Анотація:
Background: Diagnosis of Spontaneous Bacterial Peritonitis (SBP) depends mainly on ascetic fluid culture which may be negative in spite of the clinical suggestion of SBP and high ascetic fluid neutrophilic count. Aim: This study aimed to evaluate the biological importance of amyloid A biomarker in both serum and ascetic fluid to diagnose SBP as early as possible and to compare it to other markers (C-reactive protein (CRP), and the neutrophil-to-lymphocyte ratio (NLR)). Methods: This study included 37 patients with hepatic ascites; twenty-two of them had SBP, and 15 patients did not have SBP. Serum and ascetic fluid amyloid A, ascetic fluid neutrophil, C-reactive protein, and neutrophil-to-lymphocyte ratio were measured in all subjects before the start of antimicrobial chemotherapy to the infected ones. Results: Both the serum and ascetic fluid amyloid and also, CRP were significantly higher in patients infected with ascetic fluid than others. The cut-off point of serum amyloid A for early detection of SBP was 9.25ug/ml with the high sensitivity and specificity. For ascetic amyloid A, the sensitivity and specificity were 90.09% and 60% at cut-off point 2.85ug/ml, respectively. Conclusion: Amyloid A in serum and ascitic fluid can be considered as a good biomarker for early diagnosis of SBP.

The significance of the snowstorm image in B. Pasternak’s novel “Doctor Zhivago” is characterized. A comparative analysis of the positions of B. Pasternak and A. Blok in relation to the revolutionary events of 1917 and their reflection in “Doctor Zhivago” and in the poem “Twelve” is carried out. The image of Yuri Zhivago is investigated from the point of view of the type of cultural hero - the Ascetic. The characteristic is given to the image of Antipov - Strelnikov as a representative of the type of cultural hero - Conquistador.

Анотація:
The article raises the question of the cultural landmarks of the heroes of the novel B.L. Pasternak “Doctor Zhivago”. As is known, the plot of the work takes place at the turn of the eras, and for some heroes this becomes an incentive to think about their cultural self-determination. Their actions and behavior model are directly related to the attitude to cultural reality described in the work, and sometimes even aimed at its transformation by practical, revolutionary methods. A certain catalytic role in relation to culture is one of the criteria of a cultural hero. From this point of view, a comparative analysis of the positions of B.L. Pasternak and A.A. Blok in relation to the revolutionary events of 1917, their reflection in “Doctor Zhivago” and in the poem “Twelve” is carried out. The image of Yuri Zhivago is investigated from the point of view of the type of cultural hero - the Ascetic. The characteristic is given to the image of Antipov (Strelnikov) as a representative of the type of cultural hero - Conquistador. It has been established that Zhivago’s ascetic cultural position is based on loyalty to timeless ideals, an idea of the aesthetic potential of the Personality in relation to reality, while Antipov's (Strelnikov) actions are aimed at suppressing the individual human principle and, therefore, are devoid of a genuine creative component that nourishes the culture of mankind.

Анотація:
The article focuses on three biographies of Josef Knecht from the novel by Hermann Hesse “The Glass Bead Game” with regard to the postulates of transpersonal psychology. A thorough study provides insight into the peculiarities of a unique literary technique – the transfer of the “self” into different historical periods by means of imagination, as a kind of “way to self”, devotion to truth through pagan voluntary sacrifice (“Rainmaker”), the ascetic self-denial of a Christian schemamonk (“The Father Confessor”), the life of an Indian ascetic (“Siddhartha”), understanding that all being is illusionary. The research highlights the similarity of plot events, names, integration of esoteric and exoteric perspectives. The study reviews a holotropic state of consciousness, which can develop into psychoid experience, accounting for the impossible to become possible. Based on the judgments of the direct relationship between the concepts of the macrocosm and the microcosm, the perception of reality as a result of human intellectual activity, it is specified that the efforts of the “possible human”, “virtual human” to liberate themselves from the dualistic perception of reality is accomplishable by acquiring the unity of consciousness. The authors point out that the review of the motive of “awakening” indicates Hesse’s intention to embody absolute transcendent reality in his characters, which could manage the crisis of splitting consciousness.

Анотація:
This research aims to know and understand more deeply about mystical values and transformative pedagogic values in the Syaikh Siti Jenar novel written by Agus Sunyoto. The focus of this study was the intrinsic and extrinsic structure of meaning in the novel. The method applied in this study is content analysis using genetic structure by Lucien Goldmann. The results of the study show a number of sufistic values and transformative pedagogic values. Sufistic values include: repentance, ascetic, indigent, patience, gratitude, pleasure, and trust. While the transformative pedagogic values include altruism, egalitarian, pluralism, and eclectic. The findings of the author's world view includes figures deconstruction of the teachings of Syaikh Siti Jenar, cultural missionary endeavor, the concept of superhuman, mystical literature, and literary history.

