Resumo:
This work examines the country house libraries of Great Britain and Northern Ireland. Hitherto this is an area largely overlooked both in terms of librarianship and social history. The first part of the thesis examines the historical development of book collections within country houses. In addition to pure historical bibliography it also examines the two essentials behind their development, the people and the houses. The work goes on to focus on the current situation within the United Kingdom. By means of a near comprehensive survey of extant collections, the current picture is identified in terms of the types of material, the collectors, the condition of material, the methods of preservation and, significantly, the willingness of owners to concede access. All of these issues are fundamental components in the study and have been investigated further in order to enable the attainment of the final objective, the creation of a national heritage policy for these collections. This final element focuses particularly on methods of preservation, the potential for funding and the question of access to collections. The policy relies on evidence acquired at the earlier stages of the research as well as that acquired during a second survey dealing predominantly with preservation, access and funding. This is coupled with the identification of current best practice within the library preservation field. Extensive use has been made of contact with owners and administrators of collections by means of the surveys, interviews, observational visits and correspondence. In addition, heritage organisations and structures within the United Kingdom have been analysed, with emphasis on those with direct impact on the country house. This research is fundamentally about two things: the historical development of collections and the approaches likely to ensure their survival.

The purpose of this study was to describe the home literacy environments and literacy experiences of a select group of Asian American children, and to recommend an intervention program based on the findings. The target population was the families which sent their children to a Saturday Asian language and culture school while sending them to public schools during the week, because of their expressed interest in literacy and the probability of their being the group to most likely benefit from intervention. The Home Literacy Environment and Literacy Experiences survey was initially sent out and results tallied and quantified. Upon placing the returned surveys into groups of "high," "middle," and "low" home literacy environment and literacy experiences, a sample of five "high" and five "low" families was selected for further study. Home visits, interviews, field notes, collection of artifacts and other methods of data collection provided a clearer picture of the state of the home literacy environment and literacy experiences of the families studied. Families rated as having "high" home literacy environment and experiences were found to have a larger number of literacy-related materials and higher frequency of literacy-related activities. Bilingualism and education were perceived as being important. The families also exhibited a strong interest in music and music lessons. Parents expressed a desire for two two-hour training sessions which would be held at the Saturday school location while their child attended classes there. It would be ideally held in the native language of the parents by a speaker from the native country. The parents preferred workshops with actual practice and examples which could be seen, accompanied by reading materials. Topics in which parents expressed interest include, in descending order: (a) 'selection of books for and with their child, (b) how to encourage their child to read, (c) how to discuss stories with their child, and (d) how to read aloud to their child.

ENGLISH ABSTRACT: Although there are serious hurdles to overcome before green, energy-efficient homes become common; the technological and market-based foundations are already in place to support a shift in standard practice. Many organizations, from the federal government to local non-profits, are driving the transition to more efficient homebuilding practices through research, market-based competition, and tax incentives. However, many builders are resisting the transition, due to the fragmented nature of the building industry and a perceived lack of consumer demand. Because of the nature of the US economy, until American consumers understand green homes and demand builders to build them, green homebuilding will not reach its full potential. If building practices are left unchanged, inefficient homes will continue to cause dire consequences to the world because of their contribution to global climate change.
AFRIKAANSE OPSOMMING: Hoewel daar ernstige struikelblokke bestaan, wat oorkom moet word alvorens groen, energie-doelmatige wonings algemeen raak, is die tegnologiese en markgerigte grondslae reeds gelê om ’n verskuiwing in standaard-praktyk te onderskraag. Verskeie instansies – van die federale regering tot nie-winsgewende organisasies – verleen stukrag deur middel van navorsing, markgerigte mededinging en belastingaansporings aan die oorgang na meer doelmatige gebruike rondom praktyke ten opsigte van die konstruksie van huise. Weens die gefragmenteerde aard van die boubedryf en die waarneembare gebrek aan verbruikersaanvraag staan talle bouers egter die vermelde oorgang teen. Weens die aard van die VSA se ekonomie en totdat Amerikaanse verbruikers groen tuistes kan verkoop en by bouers aandring om hulle op te rig, sal groen woningkonstruksie nie sy volle potensiaal bereik nie. Indien boupraktyke onveranderd gelaat word, sal ondoelmatige wonings as gevolg van hulle bydrae tot globale klimaatsverandering steeds aaklige gevolge vir die wêreld tot gevolg hê.

