Résumé Pour des maladies qui exigent des délais d’intervention courts telles que l’hémophilie, l’accessibilité aux soins revêt une importance capitale. Les caractéristiques sociales, thérapeutiques et cognitives des patients peuvent jouer un rôle de barrière ou, au contraire, faciliter l’accès aux services de santé. L’analyse des distances-temps aux lieux de soins réalisée en Bretagne montre que les hémophiles sévères sont proportionnellement plus nombreux dans les zones proches des hôpitaux. Les temps d’accès jouent un rôle négatif sur la satisfaction des patients pour qui ces déplacements ne sont pas routiniers. En pratique, les patients ne recherchent pas forcément le lieu de soins le plus proche et établissent leur parcours de soins en fonction de la qualité perçue des soins ou des relations avec le personnel. L’auto-traitement fait diminuer les déplacements, mais génère des contraintes matérielles qui freinent la mobilité quotidienne, alors qu’à l’échelle des parcours de vie, c’est la maladie elle-même qui est déterminante.

Stress, conflits, surcharge, pression et contrôle, mais aussi gain de temps, motivation, accessibilité… sont autant de mots souvent prononcés à l’évocation de la messagerie électronique, tout particulièrement lorsqu’elle est associée aux relations hiérarchiques. Bien que les chercheurs et praticiens reconnaissent les effets du média sur la relation hiérarchique, la recherche académique peine à expliquer la nature de leur relation ainsi que les variables explicatives de cette même relation. Visant à répondre à ces questions, la problématique centrale de notre étude consiste à examiner la relation entre l’utilisation de la messagerie électronique et la qualité de la relation hiérarchique en mettant l’accent sur trois variables médiatrices : la reconnaissance, le contrôle et l’accessibilité. L’étude empirique a été réalisée au sein d’une banque de réseau française. Une étude qualitative exploratoire a contribué à la construction d’échelles de mesure utilisées ensuite dans un dispositif de recherche quantitatif : 478 questionnaires valides ont permis une analyse du modèle par la méthode des équations structurelles. Les résultats révèlent que l’intensité perçue d’utilisation du média est liée positivement à la qualité de la relation hiérarchique lorsque la relation est considérée isolément des variables médiatrices. En revanche, lorsque nous traitons le modèle dans son ensemble — en introduisant les variables médiatrices de reconnaissance, contrôle et accessibilité — la relation directe entre utilisation de la messagerie électronique et qualité de la relation hiérarchique n’est plus validée. La reconnaissance et le contrôle sont des mécanismes explicatifs de cette relation, tandis que l’accessibilité n’explique que partiellement la relation entre messagerie électronique et qualité de la relation hiérarchique. Outre les apports méthodologiques que procure la construction du modèle et celle des échelles de mesure, la présente recherche vise à fournir des pistes pour une amélioration des pratiques de communication électronique au sein de la dyade supérieur hiérarchique/subordonné.

Les élèves présentant des difficultés comportementales sont généralement perçu⋅e⋅s comme plus difficiles à scolariser en classe ordinaire. À l’aide du modèle de causalité triadique réciproque, cet article présente les résultats d’une recherche visant à décrire l’influence des pratiques de gestion de classe sur l’expérience scolaire de ces élèves et à documenter les enjeux liés à leur scolarisation en classe ordinaire. Les entrevues menées auprès de quatorze élèves présentant des difficultés comportementales scolarisé⋅e⋅s en classe d’adaptation scolaire au secondaire révèlent que la manière dont les enseignant⋅e⋅s gèrent la classe influence significativement leur expérience scolaire. De nombreux leviers susceptibles de favoriser leur réintégration au secteur régulier ont aussi été identifiés.

