceduren. Totalt fick 50 barn (88%) information med bildstöd och 7 barn (12%) erhöll endast muntlig information. Samband kunde ses mellan hur föräldrar skattade på en tiogradig skala att informationen var anpassad till barnets ålder och i vilken utsträckning de uppfattade att barnet förstått informationen i bildstödet (rs=0.396, p=0.008). Öppna frågorna resulterade i två huvudkategorier implementering av bildstöd och barns rättigheter inom sjukvården. Slutsats: Studien visade att bildstödet hade implementerats och att bildstöd kan hjälpa barn att förstå och ge dem möjlighet att vara involverade före och under proceduren. Det är viktigt att informationen är anpassad till barnets individuella förutsättningar och att föräldrarna kan delta i bedömningen av detta.
Background: The child perspective is fundamental to a child-centered approach. The care must be provided by inviting the child and the parents to participate. This places demands on the healthcare professionals, who must provide information based on the child’s needs. Aim: To evaluate the use of picture support in procedures at the Children’s hospital in Västerås. A secondary aim was to investigate parents’ experiences of the use of picture support in procedures. Methods: Parents completed a questionnaire with multiple choice alternatives, ten-point scales questions and open-ended questions. Quantitative data were analyzed with descriptive and comparative statistics and the open-ended questions were analyzed with qualitative content analysis. Results: A total of 50 children (88%) who were planned to undergo peripheral venous catheters insertion or blood sampling received picture support prior to the procedure while 7 children (12%) received only verbal information. There was a correlation between how parents estimated that the information was adapted to the child's age and to what extent they perceived that the child understood the information in the picture support (rs=0.396, p=0.008). The open-ended questions resulted in two main categories, the implementation of visual support and children's rights in healthcare. Conclusion: The study showed that the picture support had been implemented and that picture support can help children understand and give them the opportunity to be involved before and during the procedure. It is important that the information is adapted to the child's conditions and the parents can be involved in the assessment of this.

Syfte: Syftet med föreliggande studie var att beskriva förberedelser och distraktionsmetoder som sjuksköterskor använder sig av i samband med smärtsamma omvårdnadsprocedurer för att hjälpa barn i åldern 4-6 år samt vilka fördelar detta hade. Metod: En deskriptiv design användes till den kvalitativa studien. Materialet inhämtades genom semistrukturerade intervjuer med nio sjuksköterskor som arbetade på barnkliniker. Det insamlade materialet analyserades genom kvalitativ innehållsanalys. Huvudresultat: Resultatet av berättelserna kunde kategoriseras i ”Sociala och praktiska förberedelser”, ”Ickefarmakologiska distraktionsmetoder”, ”Farmakologiska distraktionsmetoder” samt ”Distraktionen kan hjälpa barnet att främja dess självkänsla”. Att verbalt och praktiskt förbereda barn inför smärtsamma omvårdnadsprocedurer ansågs underlätta barnets besök på kliniken. Både farmakologiska och ickefarmakologiska distraktionsmetoder användes med gott resultat för att distrahera barnets uppmärksamhet från den smärtsamma omvårdnadsproceduren. Distraktionen ansågs även hindra utvecklingen av en långvarig stick- och sjukhusrädsla genom att barnet fick ett positivt minne och en stärkt självkänsla. Slutsats: Verbal och praktisk information till barn och föräldrar var viktiga aspekter i förberedelsen inför den smärtsamma omvårdnadsproceduren. Såväl farmakologiska som ickefarmakologiska distraktionsmetoder användes i stor utsträckning med gott resultat vilket kunde ha många fördelar på både kort och lång sikt.
Aim: The aim of this study was to describe preparations and distraction techniques that nurses use when dealing with painful care procedures to help children aged 4-6 years, and the benefits they have. Methods: A descriptive design was used for the qualitative study. The materials have been collected by semi-structured interviews with nine nurses working in paediatric clinics. The collected data was analyzed through a qualitative content analysis. Results: The results of the interviews were categorized in “Social and practical preparations”, “Non-pharmacological distraction techniques”, “Pharmacological distraction techniques” and “Distractions can help the child promote their self esteem”. To verbally and practically prepare the child for painful procedures was considered to facilitate the child’s visit to the clinic. Both pharmacological and non pharmacological distraction techniques were used successfully to distract the child’s attention from the painful procedure. Distraction could also prevent the development of a long term fear of hospitals and needle related procedures if the child received a positive memory and there by an enhanced self esteem. Conclusion: Verbal and practical information to children and parents were important aspects in the preparation for the painful procedure. Both pharmacological and non pharmacological distraction techniques were used extensively with good results, which could have many benefits, in both short and long term.