The British news media often describes Eastern European nationals coming to work in the UK as unskilled economic migrants, framed as ‘unwanted’ and as jeopardising British culture and economy. Often overlooked in news media and scholarship are alternative examples of human geographic mobilities out of and into Eastern Europe, such as individuals who are working in the cultural sector, namely, visual artists. Many artists from the Baltic States must go abroad in order to get onto the global art market; although, they stay connected or return to their homelands, shaping these art scenes through their cultural remittances and transnational networks. It is important to investigate the Baltic States, as it has been 26 years since their independence from the Soviet Union and 12 years since their accession into the EU. The Baltic States are now established members of the EU, after becoming members of the Eurozone and part of the Schengen Area. Visual artists from Latvia, Lithuania and Estonia are crisscrossing the EU, taking part in multi-directional routes and multi-cross-cultural connections for work. Often having multiple bases either at once or throughout their career, a lot of respondents’ feeling of home is spatial and mobile. The feelings of home are a mosaic, constutited by these factors of cross-border communications, regular travel, and having several bases for work. The meaning of home, then, is associated with their artistic practice and about relations to people rather than associated with a fixed, physical place. They are not an ethnic diaspora, as what holds them together is their art - it is about what they all ‘do’ in common. This provides a different understanding of the meaning of diaspora, as not defined only by ethnicity. Together, this study explores individuals who move regularly, working and communicating across territorial borders and across ethnic ‘borders’. In a multi-sited study across Tallinn, Riga and Vilnius and other EU cities, this research uses an ethnographic methodology in order to devise a multi-sited and multi-temporal approach for studying travelling individuals. This research uses in-depth interviews with artists and semi-structured interviews with arts professionals; participant observation with an artist in Vienna, at an art institution in Vilnius, and through communication with three artists online over three months; and a visual analysis of artworks.

Background: Opioids are often used in nursing homes to manage non-malignant pain, but little is known about their long-term use, initiation, and comparative safety. Methods: We used the Minimum Data Set 3.0 from 2011-2013 merged to Medicare and facility characteristics data to study opioid use and safety among older, long-stay residents. The specific aims were to examine the 1) prevalence of long-term opioid use; 2) geographic variation in the initiation of commonly used opioids (oxycodone, hydrocodone, tramadol); and 3) comparative safety of commonly used opioids and fracture hospitalizations. Results: One in seven long-stay residents were prescribed opioids long-term. There was extensive geographic variation in the initiation of commonly used opioids, with oxycodone (9.4%) initiated less frequently than hydrocodone (56.2%) or tramadol (34.5%) but varying most extensively across the United States, with the majority of variation in prescribing explained by state of residence. Compared to hydrocodone initiators (7.9 fracture hospitalizations per 100-person years), those initiating tramadol had lower rates of fracture hospitalizations (subdistribution hazard ratio [HRSD] = 0.67, 95% Confidence Interval [CI]: 0.56-0.80), whereas oxycodone initiators had similar rates of fracture hospitalizations (HRSD=1.08, 95% CI: 0.79-1.48). Conclusion: The prevalence of long-term opioid use was twice as common in nursing homes as community settings, with initiation patterns varying extensively by region and being strongly driven by state of residence. Although initiating tramadol was associated with lower rates of fractures than hydrocodone, questions on opioid risks and benefits remain and are especially pertinent given the high mortality rates in this population.

