Resumo:

The demand for performing intravitreal injections has increased in recent years, prompting the need for more nurse training in their administration. The Great Western Hospitals NHS Trust in Swindon has developed a structured nurse training programme and now has 8 independent nurse injectors trained to undertake injections independently; nurse practitioners now contribute upwards of 85% of the total number of injections. The authors have also demonstrated the financial benefits of using injection assistant devices and shown the positive impact such devices have on training. In September 2019, the authors organised the first course to offer nurses and doctors hands-on experience in administering injections, using the Swindon training model to provide participants with a structured approach to learn how to perform intravitreal injections safely. Nurses made up 96% of participants; the remainder were doctors and managers; 6% had never performed an intravitreal injection; of units where they had, disposable drapes and a speculum were used in 71% of these. The number of injections performed per session at participants' units at the time they attended the course was: 17 or more injections=46%, 13–14=39%, and 11–12=15%. The course was rated 8.9/10 overall for content, with 85% very likely to recommend it to colleagues. All participants indicated that using the Swindon model made them feel confident to deliver injections safely. The authors demonstrated that using a structured training protocol and intravitreal assistant device improves the quality of nurse training and increases confidence in administering intravitreal injections.

SUMMARYAn outbreak of cryptosporidiosis resulted in 516 cases in Wiltshire and Oxfordshire. The outbreak caused widespread interest and led to an official inquiry. The majority of cases were in children; 8% of cases were admitted to hospital and the median duration of illness was 3 weeks. The geographical distribution of cases matched the distribution of water supplies from three treatment works and eryptosporidium oocysts were found at these works and in the treated water. Attack rates in electoral wards supplied by the three treatment works were significantly higher than in other wards. The cause of the outbreak appeared to be the failure of normal treatment to remove oocysts. Measures at the treatment works reduced the number of oocysts detected in treated water, after which the outbreak came to an end. The conclusion of the investigations was that cryptosporidiosis is a risk of conventionally treated public water supplies.

The Great Western Hospital was opened in 2002. It was built as a PFI hospital, moving services from the old Princess Margaret Hospital situated in central Swindon. The Great Western Hospital is conveniently situated near junction 15 of the M4 and therefore has excellent transport links. The Acute Medical Unit (AMU) was purpose built adjacent to the Emergency Department and in close proximity to Emergency Department Radiology. Details of the Acute Medical Unit layout are summarised in Table 2.

An outbreak of listeriosis occurred in the Swindon area of the UK in autumn 2003. Five cases were detected in pregnant women. Four of these women were thought to have eaten prepacked sandwiches from a retail outlet in one particular hospital. Sampling at the supplier detected Listeria monocytogenes, which was indistinguishable on molecular testing from the patients’ isolates. Recent changes in UK food legislation should help diminish the risk of further outbreaks/cases such as ours occurring.

In 1990 I saw every member of the Child and Family Guidance Service within the Swindon Health Authority to discuss children from their last (unselected) six or 12 families in relation to child protection issues. All 19 professionals were part-time workers, some only doing three, two, or one sessions per week of child and family guidance work. They were seven psychiatrists (consultants and a senior registrar), two psychologists, six psychiatric social workers (including family therapists), and four specialised therapists (family, child psychotherapy, nurture-group and art). There were 14 female and five male professionals.

in 1997 the teleradiology facilities at princess margaret hospital swindon were upgraded to a modern dicom compliant system. we describe the practicalities rather than the technical issues of setting up and running a teleradiology system that can send images to the radiologists' homes on call obviating the need for them in many cases to attend the hospital. we list the initial doubts and concerns voiced by both radiologists and radiographers and describe the means of overcoming the problems that arose. from these reflections we have obtained a deeper insight into the use of teleradiology in a district general hospital (dgh) setting. our conclusions are described.

Objectives. Despite increased awareness of the high prevalence and significance of foot and ankle problems in rheumatoid arthritis (RA), feet remain neglected. Reasons may include the perception that feet are difficult to assess, they are not included in the DAS28, and lack of freely available foot screening tools specific for RA. Methods. The Swindon Foot and Ankle Questionnaire (SFAQ) is a simply worded 10-point foot and ankle screening questionnaire with diagrams of feet and ankles for use in general rheumatology outpatients. All RA patients on our electronic database were invited to complete the questionnaire and attend clinic for assessment. Patients assessed clinically were scored out of 10 using the parameters from the questionnaire. The SFAQ was compared to the Manchester Foot Pain and Disability Index (MFPDI), DAS28, HAQ, HAD, and OSRA scores. Results. 597 questionnaires were sent, 301 (50%) returned, and 137 seen in clinic. There was good correlation between the postal SFAQ score, clinic score (), and the MFPDI (). Neither of the foot scores correlated with other RA disease outcome measures. 75% patients completed the picture. 73% corresponded to clinical findings. 45% of patients required an intervention following clinical review and trended towards higher scores. Conclusions. The SFAQ was quick to complete and correlated with the MFPDI. Lack of association with standard RA outcome measures suggests that relying on these scores alone may miss foot pathology. The diagrams were a useful complement. This simple screening tool could aid identification of RA foot and ankle problems.

