AbstractThis chapter deals with the organisation of care work by Polish parents. Using the data from in-depth interviews and survey data, I demonstrate cultural norms about care that prevails in Polish society. Strong gendered norms and instruments of family policy shape different opportunity structures for men and women. I focus on how parental leaves are used and perceived by Polish parents. I argue that they are still seen primarily as women’s right. I analyse the reasoning lying behind such thinking, but also show the experiences of parents who decided to share the leave. Then I proceed to the organisation of care in the context of so-called care gap. The Polish system of parental leaves is incompatible with the system of institutional care for children. Consequently, in the period between the end of paid leave and the time when a child can go to a kindergarten parents have to develop different strategies how to provide care for their children. I show how these strategies differ in the context of economic inequalities, as well as what consequences care gap has on gender inequalities. Finally, the chapter finishes with the analysis of how care work is perceived by parents.

Outcome and cost measurement in the value-based health care framework, centered around a patient condition or segment of the population, depends on data in every step towards healthcare system redesign. Technological and service delivery innovations are key components of driving transformation towards high-value health care. AbstractCerebral palsy is a pathological condition whose prevention and treatment have been immensely studied by experts in perinatal medicine and pediatric neurology. Despite this, it is still one of the main concerns nowadays in many countries, not only for scientific reasons but for legal ones also. For instance, in Japan, an increase in lawsuits relating to cerebral palsy was observed more than a decade ago, after healthcare resources in perinatal medicine had increasingly shrunk and had become fragile under the growing burden for physicians and midwives to provide advanced treatment, emergent care, high-risk treatment, and so on. Young physicians did not specialize in obstetrics because of the increased burden, which gave rise to a vicious cycle of shrinking resources in perinatal medicine. To address this issue, the Japan Obstetric Compensation System for Cerebral Palsy (JOCS-CP) was urgently introduced in 2009 to investigate, develop preventive measures, and award monetary compensation on a no-fault basis, with the Japan Council for Quality Health Care (JQ) as its operating organization (Fig. 33.1). It has so far produced annual reports on the prevention of cerebral palsy for nine consecutive years including numerical data and specific themes relating to the occurrence and prevention of cerebral palsy. The success of the system is a good reference for responding to adverse events which may happen in and have a vast impact on perinatal care. Therefore, this chapter focuses on cerebral palsy with primary reference to materials published by the JOCS-CP in the field of perinatal medicine. The aim of this chapter is to learn about the issues mentioned above and to discuss the significance and impact of introducing a nationwide system like the JOCS-CP. It describes knowledge and idea to questions of “Why cerebral palsy is highlighted among adverse event in obstetrics?”, “How the no-fault compensation/investigation/prevention system could be introduced?”, “What has been achieved by the system?”, and “How cerebral palsy is prevented?”.

This chapter examines the continuum of care in use at the Yee Hong Centre for Geriatric Care in Toronto, Ontario, Canada. The service continuum is a response to the needs of a diverse Chinese Canadian population, where services in the appropriate language and culture are limited. Community agencies must find a way to adapt to changing systems as the Ontario government moves away from institutionalization and toward community care. This chapter explores challenges that are faced by a socially-minded organization within a medically-minded funding system. It also addresses ways to cope with the constraints.

Resumo:

One cannot discuss the ethics of health policy without understanding how health systems work, so this chapter provides background on the American health system before and after the Affordable Care Act. It also describes two universal health insurance systems the U.S. could adopt. In Canada’s single-payer system, the government serves as the basic insurer for the entire population. In the regulated-market systems of Switzerland and the Netherlands, citizens must purchase health insurance through private companies, and the government’s main jobs are providing subsidies to less wealthy individuals and ensuring that insurance companies deal fairly with citizens. The chapter concludes by examining the core ideas behind consumer-driven health care, a set of specific policies that conservatives often add to their health care proposals.

This work presents Falibras, a translation system between Portuguese and Brazilian Sign Language (LIBRAS) that aims to help both, deaf people and hearers, in trying to communicate. New variants of concern (VOCs) now account for 67% of all Ontario SARS-CoV-2 infections. Compared with early variants of SARS-CoV-2, VOCs are associated with a 63% increased risk of hospitalization, a 103% increased risk of intensive care unit (ICU) admission and a 56% increased risk of death due to COVID-19. VOCs are having a substantial impact on Ontario’s healthcare system. On March 28, 2021, the daily number of new SARS-CoV-2 infections in Ontario reached the daily number of cases observed near the height of the second wave, at the start of the province-wide lockdown, on December 26, 2020.
