Malignancy in ectopic thyroid tissue is reported in <1% of these cases.[1] We report a case of aggressive papillary carcinoma in ectopic thyroid gland in a patient with Graves’ disease. Case: A 65-year old woman was incidentally found to have a 3.1 cm mass with coarse calcifications in the superior mediastinum on CT scan of chest. We present a case of incidental finding of ectopic thyroid tissue within mediastinum in a 61-year-old female patient with a history of total thyroidectomy for thyroiditis and nodules. The patient presented to emergency room with cough and right chest pain and underwent a chest computed tomographic angiogram (CTA) to exclude pulmonary embolism as part of chest pain workup. One right paratracheal mediastinal soft tissue nodule was visualized on the images of CTA.

Ectopic thyroid tissue located in or adjacent to the gallbladder wall is extremely rare with only two cases reported since 1969. These cases were all identified incidentally by pathological examination after cholecystectomy for acute or chronic cholecystitis. We here report a case of ectopic thyroid tissue presenting as a gallbladder mass in a 60-year-old woman who underwent cholecystectomy. Ectopic thyroid tissue in the gallbladder wall was confirmed by histopathology.

Ectopic thyroid tissue is ubiquitous in the body, most frequently seen at the base of tongue, accounting for approximately 95% of reported cases. Branchial cleft cyst, a congenital abnormality, can occasionally harbor thyroid tissue inside the cyst. These ectopic thyroid tissues can develop the same disease as the orthodox thyroid gland, including the malignant diseases. Papillary thyroid carcinoma arising in the branchial cleft cyst was extremely rare, total of only 14 cases were reported in the literature. Herein, I report additional case which was discovered incidentally after surgery.

The presence of two ectopic foci of thyroid tissue in a single patient is an uncommon presentation. Ectopic thyroid reportedly has a low incidence. We are reporting here a case of dual ectopic thyroid present in the lingual and sub hyoid region in a 33 years old lady with short stature and hypothyroidism. Ultrasonography of the neck had revealed no thyroid gland in its normal anatomical position. The presence of ectopic thyroid tissue in the lingual and sub hyoid region were confirmed with a technetium-99m sodium per technetate thyroid scan.

Ectopic lingual thyroid glands may occasionally undergo massive hypertrophyand be a source of airway obstruction. Securing the airway prior to surgery in such cases may be difficult. In the present paper, we report on two cases of obstructive ectopic lingual thyroids which were successfully managed endoscopically using CO2 laser. The use of CO2 laser for removal of ectopic lingual thyroid tissue offers advantages over traditionalsurgical techniques. In our opinion, removal of an appropriate amount of gland to secure an adequate airway is all that is required in most cases.
