BackgroundIncreased rates of psychiatric disorder have previously been reported in those diagnosed with chronic fatigue syndrome (CFS) or myalgic encephalomyelitis (ME), although the direction of causation in this relationship has not been established. We aimed to test the hypothesis that individuals with self-reported CFS/ME have increased levels of psychiatric disorder prior to the onset of their fatigue symptoms.MethodA total of 5362 participants were prospectively followed with various measures of personality, psychiatric disorder and fatigue levels collected over the first 43 years of their life. CFS/ME was identified through self-report during a semi-structured interview at age 53 years.ResultsThirty-four (1.1%) of the 3035 subjects assessed at age 53 years reported a diagnosis of CFS/ME. CFS/ME was more common among females, but there was no association between CFS/ME and either social class, social mobility or educational level. Those with psychiatric illness between the ages of 15 and 36 years were more likely to report CFS/ME later in life with an odds ratio (OR, adjusted for sex) of 2.65 [95% confidence interval (CI) 1.26–5.57, p=0.01]. Increased levels of psychiatric illness, in particular depression and anxiety, were present prior to the occurrence of fatigue symptoms. There was a dose–response relationship between the severity of psychiatric symptoms and the likelihood of later CFS/ME. Personality factors were not associated with a self-reported diagnosis of CFS/ME.ConclusionsThis temporal, dose–response relationship suggests that psychiatric disorders, or shared risk factors for psychiatric disorders, are likely to have an aetiological role in some cases of CFS/ME.

AbstractThe current state of the art in field experiments does not give me any confidence that we should be assuming that we have anything worth scaling, assuming we really care about the expected welfare of those about to receive the instant intervention. At the very least, we should be honest and explicit about the need for strong priors about the welfare effects of changes in averages of observables to warrant scaling. What we need is a healthy dose of theory and the implied econometrics.

BackgroundArthrocentesis is a procedure performed to aspirate synovial fluid from a joint cavity. It has both diagnostic and therapeutic utilization in outpatient and inpatient settings. While arthrocentesis is considered a minor surgical procedure, there is always potential to injure blood vessels, nerves, and tendons1. Although joint arthrocentesis is not an Accreditation Council for Graduate Medical Education procedural requirement for Internal Medicine (IM) residents in the United States, several studies have revealed that training on knee arthrocentesis and injection provided a solid foundation to medical residents toward increasing their confidence and refining motor skills2.ObjectivesTo evaluate the current Roger Williams Medical Center (RWMC) IM residents’ confidence level in performing intra-articular injection. Results from this study will inform the design of resident-orientated quality improvement arthrocentesis teaching series in RWMC.MethodsProject was incepted and designed by one rheumatology faculty, one fellow and five IM residents of RWMC. We selected the survey as a tool to elicit information from all current IM residents who filled out a 7-question survey regarding their residency experience and exposures to knee aspiration and injection. We used Survey Monkey as an anonymous survey tool (see Table 1). More than half of the resident population (51.28%) do not think they have adequate exposure to knee arthrocentesis. 73.68% respondents have never performed knee injections/arthrocentesis. According to the survey, 94.87% of the residents would be interested in learning to do knee procedures. Having exposure to knee arthrocentesis during residency was important to 36.84% and very important to 34.21% of the trainees.ConclusionThe survey results demonstrate that most of the IM residents had no prior exposure and are uncomfortable with knee arthrocentesis. However we proved that there is high interest in learning this procedure. Literature review shows that structured training sessions have a long-term effect in competency and confidence with similar procedures. PURPOSE This pilot, open-label study evaluates the antiemetic efficacy and safety of a single 20-mg intravenous (IV) dose of dexamethasone combined with a single IV dose of ondansetron (32, 24, or 8 mg) in patients receiving highly emetogenic (HE), moderately high emetogenic (MHE), or moderately emetogenic (ME) chemotherapy, respectively. PATIENTS AND METHODS One hundred forty-six patients received a single 20-mg IV dose of dexamethasone over 15 minutes beginning 45 minutes before chemotherapy and either a single 32-, 24-, or 8-mg IV dose of ondansetron over 15 minutes beginning 30 minutes before chemotherapy. Patients were evaluated for emetic episodes, extent of nausea, and adverse events for 24 hours after chemotherapy. RESULTS Complete response (no emetic episodes) was noted in 72% (95% confidence interval [CI], 60% to 84%), 88% (95% CI, 79% to 97%), and 77% (95% CI, 63% to 92%) of patients in the HE, MHE, and ME categories, respectively. The proportion of patients who experienced no nausea on the posttreatment assessment was 51% (95% CI, 37% to 64%), 69% (95% CI, 56% to 81%), and 47% (95% CI, 29% to 65%), respectively. The antiemetic regimens were all well tolerated. The proportion of patients with any drug-related adverse events did not vary across the three study groups despite the range of ondansetron doses and variety of chemotherapy regimens. Mild headache was noted in 28% of patients. Other adverse events, all of which were noted in fewer than 10% of patients, included lightheadedness, fatigue, dizziness, and constipation. CONCLUSION A single IV dose of either 8, 24, or 32 mg of ondansetron combined with a single 20-mg IV dose of dexamethasone resulted in good control of acute emesis across a wide spectrum of chemotherapy regimens. Nausea control proved somewhat more difficult, with approximately 50% of patients in the HE and ME emetogenic categories experiencing some degree of nausea. The results of our pilot study suggest that adjusting the dose of ondansetron to the intrinsic emetogenicity of the chemotherapy regimen permits a more efficient use of ondansetron while maintaining good antiemetic control. Such an approach appears worthy of further investigation.