If the goal is to achieve better quality, merely increasing nurse staffing levels may not be effective since the variation of the quality-of-care deficiencies explained by exogenous variables was smaller than random variation 5%. If state Medicaid reimbursements can be utilized for financial incentives for better performing nursing homes, nursing homes may improve their productivity by efficiently managing organizational personnel or increasing job satisfaction among nursing practitioners. Lastly, longitudinal analysis, considering variation in length of state staffing policy implementations, is encouraged to investigate the long-term effects of state staffing standards on nurse staffing levels and quality of care.; The purpose of this dissertation is to examine nursing homes' compliance with state minimum nurse staffing standards and its relation to quality-of-care deficiencies. Specifically, this study, reviewing staffing standards from 50 states and the District of Columbia for the year 2007, proposes a unique algorithm to calculate the states' expected nurse staffing levels for individual nursing homes in order to investigate their compliance with the state nurse staffing standards. By using hierarchical linear modeling method, this study attempts to capture the impact of the staffing standards on actual nurse staffing levels under resource dependence perspectives. Path analysis using structural equation modeling was conducted to investigate both direct and indirect effects of the staffing standards on nurse staffing levels and quality-of-care deficiencies. The major findings were as follows: (1) nursing homes in states with higher state staffing standards for the categories of RN, LN, and total nurse were found to have higher RN, LN, and total staffing levels, respectively; (2) higher nurse staffing levels resulting from higher state staffing standards were significantly associated with better quality of care (less quality-of-care deficiencies cited) in nursing homes; and (3) state staffing standards were found to have much stronger contribution to nurse staffing levels than any other organizational or contextual factors while nurse staffing levels, particularly licensed staff, were found to have stronger contribution to quality-of-care deficiencies than any other organizational factors. The study findings suggest that if the goal is to increase nurse staffing levels for better quality, increasing the stringency of both federal and state nurse staffing standards would be the most effective way. However, the staffing standards first need technical changes to reduce their ambiguity and ensure their fairness.

An idea often held in the mainstream society in the United States is a phenomenon of common occurrence between tornadoes and mobile home residences. While such perceptions could be learned from media presentations of tornadic events, a spatial investigation into such events in the Southeastern United States presents other possibilities. A statistical analysis of correlation is used to investigate the nature of relationship of coincidence between the two variables. Counties with relatively high densities of each variable are used for a case study of common geofactors at the micro scale. Results of this research suggest that a regional correlation between the two variables is non-existent. Results of the case study suggest that, while some similarities in geofactors existent between counties with high densities of each variable, no definitive conclusion can be reached with the data used in this study.

Hundreds of millions of people use their home computers every day for different purposes. Many of them are connected to the Internet. Most of them are unaware of the threats or do not know how to protect themselves. This unawareness is a major threat to global computer security.

This master thesis starts by explaining some security related terms that might be unknown to the reader. It then goes on by addressing security vulnerabilities and flaws in the most popular home computer operating systems. The most important threats to home computer security are reviewed in the following chapter. These threats include worms, email worms, spyware and trojan horses. After this chapter some possible solutions for improving home computer security are presented. Finally this master thesis contains a short user survey to find out what the problems are in the real world and what can be doneto improve the current situation.

The intent of this investigation is to explore the issues related to the licensing and certification of interior designers in the United States. To achieve this, the national organizations in the field of interior design were contacted to provide information about the status of licensure on the national scene. Those organizations are: the American Society of Interior Designers; the Foundation for Interior Design Education and Research; the Institute of Business Designers; the Interior Design Educators Council, Inc.; the International Society of Interior Designers; the National Council for Interior Design Qualification; and, the National Legislative Coalition for Interior Design. In addition, an in-depth examination of the requirements of the states of California and New Mexico, and the District of Columbia provide a comparison of three different legislative programs currently in operation. This study also examines the conflicts between the fields of interior design and architecture in the context of the licensing of interior designers.

The subject of this thesis is an American Zen monastery in New York, Zen Mountain Monastery (ZMM). The study is approached through a survey of methodologies: (1) through the scholarship on American culture and religion, (2) through the sociology of the study of religious institutions and communities and (3) through a comparison with East Asian Zen monasticism. The study reveals that ZMM's monasticism: (1) is part of a systematization of Zen in America that has made Zen into a mainstream option in American society, (2) has created group practices and commitment mechanisms that put ZMM in a better position than American lay Zen centers to challenge the individualist trends of American society and spirituality and (3) is based on a conception of the self more in line with the individualism of American society than the asceticism of East Asian Zen monasticism.

