The purpose of this study was to incorporate attachment theory and psychopathy into a transactional model to explain the development of disruptive behavior disorders in children. The model tested in this study proposed two broad pathways leading to the development of disruptive behavior disorders. Each pathway was characterized by an atrisk child temperament, negative reactivity and psychopathy, which when embedded in an at-risk environment, would result in conduct problems. Hyperactivity and negative life events were hypothesized to be broad band risk factors for both pathways. The first pathway, characterized by callous-unemotional traits (CU), was hypothesized to be positively associated with thrill seeking behavior and proactive aggression in the child, and insecure attachment in the caregiver. A second pathway, characterized by child negative reactivity, was hypothesized to be positively associated with reactive aggression in the child and disorganized attachment in the caregiver. Data was collected from 48 low income caregiver/child dyads. Children were between the ages of 6 and 12 (mean age=9.3, SD=1.85), and received services from a state mental health clinic. A series of hierarchical regression analyses were performed to evaluate the relationship between the predictor variables and conduct problems. A primary finding was an extremely strong positive correlation between CU traits and conduct problems. Also, several distinct differences were found between groups of children low and high on CU traits. For those children low on CU traits, thrill seeking behaviors were positively associated with conduct problems, while negative life events, attachment insecurity, and attachment disorganization were all negatively associated with conduct problems. For the children high on CU traits, thrill seeking and attachment insecurity had no meaningful impact on conduct problems, while negative life events and attachment disorganization were positively associated with conduct problems. Hyperactivity, proactive aggression, reactive aggression, and negative reactivity were all broad risk factors for conduct problems in this study. The findings of this study suggest that several developmental pathways do exist for children who develop conduct problems, and that future research should utilize developmental models that include a number of broad risk factors, as well as factors that may be specific to certain developmental pathways.

Increasing numbers of children being referred for mental health services are exhibiting problematic behaviors that can be subsumed under the category of Disruptive Behavior Disorders. This study with its foundations in a post-positivist approach was designed to explore treatment effectiveness of cognitive behavioral intervention applied to adolescents at Oak Grove Institute, a residential treatment facility. This study hypothesized that Wexler's PRISM Model, with its integration of affect, would be instrumental in modifying disruptive behavior as measured at Oak Grove Institute. Although the small sample size precluded statistically significant findings, there were interesting results with respect to two dependant variables. Findings approached significance on measures of impulsivity and verbal aggression. That is, impulsivity and verbal aggression scores were lowered.

The purpose of this study was to investigate the effects of self-management strategies as a means of reducing off-task and disruptive behaviors of elementary school children identified as emotionally disturbed or behavior disordered (E/BD). This study provided a practical approach for classroom teachers to implement self-management strategies in classes that include children identified as having E/BD. Five elementary school children who were formally evaluated and enrolled in a special education classroom for students with E/BD were selected to participate in the study. The study also examined the effects of the self-management procedures when targeted behaviors were monitored by peers. Four resource students from the regular education class served as peer monitors. An ABAB reversal design was used to assess the effectiveness of the self-management strategy in the special education classroom. A behavior rating scale was used at the beginning of the study to develop a baseline of student behavior and during the final phase of the intervention to measure progress. The data indicated that the self-management strategies decreased the levels off-task and disruptive behaviors for all participating students. The findings of this study substantiates previous research that suggests self-management techniques help student to manage their own behaviors.

The purpose of this study was to examine the characteristics that distinguish children with behavior disorders who drop out of psychotherapy treatment from those who remain in treatment. The sample included 379 children (268 male and 111 female) who were diagnosed with a behavior disorder at Dallas County Mental Health/Mental Retardation (MH/MR), a community mental health clinic in Dallas, Texas. The results indicated that certain characteristics increased the likelihood that a child would drop out of therapy, including reliance on aid, the presence of maternal psychopathology, and more severe externalizing and internalizing behaviors. This study also found that younger children with behavior disorders had a greater probability of dropping out of treatment. Minority status, gender, parent marital status, and referral source were not found to be associated with dropping out of treatment. Future studies should focus on specific interventions that clinicians could employ to deter premature termination from treatment.