It is not so when reading the novel trilogy by Agus Sunyoto. The spiritual aspects of religion, which are presented can be said to be logical when viewed through the angle of asceticism. Research studies conducted in the form of text with the approach of critical discourse analysis (CDA). The data collected from the text unit of the novel trilogy of Sheikh Siti Jenar by Agus Sunyoto. The result of this research is, asceticism as individual morale leads to social and political morale. And the ascetic ethos of Sheikh Siti Jenar as a human person provides the impetus and limitation to act in his daily behavior. The idea of salvation in the world sets the theme of the hermitage and the world, monasticism in the world, worldly holiness in culture. Monasticism is seen as a state of mind regardless of status. The article examines the artistic images of monks in the world. Two Dostoevsky novels "The Idiot" and "Crime and Punishment" are considered. It is proved that in the novel "Idiot" the main character acts as a visionary mystic and a missionary ascetic, he is positively beautiful in everything, excessively gifted with a sense of truth, beauty, tact, harmony. The author's thought is organized multilevel and dynamically, revealed in a triad: thesis, antithesis, synthesis. In the image of Alyosha Karamazov the idea of "about a Christian" is embodied, which the historical reality and personality type did not allow the author to fully realize in Myshkin. Any creation is not only the face of the author in his mobile reality, but also the outcome from it, a creative meta-idea.

Abstract This article reads Flaubert's unfinished final novel Bouvard et Pécuchet as a two-volume epic that demonstrates the secret of the encyclopedia: the fact that different calculi of truth functions can be seen to operate independently in different areas of knowledge, that knowledge itself contains contradictions. Taking Lukács's distinction between “narrative” and “descriptive” realism as a starting point, I argue that Bouvard in fact “narrates” the very changes Lukács describes, depicting a historical shift that speaks not only to changes in literary practice but to fundamental changes in the theory and organization of knowledge itself (compare Umberto Eco's From Tree to Labyrinth). Drawing on Bernard Stiegler's theory of the “proletarianization of consumption” in the postmodern “libidinal economy,” I read Flaubert as offering a prophetic Marxist critique of the political economy (centered around the consumption of information) that determines Bouvard and Pécuchet's estate. The novel's structure as an immoralist bildungsroman—in which the protagonists’ faithful pursuit of Enlightenment ideals eventually leads them to directly reject those ideals—corresponds to and predicts the situation diagnosed by Stiegler in For a New Critique of Political Economy. Finally, I point to a cycle of sublimation and desublimation in the novel regarding knowledge and stupidity, a complex cycle that nonetheless makes it possible to read the ascetic-aesthetic copy-mapping project of volume 2 (the Sottisier) as a cynical but effective response to dehumanizing capitalist processes like proletarianization and the desublimation of knowledge.

Abstract This essay sheds new light on the spirituality and historical significance of the influential and controversial Dominican friar Battista Carioni da Crema (c.1460–1534). A popular spiritual writer, charismatic founder of devout associations such as the Barnabites, and a spiritual director of several well-known Catholic figures, including Gaetano Thiene, Battista’s significance has not yet been fully acknowledged. The essay considers his spirituality in the framework of reforming movements emerging in Italy in the first half of the sixteenth century. In dialogue with previous interpretations of Battista, the essay provides a novel and systematic analysis of his notion of perfection and concept of the Church. Synthesising ascetic and mystic spiritual influences rooted in the monastic and humanist culture of the fifteenth century, Battista presented a distinctive view of Christian life, which included an ecclesiological perspective and a new geography of the sacred. Defined as the ‘third life’ and conceived in a period of religious fluidity, it neither fitted emerging Lutheran ideas nor the orthodox Catholicism of the Roman Church. In addition, the essay argues that Battista’s proselytism can be seen as an attempt to reform society which preceded proposals for religious reform made by groups such as the Spirituali.