Dinamika pengembangan wilayah kota menimbulkan permintaan supply lahan bermukim seiring meluasnya wilayah pembangunan kota ke arah pinggiran kota.Salah satu wilayah bertetangga hinterland kota Medan dengan Kabupaten Deli Serdang adalah kawasan Tembung di Kecamatan Percut Sei Tuan yang menunjukkan gejala peri-urbanisasi menjadi kawasan cepat tumbuh desa-kota.Secara spatial arsitektural perubahan struktur kependudukan kawasan Tembung menunjukkan meluasnya pertumbuhan lahan terbangun permukiman yang menimbulkan masalah terbentuknya pola guna lahan (land use) terbangun secara acak (sprawl).Kajian ini mengungkapkan faktor-faktor daya tarik kawasan yang berpengaruh serentak terhadap terbentuknya pola guna lahan terbangun di kawasan Tembungmeliputi aspek topografi, aksesibilitas, infrastruktur, sosial-ekonomi, potensi imaji lokal.Dengan metode kuantitatif deskriptif dilakukan analisis atas data persepsi para pemukim di wilayah terbangun cepat tumbuh. Analisis deskriptif dan regresiSEM menunjukkan keseluruhan faktorberpengaruh terhadap perubahan guna lahandengan rincian hanya pada faktor aksesibilitas (P=0,039), infrastruktur (P=0,005), sosio ekonomi (P=0,038), potensi imaji lokal (P=0,007) yang signifikan berperan menjadi daya tarik kuat bagi perubahan guna lahan terbangun. Implikasi temuan perlunya pengawasan dan pengendalian pemanfaatan ruang terbangun di wilayah berdaya tarik kuat guna mengarahkan kecenderungan dampak negatif urban sprawl development. The dynamics of urban development lead to the demand for land supply along with the expansion of urban development areas towards the suburb of the cities. One neighboring area of ​​Medan City, the hinterland in Deli Serdang Regency is Tembung, located in Percut Sei Tuan Subdistrict.It shows symptoms of peri-urbanization which becomes village-city fast growth area. In architectural spatial, the changes in population structure of Tembung shows the widespread growth of settlement land built that caused problems in the formation of land use (sprawl). This study revealed regional attraction factors which had simultaneous effect on the formation of land use built patterns in Tembung area including topographic aspects, accessibility, infrastructure, socio-economic, local image potential. An analysis was condected by using quantitative descriptive method on the data of resident perceptions in rapidly growing built up area. Descriptive analysis and SEM regression showed all factors affected the change in land use with details only on accessibility factors (P = 0.039), infrastructure (P = 0.005), socio-economic (P = 0.038), local image potency (P = 0.007) which significantly attracted the changes in land use built. This study showed implications of the need for monitoring and controling land use built in regions of strong attraction area to control negative impacts of urban sprawl development.

