BackgroundThe interrater reliability of 2 new inpatient functional short-form measures, Activity Measure for Post-Acute Care (AM-PAC) “6-Clicks” basic mobility and daily activity scores, has yet to be established.ObjectiveThe purpose of this study was to examine the interrater reliability of AM-PAC “6-Clicks” measures.DesignA prospective observational study was conducted.MethodsFour pairs of physical therapists rated basic mobility and 4 pairs of occupational therapists rated daily activity of patients in 1 of 4 hospital services. One therapist in a pair was the primary therapist directing the assessment while the other therapist observed. Each therapist was unaware of the other's AM-PAC “6-Clicks” scores. Reliability was assessed with intraclass correlation coefficients (ICCs), Bland-Altman plots, and weighted kappa.ResultsThe ICCs for the overall reliability of basic mobility and daily activity were .849 (95% confidence interval [CI]=.784, .895) and .783 (95% CI=.696, .847), respectively. The ICCs for the reliability of each pair of raters ranged from .581 (95% CI=.260, .789) to .960 (95% CI=.897, .983) for basic mobility and .316 (95% CI=−.061, .611) to .907 (95% CI=.801, .958) for daily activity. The weighted kappa values for item agreement ranged from .492 (95% CI=.382, .601) to .712 (95% CI=.607, .816) for basic mobility and .251 (95% CI=.057, .445) to .751 (95% CI=.653, .848) for daily activity. Mean differences between raters' scores were near zero.LimitationsRaters were from one health system. Each pair of raters assessed different patients in different services.ConclusionsThe ICCs for AM-PAC “6-Clicks” total scores were very high. Levels of agreement varied across pairs of raters, from large to nearly perfect for physical therapists and from moderate to nearly perfect for occupational therapists. Levels of agreement for individual item scores ranged from small to very large.

The samples of p- and n-doped Fz Si were deformed in 3-point bending mode in the temperature range 800-950°C. The fine structure of dislocation related luminescence in the region of D1 and D2 bands was most pronounced at lowest rate of deformation. The temperature variation of intensity of individual lines did not reveal any thermalization effects. It implies that centers responsible for different individual lines are situated in diverse places. The most narrow lines with maximum positions of 802 meV and 807 meV (D1 band) and 869 meV and 873 meV (D2 band) are rapidly quenched with increase in temperature, while the broader background lines survive at higher temperature. The new line with maximum position at about 882 meV appears in the D2 band with enhanced carbon doping. It was found that at higher P-doping the low energy fine structure lines 802 meV and 869 meV are absent. Besides, the Fz samples with different P doping level from 6×1014 cm-3 to 1.2×1016 cm-3 demonstrate quite a different temperature variation of broad background lines. At lowest level of P-doping they move to low energy side at temperature above 30K, while at P-doping above 1015 cm-3 these lines move to higher energy in the same temperature range. A possible explanation of this observation can be related to distribution of electrons in the cband. It implies that the corresponding electronic transitions occur between the edge of conduction band and deep states.

In the title compound, C18H15NO3, the fused benzopyran and pyridine rings are essentially coplanar [r.m.s. deviation = 0.0533 Å with a maximum deviation of 0.080 (1) Å for a benzene C atom]. The cyclohexanone ring adopts an envelope conformation with the dimethyl-substituted C atom 0.660 (2) Å out of the plane formed by the remaining ring atoms (r.m.s. deviation = 0.0305 Å). The dihedral angle between the mean planes of the pyran and cyclohexanone rings is 12.95 (6)°. In the crystal, molecules are linkedviaC—H...O hydrogen bonds, leading to chains running along [011].

A considerable number of the books published in the course of the eighteenth century were printed with false details of the place of publication, authorised by the censorship offices themselves. This phenomenon, which was common throughout Europe, was particularly marked in Venice where over 800 titles, among the most sought-after of the time, were licensed exploiting this expedient to get around the bottleneck of the official publishing rules. The volume publishes the registers of the related Venetian documentation, complete with notes and indices that sketch a fairly complete picture of this aspect of the censorship system of the Ancien Régime.