Анотація:
This study is the analysis of the character Siddhartha’s pilgrimage journey in the novel entitled Siddhartha. There are three objectives of this study. The first is to describe the conflict between human’s worldly desires and his spiritual quest in Hermann Hesse’s novel, Siddhartha. The second is to explain how Siddhartha’s conflict between his worldly affairs and spiritual journey in view of Gadamer’s hermeneutics. And, the third is to explain how Siddhartha’s worldly and spiritual duality as a revelation of human’s conflict in general. The analysis and the description of the data are provided to find the conclusion. The analysis of this study is using Gadamer’s Hermeneutics theory. The study shows that the main character Siddhartha is searching for salvation through the ordeal life. He learned many teachings from Hinduism, being ascetic Samana, learned Buddha’s teachings, being worldly slave as the courtesan adherent, and finally he finds salvation from the river. In his journey to find salvation, Siddhartha is difficult to find teachings can satisfy him. He got the internal conflict between his worldly and spiritual quest when he became courtesan’s adherent. However, all the inner conflicts that Siddhartha felt have big roles and led him to make the next decision in his life. In the end of Siddhartha’s journey, finally he found salvation. The river has succeeded to change Siddhartha by its voice and “listen” became the key why he gets salvation in his life. Besides, there is the fact that Siddhartha’s story modelled itself on the journey of Siddhartha Gautama the Buddha. Hesse’s Siddhartha was written based on the author’s travel experience in India. The conflict that Hesse portrayed in that story mostly faced by people. In the novel, Hesse characterized Siddhartha as an individual who later found his life wisdom in an act of listening to the voices of nature. Listening means everything for Siddhartha. Many of the surviving texts associated with seventh-century Ireland deal with eschatology. In general, these texts convey assumptions on the fate of the soul after death that are consistent with the traditional Christian view in late antiquity in the West, namely, that the ultimate destiny of most Christians will not be determined till the great universal judgment at the end of time. To illustrate this point I will adduce the theological treatise Liber de ordine creaturarum and the moral treatise De XII abusiuis saeculi, as well as religious poetry and hagiographical works, and set them in their likely liturgical context. In contrast, the Vision of Fursey and Adomnán's Life of Columba stand out as revealing the strong influence of concepts originating in the early Christian ascetic circles of Egypt. Irish Christians were thus also exposed to the idea that the fate of all individual souls will be determined immediately after death. Awareness and even endorsement of this point of view will have prepared the way toward accepting the novel teaching that a finite period of painful purification immediately follows the death of most Christians. The survival of these two texts demonstrating the presence in seventh-century Ireland of the belief that the final determination of an individual's fate occurs at the time of death draws attention to the radical change in perspective that must have been an essential initial step in the very formulation of the doctrine of purgatory.

Анотація:
This study aims to reveal and describe the ascetic morality of Sheikh Siti Jenar in the heptalogy novels of Shekh Siti Jenar by Agus Sunyoto using the content analysis method. The problem discussed in this research is the ascetic morality of Sheikh Siti Jenar. The data source is the heptalogy novels of Shekh Siti Jenar which is divided into seven volumes, which was the result of a philology research with primary data of approximately 300 manuscripts. The research result was published by LKiS Yogyakarta in the form of stories. The results of this study indicate that the ascetic morality used in the novels is in the form of morality conditioned by humanism and bound by asceticism because it reveals the real issues of human life inreligious issues and how they are socially constructed. The ascetic morality expressed through its main character, Sheikh Siti Jenar, has its own energy to convey the side of human life that is lived like other people live it.

The article considers the connection of the works of V. G. Korolenko with the literary Western European tradition of depicting prominent people, labelled by the stamp of genius, who left a noticeable mark in science. The concept of genius is considered in the interpretation of philosophers I. Kant, A. Schopenhauer and F. Nietzsche, who saw it as consistent with their theories: contemplation and self-denial, will, energy of all previous generations. According to philosophical concepts, the following main trends in the literature of the nineteenth century were formed: the image of fanatical thieves from the works of M. Shelley, G. Wells, or infantile observers, as the heroes of the works of J. Verne – eccentrics detached from the real life. The realistic tradition in the depiction of images of scientists is considered on the example of the work of A. Chekhov «Boring Story». The image of a scientist created by a writer reflects the main contradiction in the life of a creative person – the discrepancy between life in science and real existence. There wasn’t any difference between creative and real life for V. Korolenko. He «lived as he wrote, and wrote as he lived». The great righteous man of everyday life, a missionary of spiritual deeds, the writer Korolenko created an image of the scientist Izborsky similar to his own life principles. The narrative structure of the work enabled the author to reflect on his student's past and think about his own unrealized future. It is hypothesized that Korolenko could be new Timiryazev or Mendeleev; however he chose the way forward active citizenship and revolutionary mood. The paper examines how the writer transformed the image of the scientist, which developed in European literature, into a new type of hero, due to socio-historical conditions and the spiritual search for time. The images of scientists created in the story «On both sides» and partly in the novel «The Story of My Contemporary» are analyzed from two points of view – the narrator-student and professor-teacher. A young man, who is also in a difficult mind condition, perceives personality, oratory, moral principles of the scientist Izborsky. The analysis of the narrator's structure helps to determine the conceptual features of the image of an ascetic scientist, among which is paid attention to the presence in the works of historical context, the strength of moral influence on students, admiration for the prospects of scientific discoveries.