Bakgrund Många barn upplever oro i samband med vistelse inom hälso- och sjukvården. Oron kan försämra barnets förmåga att hantera medicinska procedurer och forma negativa sjukvårdsupplevelser som kan komplicera kommande kontakter med sjukvården. Det finns också ett samband mellan oro och smärta. Lek är ett viktigt sätt för barn att uttrycka sig och olika typer av lek kan i strukturerad form användas i sjukvården för att informera och förbereda barn inför olika medicinska procedurer. Barnkompetens och kunskap kring barns svar på olika förberedande interventioner är av stor vikt för ett professionellt arbete med barn. Syfte Syftet var att undersöka effekter av förberedande lekterapeutiska interventioner på barns oro och smärta i samband med olika medicinska procedurer. Metod En litteraturöversikt av allmän karaktär genomfördes där 17 vetenskapliga artiklar av kvantitativ design utgjorde grunden för resultatet. De vetenskapliga artiklarna inhämtades från databaserna PubMed och CINAHL, och kvalitetsgranskades utifrån Sophiahemmet Högskolas bedömningsunderlag för klassificering och kvalitetsgranskning. En integrerad dataanalys genomfördes och resultatet sorterades in i kategorier och subkategorier.  Resultat Resultatet redovisas genom två huvudkategorier och fem subkategorier. De identifierade huvudkategorierna var: direkta effekter på barns periprocedurala oro och smärta och indirekta effekter på barns periprocedurala oro och smärta. Subkategorierna som identifierades var i sin tur: minskad oro, minskad smärta, ökad följsamhet, minskademaladaptiva beteenden samt minskade fysiologiska stressreaktioner.  Slutsats Resultatet visade positiva effekter av lekterapeutiska förberedelseinterventioner på barns periprocedurala oro och smärta, genom minskade oros- och smärtnivåer. Vidare kunde även indirekta effekter ses på barns periprocedurala oro och smärta genom ökad följsamhet, minskade maladaptiva beteenden och minskade fysiologiska stressreaktioner. Fler studier behövs dock för att bekräfta de indirekta effekterna.
Background Medical procedures and hospitalization can be a cause of anxiety for children. Anxiety canimpede children's ability to cope with medical treatment and complicate further treatment or contact with the healthcare system. There is also a correlation between anxiety and pain. Play is an important way for children to express themselves and different types of play can, in a structured form, be used to inform and prepare children for medical procedures. Pediatric expertise and knowledge about children's responses to various preparatory interventions is important in the professional work with children. Aim The aim of this study was to evaluate the effects of preparatory play interventions on children's anxiety and pain when undergoing medical procedures. Method A literature review was conducted, and the result was based on 17 included scientific articles of quantitative design. The scientific articles were obtained from the two databases PubMed and CINAHL, and their quality was reviewed on the basis of SophiahemmetUniversity's assessment matrix for classification and quality review. An integrated data analysis was performed, and the results were sorted into categories and subcategories. Results Two main categories and five subcategories were identified. The two main categories were: direct effects on children's periprocedural anxiety and pain and indirect effects on children's periprocedural anxiety and pain. In addition, the five subcategories were: reduced anxiety, reduced pain, increased compliance, less maladaptive behaviors and less physiological stress responses.  Conclusions The results showed positive effects of preparatory play interventions on children's periprocedural anxiety and pain. In addition, the results showed increased compliance, lessmaladaptive behaviors and less physiological stress responses, though more studies are necessary to confirm these effects.