With the rapid development of technology in the gambling industry, underage mobile gambling has become a growing concern. The present study investigated the prevalence of adolescent mobile gambling and the relationship between frequency of mobile gambling and gambling-related problems. A survey assessing past-year gambling behavior, gambling problems, perceived risk for gambling, and parental and peer disapproval of gambling was completed by 6,818 junior/senior high school students aged 10 to 19. A descriptive analysis demonstrated a 5% prevalence of mobile gambling, and regular mobile gambling (at least monthly) was associated with a higher risk of developing a gambling problem and engagement in other forms of gambling. A hierarchical logistic regression revealed that being female, younger, and perceiving higher parental disapproval of gambling is related to less past-year mobile gambling. Perceived risk of gambling-related harms and peer disapproval were not significant predictors after controlling for other factors. The results suggest that underage mobile gambling may serve as one warning sign of adolescent problem gambling. Given that mobile gambling accessibility is increasing, the results provide valuable information for early intervention and prevention.RésuméAvec le développement rapide de la technologie dans le secteur du jeu, le jeu sur appareil portable chez les personnes mineures devient une préoccupation croissante. <p><strong>ABSTRACT</strong></p><p><strong>Background:</strong> Vaccination is a safe, efficient, and cost-effective means of preventing, controlling, and eradicating many life-threatening infections and diseases. Globally, the World Health Organization estimates that vaccination saves between 2 million and 3 million lives annually. However, low immunization rates are a significant public health concern. Individual factors, along with the vaccination process and system, have been reported as perceived barriers and challenges to immunization. Lack of time, on the part of both health care professionals and patients, has also been reported as a key factor influencing patterns of immunization. Despite the accessibility of pharmacists in community pharmacies in Australia, and initiatives by other countries to introduce pharmacist vaccination services, pharmacists in Australia had not previously delivered this service. The Queensland Pharmacist Immunisation Pilot (QPIP), initially implemented for the 2014 influenza season and later expanded, as QPIP2, to include other vaccines, allowed Australian pharmacists to vaccinate for the first time.</p><p><strong>Objectives:</strong> To develop, implement, and evaluate a training program for pharmacists undertaking vaccination services in community pharmacies in Australia.</p><p><strong>Methods:</strong> Background content was developed and delivered through 2 online modules. Pharmacists were required to successfully answer a series of multiple-choice questions related to the background reading before attending a face-to-face workshop. The workshop provided practical training in injection skills and anaphylaxis management. Participants were also asked to evaluate the training program.</p><p><strong>Results:</strong> Of the 339 pharmacists who completed the training program, 286 (84%) provided an evaluation. Participants were satisfied with the training, as indicated by consistently high scores on the “overall satisfaction” question (mean 4.65/5 for the QPIP and QPIP2 training combined). Participants described the background reading as relevant to their practice and stated that it met their expectations. They also valued the opportunity to practise injections on each other during the face-to-face workshop, and this aspect was noted as a key component of the training.</p><p><strong>Conclusions:</strong> QPIP demonstrated that a pharmacist-specific training program could produce competent and confident immunizers and could be used to “retrofit” the profession, to facilitate delivery of vaccination services in Australia.</p><p><strong>RÉSUMÉ</strong></p><p><strong>Contexte :</strong> La vaccination est un moyen sécuritaire et efficient de prévenir, d’endiguer et d’éradiquer bon nombre d’infections et de maladies potentiellement mortelles. L’Organisation mondiale de la santé estime que, dans le monde, la vaccination permet de sauver entre 2 millions et 3 millions de vies par année. Cependant, les faibles taux d’immunisation représentent un enjeu de santé publique important. On a noté que des facteurs individuels ainsi que le processus et le système de vaccination sont perçus comme des obstacles à l’immunisation. Le manque de temps, tant de la part des professionnels de la santé que des patients, a aussi été présenté comme un facteur clé influençant les schémas d’immunisation. Malgré la disponibilité de pharmaciens dans les pharmacies communautaires en Australie et les initiatives lancées par d’autres pays pour mettre en place des services de vaccination offerts par les pharmaciens, les pharmaciens en Australie n’avaient pas prodigué ce service auparavant. L’étude Queensland Pharmacist Immunisation Pilot (QPIP), d’abord mise en place pour la saison de la grippe de 2014, puis reconduite en une version bonifiée nommée QPIP2 afin d’inclure d’autres vaccins, a permis aux pharmaciens australiens de vacciner pour la première fois.</p><p><strong>Objectifs :</strong> Élaborer, mettre en place et évaluer un programme de formation pour les pharmaciens qui offrent des services de vaccination dans les pharmacies communautaires en Australie.</p><p><strong>Méthodes :</strong> La matière du programme a été élaborée et offerte sous forme de deux modules en ligne. Les pharmaciens devaient répondre adéquatement à une série de questions à choix multiples portant sur les lectures préparatoires à un atelier en personne. L’atelier offrait une formation pratique sur les techniques d’injection et sur la prise en charge de l’anaphylaxie. On a aussi demandé aux participants d’évaluer le programme de formation.</p><p><strong>Résultats :</strong> Des 339 pharmaciens ayant terminé le programme de formation, 286 (84 %) ont fourni une évaluation. Les participants étaient satisfaits de la formation, comme l’indiquaient des cotes systématiquement élevées à la question concernant la satisfaction globale (moyenne de 4,65 sur 5 pour la formation des études QPIP et QPIP2 prises ensemble). Les participants ont indiqué que les lectures préparatoires étaient pertinentes pour leur pratique et qu’elles répondaient à leurs attentes. Ils ont aussi apprécié l’occasion de s’exercer à faire des injections les uns sur les autres au cours de l’atelier en personne; on a d’ailleurs noté que cet aspect représentait un élément clé de la formation.</p><p><strong>Conclusions :</strong> L’étude QPIP a permis de constater qu’un programme de formation destiné aux pharmaciens pouvait produire des vaccinateurs compétents et sûrs d’eux et qu’il pouvait servir à « moderniser » la profession afin de faciliter la prestation de services de vaccination en Australie.</p>