In his 1867 review of two recent historical novels, Henry James worried that, barring “a second Walter Scott,” no modern mind could synthesize the increasingly scientific discipline of history and the imagination of fiction. “[S]tory-tellers are, for the most part, an illogical, loose-thinking, ill-informed race” (280), James teased, while the new historian – he refers to “writers of a purely scientific turn of mind” such as “Niebuhr and Mommsen, Gizot and Buckle” (278) – “works in the dark, with a contracted forehead and downcast eyes, on his hands and knees, as men work in coal-mines” (279). James's anxious jocularity does little to disguise his interest in (and intimidation before) the ascetic historian, particularly that new professional's willingness to “say sternly to his fancy: So far thou shalt go, and no further.” Why, James subsequently wondered, should the novelist “not [also] imprison his imagination, for the time, in a circle of incidents from which there is no arbitrary issue, and apply his ingenuity to the study of a problem to which there is but a single solution” (279)?

Анотація:
A simple, accurate, precise method was developed for the estimation of Bempedoic acid and Ezetimibe in the tablet dosage form of Bempedoic acid and Ezetimibe. Optimized separation was achieved on an Ascentis C18 150x 4.6mm, 5 using mobile phase composition of phosphate buffer pH 4.0 and acetonitrile in the ratio of 700 mL:300 mL (v/v) at a flow rate of 1.0 mL/min; the injection volume is 10 μL and run time 6 minutes in isocratic elution. UV detection was carried out at a wavelength of 230 nm. The temperature was maintained at 30°C. Wellresolved peaks were observed with high numbers of theoretical plates, lower tailing factor and reproducible relative retention time. The method was validated and all the validation parameters were found to be within the acceptable limits.