Анотація:
Mozambique is among the world’s least complex economies. By systematically accounting for both supply- and demand-side factors, we identify new products and sectors that can help to diversify and upgrade its economy. In a supply-side analysis, we use network methods from the literature on economic complexity to identify a set of target products that are complex, require productive capabilities useful in the export of other products, and are close to Mozambique’s existing productive structure. In a demand-side analysis, we use gravity models to predict the export potential of target products and markets given product-specific trade resistance and geographically dispersed demand. The broad sectoral focus of Mozambique’s industrial policy is largely consistent with structural transformation and export promotion. The current prioritization of agriculture, agro-industry, and metals is especially important, while there are unexploited opportunities in machinery, vehicles, and transport equipment. We find some potential for Mozambique to export target products to neighbouring countries.

Drawing on grounded theory approach and a qualitative meta-analysis, this chapter intends to systematically review and synthesize the theories, methods, and findings of both qualitative and quantitative inquiries on computer-based instructional games. A major purpose of this literature review and meta-analysis is to inform policy and practice based on existing studies. Four major recurring themes concerning the effectiveness of computer-based instructional games have emerged from a comparative analysis with 89 instructional gaming studies and are discussed with the support of exemplar research. The chapter will assist practitioners and policymakers in understanding the “best practices” and key factors of a computer game-based learning program.

Анотація:
Despite the increase in the adoption of online educational platforms, student retention is still a challenging task with a number of students having low performance margins during these courses. This chapter intends to predict student performance based on their learning behavior on the basis of their logging data history, using the publicly available Open University Learning Analytics Dataset. To model this problem, logistic regression (LR) is used as a baseline technique. Additionally, random forest (RF), multiple layered perceptron with multiple activation functions, and Gaussian Naïve Bayes are also deployed. The results demonstrate that RF outperforms the baseline LR and other models with 89% accuracy, 89% precision, 88% recall, and 88% F1-score. Finally, the authors conclude that using the above-mentioned models, students “at-risk” can be identified which can be managed by an alert mechanism to improve student success rate by making timely interventions.

The VEGF Inhibition Study in Ocular Neovascularization (V.I.S.I.O.N.) study comprised two concurrent randomized, double-masked, sham-controlled clinical trials to evaluate the efficacy and safety of a second year of treatment with pegaptanib sodium (an anti-vascular endothelial growth factor [VEGF] agent) in patients with neovascular age-related macular degeneration (AMD). In combined analyses, 88% of patients were re-randomized at week 54, and 89% were assessed at week 102. At week 54, those initially assigned to pegaptanib were re-randomized to continue or discontinue therapy for 48 more weeks (8 injections). Those initially assigned to sham were re-randomized to continue sham, discontinue sham, or receive 1 of 3 pegaptanib doses. The study showed that in patients with a diverse mix of neovascular AMD presentations, treatment with pegaptanib every 6 weeks for 2 years reduced the risk of vision loss regardless of lesion composition or size; this benefit was maintained during year 2.

The purpose of this chapter is to list the more common accepted methods used in short-term climate prediction, explain how they are designed, how they are supposed to work, what level of skill can be expected and the references to find more about them. The emphasis is on methodology but aspects of verification and cross-validation will be mentioned as well. Most methods will be accompanied by an example. We will also mention some of the less common methods, but with less detail. We even list some methods that are not used, to delineate which are acceptable and which are not. Sections 8.1–8.6 and 8.8 are easy to read, but Sections 8.7 and 8.9 are more difficult. It will become clear by the end of the climatology section (8.1), that only the departure from climatology, the so-called anomalies, are considered worthy forecast targets. The climatology itself, including such empirically established facts as “days are warmer than nights”, and “winters are colder than summer”, is considered too obvious to be a forecast target. This is not to say that a quantitative explanation of the Earth’s climate, including daily and annual cycle, is easy. But in professionally honest verification no points are given for forecasting a correct climatology. This chapter is thus about forecasting aspects of the geophysical system that are not so obvious and more difficult. The daily and annual cycle are periodic variations controlled by external forcings such as the solar heating. Implicit in identifying a periodic phenomenon as such is that the forecast of the phenomenon is easy out to infinity. This explains a widespread search for “cycles” in early meteorological research, but very little has been found other than the obvious daily and annual cycles. By removing a climatology that accounts for daily and annual variations we in effect remove the known easy periodic part of the system. In the absence of any other information climatology is the best information available. As many travelers can attest, somebody visiting an unfamiliar location 6 months from now is well served by inspecting climatological tables.