In this work the effect of the MEA preparation techniques on the performance of DMFC was evaluated using three different methods of electrocatalyst deposition: i) catalyst coated membrane; ii) catalyst coated carbon paper; and iii) decal deposition. Optimization of the nafion content (5–15 wt. %) at anode and cathode sides of the MEA and the pressure (150–500 atm) were also performed. Activities of both supported and unsupported Pt and PtRu catalysts (Johnson Matthew) were compared in room temperature DMFC (RT-DMFC) using polarization curves. All MEAs prepared were also characterized by electrochemical (cyclic voltammetry, impedance spectroscopy) methods. It was shown that optimal nafion content is 5–10 wt. % at both anode and cathode sides, while the optimal pressure is in the 300–500 atm. range. The unsupported catalysts showed slightly higher power density at RT-DMFC (∼ 14 mW/cm2) as compared to the supported ones (∼10 mW/cm2) at the same Pt load. Variation of the wetness of MEAs upon mounting in DMFC allowed us to increase of the power density of RT-DMFC up to 32 mW/cm2.

Resumo:
This paper presents guidelines to enable a pipeline operator to assess whether the results of a metal loss in-line inspection (ILI) should be deemed acceptable. Acceptance depends on passing two checks, Verification and Validation. Verification is the process which ensures that all planning, preparation, execution, and analysis of the inspection were conducted according to ILI vendor processes to meet specification and prescribed industry standards. It assumes that a successful run is a consequence of good planning, preparation, execution and analysis. Validation is the process by which the results of an ILI run are compared to independent measurements. The Validation process depends on the results of the ILI and available information. Passing both the Verification and Validation checks is required to accept of the ILI run.

Abstract Cross-sectioning is a necessary technique for the failure analysis of integrated circuits. Historically, the majority of samples have been prepared for scanning electron microscope (SEM) analysis. Today's smaller geometry devices, however, increasingly require the improved spatial resolution afforded by the transmission electron microscope (TEM), both in imaging analysis and in elemental analysis. Specific-area cross-section TEM (SAXTEM) analysis allows the failure analyst to identify defects that may go undiscovered in the SEM. A procedure is described for a timely preparation of SAXTEM samples using a focused ion beam (FIB) instrument and a manipulator probe. This procedure extends the state-of-the-art in several key respects: A) no mechanical grinding is necessary, B) samples as large as the FIB chamber can be accommodated, e.g., whole wafers, C) multiple samples can be prepared from one die, D) the procedure is faster and more repeatable than previously reported procedures.

Abstract In order to properly characterize the entire deposition process, evaluation of the coating, including a reliable metallographic preparation technique which reveals the true microstructure, must be performed. Often, recommended metallographic sample preparation methods for thermally sprayed coatings are generic and are not tailored to specific materials. They are time-consuming and, in some cases, may provide inaccurate details (pull-outs, smearing, etc). This could lead to a wrong interpretation of the coating quality. The aim of the investigation was to develop new metallographic sample preparation procedures tailored to different types of coatings (metallic, ceramic, multilayer and composites), in order to reveal a more representative microstructure. A comparative study of different preparation procedures for the examination of various as-sprayed coatings is presented using an optical microscope. The coatings were deposited by atmospheric and vacuum plasma spray (APS and VPS) and high velocity oxygen fuel (HVOF) processes. A separate approach is recommended for choosing the right metallographic preparation procedure for ceramic, metallic, or composite coatings. Applied load and positioning of the mounted sample during preparation are identified as key factors in developing proper procedures. The microhardness of the coating must be considered when determining the applied load. Interesting practical trends in preparation procedures that may lead to superior coating representation and, in some instances, cost and time savings are presented.