Behavioral skills training (BST) has been widely used to effectively and quickly instruct learners with limited knowledge in behavior analytic skills. A training package composed of didactic instruction presented via PowerPoint, modeling, rehearsal, feedback and in vivo components were utilized to instruct two mothers of children with autism spectrum disorders to deliver learning trials with their child to contrive MOs to increase their manding repertoires. A multiple probe across participants design was used to assess the effects of BST on the participant's performance. Prior to the beginning of the study, participants were instructed to systematically identify child's reinforcers by delivering a paired choice preference assessment. Mothers were involved in choosing the manding targets used in the child's instruction. BST was effective in demonstrating rapid acquisition of skills taught to both mothers compared to baseline performances. Although child's behavior increased over pretest measures, it did not improve significantly due variable responding. Time constraints and child's excessive variable responding lead to termination of the study.

Metopic synostosis is a condition in which the metopic suture of the human cranium fuses prematurely and may be related to poor behavioral inhibition leading to behaviors commonly associated with Attention-Deficit Hyperactivity Disorder (ADHD). The purpose of this project was to examine the behavior patterns among children with a history of metopic synostosis. It was hypothesized that children with a history of metopic synostosis would exhibit many of the same behavioral patterns associated with ADHD. It was also hypothesized that children with a history of simple synostosis not involving the metopic suture would not evidence this type of behavioral pattern. In order to test these hypotheses, the behavior of three groups of children was compared including (1) children who had a history of metopic synostosis (M= 7.63 years, SD = 1.92 years), (2) children who had a history of simple craniosynostosis not involving the metopic suture (M= 7.54 years, SD = 1.88 years), and (3) a group of children diagnosed with ADHD (M=7.78 year, SD = 1.87 years). It was found using the Home and School versions of the Attention Deficit Disorders Evaluation Scale (ADDES) that children with a history of metopic synostosis demonstrate significantly more behavioral disturbances than children with a history of simple craniosynostosis not involving the metopic suture. Using the BASC Teacher Rating Form it was found that children with a history of metopic synostosis have a behavior pattern similar to children diagnosed with ADHD and a dissimilar behavior pattern compared to children who have a history of craniosynostosis not involving the metopic suture. Using the BASC Parent Rating Form it was found that children with a history of metopic synostosis have a behavior pattern dissimilar to children diagnosed with ADHD and a dissimilar behavior pattern compared to children who have a history of craniosynostosis not involving the metopic suture.

This study investigated whether behavior disordered children would decrease aggressive behavior if their social interest were developed. Three hypotheses that were tested predicted that there would be a significant difference between the control group and the experimental group on adjusted mean scores on aggressive behavior on post test scores. The measuring instruments used were the Child Behavior Checklist Parent Report Form, the Child Behavior Checklist Teacher Report Form, and the Child Behavior Checklist Director Observation Form. It was also predicted that there would be a significant difference between the control group and the experimental group on post test adjusted mean scores as measured on the Social Interest Scale. An analysis of covariance was employed to test the data. Behavior disordered students in the experimental group participated in three activities designed to develop their social interest. They participated in peer tutoring, socialization with nursing home residents, and group discussions. Data were collected from parents, teachers, and observers of behavior disordered students in an elementary school in Northwest Louisiana during the summer term of 1987. Teachers did report a statistically significant difference between the experimental and the control groups in the decrease of aggressive behavior. These results are in accord with predictions generated by Adlerian theory and with naturalistic data. Parents and observers did not report a statistically significant difference between the two groups in the decrease of aggressive behavior. Significant differences were not found between the experimental and control groups in the development of social interest. Since the teachers did report statistically significant results in this study, it is recommended that these same activities to develop social interest be repeated, that counseling sessions be designed to be more therapeutic, and that additional modeling and role playing be included. It is further recommended that an instrument be developed to measure social interest in children, and that parents and teachers of the children be provided with education and training in Adlerian theory and its application to their interactions with children.