Ukraine finds itself in the countries of the Global North, which means that it adopts the formulation of its typical research questions like the current migration research landscape. Existing research are heavily skewed towards the Global North, where governments and international organizations increasingly fund them to inform policy development. Thus, migration provides the relevance of the study of discourses around the body of concepts on it. The article describes the subtexts of media characterising the perception of migration by migrants themselves and the perception of migrants by host side. With using the Mediateka tool, an automated sample of 39,000 reports on various groups of migrants was received in the TOP-15 of the Ukrainian media (print media, TV channels, informational online media) from 2015 to 2018. The sample consisted of 12,000 messages, every tenth of which was selected for further in-depth analysis. The research was based on qualitative content analysis and critical discourse analysis. The six steps were done: looking at the description of the migration as objective necessity, subjective aspiration and something meaningless, and looking at the description of migrants as decent people, indecent people and just people with some lifestyle. The latent meanings of the messages, where the leading Ukrainian media touched on the perception of migration and migrants, were divergent and contradictory; they could not become the basis for confidence in any permanent characteristics of the images of migrants. As one of the most interesting prospects for further research, it may be the test of whether those transmitted meanings, which are usually considered positive, can interfere with social integration, and vice versa – whether negative meanings can help it. This can be done on the basis of additional empirical research using quantitative methods, such as cluster analysis to check which groups media consumers might be divided into after receiving the information about migrants.

Rice fields, especially rice straw, is one part of the agricultural sector that contributes to global warming. It is estimated that about 1% of global warming comes from rice fields. One of the efforts that can be done in reducing the contribution of paddy fields is to utilize the remaining harvest in the form of rice straw as animal feed. However, the utilization of rice straw is constrained by its low nutritional value. Therefore it is necessary to carry out ammonia to improve its quality. This study was conducted to determine the potential of rice straw to produce methane gas and to improve its quality using ammonia technology as ruminant feed. This study used a completely randomized design with four urea levels (0, 2, 4, 6% w/w) and six replications. The variables measured were gas production, methane production, total volatile fatty acids (TVFA), metabolic energy (ME), dry matter digestibility (DMD), and organic matter digestibility (OMD). The data obtained were analyzed using analysis of variance, and differences between means using Duncan's multiple range test with a confidence level of P <0.05. The results of the calculation of the production of methane (CH4) and carbon dioxide (CO2) from rice straw in Jambi Province in 2021 reached 9,024 Gg CH4 per year and Carbon 3362,167 per year. The results of ammonium hay showed that urea level had a significant effect (P<0.05) on total gas production, gas production of CH4, DMD, OMD, TVFA, and ME. The ammoniated rice straw with urea level up to 4% was the best treatment for total gas production, CH4, TVFA, and ME production. The results of the study concluded that ammoniated rice straw with 4% urea content was the best treatment to improve the quality of rice straw as ruminant feed.