Cytokine release syndrome (CRS) is among the most representative toxicities of chimeric antigen receptor T (CART) cell therapies against non-Hodgkin's lymphoma (NHL). However, the mechanism of CRS is not fully elucidated. Interleukin-6 (IL-6) has been reported as a key factor in the development of CRS. We here present a novel mechanism of CRS emphasizing a balance between IL-6 and soluble IL-6 receptors (sIL-6Rs), essential amplifiers for IL-6-mediated inflammatory responses. A 46-year-old male patient with transformed diffuse large B-cell lymphoma (Bcl-2+/C-Myc+) refractory to 3 lines of prior therapies was enrolled in our CART trial (NCT 03196830). Baseline findings included extensive disease, multiple lesions in the abdomen, bone marrow infiltration, and a large volume of chylous ascetics with malignant cells (ascitic IL-6 baseline level: 6,691 pg/ml). And the patient received a combination of CD19-BBζ CART (8 x 106 CAR+ cells/kg), CD22-BBζ CART (1 x 106 CAR+ cells/kg), and lenalidomide (25 mg, qod, Wang X, et al., Clin Cancer Res. 2018). The patient developed mild toxicities including fever, febrile, and rash on Day 3 which were completed reversed on Day 8 after supportive care without tocilizumab or steroids. Quantitative PCR analysis of the CAR transgene demonstrated rapid expansion of the CART cells in both peripheral blood and ascites. An efficacy assessment on Day 27 revealed that most lesions disappeared or significantly shrank, the ascites nearly completely disappeared, and no malignant cell in ascites or bone marrow was detected. The patient remained in remission for 10 months. The patient was graded as CRS grade 1 according to the ASBMT scale, and cytokine assays of peripheral blood at various time points before and after CART infusions revealed expected stable levels of cytokines, including IL-6, IL-1β, IL-12, IL-15, interferon-γ, monocyte chemoattractant protein 1 (MCP1), macrophage inflammatory protein 1-alpha (MIP1α), IL-8 and granulocyte-macrophage colony-stimulating factor (GM-CSF). In the ascites, however, soaring IL-6 levels were detected with a peak value of 18,516 pg/ml on Day 7, one of the highest IL-6 levels on our CART trials, whereas no compartmental CRS or other manifestation in local abdomen was observed. The trend of ascitic IL-6 coincided with that of CAR transgene numbers. Furthermore, sIL-6R levels in ascites were lower than in serum (p < 0.0001). Importantly, the ratios of IL-6:sIL-6R were 400 to 1000 fold higher in ascites compared with in serum, highlighting that sIL-6Rs were relatively depleted in the ascites. Indeed, the absolute levels of ascitic IL-6 were close to, or exceeded at some time points, those of sIL-6R, indicating that the bottleneck of the formation of IL-6/sIL-6R signaling complex might switch from IL-6 to sIL-6R in the ascites. Flow analysis demonstrated neutrophils account for 15-20% of the total cells in ascites compared with 40-75% in serum, indicating that lack of neutrophils might be a potential cause of the low sIL-6R levels. Additionally, although sgp130 levels in ascites were also lower than in serum, the absolute values were still overwhelming to those of IL-6 and sIL-6R. Lastly, the levels of MCP1, IL-8 and angiopoietin 2 were significantly elevated, and flow analysis revealed that approximately 25% of the ascitic cells were CD14+CD11b+, indicating monocytes/macrophages might be recruited and/or activated. The levels of MIP1α, GM-CSF, IL-1β, IL-12, IL-15, interferon-γ were low in ascites during the whole period. In conclusion, our data show that relative depletion of sIL-6R in ascites may completely abolish the development of local CRS in the presence of high IL-6. This is the first report to demonstrate the features of biomarkers in ascites after CART therapy, and to describe the impacts of disproportionately low sIL-6R on IL-6 signaling without drug intervention. These results imply that the unique microenvironments in exudates such as ascites may lead to distinct pathophysiological events following CART therapy, and IL-6:sIL-6R ratio should be considered as an indicative parameter in the toxicity management of CRS. Abstract: In many countries, abortion is the most contested issue being publicly discussed, in part, because it has important implications for gender equality, health, family formation, politics, and views about morality. Personal religious beliefs are often seen as having an important role in shaping disapproving attitudes. However, little attention has been given to the country religious context for understanding public opinion about abortion. Drawing on ideas from cultural sociology, the anti-ascetic hypothesis, and the moral communities hypothesis, this study investigates the individual and country religious associations with abortion attitudes. Using hierarchical modelling techniques and survey data from over 70 nations, representing the majority of the world’s population, the multilevel roles of religion are investigated. Both personal and country levels of religious importance are associated with the disapproval of abortion, but there are few differences across religions. Laws allowing for abortion are marginally associated with attitudes and there is no moral communities effect, whereby overall levels of religious importance have a moderating influence on the relationship between personal religiosity and attitudes. However, individual religious importance appears to have a greater association with disapproval in richer, rather than poorer, countries, providing novel insight into why abortion has remained controversial in many countries, especially those with higher levels of economic development.

The twisting paths of the sufis – the Turkic-Balkan motifs in the sufi ‘tariqa’ concept in selected examples of contemporary literary works The first part of this paper summarises how Sufi brotherhoods formed in the Balkans with some references to their Turkic-Ottoman sources. Islamic mystical movements constituted part of the Islamisation initiatives in the territories occupied by the Ottoman Empire: mystical teaching was apparently more successful among the local people than conservative Islam. Crypto-Christianism was a typical phenomenon among converted Slavs. Orders using the language of symbolic tales (like the Bektashi) involved some Christian rites. The orders which proved most popular were the Mevlevi, Naqshbandi, Chalwati and Bektashi, which are still active in some parts of the Balkans. Their role also reconciled the national thought of newly forming national identities in the Balkans. In the novels Death and the Dervish by Meša Selimović and Konak by Ćamil Sijarić, the mystical idea of the Arab tariqa (tarika), i.e. „path,” meets the modern literary concepts of fate and the search for truth and sense of life. The paper constitutes an attempt to present how these two aesthetics have been unwoven into modern literary texts. Kręte drogi sufich. Praca odwołuje się do znanych postaci derwiszy, zarówno tych z epoki literatury świętych podbojów islamu, jak i tych wykreowanych postaci zasłużonych derwiszy noszących cechy chrześcijańskich „świętych”. W części analitycznej tekst podejmuje interpretację współczesnych literackich kreacji ascetów i mistyków oraz realizowania przez nich koncepcji „drogi”. Służy temu analiza poszczególnych wątków z powieści Mešy Selimovicia Derwisz i śmierć i Sijaricia Ćamila Ja, eunuch. Całość jest próbą przyjrzenia się na wybranych przykładach, jak i czy współczesna literatura południowosłowiańska splata koncepcję literacką z filozofią mistycyzmu bałkańskiego, tworząc oryginalne, autorskie odsłony.