In the second quarter of 2013, 7,944 people were detained in the UK ‘for the purposes of immigration control’ (Home Office, 2013). While 1,138 of those detained were women, major shortcomings are identified in their treatment and calls made for a more gender sensitive asylum system. Although 35% of these women went on to be released there is a lack of research that investigates the on-going legacy of detention and the consequences for the sense of belonging, social integration and wellbeing of ex-detainees. This thesis draws on in-depth narrative interviews with 16 migrant women in the UK who were detained and then released from UK Immigration Removal Centres and five charity workers. Within migration scholarship the paradigm of exclusion has been traditionally adopted to understand how states seek to protect borders, keeping unwanted individuals out or contained. A spatial examination of respondents’ critical geographies of home reveals however that despite their release from detention these women continued to negotiate multiple and fluctuating boundaries. It is argued therefore that this paradigm obscures a nuanced perspective and proposes instead a discourse of precarity. Not only can the ‘state of precarity’ implicit within narratives of detention seep into and define the everyday geographies of home beyond release, respondents’ everyday negotiations with home remained central to the construction and proliferation of everyday precarity. This is achieved through a home-infused geopolitical rhetoric and interventions in the name of immigration enforcement which were revealed through (in)secure spaces of home. An exploration of emotional and embodied geographies also exposes erosive implications for feelings of belonging and health and wellbeing. A discourse of precarity therefore allows for a differentiation and critical inquiry of subjective gendered positions, citizenship and importantly, emergent accounts of resilience, reworking and resistance on predominantly social trajectories.

Abstract. The heating of residential areas contributes to over 80% of total energy consumption in Finland. This indicates huge possibilities to save energy. The objective of the thesis is to identify the factors that affect a homeowner’s decision making while choosing a heating system. Among various factors that influence the homeowners’ choice, this thesis investigates three types of determinants: the features of the heating system, the features of the building and the socio-demographic characteristics of the homeowner. The thesis uses the stated preference technique called choice experiment. In the choice experiment, respondents were presented with choice scenarios where the main heating system choices, namely ground heat, exhaust air heat pump, solid wood boiler, wood pellet boiler, electric storage heating and district heating, were described using five attributes which took various levels. In the choice analysis, the preference heterogeneity for the heating systems and attributes was modelled. The results indicate that among the attributes of the heating system, homeowners view costs as the most important ones, especially the operating costs. The results also show that their heating system choice is influenced by socio-demographic characteristics as well as building and heating system attributes. Preference heterogeneity in main heating system choices can be explained by individual characteristics such as age, education and forest ownership as well as building attributes such as energy saving capabilities of houses. Similarly, preference heterogeneity in comfort of use and environmental friendliness attributes were explained by the size of the house as well as forest ownership by the homeowner.

The purpose of this study was to identify the curricula emphases of home economics in Oyo state secondary schools, and to analyze the relationship between subject matter emphasis and selected aspects about teaching and the teacher. Sixty-two home economics teachers in Oyo state, Nigeria, responded to a mailed survey asking them to indicate the degree of emphasis given to 50 topics in five subject matter areas of home economics: Human Development and the Family, Home Management and Family Economics, Foods and Nutrition, Textiles and Clothing, and Housing. The most taught subject matter area was Foods and Nutrition followed by Home Management and Family Economics, Textiles and Clothing, Human Development and the Family, and Housing. A similar rank order was observed for subject matter competence and for preference for teaching subject matter. Scores on the topics within each subject matter area, however, indicated that all of these topics and the subject matter areas were moderately emphasized in the curriculum. The philosophical views of home economics as homemaking education; home economics as household management and home economics as cooking and sewing exist concurrently. Multiple regression analysis was used to examine the relationship between the above five subject matter areas and selected aspects about teaching and the teacher. The results of the multiple regression analysis indicated no significant relationship. Some of the problems facing home economics as a subject in the secondary schools were lack of laboratory space, equipment, finance, and shortage of home economics teachers. Collaborative curriculum development and local co-operative responses may offer the means to overcome the shortage of resources for programs in specific locales. Further research in the form of case studies of successful home economics programs could be informative in understanding better the necessary components which should be fostered in strengthening home economics programs.