Objective. To assess the effectiveness of the low-dose air ozonation for disinfection of the air in the working room. Materials and methods. We investigated 90 air samples (3 samples were taken weekly before and after the production meeting using the automatic sampling device of biological aerosols of air PU-1B). The total bacterial contamination, the content of staphylococci and mold spores were determined. Ozonation of the room (83.3 m3) was carried out for 20 minutes by means of domestic ozonator. The accumulated dose of ozone was 133.3 mg (1.6 mg/m3). Statistical data processing was carried out using the MedStat licensed program. The median, median error (Me me), left and right 95 % confidence intervals (95 % CI) were calculated. Paired comparisons were made using Wilcoxon's T-test. Results. After the meeting, the total bacterial contamination of the air was 56.0 9.3 (47.078.0) CFU. The content of staphylococci and mold spores in the air was 85.5 12.5 (76.0100.0) and 44.5 6.5 (32.054.0) CFU, respectively. After ozonation, the total bacterial contamination of the air was 14.5 3.6 (10.021.0) CFU. The content of staphylococci and mold spores in the air after ozonation was 35.5 6.7 (25.052.0) and 26.0 5.0 (18.032.0) CFU, respectively. Ozonation of the room provided a significant decrease (p 0.001) in all three of the above indicators. The room ozonation carried out promoted a reliable decrease (p 0.001) in all the above mentioned parameters. Conclusions. The above data and analysis of the literature show the possibility of using low doses of ozone for the prevention of bacterial, fungal and viral infections including SARS-CoV-2. Further study and development of reasonable modes of ozone disinfection, including low doses of ozone, is needed, as well as determination of the efficiency degree of air disinfection with non-toxic gas concentrations.

OBJECTIVE The purpose of this study was to assess the effect of low-fidelity simulation on students' confidence, knowledge, and skills in pediatric physical assessments, and to compare students' interest ratings of topics and effectiveness of learning activities between students' who experienced simulation and those who did not. METHODS Within a pediatric elective, a vital signs and physical assessment activity was re-designed to incorporate a low-fidelity heart and breath sounds simulator. Students rated their confidence in completing 9 different physical assessment skills before and after the activity and assessment. Students' perspectives of the activity were also assessed. Course evaluation surveys were compared with prior course offerings (without simulation) to determine a change in students' interest ratings of the topic and effectiveness of learning activities. The Wilcoxon signed rank test, thematic analysis, and descriptive statistics were used to analyze outcomes. RESULTS All 106 second professional year students in the elective completed the pre- and post-simulation surveys and course evaluations for 3 offerings. Students' post-simulation average confidence scores increased statistically on all 9 skills compared with pre-simulation scores. All students agreed or strongly agreed “the lecture and simulation activity done in class helped me overcome challenges I had with learning the skill.” Students (98%) successfully demonstrated competency on the formal assessment. Compared with previous course offerings, students reported higher interest ratings in the topics and instruction effectiveness when simulation was incorporated into the activity. CONCLUSIONS Low-fidelity simulation is an effective teaching and learning approach to increase students' confidence, knowledge, and interest in pediatric vital signs and physical assessment.