Pour répondre aux défis environnementaux dans les grands espaces urbains, de nombreuses politiques publiques visent la réduction de l’usage de la voiture, au profit des mobilités dites douces, mobilité partagée et mobilité active. Cependant, les utilisateurs résistent individuellement et collectivement à ces mesures : ils n’ont pas toujours les capacités (problèmes d’accessibilité), ni les motivations (valeur perçue) pour changer de pratiques de mobilité. Si le marketing s’est beaucoup intéressé à la création de valeur par l’utilisateur, l’accessibilité perçue reste un concept peu étudié et pourtant majeur pour comprendre quelles capacités favorisent le changement de pratiques. Cette recherche explique le rôle de l’accessibilité et de la valeur perçue de l’expérience – formée de la valeur dans l’usage créée à une échelle méso, et de la valeur du contexte créée à une échelle micro – dans le changement de pratiques de mobilité. Une étude qualitative auprès de 12 experts engagés dans des problématiques de mobilité urbaine, et de 10 utilisateurs a permis de proposer une définition de l’accessibilité perçue et de la valeur perçue des expériences de mobilité douce. Une étude quantitative auprès d’un panel de 883 utilisateurs a ensuite permis l’opérationnalisation des échelles de mesure et de tester les hypothèses sur les effets de l’accessibilité et de la valeur perçue sur le changement de pratiques. Le rôle modérateur des pratiques passées et de la distance aux pratiques futures est également étudié à l’aide d’analyses plus précises sur les utilisateurs réguliers de la voiture pour les engager dans une mobilité partagée, et les utilisateurs réguliers des transports en commun pour les engager dans une mobilité active. Les résultats montrent le rôle important de l’accessibilité relative sur le changement de pratiques des utilisateurs réguliers ou non réguliers de la voiture, et des utilisateurs monomodaux et multimodaux. Cette dimension de l’accessibilité perçue renvoie à la facilité globale perçue à poursuivre un objectif sans effort. A l’inverse, différentes dimensions de la valeur ont une influence positive, ou négative, sur le changement de pratiques, en fonction des pratiques passées des utilisateurs et de la distance aux pratiques futures
To meet the environmental challenges in large urban areas, many public policies aim to reduce car use in favour of so-called soft mobility, shared mobility and active mobility. However, users individually and collectively resist these measures: they do not always have the capacity (accessibility problems) or the motivation (perceived value) to change their mobility practices. While marketing has been very interested in the creation of value by the user, perceived accessibility remains an understudied concept, yet it is a major factor in understanding which capabilities encourage changes in practices.This research explains the role of accessibility and the perceived value of the experience - made up of the value in use created at a meso scale, and the value of the context created at a micro scale - in changing mobility practices. A qualitative study with 12 experts involved in urban mobility issues and 10 users allowed us to propose a definition of the perceived accessibility and perceived value of soft mobility experiences. A quantitative study with a panel of 883 users then made it possible to operationalise the measurement scales and to test the hypotheses on the effects of accessibility and perceived value on changes in practices. The moderating role of past practices and distance to future practices is also studied with the help of more precise analyses on regular car users to engage them in shared mobility, and regular public transport users to engage them in active mobility.The results show the important role of relative accessibility on the change of practices of regular or non-regular car users, and of monomodal and multimodal users. This dimension of perceived accessibility refers to the overall perceived ease of pursuing a goal without effort. Conversely, different dimensions of value have a positive or negative influence on the change of practices, depending on the past practices of the users and the distance to future practices

Background: Irresponsible self-medication is a problem for health systems in developing countries. We aimed to estimate the frequency of self-medication and associated factors in users of drugstores and pharmacies in Peru. Methods: We performed a secondary data analysis of the 2015 National Survey on User Satisfaction of Health Services (ENSUSALUD), a two-stage probabilistic sample of all regions of Peru. Non self-medication (NSM), responsible self-medication (RSM) and irresponsible self-medication (ISM) were defined as the outcome categories. Demographic, social, cultural and health system variables were included as covariates. We calculated relative prevalence ratios (RPR) with their 95% confidence intervals (95%CI) using crude and adjusted multinomial logistic regression models for complex samples with NSM as the referent category. Results: 2582 participants were included. The average age was 41.4 years and the frequencies of NSM, RSM and ISM were 25.2%, 23.8% and 51.0%; respectively. The factors associated with RSM were male gender (RPR: 1.35; 95%CI: 1.06-1.72), being between 40 and 59 years old (RPR: 0.53; 95%IC: 0.39-0.72), being 60 or older (RPR: 0.39; 95%IC: 0.25-0.59), not having health insurance (RPR: 1.89; 95%CI: 1.31-2.71) and living in the Highlands region (RPR: 2.27; 95%CI: 1.23-4.21). The factors associated with ISM were male gender (RPR: 1.41; 95%CI: 1.16-1.72), being between 40 and 59 years old (RPR: 0.68; 95%IC: 0.53-0.88), being 60 or older (RPR: 0.65; 95%IC: 0.48-0.88) and not having health insurance (RPR: 2.03; 95%CI: 1.46-2.83). Conclusion: Around half of the population practiced ISM, which was associated with demographic and health system factors. These outcomes are the preliminary evidence that could contribute to the development of health policies in Peru.

In this paper we estimate the average treatment effect from access to extension services and credit on agricultural production in selected Andean countries (Bolivia, Peru, and Colombia). More specifically, we want to identify the effect of accessibility, here represented as travel time to the nearest area with 1,500 or more inhabitants per square kilometer or at least 50,000 inhabitants, on the likelihood of accessing extension and credit. To estimate the treatment effect and identify the effect of accessibility on these variables, we use data from the Colombian and Bolivian Agricultural Censuses of 2013 and 2014, respectively; a national agricultural survey from 2017 for Peru; and geographic information on travel time. We find that the average treatment effect for extension is higher compared to that of credit for farms in Bolivia and Peru, and lower for Colombia. The average treatment effects of extension and credit for Peruvian farms are $2,387.45 and $3,583.42 respectively. The average treatment effect for extension and credit are $941.92 and $668.69, respectively, while in Colombia are $1,365.98 and $1,192.51, respectively. We also find that accessibility and the likelihood of accessing these services are nonlinearly related. Results indicate that higher likelihood is associated with lower travel time, especially in the analysis of credit.