Thesis (M.A. (Clinical Psychology) --University of Limpopo, 2007
Disruptive Behaviour Disorders are often associated with high rates of school drop outs, academic skills deficits and low achievement, drug abuse, low self esteem, depression, delinquency and incarceration. The main aim of the study was to establish whether children with DBDs are cognitively and/or neurologically impaired. The study investigated (1) whether children with DBDs show deficiencies in cognitive and motor functions and (2) whether gender and subtype influence cognitive and motor functions. Method: The sample of 137 children with DBDs (ADHD, ODD and CD) and those without a diagnosis of DBDs was drawn from children aged between 8 and 15 years. They were assessed using instruments that were selected to be measures of Executive Functions, cognitive functioning, and motor functions. The scores obtained from the administration of these measures were compared for significant differences between the DBD subtypes and a non-DBD control group as a possible function of gender. Results: The findings indicate that children with symptoms of DBDs performed poorer than the control group on all tests with the exception of the Digits backward. EF and motor impairments are associated with ADHD-C and ADHD-PI, and not with ADHD-HI, ODD and CD. Although among the DBDs, neuropsychological and cognitive impairments have been found to be severe in children with ADHD-PI and ADHD-C, the ADHD-C subtype showed qualitatively larger differences with the normal control group on most measures. There were no differences found between the genders in the performance on all tests that were administered. Conclusion Children of the ADHD-C and ADHD-PI subtypes are significantly more impaired on measures of Executive, cognitive and motor functions than those with ADHD-HI, ODD and CD and those without externalising disorders. However, the ADHD-C subtype found to be more severely impaired when compared with the ADHD-PI subtype.