Body dissatisfaction is a significant concern with severe and persistent consequences. Thus, there is a need for effective prevention and early intervention strategies that can be delivered in a timely and efficient manner, such as within the context of schools by teachers. There have been calls within the field for researchers to evaluate existing interventions under diverse conditions prior to global dissemination, and adopt an ecological approach by extending interventions to include parents. The project included two consequative studies conducted two-years apart. The first study aimed to identify effective intervention strategies for improving body image outcomes in Year 8 girls. Heeding calls for rigorous and independent evaluation of existing programs under varied conditions, Study 1 aimed to replicate the UK school-based body image program Dove Confident Me (DCM) among a selective population of adolescent girls in Australia. Expanding on Study 1, Study 2 aimed to improve body image outcomes for both Year 8 girls and their mothers. The second study evaluated a modified version of DCM alongside an investigation of Raising Confident Girls (RCG), a school- based 3-session seminar delivered to mothers. Further, the thesis aimed to understand factors contributing to improved parental uptake in body image programs and to examine whether extending classroom-based interventions to include mothers enhances the effectiveness of outcomes in daughters. Study 1, involving teacher delivery of DCM to Year 8 students (n=198) attending an independent girls’ school in Australia, hypothesized that compared to the control group (n=208), girls receiving DCM would report significant improvements in body image and psychosocial outcomes, alongside reduced severity of known eating disorder risk factors and behaviours. Multilevel mixed modeling analyses revealed significant intervention effects for social comparison and sociocultural pressure, but not in the direction hypothesized. A lack of teacher confidence with delivery, limited student engagement with the UK version of the program, and poor student-teacher relationship due to a timetable issue were highlighted as areas for improvement. Study 2, answered calls within the body image field to develop both etiological and ecological programs by conducting a second replication of a modified version of DCM and the addition of a parental intervention Raising Confident Girls (RCG) delivered to mothers. The modified DCM program was delivered to Year 8 students (n=242) and outcomes were compared with a control group (n=354). Despite significant improvements in acceptability and engagement ratings, the modified DCM program did not improve body image outcomes for participants. Interestingly, the intervention group reported a significant increase in both internalization of the thin-ideal and perceived sociocultural pressure following participation in the intervention. Raising Confident Girls (RCG), was delivered to Year 8 mothers (n=69) and outcomes were compared with a control group (n=51). Multilevel mixed modelling analyses revealed that mothers who participated in RCG reported significantly greater body esteem and body appreciation compared to the control group. Further, as predicted, participation in RCG improved a mother’s knowledge, confidence and skills parenting an adolescent girl, and improved her positive role modeling for her daughter with respect to body image. Receiving high acceptability ratings, strong engagement and low attrition rates, the RCG program appeared successful in overcoming long held difficulties with engaging parents in body image interventions. Finally, Study 2 examined whether students completing DCM benefited from having their mother attend RCG. Students whose mothers participated in RCG demonstrated a significant change in appearance-based talk at 3-month follow-up compared to students whose mothers were not involved in RCG. Additionally, there were noticeable improvements in a number of body image outcomes from pre-test to post-test for the group of students whose mothers attended RCG, however none of these findings reached significance. The study offered valuable insights towards increasing our understanding of transfer of parent intervention outcomes to daughters. The findings of the thesis contribute knowledge to the field of research regarding body-image intervention for adolescent girls and their mothers, in addition to providing practical insights for schools intending to implement body image interventions. Specifically, the study draws attention to the complexities of global dissemination and the limitations of using selective and universal programs interchangeably. The thesis highlights that while researchers are experts in etiological theory, school personnel are experts regarding their community. Cognizant of this, researchers are encouraged to work together with school personnel to develop school-based resources malleable in content and design, but robust enough to sustain effectiveness when adapted to suit diverse school environments. While the findings add to the growing body of research supporting task-shifting facilitation of body image programs to teachers, findings suggest that a strong student-teacher relationship and perceived credibility and competence of facilitator can be as essential as content of program. The thesis provides deeper insight into improving parental engagement in body image interventions delivered within the school context. Specifically, the findings emphasize the importance of tailoring the intervention to suit the needs of the parent group and suggest that the process of delivering a parent program is as important as the content of the intervention. Finally, the study reveals that providing an intervention to mothers alongside a classroom-based intervention for students enhances outcomes for daughters. Overall, the thesis supports the premise of extending classroom-based body image interventions to include parents, and identifies a number of recommendations for further research.

Uneven action characterized Federal attacks all along the Union line. In James B. McPherson’s Seventeenth Corps, which held the center of Grant’s line, the only serious attack was conducted by Thomas E. Ransom’s brigade and it failed to come closely to grips with the entrenched Confederates on its sector. In John A. McClernand’s Thirteenth Corps, holding the left of the Union line, all that was done was to move the three divisions of the corps forward to reach positions from which they could launch a serious attack. This was all that seemed possible given the rugged terrain and heavy Confederate artillery fire. The Federals had not conducted a general attack or mounted a serious effort to find and break through weak points in the enemy defence line. Their enthusiasm had dampened somewhat but they still retained strong confidence in ultimate success. The effect of the Union repulse of May 19, however, was dramatic among the Confederates. Just how much confidence should we place in published research findings, even if peer reviewed? What should we ignore, reject, modify, incorporate, pursue? To answer these questions, the sciences and the humanities must be continually in the business of keeping the record of knowledge straight at the edge, an enterprise the Internet can fruitfully enhance. Accordingly, this chapter looks at some Internet-based possibilities concerning this postpublication process: watch­dog blogs in the sciences, blogs and discussion forums in the sciences and humanities, and book and article reviews in the humanities. For these activities, as for peer review, Habermas’s ideal speech situation provides a useful theoretical framework. The goal is the same: the achievement of rational consensus concerning the originality, significance, argumentative competence, and clarity of expression of the work in question. After reading Chaucer’s Canterbury Tales—after the sweeping “Prologue,” the dramatic “Pardoner’s Tale,” the raucous “Miller’s Tale,” the sermon that is the “Parson’s Tale”—readers come upon what may well be the world’s first “Retraction Notice”: . . . Now I pray to all who hear or read this little treatise, that if there is anything in it that they like, they thank our Lord Jesus Christ for it, from whom proceeds all wisdom and goodness. And if there is anything that displeases them, I pray also that they attribute it to inadvertence rather than intent. I would have done better if I could. For the Bible says, “All that is written is written to support the teaching our faith” and that is what I wish to do. Therefore I beseech you meekly, for the mercy of God, that you pray for me that Christ have mercy on me and forgive my sins, especially my translations and works of worldly vanity, which I revoke in my retractions. . . . In acknowledging error, some editors of science journals lack the poet’s candor. One minced no words, responding to a request from the editors of the blog “Retraction Watch”—Adam Marcus and Ivan Oransky—for reasons that a paper was retracted with the following terse comment: “It’s none of your damn business.”