Abstract As part of the development of American Society of Mechanical Engineers Code Case N-809 [1], a series of sample calculations were performed to gain experience in using the Code Case methods and to determine the impact on a typical application. Specifically, the application of N-809 in a fatigue crack growth analysis was evaluated for a large diameter austenitic pipe in a pressurized water reactor coolant system main loop using the current analytical evaluation procedures in Appendix C of Section XI of the ASME Code [2]. The same example problem was previously used to evaluate the reference fatigue crack growth curves during the development of N-809, as well as to compare N-809 methods to similar methods adopted by the Japan Society of Mechanical Engineers. The previous example problem used to evaluate N-809 during its development was embellished and has been used to evaluate additional proposed ASME Code changes. For example, the Electric Power Research Institute investigated possible improvements to ASME Code, Section XI, Nonmandatory Appendix L [3], and the previous N-809 example problem formed the basis for flaw tolerance calculations to evaluate those proposed improvements [4]. In addition, the ASME Code Section XI, Working Group on Flaw Evaluation Reference Curves continues to evaluate additional research data and related improvements to N-809 and other fatigue crack growth rate methods. As a part of these Code investigations, EPRI performed calculations for the Appendix L flaw tolerance sample problem using three international codes and standards to evaluate fatigue crack growth (da/dN) curves for PWR environments: (1) ASME Code Case N-809, (2) JSME Code methods [5], and (3) the French RSE-M method [6]. The results of these comparative calculations are presented and discussed in this paper.

Background: Myasthenia Gravis (MG) is an autoimmune disease of the neuromuscular junction. In Brazil, epidemiological data regarding MG and its clinical aspects are limited. Objectives: The aim of this study was to describe the clinical and epidemiological profile of patients with MG at a referral center. Design and setting: Cross-sectional observational study performed on patients with Myasthenia Gravis age 18 years and older at Federal University of Paraná. Methods: Data were obtained through clinical evaluation and medical records. Results: The sample comprised 100 patients (67 females and 33 males). The median age at last appointment was 49.5 years (IQR 37-61). AntiAchR antibody was positive in 79 patients. Anti-MuSK antibody was positive in 6 positive patients. The Repetitive Nerve Stimulation (RNS) test presented abnormal result in 89 individuals. The main comorbidities reported were systemic arterial hypertension (SAH) (35%), mental health disorders (23%), diabetes mellitus (DM) (21%), dyslipidemia (18%), and hypothyroidism (13%). During the study period, the patients were on the following medications: pyridostigmine (88%), prednisone (42%), azathioprine (45%), cyclosporine (13%), and mycophenolate (6%). Thymectomy was performed in 30 patients, of whom 6 had a pathological diagnosis of thymoma, and five a pathological diagnosis of thymic hyperplasia. Conclusion: The patient profile as well as the prevalence of comorbidities in our MG sample were similar to previous studies. We also found that our sample presented a higher incidence of SAH, DM, mental health disorders and hypothyroidism compared with the general population.