With growing life expectancy and decreasing of fertility rates, demands of additional healthcare services is increasing day by day. This results in a rising need for additional healthcare services which leads to more medical care costs. Modern technology can play an important role to reduce the healthcare costs. In the new era of IoT, secure, fast, low energy consumption and reliable connectivity are necessary qualities to meet demands of health service. New protocols such as IEEE 802.11ax and the fifth generation of mobile broadband have a revolutionary impact over the wireless connectivity. At the same time, new technologies such as cloud computing and Close Loop Medication Management open a new horizon in the medical environment. This thesis studies different eHealth@home architectures in terms of their wireless communication technologies, data collection and data storage strategies. The functionality, benefits and gaps of current distance health monitoring architecture have been presented and discussed. Additionally, this thesis proposes solutions for the integration of new wireless technologies for massive device connectivity, low end-to-end latency, high security, Edge-Computing mechanism, Close Loop Medication Management and cloud services.

The purpose of the study was to give a description of the curricula and methods used in the home schools of home educators and to evaluate by means of case studies the effects of home education upon those included in the study.Chapter I presented an overview of the background and significance for the study and five research questions which were addressed by the study. The questions were:1. Will students being educated in the home be able to obtain academic achievement at comparable levels with those students being educated in the public school? 2. Will students being educated in the home be able to show at least one years' gain in academic achievement when scores of the previous year are compared with scores from the current year?3. What curricula and methods are being used in the home education of the children included in the study?4. What attitudes and values motivated the parents in the study to home educate their children?5. What legal actions, if any, did the parents included in the study face as a result of their choice to home educate?Chapter II provided a review of related literature. Reviewed were: compulsory attendance laws, related court decisions, and research related to home education.Chapter III presented the plan of organization and procedures used in gathering, reporting, and summarizing the data.Chapter IV contained the data collected from the four home educators. The data were presented in narrative form and in tables for each family case study.Chapter V presented a summary of the case studies, answers to research questions, observations, and recommendations. The data provided the following answers to the five research questions:1. Forty percent of the home educated students scored equal to the median national score.2. Students showed inconsistencies in average gains for the year.3. Three of the four families used curriculum materials commercially prepared for home educators.4. The parent educators appeared motivated by socialization concerns and desires for values training.5. Legal action was taken against one of the four families in the study.

This thesis explores the intersection of artistry, professionalism, and maternalism in the storytelling revival that occurred in the United States from 1890-1920, influencing a variety of child-centered reform movements. Though storytelling was practiced by men and women alike, it was portrayed as a maternal skill. However, storytelling's perceived multiplicity of uses led it to be interpreted in diverse ways. Such interpretations--particularly potent in the home, school, and library-displayed tensions inherent in the public role of these institutions, particularly in their approach to "child-centeredness." In the school, teachers embraced the nurturing potential of storytelling, arguing that it allowed them to teach more effectively. In the library, however, such an approach was rejected as antithetical to the efficient nature of the institution. The way these institutions conceived of storytelling shows that nurturing imperatives, though pervasive in childcentered reform in the late nineteenth and early twentieth century, was not the only way to conceive of child-centeredness.

Two properties of holes in InGaAs quantum wells have been investigated and are reported in the thesis - their in-plane mobility at low electric fields, and their capture by a potential well. Monte Carlo simulations of hole transport in InGaAs-AlGaAs quantum wells of different widths and alloy compositions have been carried out at 77 K, and for field strengths less than 10(^5) Vm(^-1) Valence bandstructure has been generated using a k.p method in the infinite well approximation, which accounts for mixing between heavy and light hole states. Although less accurate than more detailed k.p calculations which include mixing with the conduction, spin split-off and remote bands, it provides an adequate description of states lying close to the valence band edge, simplifies the calculation of scattering rates and is computationally efficient. The effects of alloy, impurity and phonon scattering have been included. A study of hole transport in 90 Å In(_z) Ga(_1-z)As wells (0.10 < x < 0.25) predicted that the hole mobility should increase with indium concentration, since the reduction in the effective mass of the highest HHl subband more than compensates for the greater alloy scattering rate. An analysis of wells with 18% indium content and widths in the range 50-150 Å indicated a general increase in the hole mobility with well width but with a local minimum around 120 Å due to intersubband scattering from the HHl subband to the heavier HH2 subband. A model for the 'quantum' capture of holes into a square well potential is developed in part two of the thesis. This involves the calculation of transition rates from unbound barrier states above the continuum edge to quasi-two dimensional bound states that form the quantum well subbands. The physical mechanisms and rates for the quantum capture of holes into a 30 Å In(_0.7)Ga(_0.3)As-InGaAsP quantum well (suitable for use in lasers operating at 1.55 µm) are discussed. In particular, the role of polar and non-polar optical phonon scattering, acoustic phonon and alloy scattering are considered. Bound and unbound states have been calculated using an eight band k.p band- structure model which describes mixing between the heavy and light hole, spin split-off and conduction bands. The quantum well has three subbands which derive from the HHl, HH2 and LHl zone centre states. By treating quantum capture in a similar way to a barrier transmission problem, the hole capture rate into the well can be expressed in terms of the incident particle flux density. Such local information can be readily incorporated into classical device models, for example those based on Monte Carlo simulation. Heavy and light holes of a particular energy and momentum (both transverse and parallel to the plane of the quantum well) are represented by their appropriate barrier plane wave states. The fraction of the incident amplitude which is transmitted into the well region oscillates as a function of the wavevector normal to the well, and capture is induced by a scattering process, due to phonons or alloy disorder for example. The transition rate is determined by the matrix element between the transmitted wave and the final bound state, hence the capture probability varies as a function of the in-plane wavevector and energy of the unbound state. Peaks in the capture probability are associated with transmission resonances into the well (virtual bound states), but subband mixing is also found to be an important influence.