It Was a cold evening in early December 2006, and Tracy Warner had just returned home from Willits Primary School. Ryan had just sung in the “Winter Wonderland” musical there. Christmas lights dotted F Street, adding some warmth to her modest block in the heart of Monmouth, Illinois. She looked like a new woman, and, judging by her smile, she knew it. The jeans and T-shirt—the uniform of the anxious, soon-to-be-unemployed line worker and picketer of a couple years earlier—had been replaced by a red V-neck sweater, silk blouse, and an aura of confidence. She was wrapping up four fall semester classes and a journalism internship at the school’s news­paper, the Western Courier. She had done this while raising Ryan and frantically looking for a job. She was set to graduate on the following Saturday from Western Illinois University. The dream Warner had dreamt a thousand times while piecing together refrigerator doors on the Maytag line for over fifteen years was coming true. “Look at this,” she said, handing me an essay. “It’s a paper on Rawls’ theory of justice. He said that we have to stand behind a veil of ignorance to make fair decisions.” Her reference fit the moment. John Rawls’ 1971 Theory of Justice poses a hypothetical world in which all societal roles are shuffled behind a metaphorical “veil of ignorance.” Behind this veil, one does not know to what role he or she will be assigned in the new social order. It is only from there, Rawls argues, can one truly judge the fairness of various social roles and relations. The CEO, for instance, would have to experience the lives of workers he put out of work. Warner still saw Ralph Hake as a great villain—and it was not just because of the factory closing and the gutting of her working life as well as the working lives of her friends and co-workers. Warner had embraced the changes as best she could, and she and Ryan would find a way to survive.

"the claim that if anything of the sort had occurred I would have brought a plea in bar of action against him, but that I should come to court with this plea and demonstrate to you both that I have done this man no wrong and that his prosecution of me is illegal. [4] Euergos and I loaned one hundred and five mnai to Pantainetos here, judges, on the security of a processing plant among the mine workings at Maroneia and thirty slaves. Forty-five mnai of the loan were mine, while one talent belonged to Euergos. As it happened, Pantainetos owed a talent to Mnesikles of Kollytos and forty-five mnai to Phileas of Eleusis and Pleistor. [5] The individual who sold the processing plant and the slaves to us was Mnesikles (he was the one who had bought the property for Pantainetos from Telemachos, its former owner), and Pantainetos leased it from us for the interest accruing on the money, one hundred and five drachmas per month. We made a contract in which were written the terms of the lease and a right for Pantainetos to redeem the property from us within a stated time. [6] Once this had been completed in the month of Elaphebolion in the archonship of Theophilos, I sailed off to the Black Sea, while this man and Euergos were here. As to their dealings with each other while I was away, I could not say. For their versions do not agree with each other, nor does Pantainetos’ version always agree with itself. So when all the facts were proved before the arbitrator as is happening now, and it was clearly demonstrated that he was guilty of the charges, [31] he entered a lying testimony and named as witnesses people whom I think you too will not fail to recognize, if you hear them: ‘Diotimos son of Diotimos of Ikaria, Archebiades son of Demoteles of Halai, Chairetios son of Chairimenes of Pithe, testify that they were coming away from dinner with Konon and came upon Ariston and Konon’s son fighting in the agora, and Konon did not strike Ariston.’ [32] As if you would immediately trust them and would not work out the truth, that to start with neither Lysistratos nor Paseas nor Nikeratos nor Diodoros, who have explicitly attested that they saw me being beaten by Konon and stripped."