Empathy is a construct that plays a pivotal role in the development of interpersonal relationships, and thus ones ability to function socially and often professionally. The development of empathy in children is therefore of particular interest to allow for further understanding of normative and atypical developmental trajectories. This thesis investigated the assessment of affective empathy in children aged 5-12, through the development and comparison of a multimethod assessment approach. Furthermore this thesis evaluated the differential relationships between affective empathy and global behavioural problems in children versus the presence of early psychopathic traits, such as callous-unemotional traits. The first component of this study incorporated; a measure of facial expression of affective empathy, and self-reported experience of affective empathy, as measured by the newly designed Griffith Empathy Measure - Video Observation (GEM-VO) and the Griffith Empathy Measure - Self Report (GEM-SR); the Bryant's Index of Empathy for Children and Adolescents (1982) which is a traditional child self-report measure; and a newly designed parent-report of child affective empathy (Griffith Empathy Measure - Parent Report; GEM-PR). Using a normative community sample of 211 children from grades 1, 3, 5, and 7 (aged 5-6, 7-8, 9-10, & 11-12, respectively), the GEM-PR and the Bryant were found to have moderate to strong internal consistency. As a measure of concurrent validity, strong positive correlations were found between the mother and father reports (GEM-PR) of their child's affective empathy, for grades 5 and 7, and for girls of all age groups. Using a convenience sample of 31 parents and children aged 5 to 12, the GEM-PR and the Bryant demonstrated strong test-retest reliability. The reliability of the GEM-VO and the GEM-SR were assessed using a convenience sample of 20 children aged 5 to 12. These measures involve the assessment of children's facial and verbal responses to emotionally evocative videotape vignettes. Children were unobtrusively videotaped while they watched the vignettes and their facial expressions were coded. Children were then interviewed to determine the emotions they attributed to stimulus persons and to themselves whilst viewing the material. Adequate to strong test-retest reliability was found for both measures. Using 30% from the larger sample of 211 participants (N=60), the GEM-VO also demonstrated robust inter-rater reliability. This multimethod approach to assessing child affective empathy produced differing age and gender trends. Facial affect as reported by the GEM-VO decreased with age. Similarly, the matching of child facial emotion to the vignette protagonist's facial emotion was higher in the younger grades. These findings suggest that measures that assess the matching of facial affect (i.e., GEM-VO) may be more appropriate for younger age groups who have not yet learnt to conceal their facial expression of emotion. Data from the GEM-SR suggests that older children are more verbally expressive of negative emotions then younger children, with older girls found to be the most verbally expressive of feeling the same emotion as the vignette character; a role more complimentary of the female gender socialization pressures. These findings are also indicative of the increase in emotional vocabulary and self-awareness in older children, supporting the validity of child self-report measures (based on observational stimuli) with older children. In comparing data from the GEM-VO and GEM-SR, this study found that for negative emotions the consistency between facial emotions coded and emotions verbally reported increased with age. This consistency across gender and amongst the older age groups provides encouraging concurrent validity, suggesting the results of one measure could be inferred through the exclusive use of the alternate measurement approach. In contrast, affective empathy as measured by the two measures; the accurate matching of the participant and vignette character's facial expression (GEM-VO), and the accurate matching of the self reported and vignette character's emotion (GEM-SR); were not found to converge. This finding is consistent with prior research and questions the assumption that facially expressed and self-appraised indexes of affective empathy are different aspects of a complex unified process. When evaluating the convergence of all four measures of affective empathy, negative correlations were found between the Bryant and the GEM-PR, these two measures were also found to not converge with the GEM-VO and GEM-SR in a consistent and predictable way. These findings pose the question of whether different aspects of the complex phenomena of affective empathy are being assessed. Furthermore, the validity of the exclusive use of a child self report measure such as the Bryant, which is the standard assessment in the literature, is questioned. The possibility that callous-unemotional traits (CU; a unique subgroup identified in the child psychopathy literature) may account for the mixed findings throughout research regarding the assumption that deficiencies in empathy underlie conduct problems in children, was examined using regression analysis. Using the previous sample of 211 children aged 5-12, conduct problems (CP) were measured using the Strengths and Difficulties Questionnaire (SDQ; Goodman, 1999), and the CU subscale was used from the Antisocial Process Screening Device (APSD; Caputo, Frick, & Brodsky, 1999). Affective empathy when measured by the GEM-PR and the Bryant showed differing patterns in the relationship between affective empathy, CU traits and CP. While the GEM-Father reported that neither age, CU traits nor CP accounted for affective empathy variance, the GEM-Mother report supported that affective empathy was no longer associated with CP once CU traits had been partialled out. In contrast, the Bryant reported for girls, that CU traits were not found to have an underlying correlational relationship. It can be argued from the GEM-Mother data only that it was the unmeasured variance of CU traits that was accounting for the relationship between CP and affective empathy found in the literature. Furthermore, the comparison of an altered CU subscale with all possible empathy items removed, suggests that the constructs of CU traits and affective empathy are not synonymous or overlapping in nature, but rather are two independent constructs. This multimethod approach highlights the complexity of this research area, exemplifying the significant influence of the source of the reports, and suggesting that affective empathy consists of multiple components that are assessed to differing degrees by the different measurement approaches.