This thesis examines the role of the stately home hospital during the First World War. It assesses the social and cultural importance of these institutions, as well as the place that they, and their patients, held within wartime society. It argues that the establishment of a hospital in a stately home communicated a high level of patient care, reminding people all over the Empire how much Britain valued the sacrifices of its wounded. However, some members of the soldiery misinterpreted the value bestowed upon them by their status as war heroes. Consequently, the stately home hospital became a site of physical and emotional clashes between the wounded and the medical authorities. By placing these medical establishments in their social, cultural, political, and imperial contexts, this thesis delineates the myriad of ways that the space of the stately home hospital affected the experience of wounding and how a number of different people interacted with the institution and utilised it for many different purposes. The domestic nature of these private residences meant that they straddled the military and civilian spheres, which convoluted the position of the wounded soldier, the medical staff, and ancillary workers within. In addition, the space was home to a variety of non-military personnel who presented the wounded with a variety of different opportunities that transcended normal military spaces. This thesis explores these opportunities to discuss the important position stately home hospitals held within First World War Britain. Due to the historic role of the stately home in British social, cultural and political life, the experience of recovering within these walls was socially loaded. This thesis argues that the establishment of hospitals in these buildings was an important statement to the wounded and their families.

This study looks to the social trends, needs, and definition of home for the single-mother household. People need strong families to provide them with the identity, belonging, discipline, and values that are essential for full individual development ("The American Family Crisis" 16). This requires the examination of the role that the physical home environment plays in the life of the single-parent household. Personal histories, a literary search, and research of existing examples of built facilities serve to produce a series of patterns formalized in a design matrix, investigating how architecture can foster a healthy and supportive environment for the single-parent household.The intent of this research is to define a process by which the singleparent household, headed by a single mother, may be able to become self-sufficient and empowered by their housing situation. The aim is to create More than Housing (Joan Forrester Sprague), while utilizing the architecture to foster relationships and encourage growth.A resulting programmatic guide and design development tool for supportive housing, adapted to the needs of the single-parent household, creates a framework of design ideas derived from this research. This compilation is used to define design strategies and recommendations for the form and program of support systems used to illustrate the definition, application and resolution of "home" for the single-mother household.