Introduction: Delirium has a high prevalence in hospitalized elderly patients. This is due to low hospital detection and the absence of a screening instrument. Objective: evaluate predictive variables in the development of delirium in na in-hospital environment. Methods: Cross-sectional study. Data collection was carried out between 2015-2016, with a sample of 493 elderly people. The variables used were age, sex, the reason for hospitalization, Identification of Elderly at Risk (ISAR), delirium during hospitalization using the Confusion Assessment Method, frailty using the Edmonton Scale, the impact of comorbidities by the Charlson Index and hospital immobility. Predictive variables were identified through logistic regression. Results: 469 elderly people were taken. The presence of delirium during hospitalization was mostly observed between 80 and 89 years old (n = 12), female (n = 16), with the most common reasons for hospitalization due to fractures (n = 6) and accident brain vascular (n = 11), 79% chance of surviving in one year using the Charlson Index (n = 11) and with ISAR> 2 (n = 26). There are important associations for the development of delirium for patients who have a 98% chance of surviving in one year (p = 0.05) and with ISAR <2 (p = 0.027), with a 34% increased chance and 38%, respectively. Conclusion: It is observed that, by the results, the predictive variables of inhospital delirium are patients with a 98% chance of survival and with ISAR <2.

Abstract Port Fourchon Junction is located within Chevron's Fourchon Terminal, just south of Port Fourchon and is operated by Shell Pipeline Company LP. This manifold metering station is a critical junction for the Mars Corridor oil, as oil production from Mars (MC-807), Ursa (MC-809), Titan (MC-941), Who Dat (MC-547), Medusa (MC-582), and Olympus (MC-807B) flows through this station via a 24" pipeline. Port Fourchon is at the edge of the Mississippi delta facing the sea, one of the world's most vulnerable low-elevation coastal zones. It is highly exposed to storm surge and wave-induced inundation under hurricanes which regularly visit the Gulf of Mexico. In addition, it experiences one of the largest rates of subsidence in the world, which combined with sea level rise, will increase the site vulnerability in the coming decades. This study assesses present and future scenarios of subsidence and sea level rise under extreme metocean conditions induced by hurricanes and their impact on Port Fourchon Junction. Local effects such as the differential settlement of the barrier beach have been also considered. Using results from the numerical model XBeach, a set of different present and future scenarios are modelled under extreme metocean conditions. These conditions and the subsequent design parameters calculated, are not obtained through traditional extreme value analysis methods, instead, they are estimated through the influence of boundary conditions forced with the corresponding return period values of the parameters. Boundary conditions for the simulations are extracted from Grand Isle and Port Fourchon sea level observations, and from FEMA and the Water Institute of the Gulf simulations. Port Fourchon site should be subject to flooding for 10-year return period conditions based on Grand Isle observations. For 5-6 years return period conditions some degree of milder partial flood should also be expected. This is well captured by the model. While the highest inundating level is mostly dependent on winds, waves and surge acting together, surge is the single most critical parameter that defines the asset's base inundation level. Design future conditions based on surge extreme from FEMA simulations are recommended over surge extremes derived from Grand Isle observations. The barrier beach and the breakwaters play a key factor in sheltering site from waves and surge. Even when submerged under extreme high return period conditions they dissipate the waves ensuring that the maximum water level (wave crest elevation) on site is lower than would otherwise be without them. It is then important to maintain them fit for purpose during the entire lifespan of the asset. Both Grand Isle and Port Fourchon subsidence scenarios yield similar results. Based on the importance of Port Fourchon Junction facilities, the design criteria obtained, and the higher subsidence level observed at Port Fourchon (compared to Grand Isle), it is recommended that a 1000-year return period and future scenario based on FEMA surge level and Port Fourchon Relative Sea Level Rise (RSLR) is adopted for design. The subsidence associated to this scenario is 9.8 mm/year. The sea level rise associated to this scenario is 2 mm/year.