Despite a considerable progress in developing and testing psychosocial treatments to reduce youth anxiety disorders, much remains to learn about the relation between anxiety symptom reduction and change in youth functional impairment. The specific aims of this dissertation thus were to examine: (1) the relation between different levels of anxiety and youth functional impairment ratings; (2) incremental validity of the Children Global Assessment Scale (CGAS); (3) the mediating role of anxiety symptom reduction on youth functional impairment ratings; (4) the directionality of change between anxiety symptom reduction and youth functional impairment; (5) the moderating effects of youth age, sex, and ethnicity on the mediated relation between youth anxiety symptom reduction and change in functional impairment; and (6) an agreement (or lack thereof) between youths and their parents in their views of change in youth functional impairment vis-à-vis anxiety symptom reduction. The results were analyzed using archival data set acquired from 183 youths and their mothers. Research questions were tested using SPSS and structural equation modeling techniques in Mplus. The results supported the efficacy of psychosocial treatments to reduce the severity of youth anxiety symptoms and its associated functional impairment. Moreover, the results revealed that at posttreatment, youths who scored either low or medium on anxiety levels scored significantly lower on impairment, than youths who scored high on anxiety levels. Incremental validity of the CGAS was also revealed across all assessment points and informants in my sample. In addition, the results indicated the mediating role of anxiety symptom reduction with respect to change in youth functional impairment at posttest, regardless of the youth’s age, sex, and ethnicity. No significant findings were observed with regard to the bidirectionality and an informant disagreement vis-à-vis the relation between anxiety symptom reduction and change in functional impairment. The study’s main contributions and potential implications on theoretical, empirical, and clinical levels are further discussed. The emphasis is on the need to enhance existing evidence-based treatments and develop innovative treatment models that will not only reduce youth’s symptoms (such anxiety) but also evoke genuine and palpable improvements in lives of youths and their families.

The purpose of the present study was to compare the effectiveness of three indirect models of service delivery in treating children with externalizing and internalizing behavior problems: the highly individualized behavioral consultation model (CBC); minimal consultation with videotape therapy (GVT); and self-administered videotape therapy only (VT). This study also sought to ascertain whether parent and teacher consultation increases parental involvement in their children's education. Specifically, the relationship between perceived self-efficacy and mothers' involvement in their children's education was examined. Thirty-five preschool and elementary school children, their mothers, and teachers were assigned to one of the three treatment conditions. An A-B repeated measures group research design was used to analyze the effectiveness of consultation. Outcome variables included mothers' and teachers' direct observation of target behavior, their ratings of social skills, internalizing, externalizing, and general problem behaviors, and an observational measure of child deviance behaviors and parenting skills. Results indicated that children's target behaviors improved from baseline to treatment in all three treatment conditions. Overall, children's social skills increased and behavior problems decreased over the course of treatment. Pretreatment and posttreatment self-efficacy was assessed via rating scales. Although results suggest that parent and teacher self-efficacy did not change as a result of treatment, self-efficacy ratings were associated with more hours of mother involvement in educational activities per week. These results are discussed in light of their practical and theoretical implications.

The temporal sequence of behavioral and interpersonal perceptual changes in families of problem behavior children was considered. Previous research demonstrated that both child behavior and parents' perceptions of their problem behavior children improved during parent training. A behavioral systems model employing single-case experimental design was used to test the following hypotheses: (1) change in parenting behavior would precede change in child behavior; (2) change in child behavior would precede change in parent perception of the child; and (3) change in parenting behavior would precede change in child perception of the parent.Five two-parent families, recruited through newspaper advertisement, participated in a behavior management training course. Each had a 5-11 year old problem behavior child, as qualified by the Revised Behavior Problem Checklist.Dependent variables consisted of (1) weekly in-home, observer collected parent-child behavior interaction data using a simplified version of the Family Interaction Coding System, (2) weekly questionnaires assessing parent-child interpersonal perceptions and weekly family "crises", and (3) a series of pre-post measures (Parent Attitude Test, Becker Adjective Checklist, and Child Report of Parent Behavior Inventory) to identify change at the .05 level. All subjects commenced baseline procedures simultaneously, but two families continued baseline procedures an additional four weeks before training. Graphic depiction of weekly individual parent and child behavioral and perceptual data was employed to examine the hypothesized changes. The first two hypotheses generally were supported by the results. The third hypothesis was not supported. The findings suggested that during the initial weeks of parent training, parent application of behavior change strategies resulted in improved child behavior and that by the middle stage of parent training, the parents' perceptions of their children began to improve. In addition, there was a reduction in the number of parent-child interactions interpreted as having a calming effect. This served to obscure other change such as the near elimination of aversive consequences.