The aim of this work is to discuss the concept of man on Thomas Hobbes in the three pillars that make up your theory: the state of nature, the contract and the State. First, it discusses the equality of men, the three causes of war of all against all, and how to understand the relationship between your theory and the historical moment in which he lived. Secondly, it is shown the concept of contract as a product of human capacity to transform the reality, and also the relationship between the mechanical and the moral, and the union of reason and passion in your anthropology. Finally, it analyzes the divine authority of the state and, because of this, the possibility of failure on the part of sovereign power with the rationale of the pact between individuals, which is the preservation of life. It intend to demonstrate that the seventeenth-century thinker sees the human condition as an agonizing journey, in which even the best guarantee of peace, the State, presents the possibility of serious drawbacks.
O objetivo deste trabalho é discutir a concepção de homem em Thomas Hobbes nos três pilares que compõe sua teoria: o estado natural, o contrato e o Estado. Em primeiro lugar, se analisa a igualdade entre os homens, as três causas da guerra de todos contra todos, e como compreender a relação entre a teoria do autor e o momento histórico em que ele viveu. Em segundo lugar, mostra-se a concepção de contrato como produto da capacidade humana de transformação da realidade e, ainda, a relação entre a mecânica e a moral, e a união de razão e paixões em sua antropologia. Por fim, destaca-se o Estado como detentor de uma autoridade divina e, em virtude disto, a possibilidade do descumprimento, por parte do poder soberano, com a razão de ser do pacto entre os indivíduos, a preservação da vida. Pretende-se demonstrar que o pensador seiscentista concebe a condição humana como uma trajetória agônica, na qual até mesmo a maior garantia de paz, o Estado, apresenta a possibilidade de sérios inconvenientes.

Availability of fuels, quality of light provided, daily care needed by a light source, and safety were all direct influences on the development of artificial lighting. Technological advances in response to the social needs began with the closed font and continued through the removable burner, accommodating a variety of fuels including tallow, lard, whale oil, burning fluids, kerosene, gas and ultimately electricity. Along with the increasing illuminating power, the amount of decoration, both possible and accepted, increased.

The fragmented approaches to delivering health care services in the United States, along with the associated structural inefficiencies and unsustainable increases in health care costs affecting all payers, compel the need for reform. Various federal and state-level delivery system reform models have emerged in response. The Medical Home (MH) is one of such reform models. In 2004 a national initiative entitled "The Future for Family Medicine Project" identified the lack of emphasis on comprehensive primary care, especially for chronic care patients, and proposed the introduction of MHs to improve comprehensive primary care delivery for every patient. Oregon's MH variant, the Patient-Centered Primary Care Home (PCPCH), was introduced in 2009 as part of a state-wide health reform initiative ushered in by the passage of House Bill 2009 to promote the Triple Aim. Since 2011, over 600 primary care clinics have been recognized as PCPCHs. Proponents of the model argued that it will help improve comprehensive primary care services upstream and reduce inappropriate utilization of Emergency Department (ED) and Inpatient (IP) care and expenditures downstream. Evidence on the model's application to reduce ED and IP utilization and expenditures have so far been mixed. Based on growing interests in the effects of the model's application to provide care for different types of patients, this research was designed to evaluate the policy effects of the application of PCPCHs, with a focus on PCPCHs that treat greater proportions of chronic care patients, to answer the following questions: (1) What is the average chronic disease burden of PCPCHs, and how does their average chronic disease burden compare to the communities PCPCHs are in pre-post PCPCH recognition? (2) How do primary care expenditures change based on the chronic disease burden of PCPCHs? (3) Do PCPCHs that engage more high chronic disease burden patients have more reductions in ED and IP expenditures? For this research, a chronic disease burden measure was developed from 10 markers of chronic conditions. This measure was then used to stratify PCPCH clinics and their comparators into high and low chronic disease burden clinics. The research was designed as a natural experiment, utilizing difference-in-difference methods to measure outcome differences pre-post PCPCH policy implementation and comparing outcome differences between PCPCHs and their control groups. The unit of analysis was PCPCH clinics. The theoretical perspectives that informed this research were Risk Selection and Complex Adaptive Systems (CAS). Data from Oregon's All Payer All Claims (APAC) data system, which included 16 quarters of claims and eligibility data from fourth quarter 2010 to third quarter 2014, as well as PCPCH attestation data on 525 clinics were utilized for this research. The results suggest that the chronic disease burden for PCPCHs was significantly lower than their comparator groups before clinics recognition as PCPCHs, but the chronic disease burden did not change after clinics recognition as PCPCHs. Average primary care expenditures did not change after PCPCH recognition. Average ED and average IP expenditures for high chronic disease burden PCPCHs did not change but rather decreased significantly for low chronic disease burden PCPCHs. The results imply that the distribution of chronic disease burden in PCPCHs is important and related to ED and IP expenditures, but in a different direction than expected. The results also suggest that focusing on low chronic disease burden patients in PCPCHs could help reduce ED and IP expenditures in the short and medium terms. Policies to engage a broader mix of chronic disease burden patients in PCPCHs could help increase savings from ED and IP utilization. The results also suggest the need for more research to improve current understanding of how PCPCHs are impacting health care trajectories in the current delivery system environment.