Background: Pelvic radiotherapy may damage the vagina and cause vaginal stenosis. Its incidence in the literature ranges from 1.2% to 88%. To prevent vaginal stenosis, routine vaginal dilation is recommended during and after pelvic radiotherapy. Materials and Methods: The objective was to examine critically the evidence behind this guideline. Searches included the Cochrane Central Register of Controlled Trials, MEDLINE, EMBASE and Google scholarly articles. All the relevant articles were included in the study. Discussion: Various studies gave recommendations on dilation during or immediately after radiotherapy. Literature does not support routine vaginal dilatation during or immediately after pelvic radiotherapy. Occasional penetration might prevent the sides of the vagina adhering to each other, and dilation might be valuable once the inflammatory and psychological scarring has settled. Two trials demonstrated that encouraging vaginal dilation increased patient compliance, but no difference was found in sexual function scores in the first trial. One retrospective study reported that dilation lowered stenosis rates, but the control group is not comparable. One study involving 89 women revealed that the median vaginal length was 6 cm, six to ten weeks after radiation therapy, but women tolerated a 9-cm dilator/measurer after 4 months of dilation experience. One trial showed no significant advantage by inserting mitomycin C. A study of five women reported that vaginal stenosis can be treated by dilation even many years after radiotherapy. Dilation during or immediately after radiotherapy can cause damage, and there is no evidence that it prevents stenosis. Dilation might stretch the vagina if commenced after the inflammatory phase. Dilation has been associated with traumatic rectovaginal fistulae and psychological consequences. Conclusion: Vaginal dilation might help treat the late effects of radiotherapy, but it must not be assumed that this applies to the acute toxicity phase. Routine dilation during treatment is not supported by good evidence. Prophylactic and therapeutic dilation therapy needs to be considered separately and research is needed to determine when dilation therapy should start on a large population.

INTRODUÇÃO: Insuficiência cardíaca (IC) é uma síndrome cardiovascular, na qual o coração é incapaz de bombear sangue de forma a atender às necessidades fisiológicas adequadamente. Nesse sentido, a determinação dos fatores de risco do paciente acometido pela IC é de suma importância, visto que se busca reduzir a problemática e o risco de morte do paciente. OBJETIVO: Analisar a prevalência dos fatores de risco para IC em pacientes diagnosticados e atendidos em Hospital Universitário. MÉTODO: Trata-se de um estudo retrospectivo, transversal,feito entre janeiro de 2021 e fevereiro de 2022 no Hospital Universitário Presidente Dutra no estado do Maranhão. Foram incluídos pacientes diagnosticados com IC, indivíduos com idade ≥ 18 anos, com Fração de Ejeção (FE)≤ 50%, que responderam ao questionário do projeto de pesquisa. Os critérios de exclusão foram pacientes com FE ≥ 50% ,idade < 18 anos, e que não possuiam IC.Ademais, calculou-se a média ± desvio padrão para algumas variáveis quantitativas e o percentual para as qualitativas. RESULTADOS: Com 83 pacientes estudados, a média de idade foi 57,57 ± 13,26 , 51%( 42) possuíam entre 30 e 59, 43%(36) tinham idade acima ou igual a 60 anos e 6% (5) possuíam entre 18 e 29 anos e 67 % (56) eram do sexo masculino. Predominou a cor preta e parda , com 88% (73) do total e escolaridade fundamental incompleto 37%.(31) A principal etiologia encontrada foi a isquêmica 28% (23). Além disso, 42% (35) dos pacientes possuíam Hipertensão Arterial Sistêmica , 12%(10) possuíam Diabetes Mellitus, 18%(15) possuem as duas comorbidades e 28%(23) negaram as duas comorbidades ou não souberam responder. Ademais, observou-se que 43% (36) dos pacientes eram ex-fumantes, dentre estes 89%( 32) relatou ter fumado mais de 10 anos. Somado a isso, 47% (39) dos pacientes fizeram uso de álcool por mais de 5 anos. CONCLUSÃO: Dessa forma, o estudo conclui que há uma predominância de pacientes entre 30 e 59 anos, pacientes hipertensos e pacientes que faziam uso de álcool ou eram ex-fumantes. Além disso, é perceptível que o impacto do tabaco é visto mais em pacientes que fumaram por mais de 10 anos.