This paper presents the results of an original quantitative sociological study conducted in Autumn 2019 on a sample of 620 Polish migrants living in London, Oxford, and Swindon. The study was conducted using a group-administered questionnaire. It was primarily to address the question of whether they intend to return to Poland and when. We identify some significant factors influencing their choices. Those are the length of stay in England, their financial situation, their knowledge of English, their ability to assimilate culturally, their relations to the families in Poland, homesickness, and better religious education of children. Religious issues, although mentioned, are less critical; however, a deeper statistical analysis allows to understand the religious background more broadly, to provide a clearer image of the respondents’ deep motivations.

Ergonomics in industry takes many forms and a variety of techniques are applied for the data collection of ergonomic concerns. At the Rover Group (BMW) Swindon site, chosen shop floor associates are trained in the principles of ergonomics. They lead all ergonomic assessments with the full support of the local production manager known as the ABA plan owner. This Ergonomic “facilitator” uses an ergonomic assessment sheet known Associate job Analysis A.B.A. developed by BMW AG Germany. This takes the form of simple charts to store issues categorised as Red Amber or Green (RAG) concerns. It can effectively deal with local and departmental issues using an ergonomic approach. The author, who is the site ergonomist, will share results of the activities within their environment and explain the principles of Ergonomic Self-Reliance and the infrastructure that supports it.

Resumo:

The objective of this paper is to present some comparative findings on Proton, a carmanufacturer in Malaysia and Rover Group, a British automobile manufacturer. Thepaper used a â€˜Business Historyâ€™ methodology tracking the exploits of the two firmsfrom 1980 to 2000. Two in-depth interviews were conducted with two key informantsâ€“ Nadzmi Salleh, former CEO of Proton and John Towers, the former CEO of RoverGroup. Apart from that, the study also utilized secondary documents from varioussources such British Aerospace and Honda Swindon. Proton and Rover are sources ofnational pride for Malaysia and England respectively. Yet their financial performanceover time were said to be poor. Some efforts by the respective governments and themanagement of the two firms to improve the performance in both firms were highlighted.In terms of the current status of both firms, Proton is still surviving but Rover has beendefunct since 2005.

Resumo:

Poles represent one of the largest groups of economic immigrants to the UK. As a result of Brexit, many of them have redefined their migration scenarios, which has affected the economy and some areas of social and cultural life in the UK. This paper presents the results of our original quantitative study conducted in the autumn of 2019 on a sample of 620 Polish respondents living in three locations in England – London, Oxford, and Swindon. The study addresses the question Do Polish migrants intend to return to Poland, and if they do, when? and examines to what extent this decision is influenced by the length of their stay in England, by their financial situation, by their knowledge of English, by their ability to assimilate culturally, by how much they miss their family, by homesickness, and by their craving for Polish culture. The article follows the typology of attitudes adopted by Poles towards Brexit, as identified by Agnieszka Trąbka and Paulina Pustułka.

This paper is based on sociological quantitative studies carried out in 2019 on a sample of 620 Polish Catholics living in London, Swindon, or Oxford. Those studies and their findings are limited only to those Catholics who make up the communities around major Polish institutions in the UK, such as Polish parishes, Saturday schools, and community houses. The goal of this paper is to describe selected aspects of Polish migrants’ religiosity in the new social and cultural milieu. What we focus on here is how Poles themselves describe their faith, how they understand and evaluate their membership of parishes or other religious communities, and how they approach religious practices, especially Sunday Mass attendance. We address the following questions: how do the Poles living abroad describe their attitudes towards faith? How many of them are active members of Polish parishes? What do their religious practices and membership of other community organisations look like? How do specific factors affect the results across these areas?

SUMMARYWith the aim of minimizing adverse health outcomes and reducing the risk of outbreaks, we offered one dose of MMR vaccine to children known to be incompletely immunized at the time of teenage booster vaccination in secondary schools in Swindon in 2011. The Child Health Department database was queried to identify Year 10 children who had had zero or one dose of MMR vaccine previously. Of the 316 children offered vaccination, 60 received a first dose and 87 received a second dose of MMR vaccine. Fourteen children had two documented doses in the past and two had contraindications to the vaccine. Overall uptake of two doses of MMR vaccine increased from 86·3% to 90·6%. The valuable uptake achieved demonstrates that an opportunistic offer of MMR vaccine for unimmunized children at schools is feasible and beneficial. MMR vaccine should be offered routinely to unimmunized children at the time of school vaccination programmes, especially in areas with sub-optimal coverage.

The HIVE is a building designed to act as the support framework for research into minimising the environmental impact of the built environment. Located at Wroughton Airfield south of Swindon, UK, the HIVE is exposed to seasonal temperatures ranging from –10ºC to +35ºC, an annual average rainfall of 750mm and wind speeds that can exceed 85km/h. This offers the opportunity to evaluate materials and systems in extreme UK weather conditions. Researching and developing materials and products at full scale requires a number of prerequisites, including an appropriate site, planning consent, building control approval and infrastructure such as foundations, utilities and data acquisition. Since all these are applicable to every project, the concept of the HIVE was to have these available as a 'plug and play' facility eliminating expensive and time-consuming preliminaries. This will result in a more effective use of researchers' time and costs, effectively fast-tracking the research output, and ultimately bringing materials, products and systems to the market more rapidly.