This comparative research explores the work experiences of dementia care workers in nursing homes. The aim of this study is to understand concepts central to care and to gain insights from the care workers‟ perspectives. A comparative framework and symbolic interactionist approach is used to analyse data collected using ethnographic methods from 59 dementia care workers in Canada, Scotland, and the United States. The fieldwork settings are institutionalised; dominated by for-profit ownership; and provide care for a resident population with high cognitive and physical needs. The comparative findings underscore the importance of work conditions that provide care workers with sufficient resources to do their job and enough time to complete their work. The absence of these critical components creates stressful work conditions for the care workers. The lack of time, staff and supplies such as towels, wash cloths, and continence products do not allow the residents‟ choices in their care and disregard their dignity and rights. The inability to deliver care for the residents according to the guaranteed government care standards often result in the violation of human rights for the care workers and residents. The care workers are unable to supply the quality of care they know the residents need and are capable of providing given better circumstances. There are frequently not enough care workers, resources, or time to meet the level of care that relevant standards mandate or the care workers know is possible. The analysis reveals that care workers struggle to provide more than basic physical care and are seldom able to meet essential social care needs for the residents. Unwritten rules are implemented in each setting that include separating people with dementia, placing these residents out of view of the public, not allowing the residents access to go outside, and not providing them with engaging and meaningful interactions. While policies are frequently developed with good intentions, many are counter-productive without dementia knowledge. This comparative research reveals care practices and routines share strong similarities across the fieldwork sites while the care worker characteristics as a workforce vary the most between countries. Some differences involve the training required, average age, pay and mode of dress or appearances. Too often researchers frame stress issues for care workers as problems with attitudes, motivation, training and incentives. Yet, the broader social structures and conditions that set the context in which these problems have their origins are commonly ignored. Good working conditions for care workers are precursors to good care for the residents. This thesis concludes with recommendations for practice, research and policy development.

Resumo:
This dissertation builds on the current anthropological studies of care relations in practice. It draws inspiration from science and technology studies (STS) and postfeminist technoscience. A qualitative ethnographic approach grounds the empirical data collection and analysis. This entails ethnographic fieldwork with senior home care in the United States and Sweden during 2007–2008 and 2011–2012. Analytical attention centers on how movements situate various tensions of senior home care in practice. Four interrelated published works comprise the main thematic chapters. Each article exemplifies how human and nonhuman relations move and mediate care. They develop several heuristic terms that advance ideas about how older people, aging bodies, technologies, spaces, and times that tinker each other through movements of care in practice. The comprehensive summary frames these articles with an overview of the primary thematic orientations and methodological concerns. A discussion of the main contributions and implications of the dissertation concludes the work.

Scholars have traditionally regarded John Donne?s three Hymns as independent works of devotional or meditative verse. This study proposes that Donne?s Hymns are read properly as a tripartite sequence, one which addresses the poet?s desire for a spiritual union with Christ. Before that union may occur, Donne must ?tune the Instrument? of his soul by submitting his fallen will to the will of Christ. The Hymns are that ?tuning,? that labor of submission. As they unfold, the Hymns present the drama of Donne?s piety wrestling with his aggressive will, and his transformation from a self-serving rake dedicated to his ?Iland? of ?false mistresses? into a Christ-serving religious. Until the last line of the Hymns, Donne seems to have won spiritual security: he has prayed for release from his ?Iland,? he has ?embrace[d]? the body and blood of Christ, and he has confessed his ?sinne of feare,? despair. However, the final line of the Hymns, ?I have no more,? seems to unravel Donne?s spiritual work. His final perplexing, ambiguous line could indicate (among many other meanings) that he has fully transformed, or that he has abandoned the enterprise altogether. ?I have no more,? an enunciation of Donne?s fallen will, proves if anything that the work of the soul is never complete.
